Viral hepatitis represents a major danger to public health,and is a globally leading cause of *** five liver-specific viruses:Hepatitis A virus,hepatitis B virus,hepatitis C virus,hepatitis D virus,and hepatitis E virus,each have their own unique epidemiology,structural biology,transmission,endemic patterns,risk of liver complications,and response to antiviral *** remain few options for treatment,in spite of the increasing prevalence of viral-hepatitiscaused liver ***,chronic viral hepatitis is a leading worldwide cause of both liver-related morbidity and mortality,even though effective treatments are available that could reduce or prevent most patients’*** 2016,the World Health Organization released its plan to eliminate viral hepatitis as a public health threat by the year 2030,along with a discussion of current gaps and prospects for both regional and global eradication of viral ***,treatment is sufficiently able to prevent the disease from reaching advanced ***,future therapies must be extremely safe,and should ideally limit the period of treatment necessary.A better understanding of pathogenesis will prove beneficial in the development of potential treatment strategies targeting infections by viral *** review aims to summarize the current state of knowledge on each type of viral hepatitis,together with major innovations.

众所周知,采用粉煤灰制备混凝土,因粉煤灰成分中含有未燃炭等微粒子,容易造成阴离子型引气剂吸附在这些微粒子上,导致含气量经时损失。同时在混凝土的制备、运输过程中,因受采用的材料、拌和状态等物理因素影响,还会发生含气量经时增加的现象。所以在制备、管理混凝土时,如何保证出机初期一定的含气量、缓解含气量的经时损失或经时增加,是一项极其关键的技术。本文将就采用能改善上述状态的引气剂、抑泡剂拌和混凝土,对混凝土的经时变化以及抗冻融性等特征产生的影响进行论述。

Median neuropathy after multiple punctures of the forearm for catheterization:A case report

BACKGROUND Neuropathy may occur at some sites after catheterization for close examination of cardiac *** the radial artery is considered a relatively uncomplicated site for catheterization,the radial artery and median nerve are in relatively close proximity,with the risk of median nerve injury depending on the angle of *** purpose of this study was to report the outcomes of surgery performed for conservative therapy-resistant median neuropathy following forearm *** SUMMARY A 50-year-old woman experienced palsy from the right thumb to the radial side of the ring finger after catheterization from the radial artery of the right *** developed at the same site and a positive tinel-like sign was seen for the median nerve area at the high level of the puncture *** conduction study showed reduced compound muscle action potentials and loss of sensory nerve action *** did not improve despite pharmacotherapy and the patient gradually developed flexion restrictions of the index and middle *** nerve injury and associated flexor tendon adhesion was diagnosed,and the patient was referred for surgery at 3 mo after *** the same area was opened,no injury to the median nerve epithelium was obvious,but the area of the positive tinel-like sign was highly adherent to surrounding tissue and to the flexor digitorum superficialis of the index and middle *** surgery was terminated with adequate adhesion *** was initiated postoperatively,improving neurological symptoms and range of motion of the *** months after surgery,the patient returned to daily activities without *** This case provides the appropriate diagnosis and treatment for a suspected peripheral nerve injury.

Improvement of thermal environment and reduction of energy consumption for coolin8 and heating by retrofitting windows

Various techniques for creating a comfortable thermal environment and saving energy have been proposed and employed in residential buildings in many countries, including Japan. For these techniques to be introduced, existing houses should be renovated. Among the techniques available, installation of additional inner windows is effective in creating a comfortable and energy-efficient living environment. In the present research, the effect of additional inner windows on the thermal environment and energy saving was investigated by measuring indoor climates. Windows were renovated in two rooms on the fourth floor of a four-story reinforced concrete building. Air temperatures, the humidity, the global solar radiation on horizontal and vertical surfaces, radiant temperatures, and the electricity consumption of air-conditioners were measured. A comparison of these vatues before and after the installation of inner- windows showed that the thermal environment and energy saving had improved. Results obtained from a thermal model agreed well with measured results by changing the value of solar transmittance and heat transmission coefficient of the glazing forlowing renovation. Furthermore, in a questionnaire survey conducted in summer, more than half of the occupants answered ＂cornfortabte＂ to a question on the overall thermal comfort.

Triple therapy of interferon and ribavirin with zinc supplementation for patients with chronic hepatitis C:A randomized controlled clinical trial

AIM： To study the therapeutic effect of interferon （IFN） and ribavirin with zinc supplement on patients with chronic hepatitis C viral （HCV） infection. METHODS： A total of 102 patients confirmed histologically to have chronic HCV infection with genotype lb and more than 100 KIU/mL of HCV were randomly assigned to each arm of the study and each received 10 million units of pegylated interferon （IFN- alpha-2b） daily for 4 wk followed by the same dose every other day for 20 wk plus ribavirin （600 or 800 mg/d depending on body weight）, with or without polaprezinc （150 mg/d） orally for 24 wk. The primary endpoint was sustained virological response （SVR） defined as negative HCV-RNA in the serum 6 mo after treatment. RESULTS： There were no differences in the clinical background between the two groups except for more females in the dual therapy group than in the other group （P〈 0.05）. SVR was observed in 33.3% of the triple therapy group and 33.3% of the dual therapy group. The side effects were almost the same in both groups except for gastrointestinal symptoms, which were less in the triple therapy group （P= 0.019）. CONCLUSION： Considered together, triple therapy of zinc plus IFN and ribavirin for HCV infection patients with genotype lb and high viral load is not better than dual therapy except for lower incidence of gastrointestinal side effects.

We report the discovery of KMT-2020-BLG-0414 Lb,with a planet-to-host mass ratio q2=0.9-1.2×10-5=3-4 q⊕at 1σ,which is the lowest mass-ratio microlensing planet to *** with two other recent discoveries(4?q/q⊕?6),it fills out the previous empty sector at the bottom of the triangular(log s,log q)diagram,where s is the planet-host separation in units of the angular Einstein radiusθ***,these discoveries call into question the existence,or at least the strength,of the break in the mass-ratio function that was previously suggested to account for the paucity of very low-q *** to the extreme magnification of the event,Amax~1450 for the underlying single-lens event,its light curve revealed a second companion with q3~0.05 and|log s3|~1,i.e.,a factor~10 closer to or farther from the host in *** measurements of the microlens parallaxπE and the angular Einstein radiusθE allow estimates of the host,planet and second companion masses,(M1,M2,M3)~(0.3 M⊙,1.0 M⊕,17 MJ),the planet and second companion projected separations,(a⊥,2,a⊥,3)~(1.5,0.15 or 15)au,and system distance DL~1 *** lens could account for most or all of the blended light(I~19.3)and so can be studied immediately with high-resolution photometric and spectroscopic observations that can further clarify the nature of the *** planet was found as part of a new program of high-cadence follow-up observations of high-magnification *** detection of this planet,despite the considerable difficulties imposed by COVID-19(two KMT sites and OGLE were shut down),illustrates the potential utility of this program.

Endoscopic naso-pancreatic drainage for the treatment of pancreatic fistula occurring after LDLT

Pancreatic fistula is a quite rare complication in patients who undergo living donor liver transplantation(LDLT).However,in the cases that show pancreatic fistula,the limited volume of the graft and the resultant inadequate liver function may complicate the management of the *** a result,the pancreatic fistula may result in the death of the *** present 2 cases in whichendoscopic treatment was effective against pancreatic fistulas that developed after *** case 1,a 61-yearold woman underwent LDLT for primary biliary *** of a portal venous thrombus caused by a splenorenal shunt,the patient underwent portal vein reconstruction,and a splenorenal shunt was ligated on postoperative day(POD)*** main pancreatic duct was injured during the manipulation to achieve hemostasis,thereby necessitating open ***,discharge of pancreatic fluid continued even after POD *** naso-pancreatic drainage(ENPD)was performed,and this procedure resulted in a remarkable decrease in drain *** refractory pancreatic fistula healed on day 40 after *** case 2,a 58-year-old man underwent LDLT for cirrhosis caused by the hepatitis C *** the portal vein was exposed during thrombectomy,the pancreatic head was injured,which led to the formation of a pancreatic *** therapy was ineffective;therefore,ENPD was *** pancreatic fistula healed on day 38 after *** findings in these 2 cases show that endoscopic drainage of the main pancreatic duct is a less invasive and effective treatment for pancreatic fistulas that develop after LDLT.

BACKGROUND The incidence of internal hernias has recently increased in concordance with the popularization of laparoscopic *** particular concern are internal hernias occurring in Petersen's space,a space that is surgically created after treatment for gastric cancer and *** hernias cause devastating sequelae,such as massive intestinal necrosis,fatal Roux limb necrosis,and superior mesenteric vein *** addition,protein-losing enteropathy(PLE)is a rare syndrome involving gastrointestinal protein loss,although its relationship with internal Petersen’s hernias remains *** SUMMARY A 75-year-old man with a history of laparotomy for early gastric cancer developed Petersen's hernia 1 year and 5 mo after *** was successfully treated by reducing the incarcerated small intestine and closure of Petersen’s defect without resection of the small *** 3 mo after his surgery for Petersen’s hernia,he developed bilateral leg edema and *** was diagnosed with PLE with an alpha-1 antitrypsin clearance of 733 mL/24 ***-balloon enteroscopy revealed extensive jejunal ulceration as the etiology,and it facilitated minimum bowel *** analysis showed extensive jejunal ulceration and collagen hyperplasia with nonspecific inflammation of all layers without lymphangiectasia,lymphoma,or vascular *** postoperative course was unremarkable,and his bilateral leg edema and hypoalbuminemia improved after 1 *** was no relapse over the 5-year follow-up *** PLE and extensive jejunal ulceration may occur after Petersen's *** enteroscopy helps identify and resect these lesions.

反应精馏偶合了反应和精馏两种单元操作,通过精馏促进反应,可以提高反应转化率和收率,为可逆反应的化工过程生产提供了新的设计途径。基于严格热力学分析计算,利用计算机模拟和优化手段。提出了乙酸丁酯反应精馏、分离纯化的生产流程。采用UNIQUAC方程表征乙酸-正丁醇-乙酸丁酯-水四元非理想体系的汽液平衡,首先,根据实验数据回归了热力学模型中的交互作用参数,并预测了体系中5个共沸物组成,模型的计算结果与实际数据吻合。基于平衡级模型,提出了由平衡反应器、反应精馏塔、倾析器和纯化塔构成的可行流程,对提出的设计流程进行了模拟、优化,得到了操作工艺参数。模拟结果对工业过程的设计和改造具有一定的指导意义。

The influence of adiponectin G276T gene polymorphism on changes in total and high molecular weight adiponectin levels by exercise training

Adiponectin is an adipocyte-secreted hormone and regulates the metabolism of lipid and glucose. We examined the influence of adiponectin G276T gene polymorphism on changes in total or high molecular weight adiponectin levels by exercise training. A randomized parallel-design study (n = 53;40 women and 13 men;age range, 32 - 65 years) was conducted. Participants were randomly assigned to the exercise (n = 26) or control (n = 27) group and received exercise training for 70 min 2 times per week for 12 weeks and exercise advice at the baseline, respectively. Blood sample were obtained before and after the intervention. The primary outcomes were changes in total adiponectin and high molecular weight adiponectin levels. At the baseline, the participants with and without 276G allele did not differ with the total and high molecular weight adiponectin levels in the entire study population. No significant difference in the change in the total and high molecular weight adiponectin levels between the subjects with the G276G genotype and 276T allele carriers were found. This study provides evidence that the G276T polymorphism of the adiponectin gene is not associated with the magnitude of the effect of twice-weekly exercise training on the total and high molecular weight adiponectin levels.
