Pseudomonas aeruginosa-related effusive-constrictive pericarditis diagnosed with echocardiography:A case report

BACKGROUND effusive-constrictive pericarditis(eCP)is an uncommon pericardial syndrome.Careful echocardiographic examination may provide helpful information not only for diagnosing but also for managing eCP.eCP has various etiologies;however,Pseudomonas aeruginosa(P.aeruginosa)infection has not been reported as a cause to date.Herein,we present a rare case of eCP caused by P.aeruginosa infection,which was followed up using echocardiography.CASe SUMMARY A 30-year-old man was admitted to our hospital with a 2-mo history of cough,dyspnea,bloating,palpitations,and lower-extremity edema.The patient was initially diagnosed with pericardial effusion by transthoracic echocardiography.Drainage of pericardial effusion was performed to relieve the clinical symptoms.A follow-up echocardiogram showed that the pericardial effusion had decreased;however,the right atrial pressure continued to increase,and signs of constrictive pericarditis were observed upon a more comprehensive inspection.Therefore,the diagnosis of eCP was established based on the comprehensive pre-and postpericardiocentesis echocardiographic findings.An urgent pericardectomy was subsequently performed,which significantly relieved the patient's clinical symptoms,and the signs of pericardial constriction on echocardiography improved.Pericardial effusion and pericardial culture showed growth of P.aeruginosa.CONCLUSION eCP induced by P.aeruginosa infection remains a rare disease.The presence of echocardiographic features of constrictive pericarditis after pericardiocentesis therapy is highly indicative of eCP.

Right-heart contrast echocardiography reveals missed patent ductus arteriosus in a postpartum woman with pulmonary embolism: A case report

BACKGROUND As an established,simple,inexpensive,and surprisingly effective diagnostic tool,right-heart contrast echocardiography(RHCe)might help in solving a vexing diagnostic problem.If performed appropriately and interpreted logically,RHCe allows for differentiation of various usual and unusual right-to-left shunts based on the site of injection and the sequence of microbubble appearance in the heart.CASe SUMMARY A 31-year-old woman was readmitted to hospital with a 2-mo history of worsening palpitation and chest distress.Two years prior,she had been diagnosed with postpartum pulmonary embolism by conventional echocardiography and computed tomography angiography.While the latter showed no sign of pulmonary artery embolism,the former showed pulmonary artery hypertension,moderate insufficiency,and mild stenosis of the aortic valve.RHCe showed microbubbles appearing in the left ventricle,slightly delayed after rightheart filling with microbubbles;no microbubbles appeared in the left atrium and microbubbles’appearance in the descending aorta occurred nearly simultaneous to right pulmonary artery filling with microbubbles.Conventional echocardiography was re-performed,and an arterial horizontal bidirectional shunt was found according to Doppler enhancement effects caused by microbubbles.The original computed tomography angiography findings were reviewed and found to show a patent ductus arteriosus.CONCLUSION RHCe shows a special imaging sequence for unexplained pulmonary artery hypertension with aortic valve insufficiency and simultaneous patent ductus arteriosus.

BACKGROUND Lymphangioleiomyomatosis(LAM)is a rare cystic lung disease characterized by the proliferation,metastasis,and infiltration of smooth muscle cells in the lung and other tissues,which can be associated with tuberous sclerosis complex(TSC).The disorder of TSC has a variable expression,and there is great phenotypic variability.CASe SUMMARY A 32-year-old Chinese woman with a history of multiple renal angioleiomyolipoma presented with a productive cough persisting for over 2 wk.Highresolution chest computed tomography revealed interstitial changes,multiple pulmonary bullae,bilateral pulmonary nodules,and multiple fat density areas of the inferior mediastinum.Conventional and contrast ultrasonography revealed multiple high echogenic masses of the liver,kidneys,retroperitoneum,and inferior mediastinum.These masses were diagnosed as angiomyolipomas.Pathology through thoracoscopic lung biopsy confirmed LAM.Furthermore,high-throughput genome sequencing of peripheral blood DNA confirmed the presence of a heterozygous mutation,c.1831C>T(p.Arg611Trp),of the TSC2 gene.The patient was diagnosed with TSC-LAM.CONCLUSION We highlight a rare case of TSC-LAM and the first report of a mediastinum lymphangioleiomyoma associated with TSC-LAM.

Development of practical and versatile electrophilic difluoromethylating reagent is still a grand challenge so far due to its inherent instability or low reactivity.Herein,we report the design and synthesis of difluoromethyl phenoxathiinium tetrafluoroborate(PT-CF_(2)H^(+)BF_(4)^(-)) as a novel difluoromethylating reagent,which proves to be a bench-stable,general,powerful and versatile reagent with divergent·CF_(2)H,CF_(2)H^(+),and:CF_(2) reactivities.Making use of this reagent,we demonstrated a vast array of difluoromethyl radical transfer reactions via diverse pathways involving photocatalyst-free visible-light induction,visible-light photoredox catalysis and visible-light mediation single electron transfer of eDA complex.Moreover,the green and highly effective CF_(2)H^(+) and:CF_(2) transfer reactions were also readily accomplished.

Isorhamnetin Downregulates MMP2 and MMP9 to Inhibit Development of Rheumatoid Arthritis through SRC/eRK/CReB Pathway

Objective To investigate the effect of isorhamnetin on the pathology of rheumatoid arthritis(RA).Methods Tumor necrosis factor(TNF)-α-induced fibroblast-like synoviocytes(FLS)was exposed to additional isorhamnetin(10,20 and 40µmol/L).Overexpression vectors for matrix metalloproteinase-2(MMP2)or MMP9 or SRC were transfected to explore their roles in isorhamnetin-mediated RA-FLS function.RA-FLS viability,migration,and invasion were evaluated.Moreover,a collagen-induced arthritis(CIA)rat model was established.Rats were randomly divided to sham,CIA,low-,medium-,and high-dosage groups using a random number table(n=5 in each group)and administed with normal saline or additional isorhamnetin[2,10,and 20 mg/(kg·day)]for 4 weeks,respectively.Arthritis index was calculated and synovial tissue inflammation was determined in CIA rats.The levels of MMP2,MMP9,TNF-α,interleukin-6(IL-6),and IL-1β,as well as the phosphorylation levels of SRC,extracellular regulated kinase(eRK),and cyclic adenosine monophosphate response element-binding(CReB),were detected in RA-FLS and synovial tissue.Molecular docking was also used to analyze the binding of isorhamnetin to SRC.Results In in vitro studies,isorhamnetin inhibited RA-FLS viability,migration and invasion(Petin downregulated the levels of MMP2,MMP9,TNF-α,IL-6,and IL-1βin RA-FLS(Pe overexpression of either MMP2 or MMP9 reversed isorhamnetin-inhibited RA-FLS migration and invasion,as well as the levels of TNF-α,IL-6,and IL-1β(Permore,isorhamnetin bound to SRC and reduced the phosphorylation of SRC,eRK,and CReB(Perexpression reversed the inhibitory effect of isorhamnetin on RA-FLS viability,migration and invasion,as well as the negative regulation of MMP2 and MMP9(Pes,isorhamnetin decreased arthritis index scores(Peviated synovial inflammation.Isorhamnetin reduced the levels of MMP2,MMP9,TNF-α,IL-6,and IL-1β,as well as the phosphorylation of SRC,eRK,and CReB in synovial tissue(Pe inhibit

Jigsaw-Sketch: a fast and accurate algorithm for finding top-k elephant flows in high-speed networks

Finding top-k elephant flows in high-speed networks is one of the most fundamental network measurement tasks. It is more challenging than per-flow size estimation since the IDs and sizes of top-k flows must be tracked simultaneously. Most existing studies only record the IDs of a small number of elephant flows to fit their estimators in the extremely limited high-speed on-chip memory. However, these solutions need too many memory accesses when a packet arrives to track the elephant flows with high accuracy, which limits their practicability. Therefore, this paper proposes Jigsaw-Sketch, a new algorithm to find the top-k elephant flows with much fewer memory accesses while achieving high memory efficiency and accuracy. In this design, we propose a novel two-stage jigsaw storage scheme, which can capture the candidate top-k flows from massive network steams efficiently, and further find the top-k elephant flows with high memory efficiency and only a few memory accesses for each packet. extensive experimental results based on real network traces show that Jigsaw-Sketch improves the packet processing throughput by at least 86%, while achieving smaller memory footprints and higher accuracy compared to the SOTA.

A novel method for modeling the phase change of iron ore particles in the cohesive zone of a blast furnace

Cohesive zone plays a vital role in the stable operation of a blast furnace(BF),yet the complex phase change process of iron ore particles in this zone is still not well understood.In this study,a novel one-dimensional(1D)unsteady phase change model was developed to elucidate the heat transfer and melting mechanisms of iron ore particles.After model validation,the effects of several key operating parameters(e.g.,particle diameter,gas velocity,initial temperature)on the phase change behavior of iron ore particles were analyzed,and the joint effect of multiple parameters was discussed.The results show that larger-sized iron ore particles possess lower specific surface areas,which in turn reduces their convective heat absorption capacity.Consequently,the distance from the solid-liquid phase interface to the particle surface increases,thereby slowing down the movement of the phase interface and pro-longing the melting duration of the particles.Increasing the gas velocity and the initial temperature does not have a significant impact on reducing the duration of the complete melting process.Under the specified conditions,it is observed that increasing the gas velocity by 3-fold and 9-fold results in a reduction of the melting duration by 2.4%and 8.3%,respectively.elevating the initial temperature of iron ore particles results in a decrease in the core-to-surface temperature difference,a slower heating rate,and a shorter duration to achieve melting.Among the factors affecting the melting process,the particle diameter is found to be the most significant in terms of the liquid phase precipitation,mushy zone thickness,and core-to-surface temperature difference of iron ore particles.

STING signaling in islet macrophages impairs insulin secretion in obesity

The innate immune regulator stimulator of interferon genes(STING)mediates self-DNA sensing and leads to the induction of type I interferons and inflammatory cytokines,which promotes the progression of various inflammatory and autoimmune diseases.Innate immune system plays a critical role in regulating obesity-induced islet dysfunction,whereas the potential effect of STING signaling is not fully understood.Here,we demonstrate that STING is mainly expressed and activated in islet macrophages upon high-fat diet(HFD)feeding.Sting^(-/-)alleviates HFD-induced islet inflammation by inhibiting the expression of pro-inflammatory cytokines and the infiltration of macrophages.Mechanically,palmitic acid incubation promotes mitochondrial DNA leakage into the cytosol and subsequently activates STING pathway in macrophages.Additionally,STING activation in macrophages impairs glucose-stimulated insulin secretion by mediating the engulfment ofβcell insulin secretory granules.Pharmacologically inhibiting STING activation enhances insulin secretion to control hyperglycemia.Together,our results reveal a regulatory mechanism in controlling the islet inflammation and insulin secretion in dietinduced obesity and suggest that selective blocking of the STING activation may be a promising strategy for treating type 2 diabetes.

One specimen belonging to the family Comatellinae was collected from the Zhenbei Seamount(332.5–478.2 m)in the South China Sea in July 2022.Based on the morphological characters,the specimen was identified as Palaeocomatella hiwia McKnight,1977.It is first recorded from China Sea and redescribed in detail.This specimen differs from the original description from New Zealand for never showing syzygy at br4+5 or br5+6 on interior and br1+2 on exterior arms.However,it is much conform to the redescription to specimens from Indonesia,with only differences in position of the second syzygy and distalmost pinnule comb.Specimen is deposited in the Institute of Oceanology,Chinese Academy of Sciences.Phylogenetic analyses based on the mitochondrial c oxidase subunit I(COI)and 16S rRNA genes indicated that P.hiwia was nested within the tribe Phanogeniini and clustered with Aphanocomaster pulcher.Furthermore,P.hiwia showed same morphological features in terms of mouth placement,comb location,and number of comb teeth rows as other genera of Phanogeniini.Therefore,we suggest that the genus Palaeocomatella should be put in the tribe Phanogeniini.

FPR1 Antagonist (BOC-MLF) Inhibits Amniotic epithelial-mesenchymal Transition

Objective:Premature rupture of membranes(PROM)is a common pregnancy disorder that is closely associated with structural weakening of fetal membranes.Studies have found that formyl peptide receptor 1(FPR1)activates inflammatory pathways and amniotic epithelial-mesenchymal transition(eMT),stimulates collagen degradation,and leads to membrane weakening and membrane rupture.The purpose of this study was to investigate the anti-inflammatory and eMT inhibitory effects of FPR1 antagonist(BOC-MLF)to provide a basis for clinical prevention of PROM.Methods:The relationship between PROM,FPR1,and eMT was analyzed in human fetal membrane tissue and plasma samples using Western blotting,PCR,Masson staining,and eLISA assays.Lipopolysaccharide(LPS)was used to establish a fetal membrane inflammation model in pregnant rats,and BOC-MLF was used to treat the LPS rat model.We detected interleukin(IL)-6 in blood from the rat hearts to determine whether the inflammatory model was successful and whether the anti-inflammatory treatment was effective.We used electron microscopy to analyze the structure and collagen expression of rat fetal membrane.Results:Western blotting,PCR and Masson staining indicated that the expression of FPR1 was significantly increased,the expression of collagen was decreased,and eMT appeared in PROM.The rat model indicated that LPS caused the collapse of fetal membrane epithelial cells,increased intercellular gaps,and decreased collagen.BOC-MLF promoted an increase in fetal membrane collagen,inhibited eMT,and reduced the weakening of fetal membranes.Conclusion:The expression of FPR1 in the fetal membrane of PROM was significantly increased,and eMT of the amniotic membrane was obvious.BOC-MLF can treat inflammation and inhibit amniotic eMT.
