Effect of <i>Carica papaya</i>Leaf Extract on Serum Lipids and Liver Metabolic Parameters of Rats Fed a High Cholesterol Diet

Hyperlipidemia plays an important role in the development of atherosclerosis, the main cause of death in the world. In this study, the lipid-lowering effect of Carica papaya leaf in rats fed with a high cholesterol diet was evaluated. Daily doses of C. papaya extract 0, 31, 62 or 125 mg/kg body weight were orally administered in 300 μl polyethylene glycol to hypercholesterolemic rats;it was also administered 62 mg/kg body weight of the extract to rats with normal diet. After a 20-day treatment, the animals were sacrificed;blood and liver were analyzed. Hypercholesterolemic rats showed an increased serum and liver cholesterol, triacylglycerols, and atherogenic index. The C. papaya extract produced a significant decrease of serum and liver cholesterol concentrations in hypercholesterolemic rats, but did not modify serum or liver triacylglycerols;however, the extract reduced the atherogenic index in a dose-dependent manner. C. papaya treatment decreased LDL-C and increased HDL-C in serum significantly. When the oxygen consumption was evaluated in phosphorylating and resting states, the respiratory control in hypercholesterolemic rats mitochondria was lower than in normal diet rats. However, a higher respiratory control in hypercholesterolemic rats mitochondria was observed after C papaya treatment. The liver morphological data are in accordance with serum and liver biochemical values. Our data support that C. papaya has a significant hypocholesterolemic action and HDL-C raising effect on rats fed with a cholesterol-rich diet, however, the precise metabolites responsible of this effect remain unknown.

Evaluation of Mandibular Deprogramming Using TEnS (Transcutaneous Electrical nerve Stimulation) in Patients at the Specialty Clinic of the Juarez Autonomous University of Tabasco from 2022 to 2024

Temporomandibular disorders (TMD) negatively affect quality of life, causing pain and restricted jaw movements. This study evaluates the effectiveness of Transcutaneous Electrical nerve Stimulation (TEnS) on TMD patients before orthodontic treatment at the Juarez Autonomous University of Tabasco. A quantitative, quasi-experimental study was conducted with 30 patients who met the inclusion criteria: aged 18 to 65 years, with TMJ pain symptoms, diagnosed with temporomandibular disorders using the Helkimo index, and who had provided informed consent for the study. The range of mandibular movement and the Visual Analog Scale (VAS) were measured to record values before therapy. The electrodes of the TEnS therapy were positioned on both sides of the jaw in the preauricular area and the masseter muscle, the therapy was administered for 40 minutes, utilizing a frequency of 2 - 4 Hz and a pulse duration of 300 microseconds. Following this, the range of mandibular movement and the VAS were reassessed to evaluate the outcomes., significantly improved maximum mouth opening (37.7 ± 6.4 mm to 44.6 ± 5.88 mm), right laterality (3.86 ± 1.57 mm to 5.43 ± 1.21 mm), left laterality (3.13 ± 1.63 mm to 4.53 ± 1.61 mm), protrusion (4.01 ± 1.7 mm to 5.36 ± 1.32 mm), and pain perception (4.56 ± 2.045 to 2.03 ± 1.5) (P nS therapy enhances mandibular movement and reduces pain, making it a well-tolerated, non-invasive complementary treatment for TMD.

ng>Introduction:ng> The glycemic control of type 2 diabetes is a very important challenge that requires a multidisciplinary group of specialists, an adequate psychosocial environment and family support and searching for better measures for the control and prevention of complications, which encourages the patient to improve his lifestyle. ng>Method: ng>The analysis was made with two primary care medical units, the Mutual Aid Group (GAM) from the Los Pinos Medical center, and the Chronic disease Specialized Medical Unit (UnEME EC), which provided integral care in the municipality of San Cristóbal de Las Casas, Chiapas. A sample of 138 patients was obtained through a random sampling with 95% confidence and a margin of error of ±5. The recollection of data was made with the application of the questionnaire “Lifestyle and adherence to pharmacological treatment in patients with type 2 diabetes, which consists of four sections that evaluate 1) socio-demographic data;2) anthropometric and clinical;Pharmacological adherence with Morisky’s 8-item questionnaire (MMSA-8);3) Lifestyle: with the instrument to Measure Lifestyle in Diabetics (IMEVID). ng>Results:ng> 37.6% had an HbA1c lesser than 7%, A prevalence of 50.7% was obtained in patients adhering to the pharmacological treatment, and 39.8% patients with a favorable lifestyle. The association of the patients’ results with an adequate glycemic control demonstrated by an HbA1C lesser than 7% was obtained through Student’s T test in which these patients are related with a better score on questionnaires by IMEVID and MMAS-8. ng>Conclusions:ng> The units with integral care and multidisciplinary treatment are fundamental in the care of patients with type 2 diabetes;the use of questionnaires such as IMEVID or MMAS-8 is useful in the daily clinical practice.

To the Editor:We sincerely applaud the work done by Gupta et al.,in which they analyzed a single-center experience regarding gallbladder per-foration(GBP)management[1].Currently,there is a lack of cohort studies to adequately describe the approach strategies and man-agement for this pathology,with international guidelines making ambiguous recommendations[2,3].Their study stratified GBP types and provided details regarding clinical presentation,comorbidities,preoperative diagnosis,etiology,imaging techniques,and surgical *** study reported a high conversion rate when a la-paroscopic approach was attempted for cholecystectomy.

Various surgical techniques and approaches have been described to repair cleft nose deformities. It is necessary to consider that since it is a congenital deformity, surgical management must consider the patient’s growth process, as well as the healing itself from the surgery. The present study aims to evaluate an alternative solution to secondary rhinology, aesthetic, and functional alterations to unilateral or bilateral cleft lift palate to minimize deformities and ensure good results. 11 patients were studied between 1995 to 2002, ten male and one female. In 8 cases, the patients had a history of cleft lip and palate on the left side, 2 patients with cleft lift palate on the right, and 1 patient with bilateral cleft lip and palate. 100% of the patients increased their naso-labial angle. This improved their appearance and structure, starting from a preoperative arithmetic mean of 39 degrees to a postoperative arithmetic mean of 96 degrees. Such intervention increased the naso-labial angle by 57 degrees. At the base of the nose, it was possible to improve the inclination of the alar line (line B) in 10 of the cases with a variation of 2 to 3 mm in relation to the perpendicular line A and only one case remained with the same inclination.

Pancreatic Pseudocyst Rupture: Cause of Acute Abdomen

Case report of a 23-year-old male patient with a pancreatic pseudocyst, initially was treated by the Interventional Radiology service by placing percutaneous drainage with poor evolution, abruptly presented abdominal pain, Systemic Inflammatory Response Syndrome and peritoneal irritation, so an urgent exploratory laparotomy was performed with a finding of rupture of the pancreatic pseudocyst into the abdominal cavity. He presented septic shock and died in the immediate postoperative period. Computerized Axial Tomography represents the ideal diagnostic method. Spontaneous rupture is extremely rare and requires urgent surgical intervention.

One of the most prominent features of quantum entanglement is its invariability under local unitary transformations,which implies that the degree of entanglement or nonseparability remains constant during free-space propagation,true for both quantum and classically entangled *** we demonstrate an exception to this rule using a carefully engineered vectorial light field,and we study its nonseparability dynamics upon free-space *** show that the local nonseparability between the spatial and polarization degrees of freedom dramatically decays to zero while preserving the purity of the state and hence the global *** show this by numerical simulations and corroborate it *** results evince novel properties of classically entangled modes and point to the need for new measures of nonseparability for such vectorial fields,while paving the way for novel applications for customized structured light.

Pseudoaneurisms are arterial wounds that affect all layers of an artery;although very rare they are mostly lethal and most of them are iatrogenic. Herein, we present two different cases of hepatic artery pseudoaneurysms derived from previous surgeries which were assessed by computed tomography angiographies and consequent conservative endovascular approaches with arteriographies, posterior cannulations and the placement of Amplatzer Vascular Plug 4 (AVP 4) systems in order to treat the pseudoaneurysm. Computed control tomographies were conducted showing no later complications. The purpose of this article is to demonstrate the importance and current clinical applications of AVP in the field of interventional radiology.

Effect of Pregabalin and Gabapentin on nociceptive Behaviors Induced by Spinal nerve Ligation

Pain is defined as an unpleasant sensory and emotional experience, associated with actual or potential tissue damage. According to its neurobiological mechanism, pain is classified into nociceptive, inflammatory, dysfunctional, and neuropathic. neuropathic pain (nP) is caused by a lesion or disease of the somatosensory nervous system. Both pregabalin and gabapentin are pharmaceuticals used as validation drugs in experimental models of nP. Pregabalin was shown to produce significant antihyperalgesic and antiallodynic effects. Gabapentin is used as a reference compound for new analgesics and reduces tactile allodynia in rats. The aim of this work is to evaluate pregabalin and gabapentin effects on nociceptive behaviors induced by spinal nerve ligation (SnL). Female Wistar rats of 140 - 160 g were used, divided into five groups: naive, SHAM, SnL rats treated with saline solution, SnL rats treated with pregabalin 30 mg/kg p.o., SnL rats treated with gabapentin 300 mg/kg p.o. nociceptive behaviors were determined by the up and down method. In the establishment of SnL-induced allodynic behavior, a reduction in paw withdrawal threshold was observed in the time course, which was present from day 1 and it was maintained for 28 days post-ligation. With the administration of pregabalin and gabapentin, anti-allodynic behavior was observed in the time course and in the areas under the curve (AUC) of the time course of anti-allodynic behavior, significant difference was observed between pregabalin, and gabapentin groups compared to vehicle with a value of p n and gabapentin induce an antinociceptive effect in rats subjected to SnL.

Risk Groups and Mortality in Patients with SARS-CoV-2 from Family Medicine Unit no. 43 in Southeastern Mexico

SARS-CoV-2 infection is a viral disease that causes respiratory infections that can cause multiple complications. Patients with adjacent morbidities have a higher chance of complications and mortality. The national Health and nutrition Survey (EnSAnUT) in 2018 reported that in Mexico, 75.2% of adults aged 20 years and over are overweight and obese, both situations are closely related to the cases reported as severe COVID and even with a higher risk of mortality. The main objective was to evaluate the risk and mortality groups in patients with SARS-CoV-2 from family medicine in the south of México. The study was carried out during the 2020 year, in the Family Medicine Unit no. 43 of Villahermosa, Tabasco, a cross-sectional, retrospective investigation was carried out with an analytical approach, and the sample was obtained using the formula of n for infinite populations that correspond to 215, patients were included who met the selection criteria;3 groups were integrated: DM2, HAS and obesity. Descriptive statistics andχ2 formula were used, using the SPSS version 25 Windows program. The female gender presented the highest number of infections 122 (56.7%) and mortality: 5 (2.32%), obesity comorbidity presented the highest number of infections: 194 (90.2%), HAS: 153 (71.2%), and MD: 44 (20.46%). Complications were observed in 23 (10.69%), distributed in: SIRA: 7 (3.3%), multiple organ failure: 6 (2.8%), heart failure: 6 (2.8%) and pneumonia 4 (1.9%). The observed mortality was 8 (3.72%). The comorbidities of patients with the highest mortality were: SAH: 6 (75%) (χ2 = 9.169;df = 1;p = 0.010) and obesity: 6 (75%) (χ2 = 9.169;df = 1;p = 0.010). The group with the highest risk for SARS-CoV-2 was patients with overweight/obesity comorbidities. Mortality was 3.7%. The patients who died had at least one or more comorbidities. The main complications were ARIS, heart failure, and multiple organ failure.
