Background Distal pancreatic carcinoma is one of the most lethal cancers largely due to its high incidence of distant *** study aims to assess the prognostic value of splenic-vasculature involvement in resected distal pancreatic *** In this retrospective study,we collected the clinicopathologic information of 454 patients with pancreatic cancer and performed univariate and multivariate analyses to identify factors associated with progression-free survival(PFS)and overall survival(OS),with an emphasis on the prognostic value of splenic-artery and-vein *** Univariate analysis revealed that larger tumor size,non-intraductal papillary mucinous neoplasm(non-IPmN)-associated adenocarcinoma,poor differentiation,stage pT3,nodal metastasis,lymphovascular invasion,perineural invasion,and pathologic and radiographic evidence of splenic-vein invasion were significantly associated with shorter PFS and OS(all Pmultivariate analysis confirmed non-IPmN-associated adenocarcinoma,stage pT3,stage pN1-2,and postoperative adjuvant chemotherapy as independent risk factors for both PFS and OS,and larger tumor size and radiographic evidence of splenic-artery invasion as predictors of PFS *** Guidelines should be developed for a uniform approach with regard to the examination and reporting of the status of the splenic vasculature when dealing with distal-pancreatic-cancer specimens.

Regional disease characteristics and comorbidities of patients with schizophrenia in the Ziprasidone Observational Study of Cardiac Outcomes (ZODIAC): Findings from an international large simple trial

Background: Using baseline data from the Ziprasidone Observational Study of Cardiac Outcomes (ZODIAC), we assessed disease characteristics and prevalence of select comorbidities among subjects with schizophrenia in different clinical settings across 18 countries. ZODIAC was a randomized, open-label, one-year, large simple trial (LST) that enrolled 18,239 individuals with schizophrenia. methods: Subjects were randomized to open-label treatment with ziprasidone (n = 9120) or olanzapine (n = 9119) in naturalistic (usual care) settings and followed for one year. Study sites (n = 749) applied minimal selection criteria in an attempt to make the study population as representative as possible of those receiving treatment in “real world” circumstances across the countries. Results: mean patient age was 41 years, 55% were male, 34% were markedly ill or presented with more severe disease, and 66% of subjects had one or more select comorbid conditions [i.e. heart attack, stroke, hypertension, CAD/angina, high cholesterol/triglycerides, diabetes, or overweight (BmI ≥ 25)] at baseline. History of suicide attempt was greatest in the US (38%), compared with Sweden (34%), Brazil/South America (26%), Asia (23%), and Eastern Europe (20%). Overweight or obesity was the most prevalent comorbid risk factor, representing 60% of enrolled subjects, 70% of US subjects compared with 30% in Asia and 52% - 64% in the other regions studied. High cholesterol/triglycerides levels were found in 23% of US subjects compared with a relatively low prevalence in other countries (3% - 11%). History of cardiovascular or diabetes-related comorbidities was found in 31% of subjects. Current smoking (46.5%) and past smoking (11.8%) were common with men dominating the proportion of current smokers: US (61%);Asia (60%);Sweden (50%);Eastern Europe (49%);and Latin America (44%). Effect of Cosolvent in a Nucleophilic Substitution Reaction Using Organoclay in Triphase Catalytic System

A remarkable rate enhancement technique has been devised for a typical nucleophilic displacement reaction by using triphase catalytic materials based on tetraoctylammonium exchange forms of hectorite clay. Pseudo-first order rate constants (kobs) for the conversion of 1-bromobutane to the corresponding chloride under triphase conditions using the clay catalyst in the presence of various polar cosolvents have been observed. The results here have shown that the addition of a cosolvent increases the catalytic activity of the triphase system by several fold. In addition, the results have demonstrated that each cosolvent has a unique concentration for achieving an optimum reaction rate.

Biophysical mechanism of the SAHA Inhibition of Zn^（2+）-Histone Deacetylase-Like Protein(FB188 HDAH)Assessed via Crystal Structure Analysis

The zinc-containing enzyme HDAC-like amidohydrolase (FB188 HDAH), identified in the Bordetella alcaligenes bacteria, is similar to enzymes that participate in epigenetic mechanisms such as histone modifications. The X-ray crystal structure of FB188 HDAH complexed with the antagonist SAHA (suberoylanilide hydroxamic acid) has been solved (PDB ID: 1ZZ1). Notably, the complex crystallizes as a tetramer in the asymmetric unit cell of the crystal. The crystal yielded a suitable structure to analyze the dynamics of the inhibitory mechanism of SAHA on this histone deacetylase. Applying computational chemistry techniques and quantum mechanics theory, several physicochemical properties were calculated to compare the active site of the enzyme of the four monomers. Significant differences were observed in the areas and volumes of the binding pocket, as well as hydrophobic interactions, dipole moments, atomic charges and electrostatic potential, among other properties. Remarkably, a free-energy curve resulting from the evaluation of the energies of SAHA and the interacting amino acids of the four crystal monomers unveiled the biophysical mechanism of the FB188 HDAH inhibition exerted by SAHA to a greater extent. The biophysical mechanism of SAHA inhibition on FB188 deacetylase was clearly observed as a dynamic process. It is possible to define the physicochemical dynamics of the molecular complex by the application of computational chemistry techniques and quantum mechanics theory by studying the crystal structures of the interacting molecules.

milk Replacer Acidification for Free-Access Feeding: Effects on the Performance and Health of Veal Calves

Objectives of this study were to investigate the effects of milk replacer (mR) acidification for free- access feeding on pre- and post-weaning performance, morbidity and mortality of calves. Calves were randomly assigned to free-access feeding of acidified (ACID, n = 31) or non-acidified (NON, n = 31) mR, and weaned at 42 days. ACIDmR was prepared to pH 4.0 - 4.5 using formic acid. Intakes were measured daily and weights weekly. Samples of ACID and NON mR were analyzed for coliform and aerobic bacterial growth. After weaning, calves transitioned to a grain-fed veal diet, weighed every 2 weeks. At slaughter (approximately 6 months of age), lungs were evaluated for gross pathological changes and dressed carcass weights were obtained. Generalized linear mixed models were constructed to examine the effects of mR acidification. Differences by treatment group for pre- and post-weaning morbidity and mortality were tested using Pearson’s χ2 and Fisher’s exact tests. ACID calves consumed less mR than NON (10.6 vs. 11.7 L/d, P = 0.02). Acidification tended to promote earlier onset of starter ration consumption (32.0 vs. 39.5 d, hazard ratio = 1.5, P = 0.07), but did not affect average daily starter ration or water intakes across the pre-weaning period. ACID and NON calves did not differ for BW at weaning, pre- or post-weaning ADG or dressed carcass weight. ACID mR had less coliform (P mR, but odds of disease treatment and mortality did not differ. ACID calves tended to have lower odds of pulmonary lesions during post-mortem inspection than NON calves (OR = 0.3, P = 0.07). These results indicate that under free-access feeding conditions, acidification limited bacterial growth and mR intake, but there were no long-term impacts of acidification on calf performance or health. In conclusion, acidification to pH 4.0 - 4.5 will significantly reduce bacterial growth in milk fed to calves.

Huanglongbing (HLB) or citrus greening is currently the most important citrus disease, caused by the bacterium Candidatus Liberibacter asiaticus (CLas). The impossibility of isolating it causes understanding its pathogenic mechanisms to be a complicated task. Recent studies identified 16 proteins with the signal peptide needed to be secreted in the plant and cause the disease. The present study aims to perform a bioinformatic analysis of these proteins with the function prediction approach by gene ontology (GO) and the detection of conserved domains. It was observed that of the 16 proteins analyzed not all are found in different infective strains reported in the literature. The GO analysis allowed us to relate different proteins with the biological process of energy and pathogenic activity, especially CLIBASIA_03315 and CLIBASIA_05115, respectively. The domain analysis allowed the observation of a β-CA domain, tentatively related to the damage caused to the chloroplast and a PAAR domain associated with the T6SS secretory system. Our results provide information on the possible function of potential pathogenicity effectors in Cmily:Verdana;">Las.

Risk Factors for Surgical Site Infection after Hip Arthroplasty: A multicentric Study

The objective of this study is to answer three main questions: What is the risk of wound infection for patients undergoing hip arthroplasty? What are the main etiologicagents of surgical site infection (SSI)? What are the risk factors most associated with surgical site infection? method: This was a multicentric, retrospective cohort study which analyzed data collected in five general hospitals in Belo Horizonte, Brazil, between the period of January 2009 and December 2013. The continuous parameters studied were age, length of hospital stay before surgery, duration of surgery, number of professionals at surgery and number of hospital admissions. Categorical variables were surgical wound classification (clean, clean contaminated, contaminated, dirty/infected), American Society of Anesthesiologists (ASA) score (I, II, III, IV, V), type of surgery (elective, emergency), general anesthesia (yes, no), prophylactic antibiotic (yes, no), trauma surgery (yes, no) and Nosocomial Infections Surveillance (NNIS) risk index (IRIC = 0, 1, 2, 3). Results: Estimated SSI risk was 3.2% (95% C.I. = 2.6% to 4.1%) and risk of osteomyelitis was 0.6% (95% C.I. = 0.4% to 1.1%). ASA score > 2, general anesthesia, length of hospital stay before surgery higher than four days, more than two professionals at surgical field and duration of surgery higher than five hours were risk factors for SSI after hip prosthesis (p multiple logistic regression analysis indicated that the modified NNIS risk was independently associated with surgical site infection after arthroplasty of hip. Conclusion: Despite the modified NNIS index being a risk factor for SSI, none of its independent variables was statistically significantly in the logistic model (p > 0.100). Each modified NNIS risk category increased the chance of a patient being infected by almost three times, when compared with the previous category (OR = 2.82;p = 0.011).
