Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in June 2012, and the data have been released to the public as the LAMOST Pilot data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in June 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

As one of the three payloads for the Advanced Space-based Solar Observatory(ASO-S)mission,the Lyman-alpha(Lyα)Solar Telescope(LST)is composed of three instruments:a Solar Corona Imager(SCI),a LyαSolar Disk Imager(SDI)and a full-disk White-light Solar Telescope(WST).When working in-orbit,LST will simultaneously perform high-resolution imaging observations of all regions from the solar disk to the inner corona up to 2.5 R⊙(R⊙stands for the mean solar radius)with a spatial resolution of 4.8′′and 1.2′′for coronal and disk observations,respectively,and a temporal resolution of 30–120 s and 1–120 s for coronal and disk observations,*** maximum exposure time can be up to20 s due to precise pointing and image stabilization *** the three telescopes of LST,SCI is a dual-waveband coronagraph simultaneously and independently observing the inner corona in the HI Lyα(121.6±10 nm)line and white light(WL)(700±40 nm)wavebands by using a narrowband Lyαbeam splitter and has a field of view(FOV)from 1.1 to 2.5 R⊙.The stray-light suppression level can attainline and 360.0 nm wavebands,respectively,which adopt an off-axis two-mirror reflective structure with an FOV up to 1.2 R⊙,covering the inner coronal edge area and relating to coronal *** present the up-to-date design for the LST payload.

Intrahepatic cholangiocarcinoma(iCCA)can originate from the large bile duct group(segment bile ducts and area bile ducts),small bile duct group(septal bile ducts and interlobular bile ducts),and terminal bile duct group(bile ductules and canals of Hering)of the intrahepatic biliary tree,which can be histopathological corresponding to large duct type iCCA,small duct type iCCA and iCCA with ductal plate malformation pattern,and cholangiolocarcinoma,*** challenge in pathological diagnosis of above subtypes of iCCA falls in the distinction of cellular morphologies,tissue structures,growth patterns,invasive behaviors,immunophenotypes,molecular mutations,and surgical *** these reasons,this expert consensus provides nine recommendations as a reference for standardizing and refining the diagnosis of pathological subtypes of iCCA,mainly based on the 5th edition of the World Health Organization Classification of Tumours of the Digestive System.

目的研究结缔组织生长因子（CTGF）小干扰RNA（siRNA）在CCl4诱导的大鼠肝纤维化模型中的抗肝纤维化作用及其对细胞外基质积聚的影响。方法雄性SD大鼠30只，随机分成5组。模型组：皮下注射CCl4及经门静脉注射等渗盐水，3d1次，连续6周；预防组：皮下注射CCh及经门静脉注射CTGFsiRNA，连续6周；治疗组：皮下注射CCl4 2周，随后给予CTGFsiRNA及CCl4 4周；对照siRNA干预组：皮下注射CCla及经门静脉注射对照siRNA，连续6周；空白对照组：经门静脉注射等渗盐水6周。用HE和SiNusred染色评估大鼠肝组织学变化，RTPCR或（和）Westernblot检测肝组织CTGF、Ⅰ与Ⅲ型胶原及层黏连蛋白的表达，放射免疫法检测外周血Ⅲ型前胶原及透明质酸含量。结果模型组及对照siRNA组大鼠肝组织CTGF、Ⅰ与Ⅲ型胶原及层黏连蛋白基因表达显著上调；与模型组相比，预防组及治疗组由CCl4诱导的CTGFmRNA和蛋白表达及Ⅰ与Ⅲ型胶原、层黏连蛋白mRNA表达分别下调76%±8％、95％±2％、74%±8％、78％±8％、31％±7％和80％±3％、93％±3％、57％±6%、59％±10％、43％±9％（F值分别为68．630，21．234，24．219，16．315和9．716，P值均〈0．01），肝纤维化程度明显减轻，大鼠外周血Ⅲ型前胶原和透明质酸含量也显著降低。结论经门静脉注射CTGF siRNA能显著抑制实验大鼠肝CTGF基因表达，由此通过阻止细胞外基质积聚而缓解肝纤维化。

目的研究化学合成的小干扰RNA(483 siRNA)对大鼠原代肝星状细胞(HSCs)结缔组织生长因子(CTGF)蛋白表达及细胞生物学特性的影响。方法以胶原酶原位灌注消化、密度梯度离心分离大鼠原代HSCs,以脂质体Oligofectamine包裹483siRNA转染培养72 h的原代HSCs作为转染组,并设空白对照组。收集孵育96 h的HSCs及培养上清液,采用Western blotting、免疫细胞化学染色、MTT和RT-PCR法分别检测细胞CTGF蛋白表达、α-SMA表达、细胞增殖和Ⅰ、Ⅲ型胶原mRNA表达;放射免疫法检测培养上清液中透明质酸及Ⅲ型前胶原含量。结果与对照组比较,转染组HSCs的CTGF蛋白表达下调(92±5)%,α-SMA染色阳性细胞减少(58±6)%,细胞增殖活性下调(18±5)%,Ⅰ、Ⅲ型胶原mRNA表达分别下调(53±6)%和(41±7)%;培养上清中透明质酸和Ⅲ型前胶原含量分别降低(48±5)%和(33±8)%。结论483 siRNA能显著下调原代HSCsCTGF蛋白表达,并由此抑制细胞活化、增殖及细胞外基质的合成和分泌。提示针对CTGF靶位的siRNA可能具有防治肝纤维化的潜力。

[目的]通过有限元分析,研究动态锁定螺钉治疗胫骨远端骨折的力学变化。[方法]选取1例男性志愿者采集下肢CT影像数据,采用Mimics 14.0和Geomagic Studio10.0软件建立胫骨三维模型,Cero3.0软件在胫骨远端干骺端横形切除2mm宽的骨质,建立胫骨远端骨折模型,并参考临床常用的各种内固定钢板的参数,分别建立动力加压钢板(dynamic compression plate,DCP)、锁定加压钢板(locking compression plate,LCP)和动态锁定螺钉(dynamic locking screws,DLS)的三维有限元模型。采用Hypermesh13.0和Abaqus6.14软件加载分析骨折端的位移、应力及应力遮挡率。[结果]轴向载荷及弯曲载荷下,骨折端位移量由大到小依次为DCP模型>DLS模型>LCP模型,组间比较差异均有统计学意义(PDLS模型>LCP模型,差异均有统计学意义(P<0.05)。轴向载荷下,应力遮挡率依次为:DCP模型93.47%,LCP模型71.86%,DLS模型42.91%。[结论]对于治疗胫骨远端骨折,DLS能够为骨折愈合提供更为良好的力学环境。

目的探讨影响去势抵抗性前列腺癌(CRPC)预后的因素。方法收集2015年12月1日—2017年11月30日在天津中医药大学第一附属医院、天津医科大学肿瘤医院、天津医科大学总医院就诊符合纳入标准的CRPC患者。对18项纳入因素进行可靠性分析及效度分析;用Kaplan-Meier法进行单因素分析,筛选CRPC预后影响因素,绘制生存曲线;将单因素分析有意义的因素纳入COX比例风险模型,进行多因素分析,判断预后独立危险因素。结果确诊CRPC时存在血尿症状、骨痛症状、血红蛋白(HGB)5.18 mmol/L、总前列腺特异性抗原(TPSA)>10 ng/mL、游离前列腺特异性抗原与总前列腺特异性抗原的比值(f/tPSA)>0.19、气血两虚证、未进行中药联合治疗为CRPC患者总生存时间(OS)的危险因素;CHO>5.18 mmol/L为CRPC患者OS的独立危险因素。CHO>5.18 mmol/L、HGB5.18mmol/L是CRPC患者OS的独立危险因素。
