Effect of Specific Verbal instructions on the identification of Pain Location during a Passive Straight Leg Raise Test

Musculoskeletal pain is common. Because pain is subjective, objectively describing it is crucial. However, pain assessment may cause distress in patients;therefore, physical therapists (PTs) should conduct these tests quickly and accurately. Simple and clear instructions are recommended for pain assessment. However, few studies have provided evidence to support this hypothesis. Correspondingly, this study aimed to confirm the effectiveness of specific verbal instructions for pain location during five consecutive Passive Straight Leg Raise (PSLR) tests. The 28 asymptomatic participants (age 27.4 ± 9.6 years) who provided informed consent received five consecutive PSLR tests: three without and two with specific verbal instructions to ascertain pain intensity, quality, and location. The participants drew pain locations on a body chart and described the pain intensity and quality after each test. All participants were interviewed regarding the differences they noted in the presence and absence of specific verbal instructions. Each pain location was classified into one of ten areas for statistical analysis. The proportion of participants who changed the pain location was compared between the tests using McNemar’s test, and the kappa coefficient was confirmed for consistency of pain location. There was a significant difference in the proportion of participants who changed their pain location between the second and third tests and from the third to the fourth test (McNemar’s test: p = 0.003). Kappa coefficients had low consistency (κ = 0.28) just after receiving the specific verbal instructions in the fourth test compared to the third test. Consistency improved in the fifth test (κ = 0.57);93% of the participants answered that the pain location had become clearer. This study revealed the effects of specific verbal instructions in identifying pain locations. in legume plants, uricase gene （Nodulin-35） plays a positive role in metabolism of ureide and amide compounds in symbiotic nitrogen-fixing in the nodules. in this study, a pot experiment was performed to examine the effects of ammonium application on the transcription of MsU2 gene and distribution of major nitrogen compounds in alfalfa Medicago sativa. Data showed that alfalfa plant has a significant difference in contents of nitrogen compounds in xylem saps compared with soybean plant, and belongs to typical amide type legume plants with little ureide accumulation, and the accumulation of asparagines and ureide in the tissues of alfalfa is mainly gathered in the nodules. Northern blotting showed that excessive ammonium significantly inhibited the transcription of MsU2 gene in the nodules and roots, and mRNA accumulation of MsU2 gene in the plants exposed to excessive ammonium decreased gradually with culture time extension, indicating that application of ammonium significantly inhibited the transcription of MsU2 gene in the alfalfa plants. Although the application of exces- sive ammonium increased the contents of amino acids in various tissues of alfalfa, the accumulation of allantoin reflecting the strength of uricase activity is remarkably reduced in the xylem saps, stems and nodules when alfalfa plants exposed to excessive ammonium, suggesting that application of excessive ammonium generated a negative effect on symbiosis fixing-nitrogen system due to inhibition of ammonium ion on uricase activity in the nodules of alfalfa. This result seems to imply that application of excessive ammonium in legume plants should not be proposed to avoid affecting the ability of fixing nitrogen in the nodules of legume plants, and reasonable dose of ammonium should be recommended to effectively utilize the fixed N from atmosphere in legume plant production.

Background： Bovine milk contains not only a variety of nutritional ingredients but also microRNAs （miRNAs） that are thought to be secreted by the bovine mammary epithelial cells （BMECs）. The objective of this study was to elucidate the production of milk-related miRNAs in BMECs under the influence of lactogenic hormones. Results： According to a microarray result of milk exosomal miRNAs prior to cellular analyses, a total of 257 miRNAs were detected in a Holstein cow milk. Of these, 18 major miRNAs of interest in the milk were selected for an expression analysis in BMEC culture that was treated with or without dexamethasone, insulin, and prolactin （DiP） to induce a lactogenic differentiation. Quantitative polymerase chain reaction （qPCR） results showed that the expressions of miR-21-Sp （P = 0.005）, miR-26a （P = 0.016）, and miR-320a （P = 0.011） were lower in the DiP-treated cells than in the untreated cells. in contrast, the expression of miR-339a （P-- 0.017） in the cell culture medium were lower in the DiP-treated culture than in the untreated culture. intriguingly, the miR-148a expression in cell culture medium was elevated by DiP treatment of BMEC culture （P = 0.018）. The medium-to-cell expression ratios of miR- 103 （P = 0.025）, miR-148a （P 〈 0.001）, and miR-223 （P = 0.013） were elevated in the DiP-treated BMECs, suggesting that the lactogenic differentiation-induced secretion of these three miRNAs in BMECs. A bioinformatic analysis showed that the miRNAs down-regulated in the BMECs were associated with the suppression of genes related to transcriptional regulation, protein phosphorylation, and tube development. Conclusion： The results suggest that the miRNAs changed by lactogenic hormones are associated with milk protein synthesis, and mammary gland development and maturation. The elevated miR-148a level in DiP-treated BMECs may be associated with its increase in milk during the lactation period of cows.

AiM: To assess the efficacy of topical Semaphorin 3A(SEMA3A) in the treatment of allergic ***: Experimental allergic conjunctivitis(EAC)mice model induced by short ragweed pollen(SRW) in 4-week-old of BALB/c mice, mice were evaluated using haematoxylin and eosin(H&E) staining, immunofluor-escence and light microscope photographs. Early phase took the samples in 24 h after instillation and late phase took the samples between 4 to 14 d after the start of treatment. The study use of topical SEMA3A(10 U, 100 U,1000 U) eye drops and subconjunctival injection of SEMA3 A with same concentration. For comparison, five types of allergy eyedrops were quantified using clinical characteristics.· RESULTS: Clinical score of composite ocular symptoms of the mice treated with SEMA3 A were significantly decreased both in the immediate phase and the late phase compared to those treated with commercial ophthalmic formulations and non-treatment mice. SEMA3 A treatment attenuates infiltration of eosinophils entering into conjunctiva in EAC mice. The score of eosinophil infiltration in the conjunctiva of SEMA3 A 1000 U-treated group were significantly lower than low-concentration of SEMA3 A treated groups and non-treated group. SEMA3 A treatment also suppressed T-cell proliferation in vitro and decreased serum total ig E levels in EAC mice. Moreover, treatment of SEMA3 A suppressed Th2-related cytokines(iL-5, iL-13 and iL-4)and pro-inflammatory cytokines(iFN-γ, iL-17 and TNF-α)release, but increased regulatory cytokine iL-10 concentration in the conjunctiva of EAC ***: SEMA3 A as a biological agent, showed the beneficial activity in ocular allergic processes with the less damage to the intraocular tissue. it is expected that SEMA3 A may be contributed in patients with a more severe spectrum of refractory ocular allergic diseases including allergic conjunctivitis in the near future.

Combustion synthesis of YAG:Ce phosphors via the thermite reaction of aluminum

Cerium-doped yttrium aluminum garnet（YAG：Ce） as a yellow phosphor for white light-emitting diodes（LEDs） was synthesized via a facile combustion method using Y2 O3, CeO2, Al2 O3, Al,and NaClO4 as raw materials. The combustion synthesis approach utilizes the strong exothermic oxidation of aluminum to realize a self-sustaining reaction. in this study, we investigated the effects of the ratios of Al2 O3 to Ai,fluxes, and coprecipitated materials as raw materials on the luminescence properties of the synthesized YAG：Ce phosphors. When the amount of Al2 O3 x is varied, the combustion reaction proceeds at x ≤ 1.8,with x = 1.725 being the optimum condition for producing a high-performance product. When 5 wt%BaF2 is added, the luminescence intensity is significantly improved owing to a decrease of YAP（YAlO3）formation with improved uniformity. However, the addition of CaF2 and NaF does not improve the luminescence properties. To suppress the segregation of CeO2, we used the coprecipitated material Y2 O3-CeO2 as a raw material. Unlike with separate addition of Y2 O3 and CeO2, Ce ions are uniformly distributed in the coprecipitated material, resulting in improved luminescence properties. The combination of BaF2 and coprecipitated material significantly improves the internal quantum efficiency to83.0%, which is close to that of commercial phosphors.

The most prevalent issue in physical therapy is pain. Due to the subjective nature of pain, assessment tools are essential in understanding it as objective data. However, assessment of pain may result in distress for the patient. A physical therapist (PT) should conduct these tests as quickly and accurately as possible. Straightforward instructions are vital in such cases. This study aimed to clarify the effect of verbal instructions for pain assessment during a passive straight leg raise (PSLR) test for participants with chronic low back pain (CLBP). This study included 22 participants who provided informed consent and received three consecutive PSLR tests with measurement of the hip flexion range of motion (HFROM) and were instructed to cease the test at submaximal pain before the first test. Following the second and third tests, participants were given specific verbal instructions to remember pain intensity, quality, and location. After each test, participants were to circle the pain location on the body chart and rate their pain intensity on a numeric rating scale (NRS) and pain quality. All participants were then interviewed about the differences between having and not having specific verbal instructions. The results of HFROM, NRS, and pain extent were not significantly different between the first and second tests or between the second and third tests using a paired t-test. Eleven changes in pain location were found in the second test compared to those in the first test. in the third test, only three participants circled a different area than in the second test. Ten participants showed similar changes with pain location in pain quality in the three PSLR tests. This study revealed the effect of specific verbal instructions prior to PSLR tests. Particularly, participants could notice exact pain location. Our findings may help PT to understand pain cause and reduce patients’ stress during pain assessment in clinical settings.

A turbidity current is a particle-laden current driven by density differences due to suspended sediment *** currents can transport large amounts of sediment down slopes over great distances,and play a significant role in fluvial,lake and submarine *** better understand the sediment transport process,the flow system of an experimentally produced turbidity current in an inclined flume was investigated using video *** observed that the current progresses with constant frontal velocity and maintains an unchanged global interface *** addition,the spatio-temporal profiles of the inner mean and turbulence velocity obtained by ultrasound velocity profiler(UVP) showed that similar distributions were maintained,with low *** results indicate that the turbidity current progressed in a quasi-stationary state,which enabled long-distance sediment *** understand the mechanisms behind the quasi-stationary flow,we analyzed the forces acting on the turbidity *** found that under particular densities of suspended particles,the gravitational force is balanced by the viscous forces along the slope *** conclude that this specific force balance induces the quasi-stationary flow structure,enabling the long-distance transport of a substantial amount of sediment downstream with low dissipation.
