Lone-Star retractor perineal exposure method for laparoscopic abdominal perineal resection of rectal cancer

BACKGROUND The abdominal perineal resection(APR),historically referred to as Mile’s proce-dure,stands as a time-honored surgical intervention for rectal cancer *** in surgical techniques and the advent of neoadjuvant therapies have significantly improved the rate of sphincter preservation among patients afflicted with rectal cancer,including those with ultralow rectal *** these improvements,APR maintains its irreplaceable role in the clinical landscape,particularly for cases involving low rectal cancer with encroachment on the external anal sphincter or levator ani *** perineal exposure stands as a pivotal phase in APR,given that the precision of this maneuver is directly correlated with both the safety of the surgery and the patient’s subse-quent long-term *** To evaluate the value of Lone-Star retractor(LSR)perineal exposure method in the treatment for laparoscopic APR of rectal *** We reviewed the records of 38 patients with rectal cancer at Anqing Municipal Hospital from January 2020 to December 2023,including 20 patients who underwent the APR procedure with a LSR perineal exposure method(LSR group)and 18 patients who underwent the APR procedure with a conventional perineal exposure method(control group).In the LSR group,following incision of the skin and subcutaneous tissue,the LSR was placed and dynamically adjusted according to the surgical plane to fully expose the perineal operative *** A total of 38 patients underwent laparoscopic APR,none of whom were found to have distant metastasis upon intraoperative *** blood loss,the postoperative hospital stays and the wound pain scores in the LSR group were significantly lower than those in the control group.A single surgeon completed the perineal operation significantly more often in the LSR group than in the control group(P<0.05).The incidence of infection via the perineal incision in the LSR group was significantly lower

Evaluation of Nonbreaking Wave-Induced Mixing Parameterization Schemes Based on a One-Dimensional Ocean Model

Surface waves have a considerable effect on vertical mixing in the upper *** the past two decades,the vertical mixing induced through nonbreaking surface waves has been used in ocean and climate models to improve the simulation of the upper *** far,several nonbreaking wave-induced mixing parameterization schemes have been proposed;however,no quantitative comparison has been performed among *** this paper,a one-dimensional ocean model was used to compare the performances of five schemes,including those of Qiao et al.(Q),Hu and Wang(HW),Huang and Qiao(HQ),Pleskachevsky et al.(P),and Ghantous and Babanin(GB).Similar to previous studies,all of these schemes can decrease the simulated sea surface temperature(SST),increase the subsurface temperature,and deepen the mixed layer,thereby alleviating the common thermal deviation problem of the ocean model for upper ocean *** these schemes,the HQ scheme exhibited the weakest wave-induced mixing effect,and the HW scheme exhibited the strongest effect;the other three schemes exhibited roughly the same *** particular,the Q and P schemes exhibited nearly the same *** the simulation based on observations from the Ocean Weather Station Papa,the HQ scheme exhibited the best performance,followed by the Q *** the experiment with the HQ scheme,the root-mean-square deviation of the simulated SST from the observations was 0.43℃,and the mixed layer depth(MLD)was 2.0 *** a contrast,the deviations of the SST and MLD reached 1.25℃ and 8.4 m,respectively,in the experiment without wave-induced mixing.

Intrauterine adhesion is a major cause of female reproductive *** we and others uncontrolled pilot studies showed that treatment with autologous bone marrow stem cells made a few patients with severe intrauterine adhesion obtain live birth,no large sample randomized controlled studies on this therapeutic strategy in such patients have been reported so *** verify if the therapy of autologous bone marrow stem cells-scaffold is superior to traditional treatment in moderate to severe intrauterine adhesion patients in increasing their ongoing pregnancy rate,we conducted this randomized controlled clinical *** 195 participants with moderate to severe intrauterine adhesion were screened and 152 of them were randomly assigned in a 1:1 ratio to either group with autologous bone marrow stem cells-scaffold plus Foley balloon catheter or group with only Foley balloon catheter(control group)from February 2016 to January *** per-protocol analysis included 140 participants:72 in bone marrow stem cells-scaffold group and 68 in control *** ongoing pregnancy occurred in 45/72(62.5%)participants in the bone marrow stem cells-scaffold group which was significantly higher than that in the control group(28/68,41.2%)(RR=1.52,95%CI 1.08–2.12,P=0.012).The situation was similar in live birth rate(bone marrow stem cells-scaffold group 56.9%(41/72)*** group 38.2%(26/68),RR=1.49,95%CI 1.04–2.14,P=0.027).Compared with control group,participants in bone marrow stem cells-scaffold group showed more menstrual blood volume in the 3rd and 6th cycles and maximal endometrial thickness in the 6th cycle after hysteroscopic *** incidence of mild placenta accrete was increased in bone marrow stem cells-scaffold group and no severe adverse effects were *** conclusion,transplantation of bone marrow stem cells-scaffold into uterine cavities of the participants with moderate to severe intrauterine adhesion increased their ongoing pregnancy and

Stability-Considered Lane Keeping Control of Commercial Vehicles Based on Improved APF Algorithm

Regarding the lane keeping system,path tracking accuracy and lateral stability at high speeds need to be taken into account especially for commercial vehicles due to the characteristics of larger mass,longer wheelbase and higher mass *** improve the performance mentioned above comprehensively,the control strategy based on improved artificial potential field(APF)algorithm is *** the paper,time to lane crossing(TLC)is introduced into the potential field function to enhance the accuracy of path tracking,meanwhile the vehicle dynamics parameters including yaw rate and lateral acceleration are chosen as the repulsive force field *** lane keeping controller based on improved APF algorithm is designed and the stability of the control system is proved based on Lyapunov *** addition,adaptive inertial weight particle swarm optimization algorithm(AIWPSO)is applied to optimize the gain of each potential field *** co-simulation results indicate that the comprehensive evaluation index respecting lane tracking accuracy and lateral stability is reduced ***,the proposed control strategy is verified by the HiL *** provides a beneficial reference for dynamics control of commercial vehicles and enriches the theoretical development and practical application of artificial potential field method in the field of intelligent driving.

Reduced exciton binding energy and diverse molecular stacking enable high-performance organic solar cells with V_(OC)over 1.1 V

High-voltage organic solar cells(OSCs)have received increasing attention because of their promising applications in tandem devices and indoor photovoltaics,but the trade-off between energy loss and charge generation induced by exciton binding energy(E_(b))has become one of the biggest bottlenecks limiting the development of this ***,a wide bandgap(WBG)nonfullerene acceptor BTA503 with reduced E_(b) is designed by changing the phenyl side chain on the central core of Cl-BTA5 to an alkyl *** diverseπ-πinteractions and enhanced molecular stacking of BTA503 are responsible for its reduced E_(b).Furthermore,both the diminished charge recombination and the fast exciton dissociation caused by the small E_(b) favor the generation of more charge carriers for the PTQ10:BTA503 *** efficient Forster resonance energy transfer(FRET)and multiple π-π stacking patterns provide additional charge transfer and transport ***,the PTQ10:BTA503-based OSC device achieves a V_(OC)of 1.112 V and a PCE of 12.70%,which is higher than that of PTQ10:Cl-BTA5(PCE=10.92%).Simultaneously,the thick film(~300 nm)binary device of PTQ10:BTA503 achieves a PCE of 10.13% with a V_(OC)of 1.102 V,which is the best result for thick film high-voltage *** importantly,the ternary device of PTQ10:BTA503:Cl-BTA5(1:0.9:0.1)realizes a champion PCE of 13.12% with a V_(OC)of 1.126 *** study demonstrates that it is an effective strategy to reduce E_(b) of A_(2)-A_(1)-D-A_(1)-A_(2) type WBG acceptors by modulating the side chains on D unit,which further favors the corresponding devices to obtain world-record PCE and improves their potential for commercial applications.

The present letter to the editor is related to the review with the title“Past,present,and future of long-term treatment for hepatitis B virus.”Chronic hepatitis B(CHB)represents an important and pressing public health *** identification and effective antiviral therapy hold the potential to reduce liver-related mortality attributable to chronic infection with hepatitis B virus(HBV)***,the current global treatment rates for CHB remain conspicuously low,with the excessively stringent treatment criteria advocated by national CHB guidelines being a contributing factor to these low ***,recent strides in comprehending this malady and the emergence of novel antiviral agents prompt the imperative re-evaluation of treatment standards to extend the sphere of potential *** impending need arises for a novel paradigm for the classification of patients with CHB,the expansion of antiviral treatment eligibility for HBV-infected individuals,and even the streamlining of the diagnostic process for CHB to amplify cost-effectiveness and augment survival prospects.

Aiming at accessing polyethylene-bearing in-chain degradable functionality,copolymerization of carbon dioxide(CO_(2))with ethylene has long been recognized as a significant but highly challenging ***,most trials have resulted in ethylene homopolymerization because of the endothermic property and high energy barriers of CO_(2)insertion during polymer chain *** we report an unprecedented tandem reaction protocol for the synthesis of polyethylenes with in-chain carbonyls from CO_(2)and ethylene by the combination of photocatalytic CO_(2)reduction and palladium-catalyzed coordination/insertion *** protocol provides an easy way to adjust the carbonyl content(from 0.13 to 12 mol%)and insertion selectivity(up to 99%isolated carbonyls)of the polyethylene copolymers,which enables the polymers tomaintain the main properties of highdensity polyethylene and simultaneously endows them with good *** addition,besides apolar polyethylene copolymers,polar-functionalized polyethylenes with in-chain isolated carbonyls can also be synthesized from CO_(2),ethylene and a polar monomer using the same *** believe our present work sheds new light on the synthesis of degradable polyethylenes by employing CO_(2)as a green carbonyl source.

Uncovering of the formation of rare earth texture and pseudo fiber bimodal microstructure in the high ductility Mg-2Gd-0.4Zr alloy during extrusion

The aim of this research was to elucidate the underlying mechanism involved in the formation of rare earth(RE)texture and pseudo fiber bimodal microstructure in the high ductility Mg-2Gd-0.4Zr *** microstructure and texture evolution during the extrusion process were analyzed using various tech-niques,including optical microscopy(OM),scanning electron microscopy(SEM),electron backscatter diffraction(EBSD),and electron probe microanalysis(EPMA).The findings revealed that the RE texture in the extruded Mg-2Gd-0.4Zr alloy emerged during the dynamic recrystallization(DRX)process and was further strengthened during the subsequent static recrystallization and grain growth *** nu-cleation and growth of grains in the streamline region of Zr particles were delayed in comparison to other regions due to the pinning effect of Zr particles,ultimately resulting in the formation of pseudofiber bi-modal microstructure in the extruded Mg-2Gd-0.4Zr alloy.
