BACKgROUND The abdominal perineal resection(APR),historically referred to as Mile’s proce-dure,stands as a time-honored surgical intervention for rectal cancer *** in surgical techniques and the advent of neoadjuvant therapies have significantly improved the rate of sphincter preservation among patients afflicted with rectal cancer,including those with ultralow rectal *** these improvements,APR maintains its irreplaceable role in the clinical landscape,particularly for cases involving low rectal cancer with encroachment on the external anal sphincter or levator ani *** perineal exposure stands as a pivotal phase in APR,given that the precision of this maneuver is directly correlated with both the safety of the surgery and the patient’s subse-quent long-term *** To evaluate the value of Lone-Star retractor(LSR)perineal exposure method in the treatment for laparoscopic APR of rectal *** We reviewed the records of 38 patients with rectal cancer at Anqing Municipal Hospital from January 2020 to December 2023,including 20 patients who underwent the APR procedure with a LSR perineal exposure method(LSR group)and 18 patients who underwent the APR procedure with a conventional perineal exposure method(control group).In the LSR group,following incision of the skin and subcutaneous tissue,the LSR was placed and dynamically adjusted according to the surgical plane to fully expose the perineal operative *** A total of 38 patients underwent laparoscopic APR,none of whom were found to have distant metastasis upon intraoperative *** blood loss,the postoperative hospital stays and the wound pain scores in the LSR group were significantly lower than those in the control group.A single surgeon completed the perineal operation significantly more often in the LSR group than in the control group(Pgroup was significantly lower

Circular RNAs(circRNAs)are ideal biomarkers of oral squamous cell carcinoma(OSCC)because of their highly stable closed-loop structure,and they can act as microRNA(miRNA)sponges to regulate OSCC *** analyzing clinical samples,we identified circCPNE1,a dysregulated circRNA in OSCC,and its expression level was negatively correlated with the clinical stage of OSCC ***-of-function assays revealed the tumor-suppressive effect of circCPNE1,which was then identified as a miR-330-3p ***-330-3p was recognized as a tumor promoter in multiple studies,consistent with our finding that it could promote the proliferation,migration,and invasion of OSCC *** results indicated that selective inhibition of miR-330-3p could be an effective strategy to inhibit OSCC ***,we designed cationic polylysine-cisplatin prodrugs to deliver antagomiR-330-3p(a miRNA inhibitory analog)via electrostatic interactions to form PP@miR nanoparticles(NPs).Paratumoral administration results revealed that PP@miR NPs effectively inhibited subcutaneous tumor progression and achieved partial tumor elimination(2/5),which confirmed the critical role of miR-330-3p in OSCC *** findings provide a new perspective for the development of OSCC treatments.

Foam concrete is a prospective material in defense engineering to protect structures due to its high energy absorption capability resulted from the long plateau ***,stress enhancement rather than stress mitigation may happen when foam concrete is used as sacrificial claddings placed in the path of an incoming blast *** investigate this interesting phenomenon,a one-dimensional difference model for blast wave propagation in foam concrete is firstly proposed and numerically solved by improving the second-order godunov *** difference model and numerical algorithm are valida.ed against experimental results including both the stress mitigation and the stress *** difference model is then used to numerically analyze the blast wave propagation and deformation of material in which the effects of blast loads,stress-strain relation and length of foam concrete are *** particular,the concept of minimum thickness of foam concrete to avoid stress enhancement is ***,non-dimensional analysis on the minimum thickness is conducted and an empirical formula is proposed by curve-fitting the numerical da.a,which can provide a reference for the application of foam concrete in defense engineering.

Laser powder bed fusion(LPBF)in-situ alloying technology offers the possibility to construct gradient materials with varied structures and *** graded Fe-Cr-Co permanent magnetic alloys were fabricated by LPBF and in-situ alloying mixed powders of Fe,Cr,and Co *** effects of different Fe,Cr and Co contents on the microstructure,magnetic properties and hardness of Fe-Cr-Co alloys prepared by LPBF were *** as-built Fe-Cr-Co alloys present a single body-centered-cubic phase and have a homogeneous distribution of *** mechanical properties and magnetic properties of the compositionally graded sample show a gradient *** the increase in Cr content,the Vickers hardness of the sample increases,and the saturation magnetization of the sample *** optimal magnetic properties in an isotropic state are given as coercivity HcB=21.65 kA/m,remanence Br=0.70 T and energy product(BH)_(max)=5.35 kJ/m^(3),which are comparable to or higher than the reported magnetic properties in an isotropic state prepared by traditional powder *** in-situ alloying technology has the potential to further explore Fe-Cr-Co magnetic materials,such as those consisting of multiple or more constituent elements,and to maximize the compositional flexibility of magnetic materials.

BACKgROUND Prophylactic loop ileostomy is an effective way to reduce the clinical severity of anastomotic leakage following radical resection of rectal *** surgical site infection(SSI)is a common complication after ileostomy *** To evaluate the efficacy and safety of the micro-power negative pressure wound technique(MPNPWT)in preventing incisional *** This was a prospective,randomized controlled clinical trial conducted at a single center.A total of 101 consecutive patients who underwent ileostomy closure after rectal cancer surgery with a prophylactic ileostomy were enrolled from January 2019 to December *** were randomly allocated into an MPNPWT group and a control *** MPNPWT group underwent intermittent suturing of the surgical incision with 2-0 Prolene and was covered with a micro-power negative pressure *** surgical outcomes were compared between the MPNPWT(n=50)and control(n=51)*** factors for incisional SSI were identified using logistic *** There were no differences in baseline characteristics between the MPNPWT(n=50)and control groups(n=51).The incisional SSI rate was significantly higher in the control group than in the MPNPWT group(15.7%vs 2.0%,P=0.031).However,MPNPWT did not affect other surgical outcomes,including intra-abdominal complications,operative time,and blood *** hospital stay length and hospitalization costs did not differ significantly between the two groups(P=0.069 and 0.843,respectively).None of the patients experienced adverse effects of MPNPWT,including skin allergy,dermatitis,and *** also helped heal the infected *** study indicated that MPNPWT was an independent protective factor[odds ratio(OR)=0.005,P=0.025)]and diabetes was a risk factor(OR=26.575,P=0.029)for incisional *** MPNPWT is an effective and safe way to prevent incisional SSI after loop ileostomy closure.
