BACkGROUNd Pelvic fractures(PF)with concomitant injuries are on the rise due to an increase of high-energy *** of the elderly population with age related comorbidities further complicates the *** organ injuries are kindred with PF due to the proximity to pelvic *** of contrast blush(CB)on computed tomography in patients with PF is considered a sign of active bleeding,however,its clinical significance and association with outcomes is *** To analyze polytrauma patients with PF with a focus on the geriatric population,co-injuries and the value of contrast *** This retrospective cohort study included 558 patients with PF admitted to level 1 trauma center(01/2017-01/2023).Analyzed variables included:Age,sex,mechanism of injury(MOI),injury severity score(ISS),Glasgow coma scale(GCS),abbreviated injury scale(AIS),co-injuries,transfusion requirements,pelvic angiography,embolization,laparotomy,orthopedic pelvic surgery,intensive care unit and hospital lengths of stay,discharge disposition and *** study compared geriatric and non-geriatric patients,patients with and without CB and abdominal *** score matching was implemented in comparison *** PF comprised 4%of all trauma admissions.89 patients had CB.286(52%)patients had concomitant injuries including 93(17%)patients with abdominal *** patients compared to non-geriatric had more falls as MOI,lower ISS and AIS pelvis,higher GCS,less abdominal co-injuries,similar CB and angio-embolization rates,less orthopedic pelvic surgeries,shorter lengths of stay and higher *** propensity matching,orthopedic pelvic surgery rates remained lower(8%vs 19%,Pd mortality higher(13%vs 4%,Pd to without CB had more pelvic angiography(71%vs 12%,P<0.001),hi

Microscale shaping and rounding of ridge arrays and star pattern features on nickel mould via electrochemical polishing

High quality micro mould tools are critical for ensuring defect-free production of micro injection moulded *** demoulding stage of the micro injection moulding can adversely affect the surface integrity due to friction,adhesion and thermal stresses between the metallic mould and polymeric replicated *** the present work,we propose the use of precision electropolishing(EP)as a shaping and polishing process to control the draft angle and fillet radius of micro features in order to ease *** defects that occur in replicated polymer parts include cracks,burrs and distorted features.A nickel mould having multiple linear ridges and star shape patterns was designed for the present investigation to have characteristic dimensions ranging from 10μm to 150μm and with various aspect ratios to study the effect of electropolishing on modifying the shape of micro features and surface morphology.A transient 2d computational analysis has been conducted to anticipate the effect of shaping on the Ni mould after electrochemical polishing with non-uniform material removal rates,based on the distribution of current *** experimental results indicate that after shaping using EP,the draft angle of star-patterns and linear patterns can be effectively increased by approximately 3.6°,while the fillet radius increases by up to 5.0μ*** controlling the electropolishing process,the surface roughness can be maintained under 50 *** work uses a green and environmental friendly nickel sulfamate electrolyte which can be effective for shaping of nickel micro features without causing any surface deposition.

Generalized polynomial chaos expansion by reanalysis using static condensation based on substructuring

This paper presents a new computational method for forward uncertainty quantification(UQ)analyses on large-scale structural systems in the presence of arbitrary and dependent random *** method consists of a generalized polynomial chaos expansion(GPCE)for statistical moment and reliability analyses associated with the stochastic output and a static reanalysis method to generate the input-output data *** the reanalysis,we employ substructuring for a structure to isolate its local regions that vary due to random *** allows for avoiding repeated computations of invariant substructures while generating the input-output data *** substructuring with static condensation further improves the computational efficiency of the reanalysis without losing ***,the GPCE with the static reanalysis method can achieve significant computational saving,thus mitigating the curse of dimensionality to some degree for UQ under high-dimensional *** numerical results obtained from a simple structure indicate that the proposed method for UQ produces accurate solutions more efficiently than the GPCE using full finite element analyses(FEAs).We also demonstrate the efficiency and scalability of the proposed method by executing UQ for a large-scale wing-box structure under ten-dimensional(all-dependent)random inputs.

Multiple sclerosis(MS)is an autoimmune disorder of the central nervous system(CNS)and is primarily characterized by immune cell infiltration leading to relapses followed by remission phases and a disease course turning progressive over time with neurodegenerative processes taking over(Amin and Hersh,2023).Of note,beyond relapse-associated worsening early in disease progression independent of relapse activity may arise independently of relapse activity and can occur in all ***-mediated damage of myelin sheaths and the subsequent loss of mature oligodendrocytes are resulting in impaired axonal integrity,neurodegeneration and accounts for irreversible neuronal damage(kuhlmann et al.,2023).The current landscape of available disease-modifying therapies comprises mainly immunomodulatory drugs that effectively diminish relapses and slow down progression at the onset form of the disease,namely relapsing MS(RMS).In this regard,a number of drugs have been approved as disease-modifying therapies for MS by US Food and drug Administration and European Medicines Agencies(Box 1).

Ion heat transport in electron cyclotron resonance heated L-mode plasma on the T-10 tokamak

Anomalous ion heat transport is analyzed in the T-10 tokamak plasma heated with electron cyclotron resonance heating(ECRH) in second-harmonic extra-ordinary mode. Predictive modeling with empirical scaling for Ohmical heat conductivity shows that in ECRH plasmas the calculated ion temperature could be overestimated, so an increase of anomalous ion heat transport is required. To study this effect two scans are presented: over the EC resonance position and over the ECRH power. The EC resonance position varies from the high-field side to the low-field side by variation of the toroidal magnetic field. The scan over the heating power is presented with on-axis and mixed ECRH regimes. discharges with high anomalous ion heat transport are obtained in all considered regimes. In these discharges the power balance ion heat conductivity exceeds the neoclassical level by up to 10 times. The high ion heat transport regimes are distinguished by three parameters: the ratio Te/Ti, the normalized electron density gradient R/■, and the ion–ion collisionality νii~*. The combination of high Te/Ti, high νii~*, and R/■=6-10 results in values of normalized anomalous ion heat fluxes up to 10 times higher than in the low transport scenario.

Numerical calculation of Z-scan measurements for nonlinear media with large phase shift

We have reported the characteristics of a Z-scan for the poly(azaneylylidene-acylene)(dAZA) polymer, as nonlinear medium with a large nonlinear phase shift using continuous-wave(CW) laser beam. It has been verified that the Fresnel diffraction model is applicable for analyses of Z-scan measurements with dAZA polymer at high laser power. It was found that Z-scan curves with peak-to-valley features appear as the applied light intensity increases in the case of a large nonlinear phase shift. The Z-scan experiments were carried out using a CW laser to verify the theoretical calculations in the case of a large nonlinear phase shift model. Our results show good agreements between the experimental data and the proposed theoretical models.
