Time of surgery and surgeon level in supracondylar humerus fractures in pediatric patients:A retrospective study

BACKGROUND Supracondylar humerus fractures account for more than 60%of all elbow fractures and about 1/5 of all pediatric ***,these fractures can be associated with risk of complications including neurovascular injuries,malunions and limb *** exists regarding the effect of time of surgical intervention and/or level of surgeon performing the surgery on outcome of these *** To determine whether time of surgical intervention and/or surgeon level influence the outcomes of surgically managed pediatric supracondylar humerus *** We retrospectively studied 155 pediatric patients presenting with a supracondylar humerus fracture in a level 1 trauma center from January 2006 to December *** data extracted included demographic data,fracture characteristics,surgical data,and follow-up *** collected data was analyzed and P values ofmented post-operative complications,of which the majority occurred in surgeries performed after day time working hours and in fractures requiring open *** the lowest complication rate was found in surgeries performed by pediatric orthopaedic surgeons,this did not reach statistical *** In pediatric patients undergoing surgery for supracondylar fractures,we found a higher complication rate when surgeries were not performed during working *** level and training had no significant effect on the risk of post-operative complications.

Stability analysis of drugdynamics model: A mat hematical approach

In this paper, a mathematical model of drug release from polymeric matrix and consequent intracellular drug transport is proposed and analyzed. modeling of drug release is done through solubilization dynamics of drug particles, diffusion of the solubilized drug through the polymeric matrix in addition to reversible dissociation/recrystallization process. The interaction between drug-receptor, drug-plasma proteins along with other intracellular endosomal events is modeled. This leads to a mixed system of partial and ordinary differential equations with associated pertinent set of initial and boundary conditions. Furthermore, besides the stability of the proposed model, several sub-models are also studied for their stability criteria. Prominence is provided to the reduced model system having requisite relevance to the original system where quasi steady state approximation (QSSA) theory is utilized. For the model to be potent enough to generate appropriate predictive results for drug delivery, the stability properties of equilibrium in the mathematical model are analyzed both analytically and numerically. Numerical simulation in the embodiment of graphical representations speaks about various vital characteristics of the underlying physical phenomena along with the importance and sensitized impact of the model parameters controlling significant biological functions. Probed new therapies and clinical procedures could be assessed considering the present mathematical model and its analysis as the basis framework in order to effectively enhance therapeutic efficacy and improved patient compliance. The present study confirms the necessity of stability analysis study so that advocated mathematical model can effectively complement the real physiological behavior of pharmacokinetics.

Te(Ⅳ),Se(Ⅳ),V(Ⅲ),Nb(Ⅴ)and Ta(Ⅴ)complexes of indole-3-acetic acid(IAAH)ligand were synthesized,characterized by elemental analysis and various spectroscopic techniques like,IR,1H-NmR,X-ray powder diffraction,UV-Visible,thermogravimetry analysis,magnetic measurements,molar conductance and surface morphology using *** the synthesized complexes of IAAH ligand have 1∶2 stoichiometry of the types[Te(IAA)2(NH 3)2]·2Cl(Ⅰ),[Se(IAA)2(NH 3)2]·2Cl(Ⅱ),[V(IAA)2(NH 3)(Cl)](Ⅲ),[Nb(IAA)2(Cl)3](Ⅳ),and[Ta(IAA)2(Cl)3](Ⅴ).Spectral analysis indicates octahedral geometry for the Te(Ⅳ),Se(Ⅳ)and V(Ⅲ)complexes,whereas both Nb(Ⅴ)and Ta(Ⅴ)have a *** bonding sites are the oxygen atoms of carboxylate group for the deprotonated indole-3-acetic acid(IAA)*** thermogravimetry analysis studies gave evidence for the presence of other coordinated molecules(Cl or NH 3)in the composition of IAA complexes,which were further supported by IR and micro analytical *** higher molar conductance data of tellurium and selenium(Ⅳ)complexes reveal that these chelates are electrolytes,while low conductivity values for the vanadium(Ⅲ),niobium and tantalum(Ⅴ)chelates indicated a *** test the antibacterial property of the five complexes in this study,four bacterial strains Klebsiella(G-),Escherichia coli(G-),Staphylococcus aureus(G+)and Staphylococcus epidermidis(G+)were used in the *** effects of the five complexes in the cytotoxicity of Caco-2 and mcf-7 human cancer cell lines were studied Neutral red uptake assay for the estimation of cell viability/cytotoxicity protocol.

Green synthesis of bentonite-supported iron nanoparticles as a heterogeneous Fenton-like catalyst:Kinetics of decolorization of reactive blue 238 dye

This study aimed to synthesize green tea nano zero-valent iron(GT-NZVI)and bentonite-supported green tea nano zero-valent iron(BGT-NZVI)nanoparticles using green tea extracts in an environmentally sustainable *** was used as a support material because it disperses and stabilizes GT-NZVI,and it helps to reduce the cost,increase the adsorption capacity of GT-NZVI,and decrease the optimum amount of GT-NZVI used in Fenton-like oxidation.A scanning electron microscope,an atomic force microscope,and a Fourier transform infrared spectroscope were used to characterize GT-NZVI and B-GT-NZVI,while the zeta potential was measured to evaluate the stability of iron *** decolorization kinetics of reactive blue 238(RB 238)dye in the aqueous phase in the Fenton-like oxidation process were investigated as *** effects of various experimental conditions such as reaction time,dosages of catalysts,concentration of H2O2,temperature,addition of inorganic salts,and other parameters were *** results show that the oxidative degradation efficiencies of RB 238 dye catalyzed by GT-NZVI and B-GT-NZVI were 93.5%and 96.2%,respectively,at the optimum reaction conditions as follows:c(H2O2)¼5 mmol/L,r(GT-NZVI)¼0.5 g/L or r(B-GT-NZVI)¼0.5 g/L,c(RB 238 dye)¼0.05 mmol/L,and pH¼2.5 at 180 *** best catalytic performance was exhibited when B-GT-NZVI was *** kinetic models were employed,and the second-order model was found to be the best model representing the experimental kinetic data of RB 238 *** value of activation energy decreased from 38.22 kJ/mol for GT-NZVI to 14.13 kJ/mol for *** indicates that the effect of B-GT-NZVI in decreasing the energy barrier is more pronounced than that of the GT-NZVI catalyst,leading to improved Fenton-like oxidation processes.©2020 Hohai *** and hosting by Elsevier *** is an open access article under the CC BY-NC-ND license(http://***/licenses/by-nc-nd/4.0/).

Higher Order Implicit Scheme for Nonlinear Time-Dependent Convection-Diffusion- Reaction Equation

A mathematical model comprising of nonlinear reaction, diffusion, and convection mechanisms seen in natural and anthropogenic processes is numerically investigated here. It is proposed that a higher order numerical scheme of finite difference method be used in conjunction with an iterative approach in order to solve the nonlinear one dimensional convection-diffusion-reaction equation. To account for the wide variety of physical characteristics and boundary conditions, an iterative approach is presented that yields a reliable and precise solution every time. We examined the accuracy and operational efficiency of two distinct finite difference approaches. The efficiency of the system is determined by comparing the estimated results to the appropriate analytical solution by adhering to established norms. Coherence and convergence were analyzed for each approach. The simulation results demonstrate the efficacy and accuracy of these methods in solving nonlinear convection- diffusion-reaction equations. Convection-diffusion-reaction equation modeling is critical for employing the offered results in heat and mass transport processes.

An analytical study of drug release kinetics from a degradable polymeric matrix

In modern days, biodegradable polymeric matrix used as the kingpin of local drug delivery system is in the center of attention. This work is concentrated on the formulation of mathematical model elucidating degradation of drug-loaded polymeric matrix followed by drug release to the adjacent biological tissues. Polymeric degradation is penciled with mass conservation equations. Drug release phenomenon is modeled by considering solubilization dynamics of drug particles, diffusion of the solubilized drug through polymeric matrix along with reversible dissociation/recrystallization process. In the tissue phase, reversible dissociation/association along with internalization processes of drug are taken into account. For this, a two-phase spatio-temporal model is postulated, which has ensued to a system of partial differential equations. They are solved analytically with appropriate choice of initial, interface and boundary conditions. In order to reflect the potency of the advocated model, the simulated results are analogized with corresponding experimental data and found laudable agreement so as to validate the applicability of the model considered. This model seems to foster the delicacy of the mantle enacted by important drug kinetic parameters such as diffusion coefficients, mass transfer coefficients, particle binding and internalization parameters, which is illustrated through local sensitivity analysis.

Spectrum of Primary Glomerular Diseases in Patients Presenting with Urinary Abnormalities

Introduction: Primary Glomerular Diseases are a spectrum of renal disorders of unknown aetiology with distinct characteristics, specific natural history and prognosis. A thorough evaluation is prerequisite to establish the diagnosis since many systemic diseases and secondary aetiology masquerade as primary diseases. methods: This prospective observational study was conducted at a tertiary care centre and included 30 patients, with clinical features suggestive of primary glomerular diseases, of which 23 patients (76.6%) were males and 07 patients (23.33%) were females. The mean age at presentation was 37.23 ± 12.89 years. Among the observed spectrum of Primary Glomerular Diseases, IgA Nephropathy (IgA N) was seen in 26.67% patients, Focal Segmental Glomerulosclerosis (FSGS) in 20% patients and membranous Glomerulopathy (mGN) in 13.33% patients, whereas the incidence of other abnormalities had less percentage contribution. Proteinuria was the commonest presentation seen in 60% patients, followed by microscopic Haematuria in 20%. mean Serum Creatinine was 0.99 ± 0.16 mg/dl. mean Serum Albumin was 2.51 ± 0.76 gm/dl. Overall Nephrotic range proteinuria was observed in 15 (50%) patients. Results: IgA N, FSGS & mGN were the commonest observed Primary Glomerular Diseases. Proteinuria, Haematuria, Anasarca and Pedal Oedema were the commonest observed clinical presentations. Conclusions: In this studied series IgA Nephropathy, FSGS and mGN were the most prevelant diagnoses in the patients presenting with Urinary Abnormality. Nephrotic range Protenuria was the major indication for biopsy, there is a temporal variation in glomerulopathies wherein there is increase in incidence of IgA Nephropathy and decrease in incidence of FSGS.

Consensus controller for multi-UAV navigation

In this paper, we design consensus algorithms for multiple unmanned aerial vehicles （UAV）. We mainly focus on the control design in the face of measurement noise and propose a position consensus controller based on the sliding mode control by using the distributed UAV information. Within the framework of Lyapunov theory, it is shown that all signals in the closed-loop multi- UAV systems are stabilized by the proposed algorithm, while consensus errors are uniformly ultimately bounded. moreover, for each local UAV, we propose a mechanism to define the trustworthiness, based on which the edge weights are tuned to eliminate negative influence from stubborn agents or agents exposed to extremely noisy measurement. Finally, we develop software for a nano UAV platform, based on which we implement our algorithms to address measurement noises in UAV flight tests. The experimental results validate the effectiveness of the proposed algorithms.
