Impact of prehospital medical evacuation (MEDEVAC) transport time on combat mortality in patients with noncompressible torso injury and traumatic amputations: a retrospective study

Background: In combat operations, patients with traumatic injuries require expeditious evacuation to improve survival. Studies have shown that long transport times are associated with increased morbidity and mortality. Limited data exist on the influence of transport time on patient outcomes with specific injury types. The objective of this study was to determine the impact of the duration of time from the initial request for medical evacuation to arrival at a medical treatment facility on morbidity and mortality in casualties with traumatic extremity amputation and noncompressible torso injury(NCTI).Methods: We completed a retrospective review of MEDEVAC patient care records for United States military personnel who sustained traumatic amputations and NCTI during Operation Enduring Freedom between January 2011 and March 2014. We grouped patients as traumatic amputation and NCTI(AMP+NCTI), traumatic amputation only(AMP),and neither AMP nor NCTI(Non-AMP/NCTI). Analysis was performed using chi-squared tests, Fisher's exact tests,Cochran-Armitage Trend tests, Shapiro-Wilks tests, Wilcoxon and Kruskal-Wallis techniques and Cox proportional hazards regression ***: We reviewed 1267 records, of which 669 had an injury severity score(ISS) of 10 or greater and were included in the analysis. In the study population, 15.5% sustained only amputation injuries(n=104, AMP only), 10.8% sustained amputation and NCTI(n=72, AMP+NCTI), and 73.7% did not sustain either an amputation or an NCTI(n=493,Non-AMP/NCTI). AMP+NCTI had the highest mortality(16.7%) with transport time greater than 60 min. While the AMP+NCTI group had decreasing survival with longer transport times, AMP and Non-AMP/NCTI did not exhibit the same ***: A decreased transport time from the point of injury to a medical treatment facility was associated with decreased mortality in patients who suffered a combination of amputation injury and NCTI. Background: Dietary supplement use(protein/amino acids, weight-loss supplements, performance enhancers) is common among U.S. military members. Reported dietary supplement use in deployed troops is limited and is of concern in settings where troops are exposed to high ambient temperatures, increased physical demands, and dehydration. Our objective was to describe dietary supplement use and adverse events(AEs) among deployed U.S. service members compared with their pre-deployment ***: We conducted an institutional review board(IRB) approved, descriptive study in Afghanistan using a written questionnaire and collected demographic information, dietary supplement use before and during deployment, AEs associated with supplement use, and physical workout routines. Participants were U.S. military personnel of all branches of service deployed to Afghanistan. They were recruited in high-traffic areas in the combat theater. We analyzed the data with descriptive statistics. Paired t-test/Wilcoxon signed-rank test was conducted to examine the before/during deployment changes for continuous data, and Mc Nemar's chi-square test was conducted for categorical data. We constructed separate logistic regression models to determine the best predictors of increases or decreases in dietary supplement use, with demographic information, reasons for using supplements, and education requested/received as covariates in each model. All statistical tests were two-sided at a significance level of 5%(P<0.05).Results: Data were collected on 1685 participants. Ninety-seven of the participants were in the Army or Air Force. The participants were more likely to work out daily or more than once a day during deployment. Thirty-five percent of the participants reported no supplement use before or during deployment. Oncogenic role of leptin and Notch interleukin-1 leptin crosstalk outcome in cancer

Obesity is a global pandemic characterized by high levels of body fat(adiposity) and derived-cytokines(i.e., leptin). Research shows that adiposity and leptin provide insight on the link between obesity and cancer progression. Leptin's main function is to regulate energy balance. However, obese individuals routinely develop leptin resistance, which is the consequence of the breakdown in the signaling mechanism controlling satiety resulting in the accumulation of leptin. Therefore, leptin levels are often chronically elevated in human obesity. Elevated leptin levels are related to higher incidence, increased progression and poor prognosis of several human cancers. In addition to adipose tissue, cancer cells can also secrete leptin and overexpress leptin receptors. Leptin is known to act as a mitogen, inflammatory and pro-angiogenic factor that induces cancer cell proliferation and tumor angiogenesis. Moreover, leptin signaling induces cancer stem cells, which are involved in cancer recurrence and drug resistance. A novel and complex signaling crosstalk between leptin, Notch and interleukin-1(IL-1) [Notch, IL-1 and leptin crosstalk outcome(NILCO)] seems to be an important driver of leptin-induced oncogenic actions. Leptin and NILCO signaling mediate the activation of cancer stem cells that can affect drug resistance. Thus, leptin and NILCO signaling are key links between obesity and cancer progression. This review presents updated data suggesting that adiposity affects cancer incidence, progression, and response to treatment. Here we show data supporting the oncogenic role of leptin in breast, endometrial, and pancreatic cancers.

The correct use of information in science and technology is very important for its progress. Nowadays, the equipment used for the scientific and technological development provides results that are later interpreted by the researchers, in most of the above mentioned equipment the results are images full of information which has to be analyzed. A powerful stage with multiple benefits in this field is the image pre-processing by means of intelligent systems, which are capable to do image analysis throwing very useful results that enhance the scientific and technological information. There are currently more than 500 functions in the computational vision specialized open source library OpenCV, which associated with the C＋＋ programming language. These functions are used for application development in many areas of computer vision such as products inspection, medical images, safety, user＇s interfaces, camera calibration, stereoscopic vision and robotics. In this development and research work, by using the available functions and modifying the exposed methods, we present a proposal for signal detection in images originated in the transmission electron microscope （known as diffraction patterns）, which are attached to the detailed analysis of crystalline structures used in the study of the materials science, the results show a profit of at least 18% in the detection of signs by means of the method proposed in this work.

Energy Efficiency Improvement of an Industrial crystallization Process Using Linearizing Control

This paper illustrates the benefits of a multivariable linearizing control approach applied to an industrial crystallization process. This relevant approach is declined according to two different strategies: first, a setpoint tracking is proposed for the couple crystal mass/concentration, whereas a second way consists in tracking of crystal content and concentration. The controlled variables, unavailable online, are issued from an observer developed in previous works. The performance of these strategies, which application to cane sugar crystallization constitutes a real novelty, are compared with experimental data issued from a PID-controlled industrial plant. The results reveal a significant improvement of energy efficiency, leading to an economy of more than 10% of energy.

Thermoluminiscence Response of an Alloy Quaternary Ionic crystal Exposed to High Energy Radiation

The alkali halide crystals exhibit significant TL （thermoluminescence） properties when exposed to ionizing radiation. The defects in crystals can be produced in high concentration by means of chemical impurities, radiation, mechanical work and others processes. This study is concerned with the TL properties of quaternary mixtures of alkali halide crystals when they are irradiated with beta and gamma-rays. The samples were made mixing KCI, KBr, RbCI, RbBr salts doped with divalent europium （Eu2＋）, by the Czochralski method sintering at 400 ~C during 6 hours in a N2 atmosphere. The samples were exposed to beta and gamma rays. We present the TL glow curves, demonstrating that the nature of the defects in the crystals can be produced by trapping states and recombination mechanisms. The highest temperature TL curves are the most interesting due to possible applications in dosimetry and optical energy storage.

Physical and Chemical Properties and crystal Structure Transformation of Beeswax during Heat Treatment

The object of research is the wax as a waste product of bees in the steppe and mountain areas of Almaty region of Kazakhstan. Purpose: To study the structure parameters and the homologous phase composition, morphology waxes of alpine and mountain areas of Kazakhstan and the establishment temperature-dependent patterns of their atomic-crystalline state. Methods: Radiography, gas chromatography, scanning electron microscopy and chemical methods of separation of multicomponent systems. The results were obtained in the atomic crystal and molecular structures of beeswax depending on the districts collecting materials. The technique of obtaining radiographs after dissolving beeswax in benzene, hexane and xylene was used. Submitted thermo-radiographic study samples were obtained. Batch analysis thermo-radiographs waxes after treatment with benzene, hexane and xylene showed that their structure is essentially different from the source and closer to the spectra of the crystallized wax melted and due to annihilation of stacking faults waxes native atoms after their dissolution in hexane, benzene and xylene and subsequent their recrystallization. The results obtained allow expanding the amount of information on the physico-chemical processes and structural transformations in beeswax as a composite of different genesis biomaterial. Scope: Beekeeping, plant, structural and analytical chemistry, biotechnology and biocomposites.
