Use of the tumor necrosis factor-blockers for Crohn's disease

The use of anti-tumor necrosis factor-therapy for inflammatory bowel disease represents the most important advance in the care of these patients since the publication of the National Co-operative Crohn's disease study thirty years *** recommendations of numerous consensus groups worldwide are now supported by a wealth of clinical trials and several *** general,it is suggested that tumor necrosis factor-blockers(TNFBs) are indicated(1) for persons with moderately-severe Crohn's disease or ulcerative colitis(UC) who have failed two or more causes of glucocorticosteroids and an acceptably long cause(8 wk to 12 wk) of an immune modulator such as azathioprine or methotrexate;(2) non-responsive perianal disease;and(3) severe UC not responding to a 3-d to 5-d course of *** TNFBs have been introduced and the patient is responsive,therapy given by the IV and SC rate must be *** remains open to definitive evidence if concomitant immune modulators are required with TNFB maintenance therapy,and when or if TNFB may be weaned and *** supportive evidence from a single study on the role of early versus later introduction of TNFB in the course of a patient's illness needs to be *** risk/benefit profile of TNFB appears to be acceptable as long as the patient is immunized and tested for tuberculosis and viral hepatitis before the initiation of TNFB,and as long as the long-term adverse effects on the development of lymphoma and other tumors do not prone to be *** the rates of benefits to TNFB are modest from a population perspective and the cost of therapy is very high,the ultimate application of use of TNFBs will likely be established by cost/benefit studies.

Celiac disease(CD) is one of the most common diseases,resulting from both environmental(gluten) and genetic factors [human leukocyte antigen(HLA) and nonHLA genes].The prevalence of CD has been estimated to approximate 0.5%-1% in different parts of the ***,the population with diabetes,autoimmune disorder or relatives of CD individuals have even higher risk for the development of CD,at least in part,because of shared HLA *** gains access to the basal surface of the epithelium,and interact directly with the immune system,via both trans-and para-cellular *** a diagnostic perspective,symptoms may be viewed as either "typical" or "atypical".In both positive serological screening results suggestive of CD,should lead to small bowel biopsy followed by a favourable clinical and serological response to the gluten-free diet(GFD) to confirm the *** anti-tissue transglutaminase antibody or antiendomysial antibody during the clinical course helps to confirm the diagnosis of CD because of their over 99% specificities when small bowel villous atrophy is present on ***,the only treatment available for CD individuals is a strict life-long GFD.A greater understanding of the pathogenesis of CD allows alternative future CD treatments to hydrolyse toxic gliadin peptide,prevent toxic gliadin peptide absorption,blockage of selective deamidation of specific glutamine residues by tissue,restore immune tolerance towards gluten,modulation of immune response to dietary gliadin,and restoration of intestinal architecture.

In Vivo Phosphorylation Site Mapping and Functional Characterization of Arabidopsis Phototropin 1

Phototropins （phot1 and phot2） are blue-light receptor kinases controlling a range of responses that optimize the photosynthetic efficiency of plants. Light sensing is mediated by two flavin-binding motifs, known as LOVl and LOV2, located within the N-terminal region of the protein. Photoexcitation via LOV2 leads to activation of the C-terminal kinase domain and consequently receptor autophosphorylation. However, knowledge of the in-vivo phosphorylation sites for Arabidopsis phototropins is lacking and has impeded progress in elucidating the functional significance of receptor phos- phorylation. We have purified photl from Arabidopsis and identified the in-vivo sites of receptor phosphorylation by liquid chromatography tandem mass spectrometry. Arabidopsis-derived photl binds flavin mononucleotide as chromophore and is phosphorylated at four major sites located upstream of LOV2 （Ser^58, Ser^85, Ser^350, and Ser^410）, three of which are induced by blue light. Nevertheless, structure-function analysis indicates that the biological activity of photl can be attributed to a modular unit comprising the LOV2-kinase region of the protein. Thus, peptide regions upstream of LOV2, including the sites of receptor phosphorylation identified here, do not appear to be important for receptor signaling. By contrast, these regions may be necessary for maximizing stomatal performance and possibly light-induced relocalization of photl.

使用不同形状、不同磨粒尺寸的单层金刚石工具以螺旋钻孔方式加工多向碳纤维-环氧树脂层压板,比较各工具的加工性能。在复合材料上钻孔时,和传统钻削相比,螺旋钻孔是更先进和完善的方法。选择7种不同工具开展性能试验,研究金刚石磨粒尺寸、刀具槽口类型和末端几何形状对钻进力、温度、钻进缺陷和表面完整性的影响。结果发现高速旋转钻削可以显著降低钻削力。使用磨粒尺寸更小的工具进行加工,可以改善钻孔表面粗糙度,但是其钻削力更大、钻削温度更高。带有槽口的工具可以减轻工具表面堵塞。和平直末端或无槽口的工具相比,球形末端的工具性能更好。

Restricted nutrient intake does not alter serum-mediated measures of implant response in cell culture

Background： During nutritional stress, reduced intake may reduce the efficacy of anabolic implants. This study was conducted to evaluate basic cellular responses to a growth promotant implant at two intake levels. Methods： Sixteen crossbred steers （293 ± 19.3 kg） were used to evaluate the impact of anabolic implants in either an adequate or a restricted nutritional state. Steers were trained to individual Calan gates, and then randomly assigned to 1 of 4 treatments in a 2 × 2 factorial arrangement. Treatments consisted off presence or absence of an anabolic growth implant （Revalor-XS, 200 mg TBA and 40 mg estradiol; IMPLANT or CONTROL） and a moderate energy, pelleted, starting cattle diet fed at either 2.0 × or 1.0 × maintenance energy （NEM） requirements （HIGH or LOW）. Serum （d O, 14, and 28） was used for application to bovine muscle satellite cells. After treatment with the serum （20% of total media） from the trial cattle, the satellite cells were incubated for 72 h. Protein abundance of myosin heavy chain （MHC）, phosphorylated extracellular signal-related kinase （phospho-ERK）, and phosphorylated mammalian target of rapamycin （phospho-mTOR） were analyzed to determine the effects of implant, intake, and their interaction （applied via the serum）. Results： Intake had no effect on MHC （P = 0.85） but IMPLANT increased （P 〈 0.01） MHC abundance vs. CONTROL. Implant status, intake status, and the interaction had no effect on the abundance of phospho-ERK （P〉0.23）. Implanting increased phospho-mTOR （P 〈 0.01） but there was no effect （P 〉 0.51） of intake or intake x implant. Conclusions： The nearly complete lack of interaction between implant and nutritional status indicates that the signaling molecules measured herein respond to implants and nutritional status independently. Furthermore, results suggest that the muscle hypertrophic effects of anabolic implants may not be mediated by circulating IGF-1.

本试验,是确定自然传染钩端螺旋体病的生长猪,经肌肉注射一次链霉素,药量每公斤体重25毫克,对制止钩端螺旋体尿的效果。在试验开始时,检查了用于试验的54头生长猪,年龄10～12周,它们没有波摩那(pomona)或大赖索维(tarassovi)血清型钩体传染的血清学迹象。这些生长猪饲养在肥育圈舍中的围栏内,而其它围栏内的传染猪所排出的排泄物,流过生长猪的围栏。每头生长猪采集尿样,每周3次,直至20～24周龄被屠宰作感肉为止。试验用的生长猪,间接接触传染后,于16至74天之间,首先检出了钩端旋体尿。对有钩螺旋体尿的37头猪中,有21头注射链霉素。此后从21头治疗猪的185份尿样中,有11份检出钩端螺旋体(5.9%),并在屠宰时,检验了10头治疗猪,其中1头从肾脏培养出波摩那血清型钩体。检查出有钩端螺旋体尿的16头未治疗猪,在211份尿样中,有179份含有钩端螺旋体(84.8%),并从11头未经治疗的猪中,有4头从肾脏培养出钩端螺旋体。在自然传染的猪中,Ferguson等人的结论是,每吨饲料加入400克金霉素,饲喂10天,没有很好地制止钩端螺旋体尿。在实验性传染的猪中Stalheim发现,每吨饲料加入800克金霉素,饲喂14～21天,或肌肉注射一次双氢链霉素,药量每公斤体重25毫克,之后没有出现钩湍螺体尿,并且从屠宰猪的的肾脏中,没有分离出钩端螺旋体。实验性传染的钩端螺旋体病,其传染程度和病原毒力不可能比得上自然发病的。因而,在室外条件下,没有研究出抗菌素的有效治疗之前,不可能有把握地推荐使用抗菌素控制钩端螺旋体病的自然暴发。本文记载了一个商业猪场,在试验性自然传染钩端螺旋体病的生长猪中,肯定了链霉素制止钩端螺旋体尿的试验效果。

The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard Model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.

Resection of complex pancreatic injuries: Benchmarking postoperative complications using the Accordion classification

AIM To benchmark severity of complications using the Accordion Severity Grading System(ASGS) in patients undergoing operation for severe pancreatic injuries. METHODS A prospective institutional database of 461 patients with pancreatic injuries treated from 1990 to 2015 was reviewed. One hundred and thirty patients with AAST grade 3, 4 or 5 pancreatic injuries underwent resection(pancreatoduodenectomy, n = 20, distal pancreatectomy, n = 110), including 30 who had an initial damage controllaparotomy(DCL) and later definitive surgery. AAST injury grades, type of pancreatic resection, need for DCL and incidence and ASGS severity of complications were assessed. Uni-and multivariate logistic regression analysis was applied. RESULTS Overall 238 complications occurred in 95(73%) patients of which 73% were ASGS grades 3-6. Nineteen patients(14.6%) died. Patients more likely to have complications after pancreatic resection were older, had a revised trauma score(RTS) juries of the head and neck of the pancreas with associated vascular and duodenal injuries, required a DCL, received a larger blood transfusion, had a pancreatoduodenectomy(PD) and repeat laparotomies. Applying univariate logistic regression analysis, mechanism of injury, RTS < 7.8, shock on admission, DCL, increasing AAST grade and type of pancreatic resection were significant variables for complications. Multivariate logistic regression analysis however showed that only age and type of pancreatic resection(PD) were significant. CONCLUSION This ASGS-based study benchmarked postoperative morbidity after pancreatic resection for trauma. The detailed outcome analysis provided may serve as a reference for future institutional comparisons.
