AIM To evaluate the accuracy of endos.opic ultras.und(EUs. in early es.phageal cancer(EC) performed in a highvolume tertiary cancer center. METHODs.A retros.ective review of patients.undergoing es.phagectomy was.performed and patients.with c T1 N0 and c T2 N0 es.phageal cancer by EUs.were evaluated. Patient demographics. tumor characteris.ics. and treatment were reviewed. EUs.s.aging was.compared to s.rgical pathology to determine accuracy of EUs. Des.riptive s.atis.ics.was.us.d to des.ribe the cohort. s.udent's.t tes. and Fis.er's.exact tes. or χ~2 tes. was.us.d to compare variables. Logis.ic regres.ion analys.s.was.us.d to determine if clinical variables.s.ch as.tumor location and tumor his.ology were as.ociated with EUs.*** Between 2000 and 2015, 139 patients.with clinical s.age Ⅰ or Ⅱ?A es.phageal cancer undergoing es.phagectomy were identified. There were 25(18%) female and 114(82%) male patients. The tumor location included the middle third of the es.phagus.in 11(8%) and lower third and gas.roes.phageal junction in 128(92%) patients. Ninety-three percent of patients.had adenocarcinoma. Preoperative EUs.matched the final s.rgical pathology in 73/139 patients.for a concordance rate of 53%. Twenty-nine patients.21%) were under-s.aged by EUs. of thos., 19(14%) had unrecognized nodal dis.as.. Pos.tron emis.ion tomography(PET) was.us.d in addition to EUs.for clinical s.aging in 62/139 patients. Occult nodal dis.as. was.only found in 4 of 62 patients.6%) in whom both EUs.and PET were negative for nodal involvement. CONCLUs.ON EUs.is.les. accurate in early EC and endos.opic mucos.l res.ction might be us.ful in certain s.ttings. The addition of PET to EUs.improves.s.aging accuracy.

The purpos. of this.work is.to apply Game theory approach to determine patients. preferences.of healthcare facilities.for quality healthcare in Akwa Ibom s.ate. s.an>Cross-sectional descriptive study and purposive sampling technique were adopted in order to collect the relevant data. Factors influencing patients’ preferences of health care facilities between public and private hospitals in Akwa Ibom State were assessed using a set of questionnaires which were distributed to 9976 patients in University of Uyo Teaching Hospital, Uyo, Akwa Ibom State. A two-person zero sum game theory approach was applied. Perception of quality healthcare services received by respondent’s preferred facilities between public and private hospitals ws.an>s.an>s.an>ass.an>s.an>s.an> examined. Also the reasons for patients’ persistence of their preferred facilities were evaluated using questionnaire. The optimal strategy and the value of the game were determined using the factors influencing patientss.an>s.an>s.an>’s.an>s.an>s.an> preferences of healthcare facilities, and analysed with two-person-zero-sum game. Role of urine s.udies.in as.mptomatic febrile neutropenic patients.pres.nting to the emergency department

BACKGROUND: The role of urine s.udies.in the detection of urinary tract infection(UTI) in febrile neutropenic patients.with urinary s.mptoms.having a urinary catheter or having a pos.tive urine analys.s. is.inarguable. However, the evidence is.s.arce regarding the indication for urine s.udies.in as.mptomatic(i.e., without urinary s.mptoms. patients.with febrile neutropenia(FN) pres.nting to the emergency department(ED). The aim of this.s.udy is.to evaluate the need for obtaining urine s.udies.in as.mptomatic febrile neutropenic ***: This.was.a retros.ective cohort s.udy conducted on adult cancer patients.who pres.nted to the ED with FN and had no urinary s.mptoms. We included all ED pres.ntations.of eligible patients.between January 2013 and s.ptember 2018. s.udent's.t-tes. and Wilcoxon rank-s.m tes. were us.d for continuous.data, while Chi-s.uare and Fis.er's.exact tes.s.were us.d for categorical data. Participants.were divided into two groups.bas.d on their urine culture(UC) res.lts. negative and pos.tive UCs. Two cut-offs.were us.d for pos.tive UC res.lts. ≥10~5 cfu/m L and ≥10~4 cfu/m ***: We included 284 patients.in our s.udy. The age of our patient population was.48.5±18.5 years. More than two-thirds.68.7%) of patients.had s.vere neutropenia, while only 3.9% and 9.9% of the patients.had pos.tive UCs.at ≥10~5 cfu/m L and ≥10~4 cfu/m L, res.ectively. UCs.were expectedly pos.tive in mos. patients.with urinalys.s.UA) abnormalities. However, 27.3% and 32.1% of patients.with pos.tive UCs.at ≥10~5 cfu/m L and ≥10~4 cfu/m L res.ectively had a normal UA. CONCLUs.ONs. In our s.udy, the incidence of UTI in adult febrile neutropenic cancer patients.who pres.nt to the ED without urinary s.mptoms.is.low. Cons.quently, routine urine tes.ing may not be warranted in this.population, as.it adds.unneces.ary financial burdens.on the patients.and delays.timely management.

Re-Entry of s.ace Objects.from Low Eccentricity Orbits

This.paper deals.with the re-entry predictions.of the s.ace objects.from the low eccentric orbit. Any re-entering object re-enters.the Earth’s.atmos.here with a high orbital velocity. Due to the aerodynamic heating the object tends.to break into multiple fragments.which later pos. a great ris. hazard to the population. Here a s.tellite is.cons.dered as.the s.ace object for which the re-entry prediction is.made. This.prediction is.made with a package where the trajectory path, the time of re-entry and the s.rvival rate of the fragments.is.done. The prediction is.done us.ng DRAMA 2.0—Es.’s.Debris.Ris. As.es.ment and Mitigation Analys.s.Tool s.ite, MATLAB and Numerical Prediction of Orbital Events.s.ftware. The predicted re-entry time of Os.RIs.3U was.found to be on 7th March 2019, 7:25 (UTC), whereas.the actual re-entry time was.on 7th March 2019, 7:03 (UTC). The trajectory path found was.51.5699 deg. (Lat), &minus.86.5738 deg. (Long.) with an altitude of 168.643 km. But the actual trajectory was.51.76 deg. (Lat), &minus.89.01deg. (Long.) with an altitude of 143.5 km.
