The Use of Tranexamic Acid in Percutaneous Nephrolithotomy. A Randomized Controlled Study (Local Experience)

Purpose: To evaluate the efficacy and safety of Tranexamic acid in reducing blood loss during PCNL. Materials and Methods: A total of 50 patients who underwent unilateral Percutaneous Nephrolithotomy from March 2017 to November 2017 were randomized into 2 equal groups;group (A) who received 1 gm (10 cc) of tranexamic acid ampule on call to surgery and group (B) who received (10 cc) normal saline. The Patients clinical data were collected and recorded in Microsoft Excel 2016 and analyzed using Statistical Package for the Social Science (SPSS) version 20 software program. Results: Fifty patients were included in the study: (33 males and 17 female). The mean age in group A patients was (48.12 ± 13.58) years, and in group B was (48.88 ± 16.17) with P value = 0.858. In group A the number of males was 16 (64%), and female was 9 (36%), and in group B male was 17 (68%) and female was 8 (34%). Mean body mass index (BMI) in group A was (28.58 ± 4.51) and in group B was (26.72 ± 3.71), with P value = 0.119 which is statistically not significant. The mean total blood loss in milliliters in group A was (73.80 ± 60.1), while in group B it was (117.24 ± 87.9) which is statistically significant with P-Value = 0.047. The mean hemoglobin drop in group A was (0.45 ± 0.35 g/dl) while in group B was (1.00 ± 0.46 g/dl), which is statistically significant with P value = 0.0001. The mean operative time was (48.4 ± 17) minute in group A, while it was (62.4 ± 15) in group B, with P-Value = 0.005, which is statistically was significant. Post-operative hematuria in group A;20 (80%) patients had mild hematuria, 3 (12%) patients had moderate hematuria and 2 patients (8%) had no hematuria. In group B;18 (72%) patients had mild hematuria, 6 (24%) patients had moderate hematuria and one patient (4%) had no hematuria, which is statistically not significant with P-Value = 0.487 (Table 2). Herein, the fabrication of cellulose acetate (CA) silica-based nanocomposite membranes via the dry-wetphase inversion procedure for water desalination was investigated. The modified and unmodified silicananoparticles (MSNPs and SNPs) were prepared by the sol-gel technique. The effect of the SNPs andMSNPs was investigated on the CA membrane's properties and their performance for water *** CA nanocomposite membranes were characterized to study their structure, hydrophilicity, andmorphology. The fabricated nanocomposite membranes showed hydrophilic surface properties. Theperformance of reverse osmosis (RO) membranes was measured using a crossflow RO unit at 10 bar(1 bar = 0.1 MPa). The membrane with 10 mg of SNPs enhanced permeate water flux compared to thepristine CA membrane by 1.6 L/(m2·h). The effect of MSNPs on the nanocomposites' performance waslower than their counterpart in the case of adding SNPs. The membrane with 30 mg of MSNPs showedthe highest permeate water flux among other nanocomposite membranes with a value oAQSf 35.7 L/(m2·h)at 24 bar.

BACKGROUND Preemptive living donor kidney transplantation(PLDKT)is recommended as the optimal treatment for end-stage renal *** To assess the rate of PLDKT among patients who accessed KT in our center and review the status of PLDKT in *** We performed a retrospective review of the patients who accessed KT in our center from November 2015 to November *** addition,the PLDKT status in Egypt was reviewed relative to the *** Of the 304 patients who accessed KT,32 patients(10.5%)had preemptive access to KT(PAKT).The means of age and estimated glomerular filtration rate were 31.7±13 years and 12.8±3.5 mL/min/1.73 m2,***-nine patients had KT,including 3 PLDKTs only(5.1%of total KTs and 9.4%of PAKT).Twenty-nine patients(90.6%)failed to receive PLDKT due to donor unavailability(25%),exclusion(28.6%),regression from donation(3.6%),and patient regression on starting dialysis(39.3%).In multivariate analysis,known primary kidney disease(P=0.002),patient age(P=0.031)and sex(P=0.001)were independent predictors of achievement of KT in our ***,PAKT was not significantly(P=0.065)associated with the achievement of *** of the literature revealed lower rates of PLDKT in Egypt than those in the *** Patient age,sex,and primary kidney disease are independent predictors of achieving living donor *** its non-significant effect,PAKT may enhance the low rates of *** main causes of non-achievement of PLDKT were patient regression on starting regular dialysis and donor unavailability or exclusion.

山体滑坡对人类社会和人类活动构成重大风险,常常造成基础设施损坏和人员伤亡。由自然或人类活动引起的山体滑坡在全世界频繁发生,已对全世界的财产和生命造成重大危害。作为热带沙漠气候区国家,也门也经常受山体滑坡灾害影响。也门地形崎岖陡峭、地质构造不稳定、岩石松软易碎、季风期强降雨集中且持续时间长,这些因素共同导致也门山区严重的滑坡问题和相关灾害现象。因此,也门也是世界上最容易发生山体滑坡的国家之一。也门的许多偏远地区的小规模山体滑坡,但除非造成人员伤亡,否则很少有人对其负责。而且小型滑坡造成的经济损失不亚于较大型自然灾害造成的经济损失。尽管也门相关研究者开展了大量的滑坡灾害研究工作,但仍没有可用于产生可靠的滑坡灾害预测的有效方法。针对也门频发的山体滑坡灾害问题,本研究以受山体滑坡灾害威胁严重的也门Utmah地区为研究区。通过调研方法,对该地区的山体滑坡灾害进行了调查,并分析了也门山体滑坡灾害的影响因素和成因,并采用机器学习方法对研究区山体滑坡灾害的易发性进行了评估。本研究选用随机森林(RF)、极端梯度提升(XGBoost)、人工神经网络(ANN)、朴素贝叶斯(NB)和K最近邻(KNN)五种机器学习算法开展对比研究。本研究主要目标是评估和比较相对频率比模型(RFR)和五种机器学习算法(MLA)在也门Utmah地区绘制滑坡易发性(LSM)的有效性。为了创建研究区的山体滑坡地图,研究人员借助高分辨率遥感影像对研究区开展了实地调查。通过对100处滑坡样点进行识别和测绘之后,制作了一份研究区滑坡灾害清单地图。本研究将采样点随机分为训练数据集(70个滑坡)和验证数据集(30个滑坡)。本研究在调研基础上,选取了11个山体滑坡影响因子,并使用频率比(FR)方法对这些因子进行处理为模型的变量参数。最后,本研究对五个机器学习模型使用统计指标进行测试和评估。在本研究中使用了Friedman和Wilcoxon符号秩检验以及受试者工作特征(AUROC)曲线下的面积对研究结果进行评估和验证。基于训练和验证数据集分析,结果表明RF算法(AUROC,0.992)在创建磁化率图方面优于其他模型。XGBoost模型(AUROC,0.991)、NB模型(AUROC,0.970)、ANN模型(AUROC,0.922)、KNN模型(AUROC,0.877)和FR(AUROC,0.674)效果较差。通过研究对比发现,随机森林(RF)模型是在预测山体滑坡效果较好,对研究结果的评估精度较高。该研究也表明机器学习技术对未来开发精确滑坡灾害易发性图方面具有更广的潜在应用。此外,制作的滑坡灾害易发性地图也将对也门当地民众和政府开展滑坡灾害防治、重大决策制定等工作有提供科学支撑。

建筑结构常由于损伤、规范要求、功能改变等原因,需要进行加固;碳纤维聚合物（CFRP）由于强度高、质量轻、耐腐蚀性好、易于施工等优点,广泛应用于结构加固中。以经过CFRP材料加固的典型方形截面钢筋混凝土框架柱为研究对象,针对交通工具撞击作用,探讨CFRP对钢筋混凝土柱不等跨撞击作用下的动力响应,本文主要研究内容和结果如下:（1）通过试验研究,探讨CFRP材料包裹层数（1层,6层）、包裹方式（横向,纵向）等因素对撞击作用下构件动力响应性能的影响,试验研究表明:试验构件均为受弯破坏模式,冲击点处和近冲击点支座处是破坏严重区域;横向包裹的构件断裂更为突然,对纵向包裹构件,包裹层数提高可有效减少破坏程度。（2）对试验中获得的冲击力时程曲线、挠度时程曲线和应变时程曲线分析表明包裹方式、层数对冲击力峰值影响较小;纵向包裹的构件、平台段持时更长,横向包裹平台段波动大;纵向包裹层数的增大,可增大平台值。包裹层数越大,撞击产生的挠度越小,纵向包裹能够有效传递受力。（3）基于本文试验和已有试验结果,提出采用短期抗弯刚度Bs来表征CFRP加固的RC混凝土结构柱的防撞能力。基于他人推导,提出CFRP加固RC结构柱Bs计算的表达式,将本文理论计算结果和已有试验结果进行比较,验证了公式的适用性。（4）建立了有限元模型,并将有限元分析结果与试验结果进行比较,验证了有限元模型的正确性;基于有限元分析,分析了冲击速度、砼强度、纵向钢筋配筋率、CFRP层数对构件抗冲击性能的影响,提出了CFRP加固层数最优值。

First-Line Drug Resistance Patterns of Mycobacterium tuberculosis Complex Isolates from Re-Treatment Patients from Sudan

Drug susceptibility testing (DST) plays a pivotal role in TB patients’ management leading to the selection of most effective drugs. This study aimed to determine resistance patterns to first line anti-TB drugs in Mycobacterium tuberculosis isolates from re-treated patients from Sudan. A total of 239 sputum specimens were collected from smear positive re-treatment TB patients during the period from July 2009 to July 2010. Specimens were pre-treated according to Petroff method. The recovered isolates were tested for sensitivity to first line anti-TB drugs by the 1% proportion method. One hundred and forty three (143/239, 59.8%) mycobacterial isolates were successfully recovered. The majority (98.6%, 141/143) of the isolates were Mycobacterium tuberculosis complex strains. Two strains (2/143, 1.4%) were identified as RIF/INH-resistant MOTT, while fifty four isolates (38.3%, 54/141) were MDR. Multi- drug resistant Mycobacterium tuberculosis complex (MDR-TB) among re-treatment patients from national referral centers for tuberculosis diagnosis and management was considerably high in the study isolates.

The Gain Distribution According to the Theoretical Structure and Decay Dynamics of Sodium like Cu

Level structure, oscillator strengths, transition probabilities and radiative life times are evaluated for 1s2 2s2 2p63l, 4l, 5l (l = 0, 1, 2, 3, 4) states in sodium like Cu18+. The calculations are carried out using COWAN code. The calculations were made are compared with other results in literature where a good agreement is found, we also report on some unpublished energy values and oscillator strengths. Our results are used in the calculation of reduced population of 21 fine structure levels over a wide rang of electron density values (1018 to 1020) and at various electron plasma temperature. For those transitions with positive population inversion factor, the gain coefficients are evaluated and plotted against the electron density.
