The authors report a severe form of genitoperineal localisation of Verneuil’s disease managed in a surgical environment, having evolved favorably with improvement of the patient’s quality of life, but persistence of unsightly skin scars. They insist on the unexceptional nature of this disease whose prognosis is reserved, sometimes leaving severe functional sequelae. The long evolution of the disease with prolonged hospitalization in a professionally active young patient and the extension of suppurations to the perineal, scrotal and inguinal regions was the features of this observation. After wide excision of the lesions with antibiotic associated with local care, evolution had taken to complete wound healing with improved quality of life and a recovery of his professional activity.

Introduction: Percutaneous renal biopsy (PRB) is an essential procedure for the diagnosis and therapeutic management of many primary or secondary nephropathies. Objectives: To identify the indications, to determine the profile of the diagnosed nephropathies and to evaluate the short-term complications related to the practice of echo-guided PRB at the Martigues hospital center. Methodology: This was a retrospective and descriptive study carried out on the records of patients who underwent echo-guided native kidney biopsy from January 1, 2010 to December 31, 2019 in the nephrology department of the Martigues Hospital. Results: The analysis of 123 cases of echo-guided PRB involved 76 men and 47 women with a sex ratio of 1.6. The mean age was 55.92 ± 17.80 with age extremes of 16 and 87 years. Glomerular syndromes were the main indication with 42 cases of nephrotic syndrome (34.1%), 15 cases of nephritic syndrome (12.2%), 11 cases of rapidly progressive glomerulonephritis syndrome (8.9%), and 6 cases of recurrent macroscopic hematuria syndrome (4.9%). The histological findings were 47 cases of primary glomerular lesions (38.3%), 32 cases of nephroangiosclerosis lesions (26%), 24 cases of secondary glomerulopathy (19.5%), 9 cases of interstitial nephritis (7.3%), 2 cases of myelomatous nephropathy (1.6%), and 9 cases (7.3%) of unclassified histological lesions. Twenty-two hypertensive patients (40.7%) had nephroangiosclerosis lesions (p = 0.001). The follow-up was simple in 119 patients (96.7%). Macroscopic hematuria was noted in 4 patients (3.3%). It was associated with a perirenal hematoma in 2 patients including 1 transfused case. Conclusion: Our data provide an important contribution to the understanding of the prevalence and clinical presentation of renal diseases in the nephrology department of the Martigues hospital center.

目的分析社会隔离量表(Social Isolation Scale,SIS)在葡萄牙社区老年人中应用的信度和效度。方法选取250名社区老年人(≥65岁)进行横向描述性调查研究。调查工具包括社会人口学问卷、SIS、老年抑郁量表-4 (Geriatric Depression Scale-4,GDS-4)、6项卢本社会网络量表(6-item Lubben Social Network Scale,LSNS-6)、孤独自我评估问题和生活满意度量表(Satisfaction with Life Scale,SWLS)。分析结构效度(验证性因素分析)和收敛效度,评估ISI内部信度(综合信度)、外部信度(重测、组内相关系数)和评分者间信度(Cohen's kappa系数)。结果验证性因素分析显示双因素模型具有良好的拟合指数。SIS得分与LSNS-6(r=0.47)、SWLS(r=0.26)、孤独自我评估(r=0.35)和GDS-4(r=-0.16)得分相关。SIS组合信度良好(0.708)。组内相关系数0.84～0.98。Cohen's kappa系数为0.936～1。结论 SIS可作为评估葡萄牙社区老年人社会隔离的有效和可靠工具。医疗保健专业人员,特别是在社区工作的护士,可使用SIS评估老年人的社会隔离状况,以设计、实施和评估干预措施。

Inter individual differences in the metabolism of antimalarials could be due to polymorphism of NAT2 gene. The authors determined the genotypic frequencies of single nucleotide polymorphism （SNP） of NAT2 gene and it＇s implication in antimalarial treatment during a vitamin A and zinc supplementation intervention in children aged 6 to 24 months. Children were deparasitized with artesunate-amodiaquine （ASAQ）-toddler 50/135 mg. Pharmacovigilance was done for 40 days, adverse events recorded and blood was spotted on filter paper for DNA extraction by chelex method. PCR-RFLP was performed with restriction enzymes KpnI, TaqI, and BamHl for detection of SNPs of NAT2. Allelic frequencies and phenotypes were compared between participants with or without adverse drug events. The prevalence of fast, slow and intermediate acetylators was 55%, 30% and 11% respectively. There was a significant association （P = 0.035） between NAT2 slow acetylators （and susceptibility to develop skin rash. No significant difference was observed between fast and slow acetylators and susceptibility to develop fever, anorexia, cough and common cold. Slow acetylators were more susceptible, （P = 0.011） to develop any adverse event The NAT2 slow acetylator phenotype was the most predominant and individuals with this phenotype were more significantly susceptible to develop adverse events to ASAQ.

Acute Urinary Retention among Adult Men at Bobo-Dioulasso University Teaching Hospital: Epidemiology, Aetiologies and Initial Management

We conducted a cross-sectional study between February 1st, 2012 and September 30, 2012 at Bobo-Dioulasso University Teaching hospital. The target population was all patients seen at the emergency services for acute urinary retention. Among the 155 patients admitted for urological emergencies, 104 (67.1%) had acute urinary retention. The average age of patients was 65 years, ranging from 23 to 89 years and the majority was more than 60 years old (77.8%) and lived in rural areas (64.4%). Prostate tumor pathology and urethral stricture were the most frequent diagnosis, and the renal function was impaired in 33.7% of cases. Urethrovesical drainage, cystocatheterism, and suprapubic cystostomy were the treatment approach in 56.0%, 28.0% and 15.2% of the cases. Acute urinary retention is the most common urological emergency and many complications are associated with urethrovesical sounding. These complications should therefore be prevented by improving acute urinary care.

Background: The purpose of this study was to assess the efficacy, side effects and cost of misoprostol regimens in various obstetrical situations frequently occurring in deve-loping countries. Study design: One hundred and four parturient women with indications for labor induction received different regimens of misoprostol in the range of 50-800 μg according to their gestational age. Misoprostol was administered by the vaginal route, every 6 h without exceeding four doses. Results: All indications for labor induction concerned women with a gestational age of more than 30 weeks, except in the intrauterine death cases. The mean overall duration of labor was 7.8 h (±4.6 h). The mean amount of misoprostol used was 226 μg (±196 μg). The difference in the mean labor duration between the four indications for induction was statistically significant (P < 0.01). It was also significant for the mean total dose of misoprostol used. Total dose of misoprostol and Bishop score were inversely proportional. Two caesarean deliveries and two uterine ruptures were recorded, but no maternal deaths. The mean Apgar score was 8.0 (±1) at 1 min and 9.5 (±0.8) at 5 min. In our series, four fetal deaths occurred. The mean cost of misoprostol for labor induction was around US＄1, with a range of ＄0.05-4. Conclusions: The use of vaginal misoprostol appears to be relevant in developing countries in cases where labor induction is indicated. Nevertheless, the advantages of misoprostol (low cost, facility of storage) are counter-balanced by side-effects (C-section, uterine rupture) which can be harmful for the mother and also for the newborn.
