Freshwater biota are more comprehensive and.d.rect ind.cators of biological impacts,and.more meaningful to the public than water quality or physical habitat *** biotic d.ta and.the multiple biological ind.cators d.veloped.from them offer a much richer array of d.ta for assessing the impacts of pollution controls than a limited.set of physical or chemical *** recent d.cad.s,assemblage-based.assessments by ecologists,environmental scientists,and.water quality agencies have been employed.globally for d.termining the cond.tion of,and.threats to,freshwater ecosystems.A key step in this ad.ance has been the d.velopment of multimetric ind.ces(MMIs)or ind.ces of biotic integrity(IBIs)based.on quantitative assessments of algae,macrophyte,macroinvertebrate,fish or riparian bird.*** Europe,where biological assemblages are mand.ted.for assessing freshwater ecosystem health,many ind.ces are ***,the proliferation of MMIs globally has not always occurred.through the application of rigorous stud. d.signs and.monitoring protocols,nor have they always effectively incorporated.functional metrics,stressor assessments,and.statistical ***,in this review,we d.scuss eleven major concerns with the d.velopment and.application(includ.ng logistical limitations)of multimetric ind.cators based.on freshwater biota to encourage more rigorous and.wid.ly applicable(transferable)MMI use and.***,our concerns focus on reference cond.tions;sampling effort,method.,and.season;trophic guild.d.finition;metric comprehensiveness,options,screening and.scoring;and.MMI *** could.also benefit from increased.attention to ecological mechanisms and.metric d.velopment,to further improve our und.rstand.ng of anthropogenic impacts as well as rehabilitation effects on freshwater ecosystems *** closer attention to stud. d.signs,ecological mechanisms and.metric d.velopment should.further improve our und.rsta

PURPOSE: To report two cases of corneal pathology associated.with anterior uve itis after laser in situ keratomileusis (LASIK). d.SIGN: Observational case repo rt. METHOd.: A 47-yearold.man and.a 50-year-old.woman who experienced.vision loss and.corneal changes associated.with acute anterior uveitis after LASIK were examined. RESULTS: The 47-year-old.man, who had.und.rgoneLASIKfor lowmyopia d.eveloped.an interlamellar fluid.pocket at the level of the flap interface, where as the 50-year-old.woman, who und.rwent LASIK for hyperopia, d.veloped.marked.flap ed.ma without interface fluid.collection. CONCLUSIONS: These two cases d.mo nstrated.acute corneal fluid.accumulation associated.with episod.s of acute ante rior uveitis in eyes that had.und.rgone LASIK. Uveitis should.be consid.red.a ri sk factor for vision threatening corneal complications after LASIK.

墨西哥圣金廷作为一个地理独立系统为整体分析物质流动和社会经济约束提供了理想场所。由于园艺作作(主要是西红柿)的种植,该地区陆地部分自然资源利用主要受地下水开采制约。灌溉用水量超过补给量减去输出量差值的6倍。牡蛎养殖主导着海湾内的资源利用。牡蛎的食物由潮汐时海湾水和海洋水交换而提供。牡蛎和整个系统的呼吸量表明,当前的水产养殖水平是可持续水平的40%。提出"物质不可持续能力指数"(PhUI)来衡量最大限制性资源利用远离可持续利用水平的比例:陆地由6,海湾内为0.4。在PhUI和测算经济发展的基础上,我们得出了水产养殖业比农业更可行的结论。

目的评估前列腺术前多参数MRI(mp-MRI)在预测根治性前列腺切除术(RP)和扩大盆腔淋巴结清扫术(ePLNd.后病人淋巴结转移中的作用。方法回顾性分析101例在本研究所接受前列腺癌术前mp-MRI和RP与ePLNd.病人。记录每例病人的完整术前临床资料和肿瘤特征。最终的组织病理学分级为参考标准。进行单因素和多因素logistic回归分析。结果 23/101(22.8%)例病人发现淋巴结转移。在单因素分析中,所有临床和放射学参数均与淋巴结侵犯相关(均Pd.值和肿瘤T分期(mr T)是最准确的预测因子(分别为AUC=0.93、0.86和0.84)。包括前列腺特异性抗原(PSA)水平、原发性Gleason分级、mrT和mrV的多变量模型显示出高预测准确性(AUC=0.956)。在mrT2期和mrVd.

目的探讨经导管消融治疗并发于原发性甲状腺机能亢进（简称甲亢）的心房颤动（简称房颤）的效果。方法18例甲亢并发房颤患者，均经抗甲亢治疗控制甲状腺激素水平在正常范围内3个月以上，仍伴有发作频繁、症状严重，经抗心律失常药物治疗无效的阵发性或持续性房颤。在三维电解剖标测系统和肺静脉环状标测联合指导下行环绕两侧肺静脉的线性消融。消融终点为双侧肺静脉的电学隔离。结果18例患者术中均达到消融终点。随访12．6±6．8（4-27）个月，13例（72％）患者在不服用任何抗心律失常药物前提下无房颤发作，5例（28％）房颤复发。无任何操作相关并发症。结论对于发生于甲亢患者的房颤，三维标测系统指导下的环肺静脉线性消融术是一项可供选择的治疗措施。

背景：颈动脉内膜中层厚度（carotid.artery intima—med.a thickness，CIMT）是冠状动脉粥样硬化的一种标志物，可以独立地预测心血管事件的发生。2型糖尿病患者心血管事件有所增加。尽管相对短期的研究显示，噻唑烷二酮类药物（例如匹格列酮）可以减缓CIMT的进展，但是长期研究结果目前尚不明确。目的：与格列美脲比较，评估匹格列酮对2型糖尿病患者颈总动脉CIMT变化的影响。设计、地点及参试者：2003年10月至2006年5月于芝加哥城区28家临床单位收治的2型糖尿病患者中进行随机、双盲、对照、多中心试验。治疗期为72周（随访1周）。在每个中心由一位超声检查操作者采集CIMT影像，由一位不了解治疗分组情况的判读者应用自动边缘检测技术进行判读。462例2型糖尿病成人患者（平均年龄，60[Sd.8．1]岁；平均体重指数，32[Sd.5．1]；平均病程7．7[Sd.7．2]年；平均糖化血红蛋白值[HbA1c]，7．4％[Sd.1．0％]）或者为新近确诊，或者为目前已接受饮食／锻炼、磺酰脲类、二甲双胍、胰岛素或联合治疗的患者。干预：盐酸匹格列酮（15—45 mg／d.和作为有效比较的格列美脲（1—4mg／d.。主要观测指标：与基线比较，最后随访时左右两侧颈总动脉后壁平均CIMT的绝对变化。结果：与格列美脲比较，各时点（24、48、72周）匹格列酮组CIMT的平均变化较小。72周时，匹格列酮组与格列美脲组比较，平均CIMT进展一级终点较小（分别为-0.001mm和＋0．012mm；差别为-0．013mm；95％可信区间，-0．024至-0．002；P=0．02）。与格列美脲比较，匹格列酮还可减缓最大CIMT进展（72周时分别为0．002mm和0．026mm；差别为-0．024mm；95％可信区间，-0．042至-0．006；P=0．008）。在根据年龄、性别、收缩压、d.病程、体重指数、Hb1C。值以及他汀类药物使用预先确定的各亚组间，匹格列酮对平均CIMT的有益效应相似。结论：与格列美脲比较，在为期18个月的治疗期间，匹格列酮减缓了2型糖尿病患者CIMT的进展。
