Clinical analysis of patients with deep sternal wound infection-induced sepsis:a retrospective cohort study

Background:This study aimed to summarize the clinical characteristics of patients with deep sternal wound infection-induced sepsis after median sternotomy and improve the treatment outcomes of infection-related sepsis.Methods:A retrospective cohort study was conducted on 21 patients with deep sternal wound infection-induced sepsis after median sternotomy who were admitted to the Department of Critical Care.The clinical manifestations,laboratory test results,infection control,and organ and nutritional support of the patients were summarized,and the follow-up data were obtained.Results:The primary symptoms of deep sternal wound infection-induced sepsis included dyspnea,high fever,chills,and altered state of consciousness.Laboratory test results revealed increased inflammatory markers and decreased oxygenation index.Renal and liver function injury were observed in 8 and 4 patients,respectively;18 and 12 patients demonstrated elevated D-dimer and N-terminal Pro B type natriuretic peptide levels,respectively.Of the 8 patients whose wound secretions tested positive for bacteria,Acinetobacter baumannii and Staphylococcus aureus infections were present in 6 and 2 patients,respectively.One of the 6 patients whose blood cultures tested positive for bacteria demonstrated Candida albicans infection.Fifteen patients received ventilator-assisted ventilation and 2 patients received renal replacement therapy.Of all the 21 patients,17 were cured,2 died,and 2 were discharged.Conclusion:Postmedian sternotomy sepsis attributed to a deep sternal wound infection usually results from a preexisting condition.The most prominent clinical manifestation is dyspnea,which is sometimes accompanied by the impairment of organ function.Infection prevention,proper nutrition support,and maintenance of healthy organ function are the cornerstones for successful treatment outcomes.

The oxygen atom coordination inducing the structure reconstruction of the catalytic site is identified and recognized ambiguously,which is related to accurately declare the mechanism in a dynamic catalytic process.Herein,we demonstrated that the reconstructed catalytic sites would lead to a remarkable performance for photocatalytic CO_(2)reduction.At the initial 4-cycles testing,the in-situ formation of CoOx active sites on the Co(CoP)surface performed an increasing transient activity and selectivity toward CO evolution.The formation of reconstructive Co-O bond and the appearance of intermediate specie CO were simultaneously observed by the pre-operando Raman,revealing the dynamic relationship between catalytic site structure and the photocatalytic properties.Moreover,density functional theory calculations showed that the electronic structure of the reconstructive surface sites could modulate the ability of CO_(2)adsorption and CO desorption.The reduced barrier energy for the rate-determining step finally improved the activity and selectivity of CO_(2)reduction.

Optically-excited simultaneous photoacoustic and ultrasound imaging based on aflexible gold-PDMS film

We constructed a flexible gold-polydimethylsiloxane(gold-PDMS)nanocomposites film with controllable thickness and light transmittance,to realize optically-excited simultaneous photo-acoustic(PA)and ultrasound(US)imaging under a single laser pulse irradiation.Benefiting from the excellent thermoelastic properties,the gold-PDMS film absorbs part of the incident laser energy and produces a high-intensity US,which is used to realize US imaging.Meanwhile,the partly transmitted light is used to excite samples for PA imaging.By controlling the thickness of the gold-PDMS,we can control the center frequency in the US imaging.We experimentally analyzed the frequency of the produced US signal by the gold-PDMSfilm and compared it with the finite element analysis(FEA)method,where the experiments agree with the FEA results.This method is demonstrated by the experiments on phantoms and a mouse model.Our work provides a cost-effective methodology for simultaneous PA and US imaging.

A properly designed public transport system is expected to improve traffic efficiency.A high-frequency bus service would decrease the waiting time for passengers,but the interaction between buses and cars might result in more serious congestion.On the other hand,a low-frequency bus service would increase the waiting time for passengers and would not reduce the use of private cars.It is important to strike a balance between high and low frequencies in order to minimize the total delays for all road users.It is critical to formulate the impacts of bus frequency on congestion dynamics and mode choices.However,as far as the authors know,most proposed bus frequency optimization formulations are based on static demand and the Bureau of Public Roads function,and do not properly consider the congestion dynamics and their impacts on mode choices.To fill this gap,this paper proposes a bi-level optimization model.A three-dimensional Macroscopic Fundamental Diagram based modeling approach is developed to capture the bi-modal congestion dynamics.A variational inequality model for the user equilibrium in mode choices is presented and solved using a double projection algorithm.A surrogate model-based algorithm is used to solve the bi-level programming problem.

针对传统大马力拖拉机电液提升系统控制精度不足的问题,提出利用多个反馈参数进行综合调节的控制方案,通过对不同的反馈控制方式进行权重划分,可综合各反馈控制的优势以实现多性能平衡的目的。另外,考虑到常规控制的权重因子固定不可变的问题,提出权重因子基于反馈信号的自适应变化控制方案,利用3种反馈信号与设定值的偏差进行权重因子的增量调节,使得权重因子能更好地满足性能参数的变动,分别设计了3个权重因子的自适应控制规则和对应控制器的输入输出参数。选用犁耕深度、牵引力、滑转率作为控制性能的评价指标对无反馈控制、力位综合控制以及所提自适应控制进行对比,结果表明:所提控制方法的犁耕深度均匀性较高,牵引力和滑转率数值较小,且变化范围明显小于其他2种控制方式;在提高犁耕均匀性的同时也提高了燃油经济性,并降低了悬挂系统的冲击效应。

Design and experiment of seed furrow cleaning device based on throwing and sliding for no-till maize seeding

In order to solve the serious problems of seeds are covered by residual film and overhead by straw during no-till seeding,a seed furrow cleaning device for no-till maize seeding was developed,which adopted a collaborative cleaning method of rotating spring teeth and curved sliding shovel.The movement process and motion trajectory of throwing residual film and straw were constructed.The maximum distance of throwing to one side in horizontal and maximum height in vertical were obtained.The motion trajectory of adjacent spring teeth was analyzed by Matlab,the motion trajectories of adjacent spring teeth at different speeds of 120 r/min,150 r/min and 180 r/min were achieved,the theoretical analysis results showed that the area of omitted area decreased with the increase of rotation speed.Based on theoretical and simulation analysis of critical parameters,the forward speed of machine,rotation speed of spring teeth,and dip angle between spring teeth and rotary disc were selected as the influencing factor.Straw cleaning rate(SCR)and residual film cleaning rate(RFCR)were selected as the response values for three factors and three levels of orthogonal experiment design.The optimal combination of the selected parameters was obtained,and the field test verification was also conducted.The results showed that the rotation speed of spring teeth,forward speed and dip angle of spring teeth significantly affect SCR and RFCR were in decreasing order.The field test results indicated that when forward speed was 6 km/h,rotation speed of spring teeth was 180 r/min and dip angle of spring teeth was 40°,SCR and RFCR were 88.27%and 84.31%,respectively.This study provides a reference for the development of no-till seeder in Xinjiang and the northwestern regions of China.

Photosensitized heterogeneous CO_(2) reduction(PHCR)has emerged as a promising means to convert CO_(2) into valuable chemicals,however,challenged by the relatively low carbonaceous product selectivity caused by the competing hydrogen evolution reaction(HER).Here,we report a PHCR system that couples Ru(bpy)32+photosensitizer with{001}faceted LiCoO_(2) nanosheets photocatalyst to simultaneously yield 21.2 and 722μmol·g^(-1)·h^(-1) of CO,and 4.42 and 108μmol·g^(-1)·h^(-1) of CH4 under the visible light and the simulated sunlight irradiations,respectively,with completely suppressed HER.The experimental and theoretical studies reveal that the favored CO_(2) adsorption on the exposed Li sites on{001}faceted LiCoO_(2) surface is responsible for the completely suppressed HER.

Advances in novel drugs,therapies,and genetic techniques have revolutionized the diagnosis and treatment of cancers,substantially improving cancer patients’prognosis.Although rare tumors account for a non-negligible number,the practice of precision medicine and development of novel therapies are largely hampered by many obstacles.Their low incidence and drastic regional disparities result in the difficulty of informative evidence-based diagnosis and subtyping.Sample exhaustion due to difficulty in diagnosis also leads to a lack of recommended therapeutic strategies in clinical guidelines,insufficient biomarkers for prognosis/efficacy,and inability to identify potential novel therapies in clinical trials.Herein,by reviewing the epidemiological data of Chinese solid tumors and publications defining rare tumors in other areas,we proposed a definition of rare tumor in China,including 515 tumor types with incidences of less than 2.5/100000 per year.We also summarized the current diagnosis process,treatment recommendations,and global developmental progress of targeted drugs and immunotherapy agents on the status quo.Lastly,we pinpointed the current recommendation chance for patients with rare tumors to be involved in a clinical trial by NCCN.With this informative report,we aimed to raise awareness on the importance of rare tumor investigations and guarantee a bright future for rare tumor patients.

基层医疗机构是我国医疗卫生服务体系的主力军,是国家卫生保健系统的基石。在COVID-19疫情期间,我国基层医疗机构发挥了巨大作用,但同时也凸显出许多短板。本文通过系统文献检索与分析,发现我国基层医疗机构存在患者就医需求不高、卫生系统整合性不强、服务基础薄弱、突发公共卫生事件应急能力不足等问题,据此提出基于“三力理论”的基层医疗机构建设策略,以期为完善基层医疗卫生服务体系、提升基层医疗卫生服务能力、更好地完成“健康中国2030”战略目标提供参考。

For sessile plants, gene expression plays a pivotal role in responding to salinity stress by activating or suppressing specific genes. However, our knowledge of genetic variations governing gene expression in response to salt stress remains limited in natural germplasm. Through transcriptome analysis of the Global Mini-Core Rice Collection consisting of a panel of 202 accessions, we identified 22 345 and 27 610expression quantitative trait loci associated with the expression of 7787 and 9361 e Genes under normal and salt-stress conditions, respectively, leveraging the super pan-genome map. Notably, combined with genome-wide association studies, we swiftly pinpointed the potential candidate gene STG5—a major salt-tolerant locus known as q STS5. Intriguingly, STG5 is required for maintaining Na+/K+homeostasis by directly regulating the transcription of multiple members of the Os HKT gene family. Our study sheds light on how genetic variants influence the dynamic changes in gene expression responding to salinity stress and provides a valuable resource for the mining of salt-tolerant genes in the future.
