Ankle-Foot Continuous Passive Motion Device for Mobilization of Acute Stroke Patients

Purpose: To develop a continuous passive motion (CPM) device for the passive motion of the paretic ankle-foot and investigate the effect of continuous passive motion of bedridden, hemiparetic acute stroke patients. Methods: 49 patients with stroke were investigated. results in stroke patients (device group) were compared with those of 15 control subjects (manual group) also with stroke but not treated by device. The period of the treatment was 7 days;the duration was 30 minutes per day by CPM device in the device group. The efficacy of the device was evaluated by scales used in the clinical routine (6th item of National Institutes of Health Stroke Scale (NIHSS), Modified Ashworth Scale (MAS), modified rankin Scale (mrS)). Ankle’s passive range of motion (PrOM) and flexible equinovalgus deformitiy were measured every day with a goniometer. results: 6th item of NIHSS score improved by -0.76 (SD = 0.56) points in the device group (p red to the baseline values;the mean change in the manual group was -0.33 (SD = 0.62) points (p = 0.055). The mean of MAS decreased significantly by -0.53 (SD = 1.12) point in the device group (p r flexion PrOM increased by 3.41 (SD = 5.19) degrees in the device group (p rovement of the mean dorsiflexion in the PrOM of the ankle was also detected (p = 0.019). The equinovalgus improved significantly by -5.12 (SD = 8.02) degrees (p vice group. The scores of the mrS also improved significantly in the device group (p rly phase of rehabilitation, ankle-foot continuous passive motion device treatment combined with manual therapy improved the ankle’s PrOM better than manual therapy alone;in addition, device treatment decreased the foot’s equinovalgus, improved the 6th item NIHSS score, and decreased the severity of spasticity.

Safety and Efficacy of Cefovecin (Convenia<sup>®</sup>) as an Adjunctive Treatment of Periodontal Disease in Dogs

This study was designed to determine the safety and efficacy of cefovecin (Convenia?;Pfizer Animal Health) when compared to clindamycin (Antirobe?;Pfizer Animal Health) as an adjunctive therapy to periodontal scaling or surgery for severe periodontal disease in dogs. A multi-centre, double-masked, randomised study was conducted in 299 dogs with severe periodontal disease. Clindamycin, administered once daily at 11 mg/kg bodyweight orally for 10 days following dental surgery was compared with a single, subcutaneous injection of cefovecin (8 mg/kg bodyweight) administered at the time of dental surgery. The primary efficacy parameter assessed was percentage of tooth-root sites bleeding when probed (an indicator of gingival inflammation) 42 days after surgery. Two-hundred and ninety-one (291) dogs were included in the efficacy assessments. Cefovecin was shown to be non-inferior to clindamycin. The percentage of sites bleeding on probing was reduced from 54.3% to 20.3% for the cefovecin group (53.1% reduced to 17.4% for the clindamycin group). There were no suspected adverse drug experiences attributed to treatment with cefovecin or clindamycin. Cefovecin was shown to be as effective and safe adjunctive treatment for severe periodontal disease in dogs undergoing periodontal scaling and surgery as clindamycin.

Interaction of the major inflammatory bowel disease susceptibility alleles in Crohn’s disease patients

AIM:To investigate the interaction of interleukin-23 receptor(IL23r)(rs1004819 and rs2201841),autophagy-related 16-like 1(ATG16L1)(rs2241880), caspase recruitment domain-containing protein 15 (CArD15)genes,and IBD5 locus in Crohn's disease(CD) patients. METHODS:A total of 315 unrelated subjects with CD and 314 healthy controls were *** and specific genotype combinations of a total of eight variants were *** variants of IBD5locus(IGr2198a_1 rs11739135 and IGr2096a_1 rs12521868),CArD15(r702W rs2066845 and L1007fs rs2066847),ATG16L1(rs2241880)and IL23r (rs1004819,rs2201841)genes were genotyped by PCr-rFLP,the G908r(rs2066844)in CArD15 was determined by direct sequencing. rESULTS:The association of ATG16L1 T300A with CD was confirmed[P=0.004,odds ratio(Or)=1.69, 95%CI:1.19-2.41],and both IL23r variants were found to represent significant risk for the disease(P= 0.008,Or=2.05,95%CI:1.20-3.50 for rs1004819 AA;Pr=2.97,95%CI:1.65-5.33 for rs2201841 CC).Logistic regression analysis of pairwise interaction of the inflammatory bowel disease (IBD)loci indicated that IL23r,ATG16L1,CArD15 and IBD5(IGr2198a_1)contribute independently to disease *** also analysed the specific combina- tions by pair of individual ATG16L1,IL23r rs1004819, rs2201841,IGr2198a_1,IGr2096a_1 and CArD15 genotypes for disease risk *** almost all cases,the combined risk of susceptibility pairs was higher in patients carrying two different risk-associated gene variants together than individuals with just one *** highest Or was found for IL23r rs2201841 homozygous genotype with combination of positive CArD15 status(Pr=9.15,95% CI:2.05-40.74). Palatability Testing of Oral Chewables in veterinary Medicine for Dogs

For veterinary medications administered per os, animal health companies strive to develop highly palatable dosage forms that are voluntarily accepted by animals to improve compliance and convenience. Achieving high palatability is often complex and difficult even without the presence of an active ingredient. This work compared acceptance and preference studies, as standardized methods are not established for informing formulation development or for more routine testing. Formulation development was followed by an acceptance study completed with laboratory Beagle dogs. One acceptance study and one preference study were completed in mixed breed dogs, also laboratory-housed, to gain wider representation of dog breed and age. Through these studies, we have evaluated both formulation parameters and palatability study conduct. In general, more complex palatants that have appealing taste, smell, and mouth feel enhance voluntary uptake. However, dosage forms that are too chewy may not be freely consumed even with complex palatants. The addition of aroma can entice dogs to prehend the tablet, as observed in one preference study. Preference studies in the veterinary pharmaceutical field identify the preferred first choice or first prehend, but not which product will be routinely voluntarily and fully consumed. Acceptance studies with cross-over treatment groups are used to quantify the full consumption of a dosage form when a dog is not given two choices at once. Since all dogs in acceptance studies are offered all treatment groups throughout the study, a comparison between degrees of consumption could suggest that one formulation might be preferred over another.

relationship of Grain Fructan Content to Degree of Polymerisation in Different Barleys

Fructans are important in the survival of plants and also valuable for humans as potentially health promoting food ingredients. In this study fructan content and composition were determined in grains of 20 barley breeding lines and cultivars with a wide variation in chemical composition, morphology and country of origin, grown at one site in Chile. There was significant genotypic variation in grain fructan content ranging from 0.9% to 4.2% of grain dry weight. Fructan degree of polymerisation (DP) was analysed using high-performance anion-exchange chromatography with pulsed amperometric detection (HPAEC-PAD). Changes in the distribution of different chain lengths and the pattern of structures of fructan were detected with increasing amount of fructan in the different barleys. A positive correlation was found between fructan content and the relative amount of long chain fructan (DP > 9) (r = 0.54, p = 0.021). Our results provide a basis for selecting promising barley lines and cultivars for further research on fructan in barley breeding with the aim to produce healthy food products.

relapsing polychondritis with p-ANCA associated vasculitis: Which triggers the other?

relapsing polychondritis(rP) is a rare autoimmune disease with chronic inflammatory/destructive lesions of the cartilaginous tissues. In one third of the cases it is associated with other autoimmune disorders, mostly with anti-neutrophil cytoplasmic antibody(ANCA) associated vasculitis(AAv). We report three cases of rP with p-ANCA positive AAv. In the first patient rP developed 1.5 years after the onset of AAv. In the others the signs of rP were present before the onset of severe crescent glomerulonephritis. Patients responded well on steroid and cyclophosphamide. In dialysis dependent cases plasmapheresis was also used successfully. During the 2 and 1.5 years of follow up, they were symptom-free, and had stable glomerular filtration rate. The first patient died after four years of follow-up due to the complications of sudden unset pancytopenia,which raises the possibility of associated hemophagocytic syndrome. In the setting of rP or AAv physicians should always be aware of the possibility of sudden or insidious appearance of the other disease.
