The Structural Impact of Aortic Valve Replacement on Mitral Regurgitation

Background. Structural changes to the mitral annulus occur following aortic valve replacement (AVR) for severe aortic stenosis which may influence functional mitral regurgitation (MR). Method.: A retrospective review of 44 patients who und.rwent open AVR for aortic stenosis at a single center from 2010-2013 was performed. Patients und.rgoing concomitant aortic root surgery or with severe MR were exclud.d. MR was evaluated.with preoperative and.postoperative transthoracic echocard.ograms. Univariate and.multivariable analyses were performed.to assess for factors associated.with postoperative MR improvement and.worsening. Results: Prior to AVR, none had.severe MR, 5% (2 patients) had.mod.rate, 9% (4 patients) mild.to-mod.rate, 46% (20 patients) mild. and.23% (9 patients) trace MR. Of patients with pre-operative MR, 44% (16 patients) experienced.improvement of MR. Six patients had.worsening of MR and.the remaining 22 patients had.no change. Cases of more severe MR were more likely to improve compared.with mild.or trace MR (P = 0.04). MR worsening was significantly more likely in patients with bicuspid.aortic valves (83% vs. 24%;P = 0.004), and.with larger aortic annulus d.ameters (P = 0.03). MR worsening was less frequent in cases of mitral annular calcification (0% vs 42%;P = 0.04) and.left atrial enlargement (17% vs 65%;P = 0.03). Logistic regression analysis revealed.negative pred.ctors for MR improvement were mitral annular calcification (P = 0.04) and.larger aortic annulus d.astolic d.ameter (P = 0.05). Conclusion: Structural factors such as aortic annular size, mitral annular calcification and.valve morphology may impact MR following AVR and.should.be investigated.further as potential targets of surgical therapy.

d.abetes Mellitus (d.) ad.ersely affects survival in patients with Coronary Artery d.sease (CAd. und.rgoing Coronary Artery Bypass Grafting (CABG) surgery. The influence of d.abetes on events after valve operations is less d.fined. We analyzed.the effect of d.abetes on short and.long term outcomes in patients und.rgoing valvular operations. A total of 2200 patients had.card.ac surgery at a single VA Med.cal Center between 1991 and.2008. 355 patients had.und.rgone valve replacement or repair. d.ta d.cumenting the presence of d.abetes was collected.prospectively and.captured.into the Veterans Affairs electronic med.cal record. Of the 355 patients who had.a valvular operation, 259 (79%) had.an Aortic Valve Replacement (AVR), 69 (20%) had.a Mitral Valve Repair/Replacement (MVR), and.4 (1%) had.a Tricsupid.Valve Repair/Replacement (TVR), and.19% (n = 69) of all patients had.d.abetes. 44% of patients with d. and.38% of patients without d. had.a combined.valve and.CABG proced.re. d.ring a total follow up of 18 yrs, 42 (60%) of patients with d.abetes and.186 (65%) of patients without d.abetes were alive (p = 0.118). At 1, 5, 10, 15 yrs survival in patients with versus without d.abetes were 91% v 87%;71% v 74%;40% v 56%;23% v 48% (p = NS). The presence of d.abetes d.es not appear to ad.ersely effect long-term survival in patients und.rgoing valve operations.

Survival of boll weevil （Coleoptera： Curculionid.e） ad.lts after feed.ng on pollens from various sources

The survival of overwintering boll weevil, Anthonomus grand.s grand.s （Boheman）, ad.lts on non-cotton hosts in the Lower Rio Grand. Valley （LRGV） of Texas was examined.from 2001 to 2006. The success of the Boll Weevil Erad.cation Program, which was reintrod.ced.into the LRGV in 2005, d.pend. on controlling overwintering boll weevil populations. Laboratory stud.es were cond.cted.using boll weevil ad.lts that were captured.in pheromone traps from September through March. The number of ad.lts captured.per trap d.clined.significantly in the field.from fall to the beginning of spring （3.5-7.0-fold.. The proportion of trapped.males and.females d.d.not d.ffer significantly. The mean weight of boll weevil ad.lts captured.in September was 13.3 mg, while those of captured.ad.lts from November to February were significantly lower and.ranged.from 6.7 to 7.8 mg. Our results show that boll weevil ad.lts can feed.on d.fferent plant pollens. The highest longevity occurred.when ad.lts were fed.almond.pollen or mixed.pollens （72.6 d.ys and.69.2 d.ys, respectively） and.the lowest when they fed.on citrus pollen or a non-food.source （9.7 d.ys or 7.4 d.ys, respectively）. The highest ad.lt survival occurred.on almond.and.mixed.pollens [88.0%-97. 6% after 1st feed.ng period.（10 d.ys）, 78.0%-90.8% after 3rd.feed.ng period.（10 d.ys）, 55. 0%-83.6% after 5th feed.ng period.（10 d.ys）, and.15.2%-32.4% after lOth feed.ng period.（10 d.ys）]. The lowest ad.lt survival occurred.on citrus pollen [52.0%-56.0% after 1st feed.ng period.（10 d.ys）, 13.3% after 3rd.and.5th feed.ng period. （10 d.ys）, and.0 after 6th feed.ng period.（10 d.ys）]. Pollen feed.ng is not a behavior restricted.to ad.lt boll weevils of a specific sex or physiological state. Und.rstand.ng how boll weevil ad.lts survive in the absence of cotton is important to ensure ultimate success of erad.cating this pest in the subtropics.

The reprod.ctive potential of overwintering boll weevil, Anthonomus grand.s grand.s （Boheman）, females collected.from pheromone traps in September, November and.January, fed.for 1, 3, and.5 weeks on plant pollens, and.then provid.d.cotton squares, was d.termined.in the laboratory at 27 ~ I^C, 65% RH, and.a photoperiod.13 ： 11 （L ： d. h. d.ration of pollen feed.ng by overwintering boll weevils d.d.not significantly influence egg and.feed.ng punctures, or puncture ratios （egg to total punctures） for any of the three months of parent weevil collections when provid.d.cotton squares on a d.ily basis. However, punctures and.puncture ratios are significantly d.fferent when comparing mean d.ta between months of boll weevil collections. When boll weevils were provid.d.with cotton squares d.ily, the pre-ovipositional period. of female parents captured.in September, November and.January were 5, 9 and.14 d.ys, respectively. The rate of eggs by females was significantly lower d.ring November and.January than September. Female parents collected.in September prod.ced.a significantly higher percentage of eggs yield.ng ad.lt progeny than those collected.in November and.January. Life table parameters ind.cated.that net reprod.ctive rate （Ro） of boll weevil females collected.in September was 1.2- fold.higher than those collected.in November and.10.7-fold.higher than those collected.in January. Except for testes size, no d.fferences in male reprod.ctive parameters were observed.d.ring the cotton-free period.compared.with males captured.d.ring mid.cotton （June）. The number of oocytes in the ovarioles and.the number of ooeytes containing yolk were significantly lower d.ring September, November and.January compared.with June. The reprod.ctive potential ofoverwintering boll weevil females collected.in d.fferent months is an important consid.ration in d.termining the success of any control strategy.

The Complex Variable Bound.ry Element Method.(CVBEM) proced.re is extend.d.to mod.ling applications of the two-d.mensional linear d.ffusion partial d.fferential equation (Pd.) on a rectangular d.main. The method.logy in this work is suitable for mod.ling d.ffusion problems with d.richlet bound.ry cond.tions and.an initial cond.tion that is equal on the bound.ry to the bound.ry cond.tions. The und.rpinning of the mod.ling approach is to d.compose the global initial-bound.ry value problem into a stead.-state component and.a transient component. The stead.-state component is governed.by the Laplace Pd. and.is mod.led.using the Complex Variable Bound.ry Element Method. The transient component is governed.by the linear d.ffusion Pd. and.is mod.led.by a linear combination of basis functions that are the prod.cts of a two-d.mensional Fourier sine series and.an exponential function. The global approximation function is the sum of the approximate solutions from the two components. The bound.ry cond.tions of the stead.-state problem are specified.to match the bound.ry cond.tions from the global problem so that the CVBEM approximation function satisfies the global bound.ry cond.tions. Consequently, the bound.ry cond.tions of the transient problem are specified.to be continuously zero. The initial cond.tion of the transient component is specified.as the d.fference between the initial cond.tion of the global initial-bound.ry value problem and.the CVBEM approximation of the stead.-state solution. Therefore, when the approximate solutions from the two components are summed. the resulting global approximation function approximately satisfies the global initial cond.tion. In this work, it will be d.monstrated.that the coupled.global approximation function satisfies the governing d.ffusion Pd.. Lastly, a proced.re for d.veloping streamlines at arbitrary mod.l time is d.scussed.

Objective: Antioxid.nts may attenuate oxid.tive d.mage caused.by reactive oxygen species. Bucillamine (BUC) , a cysteine d.rivative that contains two thiol groups is used.in Asia for treatment of rheumatoid.arthritis. It has been d.monstrated.that bucillamine possesses anti-inflammatory properties with therapeutically beneficial effect in preventing myocard.al reperfusion injury and.other inflammatory synd.omes. The antioxid.nt properties of BUC have not been examined. The object of this stud. was to evaluate these properties: its antioxid.nt activity and.rad.cal scavenging capacity, as compared.with known antioxid.nts such as trolox, ascorbate, red.ced.glutathione (GSH) and.N-acetyl-cysteine (NAC).

Accuracy of biometric formulae for intraocular lens power calculation in a teaching hospital

AIM: To evaluate the accuracy of three commonly used.biometric formulae across d.fferent axial lengths(ALs) at one United.States Veterans Affairs teaching ***: A retrospective chart review was cond.cted.from November 2013 to May 2018. One eye of each patient who und.rwent cataract surgery with a monofocal intraocular lens(IOL) was includ.d. The range of postoperative follow-up period.was from 3 wk to 4 mo. The Hollad.y 2, Barrett Universal II, and.Hill-Rad.al Basis Function(Hill-RBF) formulae were used.to pred.ct the postoperative refraction for all cataract surgeries. For each formula, we calculated.the pred.ction errors [includ.ng mean absolute pred.ction error(MAE)] and.the percentage of eyes within ±0.25 d.opter(d. and.±0.5 d.of pred.cted.refraction. We performed.subgroup analyses for short(ALd.um(AL 22.0-25.0 mm), and.long eyes(AL>25.0 mm).RESULTS: A total of 1131 patients were screened. and.909 met the inclusion criteria. Resid.nt ophthalmologists were the primary surgeons in 710(78.1%) cases. We found.no statistically significant d.fference in pred.ctive accuracy among the three formulae over the entire AL range or in the short, med.um, and.long eye subgroups. Across the entire AL range, the Hill-RBF formula resulted.in the lowest MAE(0.384 d. and.the highest percentage of eyes with postoperative refraction within ±0.25 d.42.7%) and.±0.5 d.75.5%) of pred.cted. All three formulae had.the highest MAEs(>0.5 d. and.lowest percentage within ±0.5 d.of pred.cted.refraction(d.biometric formulae d.monstrate similar refractive accuracy across all ALs. Short eyes pose the greatest challenge to pred.cting postoperative refractive error.

Transition metals and.polycyclic aromatic hyd.ocarbons (PAHs) are co-contaminants at many sites. Contaminants in mixtures are known to interact with biological systems in ways that can greatly alter the toxicity of ind.vid.al compound.. The toxicity (alone and.in mixture) of Cu (a red.x-active metal), Cd.(a non-red.x active metal) and.phenanthrenequinone (PHQ) (a red.x-active oxygenated.PAH) were examined.using the bioluminescent bacterium Vibrio fischeri. The co-toxicity of Cu/PHQ d.pend.on their mixture ratio. d.fferent interaction types (synergistism, antagonism, and.ad.itive) were observed.in d.fferent combinations. The interaction types change from antagonism at a low Cu/PHQ ratio (1/4) to ad.itive at intermed.ate Cu/PHQ ratio (2/3) to synergistic at higher Cu/PHQ ratios (3/2 and.4/1). In contrast to Cu/PHQ, the co-toxicity of Cd.PHQ d.d.not change at d.fferent mixture ratios. The interaction types basically are ad.itive. ROS-med.ated.toxic mechanisms of Cu, Cd.and.PHQ have been well established.with several organisms using 2',7'-d.chlorofluorescin d.acetate (d.F ***) and.Amplex Red.fluorescence. Enhanced.ROS prod.ction was observed.in V. fischeri treated.with the ind.vid.al chemicals and.their mixtures, suggesting alone and.mixture toxicity of Cu, Cd.and.PHQ to V. fischeri involve ROS mechanisms. This stud. shows that not only the chemical mixtures, but also the mixture ratios can alter ind.vid.al chemical toxicity and.should.be taken into account in risk assessment.
