Effect of a Single Shot Sciatic Nerve Block Combined with a Continuous Femoral Block on Pain Scores After Knee Arthroplasty.A Randomized Controlled Trial

Background and Purpose: Postoperative pain after knee arthroplasty (TKA) is reported as severe in up to 60% of patients. Continuous femoral nerve blocks (CFNB) are a choice for major knee repair, but controversies remain about the need of supplemental sciatic nerve blocks (SNB) for better analgesia. our aim is to assess the effect of the association of a SNB to a CFNB to reduce postoperative pain after TKA. Methods: A prospective randomized, single blinded, controlled study, on 50 patients undergoing TKA. Control group received a CFNB before general anesthesia;in the intervention group a single shot SNB was added after the CFNB was done. After the end of surgery all patients started a continuous local anesthetic infusion through the femoral catheter in the PACU (post-anesthesia care unit). Pain scores were measured in the PACU and at 12 h and 24 h postoperative using a visual analog scale (VAS). Results: VAS pain scores (mm) were lower and statistically significant for the intervention group up to 12 h postoperative: PACU admission mean VAS = 59.4 vs 30.2, P = 0.001;at 12 h mean VASr = 26.1 vs 9.2, P = 0.006;at 24 h mean VASr = 30.1 vs 32.7, P = 0.723. Conclusions: The association of a single shot SNB with a CFNB significantly reduces postoperative pain scores after TKA up to 12 h. At 24 h there are no differences between groups.

Residual Effects of the organic Amendments Poultry Litter,Farmyard Manure and Biochar on Soybean Crop

The use of organic wastes, as an alternative to inorganic fertilizer, can be an important strategy for Brazilian and tropical agriculture. despite the importance, few field studies have been done for evaluating organic amendments on soybean crops in Brazil. The study aimed to evaluate the residual effects of the organic amendments poultry litter, farmyard manure and biochar combined with mineral fertilizer on some agronomic attributes of a soybean crop. A field experiment was carried out in a split-split-plot scheme, with three replicates in a randomized block experimental design. The organic sources (plots) at rates of 0, 3, 6 and 9 Mg·ha-1 (subplots) combined with 0, 100, 200, 300 and 400 kg·ha-1 (sub subplots) of a mineral fertilizer were applied in 2008. In 2009, only the mineral fertilizer was used on the soybean crop. As a?result, all evaluated attributes were influenced by the treatments, except for the number of grains per pod. The application of poultry litter provided the highest yield (3715 kg·ha-1 using 9 Mg·ha-1 of the source). A synergistic effect between organic amendments and mineral fertilizer was observed. It was found the possibility of decrease doses of mineral fertilizers by prior use of organic amendments. The most effective dose combination application is 5.5 Mg·ha-1 of organic amendments associated with 200 kg·ha-1 of mineral fertilizer to provide optimum yield. The use of organic amendments, rich in nutrients, is a technology to sustainably increase the soybean grain yield.

Contact Precaution: Acceptance of Health Professionals to Teaching Hospital

The adoption by health professionals to the practice of patient isolation is a decision that reduces patients’ risks of occupational exposure to potentially contaminated biological material and prevents nosocomial infections. Verify the compliance of health professionals to the practices of contact precautions in patients colonized by microorganisms in a teaching hospital. This was a prospective observational cohort study in a special-sized hospital located in southeastern Brazil. The observation of health professionals in delivery of health care to patients with contact precaution was performed regarding hand hygiene, use of overcoat, glove, and mask. We carried out 1502 observations involving the following professional categories: nursing technicians and/or nursing assistants (n = 1028;68.4%), nurses (n = 200;13.3%), physical therapists (n = 185;12.3%), and physicians (n = 89;5.9%), totaling 971.8 hours. Regarding the level of compliance of professionals to contact precaution practices with hand hygiene after the procedures, the use of overcoats, gloves, and masks, all were statistically significant (p Klebsiella pneumoniae was the most positive pathogen identified in laboratory tests. This study provided the mapping of the compliance of health professionals to the practices of contact precautions in order to support a safer management of patient care reducing the risks of Healthcare-Associated Infections (HAI). This study also showed that health professionals are using practice management to deal with their behavior to protect their health.

Virtual chromoendoscopy in small bowel capsule endoscopy: New light or a cast of shadow?

AIM: To evaluate whether virtual chromoendoscopy can improve the delineation of small bowel lesions previously detected by conventional white light small bowel capsule endoscopy(SBCE). METHodS: Retrospective single center study. one hundred lesions selected from forty-nine consecutive conventional white light SBCE(SBCE-WL) examinations were included. Lesions were reviewed at three Flexible Spectral Imaging Color Enhancement(FICE) settings and Blue Filter(BF) by two gastroenterologists with ex-perience in SBCE, blinded to each other's findings, whoranked the quality of delineation as better, equivalent or worse than conventional SBCE-WL. Inter-observer percentage of agreement was determined and analyzed with Fleiss Kappa(k) coefficient. Lesions selected for the study included angioectasias(n = 39), ulcers/ero-sions(n = 49) and villous edema/atrophy(n = 12). RESULTS: overall, the delineation of lesions was im-proved in 77% of cases with FICE 1, 74% with FICE 2, 41% with FICE 3 and 39% with the BF, with a percent-age of agreement between investigators of 89%(k = 0.833), 85%(k = 0.764), 66%(k = 0.486) and 79%(k = 0.593), respectively. FICE 1 improved the delineation of 97.4% of angioectasias, 63.3% of ulcers/erosions and 66.7% of villous edema/atrophy with a percentage of agreement of 97.4%(k = 0.910), 81.6%(k = 0.714) and 91.7%(k = 0.815), respectively. FICE 2 improved the delineation of 97.4% of angioectasias, 57.1% of ulcers/erosions and 66.7% of villous edema/atrophy, with a percentage of agreement of 89.7%(k = 0.802), 79,6%(k = 0.703) and 91.7%(k = 0.815), respectively. FICE 3 improved the delineation of 46.2% of angioecta-sias, 24.5% of ulcers/erosions and none of the cases of villous edema/atrophy, with a percentage of agreement of 53.8% [k = not available(NA)], 75.5%(k = NA) and 66.7%(k = 0.304), respectively. Fluorescence of Chlorophyll a for discovering Inhibitors of Photosynthesis in Plant Extracts

Aims: Twelve crude extracts were prepared from different parts of Pluchea sagittalis, Cecropia palmata and Brachiaria brizantha, and their effect on the light reaction of photosynthesis. Study design: Use of fluorescence of ChL a (Chlorophyll a) to detect inhibitors of photosynthesis in plant extracts. Place and duration of Study: Instituto de Ciências Ambientais, Químicas e Farmacêuticas, departamento de Ciências Exatas e da Terra, Universidade Federal de Sao Paulo (UNIFESP), between July 2015 and January 2016. Methodology: The samples had their effect on the light reaction of photosynthesis studied by Chlorophyll a (Chl a) fluorescence transient by the use of chloroplasts isolated from spinach leaves. As negative controls we used the same amounts of dMSo that have been employed to solubilize the samples, and as positive control we have used 10 μM of the commercial herbicide dCMU (diuron). Results: The extracts inhibited the photosynthesis, and their inhibition sites were located in the range of electron flow from oEC complex and between P680 to QA of PS II, and inhibited the photosystem II (PS II) by inducing the appearance of the transient bands K and J. Conclusion: our results suggest an indicative that the photochemical apparatus probably is failing at the donor and acceptor sides of PSII, causing damage to electron transport during the photosynthesis phenomenon.

Transcranial direct Current Stimulation: Effects on Motor and Non-Motor Symptoms of Parkinson’s disease

Introduction: In the last thirty years, brain neuromodulation techniques have been used as an alternative to pharmacological treatment of neurological disorders. Parkinson’s disease (Pd) is a neurodegenerative disorder leading to bradykinesia, rest tremor, postural changes, and non-motor symptoms such as depression, anxiety, sleep disorders, pain, and cognitive decline that compromises executive functions (EFs), responsible for the orderly execution of behaviors and tasks of daily life and intentional and directed actions. To this date, a few studies with transcranial direct current stimulation (tdCS) have shown beneficial effects in Pd patients concerning specific motor and non-motor symptoms, targeting the motor cortex and/or prefrontal regions. objective: The main objective of this study was to evaluate the effects of left prefrontal tdCS across a broad spectrum of motor and non-motor symptoms of Pd using established validated scales. Method: Single-blind randomized clinical trial with 18 volunteers with Pd, aged between 45 and 80 years (66.1 ± 9.65), who met inclusion and exclusion criteria. Participants were submitted to assessments of motor and non-motor functions employing psychometric scales and tests to evaluate EFs and were randomly divided into two groups: control (sham stimulation) and experimental (active stimulation). All participants were involved in three separate tdCS sessions. The anode was positioned over the left dorsolateral prefrontal cortex and the cathode over the right supraorbital region, with a direct current intensity of 2 mA, lasting 20 minutes. At the end of the three sessions, all participants were reassessed. Results: Significant effects of tdCS on non-motor functions were observed for cognition (verbal fluency of actions, clock copy test, appointment by visual confrontation, and verbal memory with immediate free recall) and subjective assessment of sleep quality (overall restlessness and discomfort in the arms and legs at night, leg
