The Rheic Ocean was one of the most important oceans of the Paleozoic Era. It lay between Laurentia and Gondwana from the Early Ordovician and closed to produce the vast Ouachita- Alleghanian-Variscan orogen during the assembly of Pangea. Rifting began in the Cambrian as a continua- tion of Neoproterozoic orogenic activity and the ocean opened in the Early Ordovician with the separation of several Neoproterozoic arc terranes from the continental margin of northern Gondwana along the line of a former suture. The rapid rate of ocean opening suggests it was driven by slab pull in the outboard Iapetus Ocean. The ocean reached its greatest width with the closure of Iapetus and the accretion of the peri- Gondwanan arc terranes to Laurentia in the Silurian. Ocean closure began in the Devonian and continued through the Mississippian as Gondwana sutured to Laurussia to form Pangea. The ocean consequently plays a dominant role in the Appalachian-Ouachita orogeny of North America, in the basement geology of southern Europe, and in the Paleozoic sedimentary, structural and tectonothermal record from Middle America to the Middle East. Its closure brought the Paleozoic Era to an end.

β-Glucans, mostly from oats, have been shown to reduce blood concentrations of total and LDL-cholesterol in animals and humans. After processing, spent brewer’s yeast, a by-product of the fermentation process, contains 85% - 90% β-glucans. The purpose of this study was to evaluate the effect of yeast-derived β-glucan fiber on plasma lipids and early atherosclerosis development in hamsters consuming a semi-purified hypercholesterolemic diet (HCD). Animals were fed either the HCD or the HCD containing psyllium or β-glucan fiber from yeast for 12 weeks. Both the psyllium and β-glucan fiber from yeast showed significant decreases in plasma total cholesterol, non-HDL-C, triacylglycerol, and aortic fatty streak area when compared to the HCD. Also, the β-glucan fiber from yeast had significantly less aortic fatty streak area compared to the psyllium diet. Findings from this study show that while both β-glucan fiber from yeast and psyllium produced similar reductions in plasma lipid and lipoprotein concentrations, the β-glucan fiber from yeast prevented the development early atherosclerosis better than psyllium in the hamsters.

Prevalence of eosinophilic oesophagitis in adults presenting with oesophageal food bolus obstruction

AIM: To look at the relationship between eosinophilic oesophagitis(EO) and food bolus impaction in adults. METHODS: We retrospectively analysed medical records of 100 consecutive patients who presented to our hospital with oesophageal food bolus obstruction(FBO) between 2012 and 2014. In this cohort, 96 were adults(64% male), and 4 paediatric patients were excluded from the analysis as our centre did not have paediatric gastroenterologists. Eighty-five adult patients underwent emergency gastroscopy. The food bolus was either advanced into the stomach using the push technique or retrieved using a standard retrieval net. Biopsies were obtained in 51 patients from the proximal and distal parts of the oesophagus at initial gastroscopy. All biopsy specimens were assessed and reviewed by dedicated gastrointestinal pathologists at the Department of Pathology, University Hospital Geelong. The diagnosis of EO was defined and established by the presence of the following histological features:(1) peak eosinophil counts > 20/hpf;(2) eosinophil microabscess;(3) superficial layering of eosinophils;(4) extracellular eosinophil granules;(5) basal cell hyperplasia;(6) dilated intercellular spaces; and(7) subepithelial or lamina propria fibrosis. The histology results of the biopsy specimens were accessed from the pathology database of the hospital and recorded for analysis. RESULTS: Our cohort had a median age of 60. Seventeen/51(33%) patients had evidence of EO on biopsy findings. The majority of patients with EO were male(71%). Classical endoscopic features of oesophageal rings, furrows or white plaques and exudates werefound in 59% of patients with EO. Previous episodes of FBO were present in 12/17 patients and 41% had a history of eczema, hay fever or asthma. Reflux oesophagitis and benign strictures were found in 20/34 patients who did not have biopsies. CONCLUSION: EO is present in approximately one third of patients who are admitted with FBO. Satellites have provided high-resolution(<100 m)water color(i.e.,remote sensing reflectance)and thermal emission imagery of aquatic environments since the early 1980s;however,global operational water quality products based on these data are not readily available(e.g.,temperature,chlorophyll-a,turbidity,and suspended particle matter).Currently,because of the postprocessing required,only users with expressive experience can exploit these data,limiting their ***,we provide paths(recipes)for the nonspecialist to access and derive water quality products,along with examples of applications,from sensors on board Landsat-5,Landsat-7,Landsat-8,Landsat-9,Sentinel-2A,and *** emphasize that the only assured metric for success in product derivation and the assigning of uncertainties to them is via validation with in situ *** hope that this contribution will motivate nonspecialists to use publicly available high-resolution satellite data to study new processes and monitor a variety of novel environments that have received little attention to date.

Androgens and esophageal cancer: What do we know?

Significant disparities exist between genders for the development and progression of several gastrointestinal(GI) diseases including cancer. Differences in incidence between men vs women for colon, gastric and hepatocellular cancers suggest a role for steroid sex hormones in regulation of GI carcinogenesis. Involvement of intrinsic gender-linked mechanisms is also possible for esophageal adenocarcinoma as its incidence is disproportionally high among men. However, the cause of the observed gender differences and the potential role of androgens in esophageal carcinogenesis remains unclear, even though the cancer-promoting role of androgen receptors(AR) shown in other cancers such as prostate and bladder suggests this aspect warrants exploration. Several studies have demonstrated expression of ARs in esophageal cancer. However, only one study has suggested a potential link between AR signaling and outcome- poorer prognosis. Two groups have analyzed data from cohorts with prostate cancer and one of these found a decreased incidence of esophageal squamous and adenocarcinoma after androgen deprivation therapy. However, very limited information is available about the effects of androgen and AR-initiated signaling on esophageal cancer cell growth in vitro and in vivo. Possible mechanisms for androgens/AR involvement in the regulation of esophageal cancer growth are considered, and the potential use of AR as a prognostic factor and clinical target is highlighted, although insufficient evidence is available to support clinical trials of novel therapies. As esophageal adenocarcinoma is a gender linked cancer with a large male predominance further studies are warranted to clarify the role of androgens and ARs in shaping intracellular signaling and genomic responses in esophageal cancer.

目的　利用人类免疫缺陷病毒 (HIV)低变异的调节蛋白Rev与Rev反应区 (RRE)的高亲和性 ,尝试合成一种不受变异影响的特异性病毒表达检测结构。方法　采用分子克隆技术合成含有RRE和增强的绿荧光蛋白 (EGFP)的Rev依赖性荧光表达质粒 ,采用流式细胞仪检测新质粒的表达功能。结果　新合成的质粒pDM 12 8 EGFP (pE12 8)含氨苄青霉素受体 (AmpR)选择子、SV40 启动子、EGFP基因及HIV 1 RRE片段 ,BssSI酶切后产生 3 .6、1.9和 1.5kb片断 ,HindⅢ酶切后产生 3 470、2 72 5和 879bp 3个片断 ,PCR扩增提示EGFP基因完整插入。pE12 8在Hela细胞中显示Rev依赖性表达 ,pRev不存在时 ,几乎没有EGFP表达 ;pRev存在时 ,EGFP显著表达 (P <0 .0 1)。绿荧光Hela细胞百分比随pRev比例的增加而渐次增加。结论　pE12 8能特异、敏感、快速和直观地检测HIV的表达 ,对反映HIV的感染性及病情的转归及对获得性免疫缺陷综合征 (AIDS)

Possible role of human cytomegalovirus in pouchitis after proctocolectomy with ileal pouch-anal anastomosis in patients with ulcerative colitis

AIM: To detect the presence of human cytomegalovirus (HCMV) proteins and genes on the ileal pouch of patients with ulcerative colitis who have undergone proctocolectomy with ileal pouch-anal anastomosis (iPAA). METHODS: immunohistochemistry, polymerase chain reaction (PCR) and PCR sequencing methods were utilized to test the presence of HCMV in pouch specimens taken from 34 patients in 86 endoscopies. RESULTS: HCMV genes and proteins were detected in samples from 12 (35.2%) patients. The rate of detection was significant in the endoscopies from patients diagnosed with pouchitis (5 of 12, 41.6%), according to the Japanese classification of pouchitis, in comparison to patients with normal pouch (7 of 62, 11.2%; P = 0.021). in all patients with pouchitis in which the HCMV was detected, it was the first episode of pouchitis. The virus was not detected in previous biopsies taken in normal endoscopies of these patients. During the follow- up, HCMV was detected in one patient with recurrent pouchitis and in 3 patients whose pouchitis episodes improved but whose positive endoscopic findings persisted. CONCLUSION: HCMV can take part in the inflammatory process of the pouch in some patients with ulcerative colitis who have undergone proctocolectomy with iPAA.

Portal vein thrombosis in cirrhosis: Controversies and latest developments

Portal vein thrombosis(PVT) is encountered in livercirrhosis, particularly in advanced disease. It has been a feared complication of cirrhosis, attributed to significant worsening of liver disease, poorer clinical outcomes and potential inoperability at liver transplantation; also catastrophic events such as acute intestinal ischaemia. Optimal management of PVT has not yet been addressed in any consensus *** review current literature on PVT in cirrhosis; its prevalence, pathophysiology, diagnosis, impact on the natural history of cirrhosis and liver transplantation,and management. Studies were identified by a search strategy using MEDLINE and Google Scholar. The incidence of PVT increases with increasing severity of liver disease: less than 1% in well-compensated cirrhosis, 7.4%-16% in advanced cirrhosis. Prevalence in patients undergoing liver transplantation is 5%-16%.PVT frequently regresses instead of uniform thrombus progression. PVT is not associated with increased risk of mortality. Optimal management has not been addressed in any consensus publication. We propose areas for future research to address unresolved clinical questions.
