This study tried to apply biomass fuel to clean burner combustion under high load conditions by water addition. A newly developed injector mixes fuel rapidly with water inside of the injector with support of atomizing air. The mixture composed of three-fluid is injected as spray into a flame stabilizer of burner. Investigation of emission performance of this injector shows that the internally rapid mixing type of injector is hopeful technique to introduce water into burner combustion. Combustion by this injector emits exceedingly less particulate matters at high load. NO x emission is strongly dependent on water flow rate.

The microstructure and hardness on and below the surface of Cr-Mo steel (SCM435) treated by water jet peening (WJP) were investigated using scanning electron microscopy and micro Vickers hardness measurements. The change of the surface residual stress caused by the WJP treatment influenced the surface microstructure and surface hardness of the SCM435 steel. Cementite in the pearlite phase tended to protrude as the duration of WJP was increased. Voids were formed in the area 0.5 - 1.0 mm below the surface and also at grain boundaries between ferrite and pearlite grains, whereas no voids were formed in the depth range from 2.0 to 3.0 mm below the surface.

BACKGROUND Solitary fibrous tumor(SFT)is a rare fibroblastic mesenchymal neoplasm that affects spindle cell soft tissues with broad-spectrum biological behavior;it is predominantly benign,and rarely *** occurs mainly in the tissue structure of the serosa in the pleura and the thorax,and can be found throughout the body,though extra-thoracic localization,including the cephalic region,is *** reported the first case of intracranial malignant SFT metastasized to the chest *** SUMMARY An 81-year-old Japanese man was referred to our hospital due to progressive gait disturbance and appetite *** medical history included partial resection due to brain tumor,four times,and 50-Gray radiation therapy at another hospital,starting when he was 74 years *** unenhanced head computed tomography(CT)scan revealed an 8 cm×5.1 cm×6.5 cm mixed-density mass at the left frontal lobe,accompanying a midline shift,and an unenhanced chest-abdomen CT scan revealed a 6 cm×4.1 cm×6.5 cm low-density mass in the left chest wall.A CT-guided percutaneous lung biopsy was performed,and the pathological findings were SFT corresponding to brain ***,the correct diagnosis of his brain tumor in history of past illness revealed to be SFT,and the unremovable tumor,namely present brain lesions enlarged and metastasized to the chest *** established a definitive diagnosis of intracranial malignant SFT metastasized to the chest *** notified him and his family of the disease,and offered palliative *** passed away on the 29 th hospital *** This case suggests the need for careful,detailed examination,and careful followup when encountering patients presenting with a mass.

Hepatic resection (HR) and radiofrequency ablation (RFA) are popular local therapies for early-stage hepatocellular carcinoma (HCC).Alpha-fetoprotein,Lens culinaris agglutininreactive fraction of alpha-fetoprotein,and des-c-carboxy prothrombin are well-known and useful tumor markers for *** positive number status of these tumor markers has recently been demonstrated as beneficial for predicting outcome for HCC patients treated with local *** the normal ranges reported have differed by institution,the positivity of tumor markers is consistent and can easily be *** and Wakayama's group clearly demonstrated the following:1) Regardless of the degree of tumor stage,a triple-positive tumor marker profile can predict poor outcome in HCC patients undergoing HR;2) For RFA alone,HCC patients with double-and triple-positive status,having less than three lesions and lesions ≤3 cm in diameter show comparably insufficient outcomes;3) For HCC patients with lesions ≤5 cm in Child-Pugh grade A,HR is preferred over RFA;4) Microvascular invasion rates increased even in the double-positive patients,while poorly differentiated HCC was frequently observed only in the triple-positive patients;and 5) RFA with chemoembolization,anatomical liver resection,and postoperative adjuvant chemoembolization or hepatic arterial chemotherapy might improve the outcome for patients with highly malignant HCC with multiple positive tumor ***,the impacts of these therapies still need to be evaluated in prospective comparative studies.

Background: The ability to climb stairs (ascending and descending stair without using a handrail) and rise from a chair (rising from chairs without using an elbow rest) are among the most important measures of physical function for ADL evaluation for the independent living, and assessed by the questioners on many epidemiological studies in elderly. But little is known about the relationship between the self-reported performance level of the tasks and lower leg strength and power in very elderly people. The purpose of this study was to ascertain the relationship between the self-reported performance level of two tasks and the lower leg function in community-dwelling 80-year-old population. Methods: Out of 994 persons who were 80 years old living in Morioka City, Iwate Prefecture, Japan, 607 individuals (236 men and 371 women) underwent a physical fitness test that included measurements of leg extensor power and knee extensor strength. The ability to climb up stair and to rise from a chair was assessed by self- reported questionnaire which was ranked in three levels. The area under the receiver-operating characteristic (ROC) curve (AUC) was used to evaluate the measurements. Results: The cut-off points for the leg extensor power of subjects who could completely perform the stair-climbing and chair-rising functions were determined to be approximately 8.6 watt/kg body mass for men and 5.6 watt/kg body mass for women. In addition, the cut-off points for the knee extensor strength of subjects who could completely perform the stair-climbing and chair-rising functions were determined to be approximately 0.97 kg/kg body mass for men and 0.84 kg/kg body mass for women. Conclusions: From a practical viewpoint, the present study suggested that the cut-off points of leg extensor power and knee extensor strength can be used as targets in simple self-reported questionnaires to help in screening for mobility in 80-year-old

AIM:To investigate the optimum period of treatment for post endoscopic submucosal dissection(ESD)***:Patients who underwent ESD for gastric cancer were randomized to two groups and treated with esomeprazole 20 mg per day for 4 wk(4W group)or 2 wk(2W group).At 4 wk after ESD,we measured the size of the artificial ulcers by endoscopy and determined the ulcer healing rate,compared with the size of the ESD *** randomized controlled trial study was approved by our ethics committee and registered in the UMIN Clinical Trial ***:A total of 60 consecutive patients were included in the *** patients received rebamipide 300 mg per day for 4 *** patient in 2W group who showed bleeding within two weeks and received endoscopic treatment was excluded from further *** numbers of patients with ulcers in the healing/scar stage in the 2W and 4W groups at 4 wk after ESD were 20/6 and 28/5,respectively,with no significant *** ulcer healing rate in the 2W and 4W groups were 96.1%[95%confidence interval(CI):94.6%-97.55]vs 94.8%(95%CI:92.6%-97.1%),respectively,with no statistical difference(UMIN000006951).CONCLUSION:Two-wk treatment with a proton pump inhibitor is as effective as four-week treatment for healing post ESD ulcers.

AIM:To investigate whether flexible spectral color enhancement(FICE) improves diagnostic yields of capsule endoscopy(CE) for obscure gastro-intestinal bleeding(OGIB).METHODS:The study subjects consisted of 81 *** FICE,there were three different sets with different *** randomly selected sets of FICE,images of CE were evaluated again by two individuals who were not shown the conventional CE reports and *** difference between FICE and conventional imaging was ***:The overall diagnostic yields in FICE sets 1,2,3 and conventional imaging(48.1%) were 51.9%,40.7%,51.9% and 48.1%,respectively,which showed no statistical difference compared to conventional *** total numbers of detected lesions per examination in FICE imaging and conventional imaging were 2.5 ± 2.1 and 1.8 ± 1.7,respectively,which showed a significant difference(P = 0.01).CONCLUSION:The diagnostic yield for OGIB is not improved by ***,FICE can detect significantly more small bowel lesions compared to conventional imaging.
