Audits of death in HIV-Infected Children and Adolescents Followed up in the Pediatric department of the Regional Teaching Hospital of Borgou/Alibori from 2005 to 2020

Introduction: Human immunodeficiency virus (HIV) is a major public health problem with high morbidity and mortality among children. The objective of this work was to audit the deaths of children and adolescents with HIV infection followed up in the pediatric department of the Regional Teaching Hospital of Borgou/Alibori (CHUdB/A) the from 2005 to 2020. Patients and Method: This was a retrospective and descriptive study conducted in the pediatric department of CHUd/B-A in Parakou. All children with HIV infection who died from January 1, 2005 to August 31, 2020 were included. data collection was carried out in three stages: a phase of medical records processing, a phase of community survey and a phase of death audits. The variables studied were sociodemographic, clinical, biological, therapeutic and evolutionary. Results: Over the study period, the data of 464 infected children were recorded, including 92 deaths, representing a case fatality rate of 19.83%. severe acute malnutrition (69.23%), gastro-intestinal tract infections (43.58%) and serious opportunistic pulmonary infections (24.36% pulmonary tuberculosis and 19.23% pneumocystis) were the main causes of death. The main dysfunctions found were: the delayed diagnosis of HIV infection (79.35%), the absence or delay in consultation when the child’s clinical condition deteriorates (32.61% and 47.83%), delayed initiation of antiretroviral treatment (42.39%) and non-adherence to treatment (38.04%). Non-adherence to treatment was predominant in adolescents (90.49%). Conclusion: specific interventions for early detection, adequate nutritional care, psychosocial support for adolescents and mothers of children are necessary to reduce mortality due to HIV among children and adolescents.

In this editorial,we discuss the article by Peng et al in the recent issue of the World Journal of Gastrointestinal surgery,focusing on the evolving role of endoscopicultrasound-guided biliary drainage(EUs-Bd)with electrocautery lumen apposing metal stent(LAMs)for distal malignant biliary *** endoscopy has rapidly advanced in decompression techniques,with growing evidence of its safety and efficacy surpassing percutaneous and surgical *** endoscopic retrograde cholangiopancreatography(ERCP)has been the gold standard for biliary decompression,its failure rate approaches 10.0%,prompting the exploration of alternatives like *** random-effects meta-analysis demonstrated high technical and clinical success of over 90.0% and an adverse event rate of 17.5%,mainly in the form of stent *** based on stent size were not reported but the majority used 6 mm and 8 mm *** the body of literature continues to demonstrate the effectiveness of this technique,the challenges of stent dysfunction need to be addressed in future *** strategy that has shown promise is placement of double-pigtail stents,only 18% received the prophylactic intervention in this *** expect this to improve with time as the technique continues to be refined and *** results above establish EUs-Bd with LAMs as a reliable alternative after failed ERCP and considering EUs to ERCP upfront in the same session is an effective *** the promising results,studies must explore the role of EUs-Bd as first-line therapy for biliary decompression.

Prospective randomized study to evaluate and compare the post-procedural sexual function in patients undergoing semi-rigid ureterorenoscopy for distal ureteric stones in three different operating room settings

Water stress is a critical environmental adversity that significantly impacts the growth, development, and yield of flax plants. In this study, flax seeds were cultivated under different water irrigation requirements (WIR) (100%, 75%, and 50%) to investigate the effects of exogenously supplied nitric oxide (NO) donor sodium nitroprusside (sNP) as foliar treatments at concentrations of 0.0 mmol/L, 0.5 mmol/L, 1.0 mmol/L, and 2.0 mmol/L. drought stress led to a significant decrease in plant growth, photosynthetic pigments, yield components such as oil and total carbohydrate percentage. It also resulted in an increase in leaf H2O2 production, lipid peroxidation levels and activities of enzymatic antioxidants including polyphenol oxidase, superoxide dismutase, and nitrate reductase enzymes. However, foliar application of sNP improved photosynthetic pigments and antioxidant defense system which mitigated the negative impact of water stress on growth and yield productivity by reducing oxidative damage caused by reactive oxygen species accumulation. The use of sNP also decreased H2O2 accumulation levels, lipid peroxidation levels, and improved membrane stability. sNP treatment at concentration of 2 mmol/L showed superior results compared to other concentrations with extremely significant increases observed in yield characteristics such as oil content, total carbohydrate percentages, and unsaturated fatty acids to saturated fatty acids ratio. © 2024 the Oil Crop Research Institute, Chinese Academy of Agriculture sciences

An Improved Image steganography security and Capacity Using Ant Colony Algorithm Optimization

This advanced paper presents a new approach to improving image steganography using the Ant Colony Optimization (ACO) algorithm. Image steganography, a technique of embedding hidden information in digital photographs, should ideally achieve the dual purposes of maximum data hiding and maintenance of the integrity of the cover media so that it is least suspect. The contemporary methods of steganography are at best a compromise between these two. In this paper, we present our approach, entitled Ant Colony Optimization (ACO)-Least significant Bit (LsB), which attempts to optimize the capacity in steganographic embedding. The approach makes use of a grayscale cover image to hide the confidential data with an additional bit pair per byte, both for integrity verification and the file checksum of the secret data. This approach encodes confidential information into four pairs of bits and embeds it within uncompressed grayscale images. The ACO algorithm uses adaptive exploration to select some pixels, maximizing the capacity of data embedding while minimizing the degradation of visual quality. Pheromone evaporation is introduced through iterations to avoid stagnation in solution refinement. The levels of pheromone are modified to reinforce successful pixel choices. Experimental results obtained through the ACO-LsB method reveal that it clearly improves image steganography capabilities by providing an increase of up to 30% in the embedding capacity compared with traditional approaches; the average Peak signal to Noise Ratio (PsNR) is 40.5 dB with a structural Index similarity (ssIM) of 0.98. The approach also demonstrates very high resistance to detection, cutting down the rate by 20%. Implemented in MATLAB R2023a, the model was tested against one thousand publicly available grayscale images, thus providing robust evidence of its effectiveness. Copyright © 2024 The Authors.
