Extracorporeal blood therapy in sepsis and acute respiratory distress syndrome： the ＂purifying dream＂

Objective To discuss the rationale, hypothesis, modality of extracorporeal blood purification （EBP） techniques for the critically ill animal models or patients, and to summarize the experimental and clinical studies with inconsistent data which explored the EBP＇s efficacy in the areas of critical care medicine. Data sources Articles referred in this review were collected from the database of PubMed published in English up to June 2014. Study selection We had done a literature search by using the term ＂（sepsis OR acute lung injury OR acute respiratory distress syndrome） AND （extracorporeal blood purification OR hemofiltretion OR hemoperfusion OR plasma exchange OR plasmapheresis OR adsorpiton）＂. Related original or review articles were included and carefully analyzed. Results Acute cellular and humoral immune disturbances occur in both sepsis and acute respiratory distress syndrome （ARDS）. Treatments aimed at targeting one single pro-/anti-inflammatory mediator have largely failed with no proven clinical benefits. Such failure shifts the therapeutic rationale to the nonspecific, broad-spectrum methods for modulating the over-activated inflammatory and anti-inflammatory response. Therefore, EBP techniques have become the potential weapons with high promise for removing the circulating pro-/anti-inflammatory mediators and promoting immune reconstitution. Over the years, multiple extracorporeal techniques for the critically ill animal models or patients have been developed, including hemoflltration （HF）, high-volume hemofiltration （HVHF）, high-cutoff hemofiltration （HCO-HF）, hemo-perfusion or -adsorption （HP/HA）, coupled plasma filtration adsorption （CPFA）, and plasma exchange （PE）. These previous studies showed that EBP therapy was feasible and safe for the critically ill animal models or patients. However, data on their efficacy （especially on the clinical benefits, such as mortality） were inconsistent. Epidemiological characteristics of pulmonary tuberculosis in patients with pneumoconiosis based on its social determinants and risk factors in China:a cross-sectional study from 27 provinces

Background:Patients with pneumoconiosis have an elevated risk of contracting pulmonary tuberculosis(PTB)and need particular attention.However,extensive population-based studies on the prevalence of PTB in patients with pneumoconiosis have not been reported in China since 1992.This study aimed to investigate the epidemiological characteristics of PTB in patients with pneumoconiosis based on its social determinants and risk factors in China.Methods:Based on the Commission on Social Determinants of Health(CSDH)framework,data were obtained from a questionnaire survey of patients with pneumoconiosis from China’s 27 provinces(autonomous regions,municipalities)from December 2017 to June 2021.By chi-square and multivariate logistic regression analyses,the epidemiological characteristics of PTB in the patients were identified based on its prevalence and odds ratio(OR)and associated social determinants and risk factors.The population attributable fractions(PAFs)of significant risk factors were also calculated.Results:The prevalence of PTB in patients with pneumoconiosis(n=10,137)was 7.5%(95%confidence interval[CI]:7.0-8.0%).Multivariate logistic regression analysis showed that risk factors included in-hospital exposure to patients with PTB(OR=3.30,95%CI:2.77-3.93),clinically diagnosed cases(OR=3.25,95%CI:2.42-4.34),and northeastern regions(OR=2.41,95%CI:1.76-3.31).In addition,lack of work-related injury insurance(WRII),being born in a rural area,being unemployed,living in western regions,household exposure to patients with PTB,smoking,being underweight,complications of pulmonary bullae or pneumothorax,hospitalization history,and former drinkers among the rural patients were also statistically significant risk factors.Being born in a rural area,lack of WRII and in-hospital exposure to patients with PTB had higher PAFs,which were 13.2%(95%CI:7.9-18.5%),12.5%(95%CI:8.3-16.7%),and 11.6%(95%CI:8.8-14.3%),respectively.Conclusion:The prevalence of PTB in pneumoconiosis remains high in China;it is basically in line with the CSDH m

Cigarette smoking contributes to idiopathic pulmonary fibrosis associated with emphysema

Background Combined emphysema and pulmonary fibrosis,including idiopathic pulmonary fibrosis （IPF）,is a distinct disorder described with upper-lobe emphysema and lower-lobe fibrosis on chest computed tomography.Smoking appears to be the predominant risk factor for this disorder.We aimed to compare clinical features,smoking history,physiological and radiological findings between IPF with and without emphysema.Methods A sample of 125 IPF patients over a period of 48 months were evaluated.High resolution CT scans were reviewed blinded to clinical data.The IPF patients with or without emphysema were classified accordingly.Results The prevalence of emphysema in this IPF sample was 70/125.IPF with emphysema was significantly associated with smoking status （OR 63; 95％ CI 4.4 to 915; P=0.002） and smoking pack year （OR 1.1; 95％ CI 1.05 to 1.13; P=-0.000）.The patients with IPF and emphysema had a higher decrease in carbon monoxide diffusing capacity adjusted for alveolar volume （（58±19）％ pred vs.（66：±：21）％ pred; P=-0.021） and a higher prevalence of pulmonary hypertension （24/70 vs.7/55; P=0.006）.The two groups of patients had similar forced and residual volumes.No significant differences were found in cell differentials of bronchoalveolar lavage or the scores of fibrosis on chest CT.Survival of the patients with emphysema was significantly less than that of patients with IPF alone.Conclusions Cigarette smoking induces IPF combined with emphysema.Emphysema further impairs physiological function and increases the prevalence of pulmonary hypertension that leads to poor prognosis.The inclusion of the patients with combined pulmonary fibrosis and emphysema in IPF clinical trials may lead to under evaluation of the effect of treatment in patients.

Single-cell RNA sequencing profiling of the effects of aging on alveolar stem cells

The aging of alveolar stem cells has been linked to many chronic lung diseases, including pulmonary fibrosis. However, the effects of aging on alveolar stem cells during homeostasis and post-injury alveolar repair have not been well characterized. Here we conducted a single-cell RNA sequencing(scRNA-seq) analysis of alveolar stem cells of 3-month-old and 12-month-old mice to characterize the aging effect on alveolar stem cells. Our results have shown that the transcriptomes of alveolar stem cells of 3-month-old and 12-month-old mice are not significantly different under the steady condition. However, after a bleomycin-induced lung injury, the alveolar stem cells of 12-month-old mice show enhanced inflammatory responses and decreased lipid metabolism. Our study suggests a close relationship among aging, lipid metabolism, inflammatory responses and chronic lung diseases.

Clinical features and outcomes of 210 patients with idiopathic pulmonary fibrosis

Background Idiopathic pulmonary fibrosis （IPF） is a lethal chronic interstitial lung disease （ILD） of unknown cause and having a variable and unpredictable course.This study aimed to summarize the clinical features and follow-up outcomes and to identify potential factors useful for the assessment of prognosis in IPF.Methods Two hundred and ten patients hospitalized and diagnosed as IPF in our unit from January 1999 to June 2007 were enrolled into this study.The baseline demographic,clinical,radiologic and physiologic characteristics were summarized.Clinical follow-up data until February 2010 were collected,and the median survival time and 1-,2-,and 5-year survival rates,as well as the influences of the summarized baseline variables on the prognosis were analyzed.Results The age at diagnosis as IPF was （64±10） years,the duration before diagnosis of 106 patients （50％） was shorter than 2 years,and 73％ were males.One hundred and forty-five patients （69％） had a history of smoking with a median pack-year of 18.Eighty-nine patients （42％） had emphysema and 62 patients （29％） pulmonary arterial hypertension （PAH）.One hundred and twenty-four patients were followed up,of which 99 patients died from various causes including respiratory failure related to IPF （93％）.The follow-up period was （21±23） months.The median survival time was 38months.The 1-,2-,and 5-year survival rates were 61％,52％,and 39％,respectively.Multivariate analysis showed clubbing,PAH,duration from initial onset to diagnosis,and forced expiratory volume in 1 second （FEV1）/forced vital capacity （FVC） were independent prognostic indicators of IPF.Conclusion IPF patients who have clubbing,PAH,a higher FEVJFVC,and a short duration from initial onset to diagnosis have a poorer outcome.

间质性肺疾病(ILD)是一组由各种已知或未知原因导致的以肺泡单位炎症和(或)纤维化为基本病理特征,进而引起肺脏结构破坏和功能丧失的异质性疾病。其病因复杂,临床表现缺乏特异性,影像、病理改变多样,涉及多学科,疾病的诊断和治疗需要多学科参与。多学科讨论(MDD)确实能够提高ILD诊断和治疗的准确性,因而MDD被国内外的ILD临床实践指南推荐。然而,ILD-MDD缺乏规范,如何有效落地是关键。为此,本研究团队自2018年起在中日友好医院探索性地开展了针对ILD的MDD,也通过线上会议形式联合全国多家中心开展ILD-MDD示范。在此基础上,本研究团队在中华医学会呼吸病学分会和中国医师协会呼吸医师分会的支持下,组织相关领域专家制定了《间质性肺疾病多学科讨论规范中国专家共识》,以指导ILD-MDD规范进行,提高ILD的诊治水平。

To the Editor:Coronavirus disease 2019(COVID-19),which broke out in 2019,has become a global pandemic.Similar to severe acute respiratory syndrome coronavirus(SARS-CoV)in 2003,SARS-CoV-2 could cause acute lung injury and cytokine storms characterized by the increased interleukin(IL)-8,IL-6,and tumor necrosis factorα(TNF-α).[1]Perspective studies in those survivors from the severe acute respiratory syndrome(SARS)epidemic in 2003 revealed that those SARS patients manifested varying degrees of pulmonary interstitial fibrosis.[2]Similarly,patients with severe COVID-19 are also featured by the diffuse alveolar damage along with alveolar interstitial fibrosis.[3]

Nuclear miR-665 aggravates heart failure via suppressing phosphatase and tensin homolog transcription

Although numerous miRNAs have been discovered,their functions in the different subcellular organelles have remained obscure.In this study,we found that miR-665 was enriched in the nucleus of cardiomyocytes,and then investigated the underlying role of nuclear miR-665 in heart failure.RNA fluorescence in situ hybridization assays in human heart tissue sections and primary cardiomyocytes showed that miR-665 was localized in the nucleus of cardiomyocytes.Increased expression of nuclear miR-665 was observed not only in the cardiomyocytes isolated from the heart of mice treated in vivo by transverse aortic constriction(TAC),but also in phenylephrine(PE)-treated cultured cardiomyocytes in vitro.To further explore the role of miR-665 in heart failure,a type 9 recombinant adeno-associated virus(rAAV)system was employed to manipulate the expression of miR-665 in mice.Overexpression of miR-665 aggravated TAC-induced cardiac dysfunction,while down-expression of miR-665 showed opposite effects.Bioinformatic prediction and biological validation confirmed that the PTEN(phosphatase and tensin homolog)gene was one of the targets of miR-665 in the nucleus.Furthermore,restoring PTEN expression significantly eliminated the destructive effects of miR-665 over-expression in TAC-induced cardiac dysfunction.Our data showed that nuclear miR-665 aggravates heart failure via inhibiting PTEN expression,which provided a therapeutic approach for heart failure.
