The first-line management of psychogenic non-epileptic seizures(PNES)in adults in the emergency:a practical approach

Distinguishing non-epileptic events,especially psychogenic non-epileptic seizures(PNES),from epileptic seizures(ES)constitutes a diagnostic *** are frequent,especially when video-EEG recording,the gold-standard for PNES confirmation,cannot be *** issue is further complicated in cases of combined PNES with *** emergency units,a misdiagnosis can lead to extreme antiepileptic drug escalade,unnecessary resuscitation measures(intubation,catheterization,etc.),as well as needless biologic and imaging *** of the acute window,an incorrect diagnosis can lead to prolonged hospitalization or increase of unhelpful antiepileptic drug *** recognition is thus desirable to initiate adequate treatment and improve *** experience-based strategies and a thorough review of the literature,we aimed to present the main clinical clues for physicians facing PNES in non-specialized units,before management is transferred to epileptologists and *** such conditions,patient recall or witness-report provide the first orientation for the diagnosis,recognizing that collected information may be *** analysis of an event(live or based on home-video)may lead to a clinical diagnosis of PNES with a high confidence ***,a fluctuating course,crying with gestures of frustration,pelvic thrusting,eye closure during the episode,and the absence of postictal confusion and/or amnesia are highly suggestive of ***,induction and/or inhibition tests of PNES have a good diagnostic value when *** may also be a useful biomarker to distinguish PNES from epileptic seizures,especially following bilateral tonic-clonic ***,regardless the level of certainty in the diagnosis of the PNES,it is important to subsequently refer the patient for epileptological and neuropsychiatric follow-up.

Essential tremor:A three-dimensional neurosphere in vitro model to assess the neurotoxicity of harmane

Objectives: To use a novel in vitro model of three-dimensional(3D) neurosphere cultures to assess neurotoxic or neuroprotective effects with harmane as a model ***: A reproducible model of 3D spheroids was developed from embryonic mouse cortical neurons,using molded agarose micro-wells;this method seems particularly practical as it is customizable and widely available and does not require specific cell treatments or assay components different from 2D cultures, allowing for the easy transposition of routine protocols. To assess the neurotoxic effects of harmane, a resazurin assay was performed to measure cell viability, and a highly sensitive fluorometric method, based on the oxidation of dichlorodihydrofluorescein, was applied to measure eventually induced reactive oxygen species(ROS) after exposure to harmane at increasing concentrations of 50 100,and 250 μ***: Hydrogel microwells facilitated the assembly of spheroids containing neurons and glial cells into a complex 3D structure and prevented the agglomeration of spheroids. Exposure to harmane induced cytotoxicity in 3D neural spheroids, which was correlated with harmane concentrations, with a 27%reduction in viability at 250 μm. Harmane that did not induce significant levels of oxidative stress was detected for all tested ***: This 3D neurosphere model mimics a neuronal microenvironment, allowing a fine study of neurodegenerative disorders and the effects of chemicals on the brain. This model opens novel opportunities, not only from a pathogenetic point of view but also from a therapeutic perspective.

Characterization of phosphorus interaction with sediments affected by acid mine drainage-relation with the sediment composition

Phosphorus sorption capacity was investigated in surface sediments derived from an abandoned zinclead mine area located in northeastern Algeria. The forms and the distribution of phosphorus in the raw sediment were identified using the sequential chemical extractions method. Batch experiments were done to study the adsorption kinetics and isotherms. The pH effect was evaluated by macroscopic and infrared analyses. In raw sediment, speciation results show that phosphorus is dominantly bound to oxyhydroxides. Sorption experiments demonstrate that phosphorus uptake is principally related to sediment composition. The nature of the dominant iron oxyhydroxide has a substantial role in the adsorption capacity and the mechanism interaction. The adsorption kinetics can be described by the second order and Elovich models. The isotherms data are successfully modeled by the Temkin *** maximum phosphorus removal is reached under acidic pH. Spectroscopic analyses reveal that the predominance of jarosite implies electrostatic interaction with sediment particles. However, in the case of schwertmannite predominance, phosphate ions are adsorbed by the ligand exchange mechanism.

Predation is a pervasive selection pressure,shaping morphological,physiological,and behavioral phenotypes of prey *** studies have begun to examine how the effects of individual experience with predation risk shapes the use of publicly available risk assessment ***,we investigated the effects of prior predation risk experience on disturbance cue production and use by Trinidadian guppies Poecilia reticulata under laboratory *** our first experiment,we demonstrate that the response of guppies from a high predation population(Lopinot River)was dependent upon the source of disturbance cue senders(high *** predation populations).However,guppies collected from a low predation site(Upper Aripo River)exhibited similar responses to disturbance cues,regardless of the sender *** our second experiment,we used laboratory strain guppies exposed to high versus low background risk *** results show an analogous response patterns as shown for our first *** exposed to high background risk conditions exhibited stronger responses to the disturbance cues collected from senders exposed to high(***)risk conditions and guppies exposed to low risk conditions were not influenced by sender ***,our results suggest that experience with background predation risk significantly impacts both the production of and response to disturbance cues in guppies.

Comparing corneal outcome between femtosecond laser-assisted cataract surgery and conventional phaco surgery in Fuchs’endothelial dystrophy patients:a randomized pilot study with 6mo follow up

AIM:To compare the corneal outcome in Fuchs’endothelial dystrophy(FED)patients between femtosecond laser-assisted cataract surgery(FLACS)and conventional phaco surgery(CPS).METHODS:This was a randomized controlled study comparing one eye surgery by FLACS and the contralateral eye operated by CPS(stop and chop technique)in FED *** corneal thickness,corneal light backscatter,corneal densitometry,and central corneal endothelial cell count and hexagonality(noncontact endothelial cell microscope),and corrected distance visual acuity(CDVA)were assessed preoperatively and at day 1,40,and 180 ***:Totally 31 patients(16 women)were *** day 40 postoperatively,the mean endothelial cell loss(ECL)was 23.67%by FLACS and 17.30%by CPS(P=0.53).At day 180 postoperatively,ECL was 25.58%in FLACS and 21.32%in CPS(P=0.69).Densitometry data in all layers and all annuli from anterior layer to posterior layer in annuli 0-2,2-6,6-10 and 10-12,total densitometry with all layers and all annuli was performed.A significant difference was found in 6-10(posterior layer)at day 1 with-1.42 grayscale units(GSU;95%CI:-2.66 to-0.19,P=0.02).In 10-12(anterior layer,central layer and all layers)at day 40 were significant different with 7.7(95%CI:1.89 to 13.50,P=0.009),3.97(95%CI:0.23 to 7.71,P=0.03),4.73 GSU(95%CI:0.71 to 8.75,P=0.02),*** the remaining parameters we found no difference between the two groups(P>0.05).Three CPS eyes suffered from corneal ***:There is no significant difference in corneal outcome between FLACS and *** cell density and pentacam corneal outcome may be inadequate as outcome parameters in FED patients.

αV integrin:A new gastrin target in human pancreatic cancer cells

AIM： To analyse αv integrin expression induced by gas- trin in pancreatic cancer ***： αv integrin mRNA expression in human pan- creatic cancer cells was analysed using a ＂cancer genes＂ array and confirmed by real-time reverse transcription- polymerase chain reaction （PCR）. Western blotting and semi-quantitative immunohistochemistry were used to examine protein levels in human pancreatic cancer cell lines and pancreatic tissues, respectively, The role of αv integrin on gastrin-induced cell adhesion was examined using blocking anti-αv integrin monoclonal antibodies. Adherent cells were quantified by staining with crystal ***： Using a ＂cancer genes＂ array we identi- fied c^v integrin as a new gastrin target gene in human pancreatic cancer cells. A quantitative real-time PCR approach was used to confirm αv integrin gene expression. We also demonstrate that Src family kinases and the PI 3-kinase, two signalling pathways specifically activated by the CCK-2 receptor （CCK2R）, are involved in gastrin-mediated αv integrin expression. In contrast, inhibition of the ERK pathway was without any effect on αv integrin expression induced by gastrin. Our results also show that gastrin modulates cell adhesion via αv integrins. Indeed, in vitro adhesion assays performed on fibronectin show that gastrin significantly increases adhesion of pancreatic cancer cells. The use of blocking anti-αv integrin monoclonal antibodies completely reversed the increase in cell-substrate adhesion induced by gastrin. In addition, we showed in vivo that the targeted CCK2R expression in the pancreas of Elas-CCK2 mice, leads to the overexpression of αv integrin. This process may contribute to pancreatic turnout development observed in these transgenic ***： αv integrin is a new gastrin target in pancreatic cancer models and contributes to gastrin effects on cell adhesion.

c-Fos overexpression increases the proliferation of human hepatocytes by stabilizing nuclear Cyclin D1

AIM： To investigate the effect of stable c-Fos over- expression on immortalized human hepatocyte （IHH） proliferation. METHODS： IHHs stably transfected with c-Fos （IHH- Fos） or an empty vector （IHH-C） were grown in me- dium supplemented with 1% serum or stimulated with 10% serum. Cell proliferation was assessed by cell counts, 3H-thymidine uptake and flow cytometry analyses. The levels of cell cycle regulatory proteins （Cyclin DI, E, A） cyclin dependent kinases （cdk） cdk2, cdk4, cdk6, and their inhibitors p15, p16, p21, p27, total and phosphorylated GSK-3β and epidermal growth factor receptor （EGF-R） were assayed by Western blotting. Analysis of O/c/in D1 mRNA levels was performed by reverse transcription-polymerase chain reaction and real-time polymerase chain reaction （PCR） analysis. Stability of Cyclin DI was studied by cycloheximide blockade experiments. RESULTS： Stable c-Fos overexpression increased cell proliferation under low serum conditions and resulted in a two-fold increase in [3H]-thymidine incorpora- tion following serum addition. Cell cycle analysis by flow cytometry showed that c-Fos accelerated the cell cycle kinetics. Following serum stimulation, Cyclin D1 was more abundantly expressed in c-Fos overexpress- ing cells. Cyclin D1 accumulation did not result from increased transcriptional activation, but from nuclear stabilization. Overexpression of c-Fos correlated with higher nuclear levels of inactive phosphorylated GSK- 3β, a kinase involved in Cyclin D1 degradation and higher levels of EGF-R mRNA, and EGF-R protein com- pared to IHH-C both in serum starved, and in serum stimulated cells. Abrogation of EGF-R signalling in IHH- Fos by treatment with AG1478, a specific EGF-R tyro- sine kinase inhibitor, prevented the phosphorylation of GSK-3β induced by serum stimulation and decreased Cyclin D1 stability in the nucleus.
