Objective: To examine the effect of the Baby Friendly Hospital Initiative on breast feeding rates in Scotland. Design: Observational study using an annual su rvey of progress towards the WHO/UNICEF Baby Friendly Hospital Initiative and ro utinely collected breast feeding rates gathered on the Guthrie Inborn Errors Scr eening card at 7 days of postnatal age. Setting: Scotland, UK, population 5.1 mi llion, with about 53 000 births a year. Participants: All 33 maternity units wit h over 50 births per annum and 464 246 infants born in Scotland between 1995 and 2002. Main outcome measures: Baby Friendly status of each maternity unit at the time of an infant’s birth: certificate of commitment, UK standard award, and breast feeding at 7 days postnatal age. Results: Babies born in a hospital with the UK Baby Friendly Hospital Initiative standard award were 28% (p justment for mother’s age, deprivation, ho spital size, and year of birth. From 1995, breast feeding rates had increased si gnificantly faster in hospitals with Baby Friendly status by 2002: 11.39% (95 % confidence interval 10.35 to 12.43) v 7.97% (95% confidence interval 7.2 1 to 8.73). Conclusion: Being born in a hospital that held the award increased t he chance of being breast fed. All maternity units should be encouraged to under take the significant strategic and prac tical changes required to achieve UK Baby Friendly Hospital Initiative standa rd status.

二、桡骨头切除的影响对于Mason Ⅲ型骨折,特别是不伴韧带损伤时,如何进行外科治疗还没有得到彻底解决。桡骨头切除对肘部运动及稳定的影响已有大量研究。可以明确的是,桡骨头的缺失将导致肘部的外翻稳定性降低,是仅次于MCL的一个重要约束。我们的内翻-外翻松弛测试显示,桡骨头的缺失导致松弛显著增加。然而,相对于MCL,桡骨头的作用要低得多,对最大内翻-外翻松弛起24%的作用(先切除桡骨头)。

目的　评价妇女在妊娠前后服用单纯 40 0 μg叶酸增补剂对胎 /婴儿神经管畸形 (NTDs)的预防效果。方法　 1993～ 1995年在中国北方的NTDs高发地区和南方的NTDs低发地区妇女增补叶酸的推广项目中 ,共募集从孕前或孕后任何时间开始服药的妇女 130 142名 ,未服药的妇女 1176 89名 ;设计的服药方法从婚检时开始到孕满 3个月为止 ,每天服用单纯叶酸片 40 0 μg ;最后对妇女的分娩结局进行监测并进行预防效果的对比评价研究。结果　服药组妇女生育的胎婴儿中共发现 10 2例NTDs ,对照组胎婴儿中共发现 173例NTDs。末次月经前募集的未服药妇女在孕 2 0周以后分娩的胎婴中NTDs发生率 ,北方为 4 8‰ (16 / 3 318) ,南方为 1 0‰ (2 8/ 2 82 6 5 ) ,而妊娠前后服药妇女组则分别为1 0‰ (13/ 13 0 12 )和 0 6‰ (34/ 5 86 38)。与末次月经前募集的未服药妇女组相比 ,北方服药妇女NTDs发生率明显降低 ,其中依从性大于 80 %的服药组妇女预防率达 85 % (95 %CI为 6 2 %～ 94% ) ,南方地区服叶酸的预防率为 41% (95 %CI为 3 %～ 6 4% )。结论　妇女在妊娠前后每天服用单纯叶酸 40 0

Introduction: Hepatic magnetic resonance elastography (MRE) allows for noninvasive assessment of liver fibrosis. The purpose of this study was to evaluate the usefulness of MRE in detecting and quantifying liver fibrosis in patients with rheumatoid arthritis (RA) who have received methotrexate (MTX). Methods: The association between mean liver stiffness value as determined by MRE and variables of interest was determined. The decision for a liver biopsy in participants with an abnormal liver stiffness was made based on clinical judgment. Results: Sixty-five RA patients were enrolled. Mean liver stiffness value by MRE was abnormal in 7 patients, suggestive of hepatic injury. As a result of findings from the MRE, biopsies were performed in 5 patients and all correlated with elevated liver stiffness values. Elevated mean liver stiffness values were associated with body mass index (BMI) (OR = 1.18 per 1 kg/m2;95% CI: 1.03, 1.36;p = 0.017). Neither the total MTX dose nor the duration of MTX treatment was associated with mean liver stiffness value (p = 0.51 and P = 0.20, respectively). Conclusion: MRE provides a reliable, non-invasive assessment of liver fibrosis in patients with RA receiving MTX. Patients with RA receiving MTX who have an elevated BMI may be at increased risk for chronic hepatic injury, regardless of MTX cumulative dose or duration of treatment.

Objectives of this study were to investigate the effects of milk replacer (MR) acidification for free- access feeding on pre- and post-weaning performance, morbidity and mortality of calves. Calves were randomly assigned to free-access feeding of acidified (ACID, n = 31) or non-acidified (NON, n = 31) MR, and weaned at 42 days. ACIDMR was prepared to pH 4.0 - 4.5 using formic acid. Intakes were measured daily and weights weekly. Samples of ACID and NON MR were analyzed for coliform and aerobic bacterial growth. After weaning, calves transitioned to a grain-fed veal diet, weighed every 2 weeks. At slaughter (approximately 6 months of age), lungs were evaluated for gross pathological changes and dressed carcass weights were obtained. Generalized linear mixed models were constructed to examine the effects of MR acidification. Differences by treatment group for pre- and post-weaning morbidity and mortality were tested using Pearson’s χ2 and Fisher’s exact tests. ACID calves consumed less MR than NON (10.6 vs. 11.7 L/d, P = 0.02). Acidification tended to promote earlier onset of starter ration consumption (32.0 vs. 39.5 d, hazard ratio = 1.5, P = 0.07), but did not affect average daily starter ration or water intakes across the pre-weaning period. ACID and NON calves did not differ for BW at weaning, pre- or post-weaning ADG or dressed carcass weight. ACID MR had less coliform (P < 0.001) and aerobic bacterial growth (P < 0.001) than NON MR, but odds of disease treatment and mortality did not differ. ACID calves tended to have lower odds of pulmonary lesions during post-mortem inspection than NON calves (OR = 0.3, P = 0.07). These results indicate that under free-access feeding conditions, acidification limited bacterial growth and MR intake, but there were no long-term impacts of acidification on calf performance or health. In conclusion, acidification to pH 4.0 - 4.5 will significantly reduce bacterial growth in milk fed to calves.

AIM: To evaluate pretreatment hepatitis B virus(HBV) testing, vaccination, and antiviral treatment rates in Veterans Affairs patients receiving anti-CD20 Ab for quality ***: We performed a retrospective cohort study using a national repository of Veterans Health Administration(VHA) electronic health record data. We identified all patients receiving anti-CD20 Ab treatment(2002-2014). We ascertained patient demographics, laboratory results, HBV vaccination status(from vaccination records), pharmacy data, and vital status. The high risk period for HBV reactivation is during antiCD20 Ab treatment and 12 mo follow up. Therefore, we analyzed those who were followed to death or for at least 12 mo after completing anti-CD20 Ab. Pretreatment serologic tests were used to categorize chronic HBV(hepatitis B surface antigen positive or HBs Ag+), past HBV(HBs Ag-, hepatitis B core antibody positive or HBc Ab+), resolved HBV(HBs Ag-, HBc Ab+, hepatitis B surface antibody positive or HBs Ab+), likely prior vaccination(isolated HBs Ab+), HBV negative(HBs Ag-, HBc Ab-), or unknown. Acute hepatitis B was defined by the appearance of HBs Ag+ in the high risk period in patients who were pretreatment HBV negative. We assessed HBV antiviral treatment and the incidence of hepatitis, liver failure, and death during the high risk period. Cumulative hepatitis, liver failure, and death after anti-CD20 Ab initiation were compared by HBV disease categories and differences compared using the χ2 test. Mean time to hepatitis peak alanine aminotransferase, liver failure, and death relative to anti-CD20 Ab administration and follow-up were also compared by HBV disease ***: Among 19304 VHA patients who received anti-CD20 Ab, 10224(53%) had pretreatment HBs Ag testing during the study period, with 49% and 43% tested for HBs Ag and HBc Ab, respectively within 6 mo pretreatment in 2014.
