Delayed referral for liver transplant evaluation worsens outcomes in chronic liver disease patients requiring inpatient transplant evaluation

Simulation of the deformable lateral boundaries in biaxial test using DEM

Performance of a three-phase shunt active power filter(SAPF)relies on the capability of the controller to track the reference ***,designing an accurate current controller is crucial to guarantee satisfactory SAPF *** paper presents a model predictive current controller(MPCC)for a low-cost,four-switch,shunt active power filter for power quality improvement.A four-switch,B4,converter topology is adopted as an SAPF,hence offering a simple,robust,and low-cost *** addition,to further reduce overall cost,only two interfacing filter inductors,instead of three,are used to eliminate switching current *** proposed SAPF model MPCC is detailed for implementation,where simulation and experimental results validate effectiveness of the proposed control algorithm showing a 20%improvement in total harmonic distortion compared with a conventional hysteresis band current controller.

Intermodal Competition: Cargo Airships versus Long-Haul Trucking for Perishable Commodities

Intermodal competition changes with changes in technology, economics, and environmental concerns. Trucks and airships are generally considered not to be competitors, but this depends on the distance of haul. The tonne-kilometer cost of trucking rises much more quickly with distance than it does the cost of a cargo airship. At some distance, the two modes are direct substitutes. The costs of the Mexico-Canada refrigerated truck supply chain are compared with the costs of a 100t-lift, electrically-powered airship. The flight characteristics of the Hindenburg Zeppelin are used as a model for a modern cargo airship. The supply chain cost of trucking tomatoes is used to test the theorical proposition. The cost difference works out to about US10￠/kg (5￠/lb) advantage for trucking Mexican tomatoes to Canada. However, this cost disadvantage of the airship could be made up by their vibrationless ride, better air circulation and one-day service versus four days by truck. This alternative form of transportation could have a positive impact on worldwide north-south distribution of food. Airships can overcome trade barriers and distance to open new markets for perishable food exports. In addition, they would reduce the carbon emissions of transport. Canada imports 160,000 refrigerated truckloads of fruits and vegetables by from the southern US and Mexico. With an average driving distance of 3,000 km, these trucks emit 606,000 MT of CO2 annually. Airships powered by hydrogen fuel cells would have zero-carbon emissions. Markets are not yet incorporating the environmental advantage of airships in any freight comparison, but inevitably this will be important.

Ruptured Ectopic Pregnancy: Epidemiology and Management in the General Surgery Department of the Ignace Deen National Hospital, Conakry University Hospital

Introduction: The aim of this study was to contribute to improving the quality of GEUR management in the general surgery department and in the maternity ward of the Ignace Deen national hospital, Conakry University Hospital. Methodology: This was a retrospective, descriptive study lasting two years (January 1, 2020 to December 31, 2022), carried out in the general surgery and gyneco-obstetrics departments of the Ignace Deen national hospital, Conakry University Hospital. We included all patients with a ruptured ectopic pregnancy who received surgical treatment during the study period. Results: We collected 13,524 cases of surgical interventions in the two services, among them, we recorded 89 cases or 0.66% GEUR. The average age of the patients was 24.26 years. Brides were the most represented with 80.96% of cases. Women practicing a liberal profession were 51.69% (n = 46) and housewives 26.97% (n = 24). Clinically, amenorrhea was noted in all patients, i.e. 100%, abdominal-pelvic pain in 95.2% (n = 85) of cases, metrorrhagia in 94.08% (n = 84), abdominal-pelvic sensitivity in 97.44% (n = 87) of cases and anemia in 85.39% of cases. The GEUR was ampullary in 69.66% (n = 62) cases. Salpingectomy was performed in 90.72% (n = 81). The surgical outcomes were satisfactory in 98.87% (n = 88) of cases. We recorded one case of surgical site infection. We have not recorded any deaths. The average length of hospitalization was 4 days. Conclusion: GEUR is relatively high in our context. A good understanding of the prognostic factors of GEUR, awareness and family planning could reduce GEUR.

Splenectomy: Indications in the General Surgery Department of Ignace Deen Hospital

Introduction: Splenectomy is the surgical removal of the spleen. It can be performed during various pathologies, ranging from abdominal trauma to hemoglobinopathies. The progress made in the knowledge of the immune functions of the spleen and the fear of post-splenectomy infectious complications have favored the development of surgical or non-surgical splenic preservation techniques calling into question the dogma of splenectomy. The aim of this study was to determine the indications for splenectomy. Methodology: This was a retrospective, descriptive study lasting 5 years in the general surgery department of the Ignace Deen National Hospital. All files of splenectomized patients were included, our variables were clinical, therapeutic and progressive. Results: We collected 42 cases of splenectomies out of the 2478 surgical procedures performed, representing 1.7% of the department’s surgical activities. The average age was 44 years. The age group of 41 to 50 years was the most represented, i.e. 26% (n = 11) of cases. Sex ratio = 1. Abdominal pain was the reason for consultation in patients, i.e. 100% (n = 42) of cases. The antecedents were dominated by recurrent malaria with 52.3% (n = 22) of cases, then recurrent anemia in 21% (n = 9), and 16.7% (n = 7) had sickle cell disease. Splenomegaly was found in 31 patients, or 73.6%. Ultrasound was performed in all patients. The indications for splenectomy were: isolated splenomegaly with risk of rupture (38%, n = 16), hypersplenism (26%, n = 11) and trauma to the spleen (19.04%, n = 8). Total splenectomy was performed in all cases. The surgical consequences were favorable in 85.7%, (n = 36) with morbidity of 14% (n = 6) and mortality of 9.52% (n = 4). The average length of hospitalization was 10.4 days with extremes of 1 and 22 days. Conclusion: Splenectomy constitutes a relatively common surgical procedure in our context. Investigation of HCAR2 antagonists as a potential strategy to modulate bovine leukocytes

Background Dairy cows experiencing ketosis after calving suffer greater disease incidence and are at greater risk of leaving the herd. In vitro administration of beta-hydroxybutyric acid(BHBA;the primary blood ketone) has inhibitory effects on the function of bovine leukocytes. BHBA is a ligand of HCAR2 and the activation of these receptors promotes an anti-inflammatory response which may be related with immunosuppression observed in transition dairy cattle. The objective of this study was to identify and test antagonists for HCAR2 in bovine immune cells cultured with *** We observed expression of HCAR2 at the protein level within lymphocytes, monocytes, and granulocytes. The proportion of cells expressing HCAR2 tended to be greater in mid-lactation compared to early lactation cows;the increase was a result of increased proportion of T and B cells expressing HCAR2. Stimulation of HCAR2 with niacin or BHBA promoted Ca^(2+) mobilization in neutrophils and mononuclear cells. Mononuclear cells treated with BHBA had diminished intracellular Ca^(2+) responses when HCAR2 was knocked down by si RNA silencing, indicating Ca^(2+) mobilization was mediated by HCAR2 signaling. Two candidate antagonists for HCAR2, synthesized from niacin(NA-1 and NA-5), were tested;monocytes and neutrophils pre-treated with NA-1 and NA-5 had reduced Ca^(2+) mobilization after incubation with BHBA. Furthermore, NA-5 but not NA-1 prevented BHBA-associated reductions in cyclic *** We demonstrated that HCAR2 is present on bovine leukocytes and has greater expression later in lactation. We confirmed that BHBA and niacin derived HCAR2 antagonists alter bovine leukocyte activity. Our results demonstrate that both BHBA and niacin affect bovine leukocyte Ca^(2+) mobilization in a HCAR2-dependent manner.
