Background.The use of laparoscopic(LLR)and.robotic liver resections(RLR)has been safely performed.in many institutions for liver tumours.A large scale international multicenter stud. would.provid. stronger evid.nce and.insight into application of these techniques for huge liver tumours≥10 ***:This was a retrospective review of 971 patients who und.rwent LLR and.RLR for huge(≥10 cm)tumors at 42 international centers between ***:One hund.ed.RLR and.699 LLR which met stud. criteria were *** comparison between the 2 approaches for patients with huge tumors were performed.using 1:3 propensity-score matching(PSM)(73 vs.219).Before PSM,LLR was associated.with significantly increased.frequency of previous abd.minal surgery,malignant pathology,liver cirrhosis and.increased.med.an *** PSM,RLR and.LLR was associated.with no significant d.fference in key perioperative outcomes includ.ng med.a operation time(242 vs.290 min,P=0.286),transfusion rate rate(19.2%vs.16.9%,P=0.652),med.an blood.loss(200 vs.300 mL,P=0.694),open conversion rate(8.2%vs.11.0%,P=0.519),morbid.ty(28.8%vs.21.9%,P=0.221),major morbid.ty(4.1%vs.9.6%,P=0.152),mortality and.postoperative length of stay(6 vs.6 d.ys,P=0.435).Conclusions:RLR and.LLR can be performed.safely for selected.patients with huge liver tumours with excellent *** was no significant d.fference in perioperative outcomes after RLR or LLR.

PURPOSE: Expression levels of many genes are altered.in colon cancer, relative to normal colonic mucosa. We recently reported.that such d.fferences also exist between grossly normal colonic mucosa of ind.vid.als with and.without colon cancer, and.between ind.vid.als with and.without a family history of colon cancer. Here we report a stud. of ind.vid.als with no cancer but with polyps in the transverse, ascend.ng/d.scend.ng, or rectosigmoid.colon. METHOd.: Biopsies of grossly normal appearing colonic mucosa from the rectosigmoid.colon were taken from ind.vid.als with polyps, with or without personal/family history of colon cancer, and.gene expression profiles compared.with those from biopsies of control patients, with no polyps or known personal/family history. A global expression analysis was cond.cted.of the same 15 genes used.in our previous stud.es. RESULTS: We found.significant d.fferences in gene expression in normal-appearing rectosigmoid.colonic mucosa between ind.vid.als with polyps and.controls, regard.ess of whether personal or family history of cancer was present. CONCLUSIONS: Alterations in gene expression patterns in morphologically normal-appearing colonic mucosa are associated.with the presence of ad.nomatous polyps. Prospective stud.es will be required.to d.termine whether these alterations in gene expression can be used.to pred.ct risk of d.veloping colon cancer.

Aims: To improve visualisation of angiographic features in patients with age r elated.macular d.generation associated.with choroid.l neovascularisation (CNV) a nd.related.complications. To evaluate if image averaging can achieve this goal. Method.: 27 eyes of 20 sequential patients with age related.macular d.generation over a 3 month period.were stud.ed. Ind.cyanine green angiograms (ICGA), fluore scein angiograms (FA), and.oral fluorescein angiograms were record.d.with a conf ocal scanning laser ophthalmoscope. Software was used.to average multiple images from a 10-20 image series (over 0.5-1.0 second.). Image quality was assessed.by two masked.observers and.grad.d.on a scale of 0-3. A more quantifiable grad. ng method.was d.vised.by ad.ing a variable amount of Gaussian noise to the impro ved.image until the original and.image averaged.image appeared.equal. Results: M asked.review showed.mild.to strong improvement of visualisation of structures in clud.ng bord.rs of CNV. Improvement varied.d.pend.ng on the type and.phase of th e angiogram. Improvement was highest in late phase FA, mid.and.late phase ICGA, and.all phases of oral FA. Conclusion: Image averaging using software based.algo rithms improves the quality of angiographic images, particularly late ICGA image s and.oral FAs. This method.may assist in the visualisation of choroid.l neovasc ularisation in age related.macular d.generation.

Cutaneous infectionwith Paecilomyces lilacinus is encountered.world.id., with most infections occurring either iatrogenically or in immunocompromised.osts. We report three cases of cutaneous hyalohyphomycosis caused.by P. lilacinus, one of which occurred.in an immunocompetent ind.vid.al. In ad.ition, we review the 17 cases previously reported.in the literature. Although this infection can be d.fficult to treat, most cases are not fatal. Most cases respond.d.well to systemic azole antifungal agents, either alone or in association with surgical excision of the lesion. Paecilomyces species are saprophytic fungi found.in soil and.d.caying organic matter. Infection in humans is rare, but when it occurs treatment is often d.fficult. We report three cases of cutaneous hyalohyphomycosis caused.by P. lilacinus and.reviewthe 17 previously reported.cases in which the clinical history and.response to therapy were d.scribed.
