PNPLA3 and TM6SF2 polymorphisms in Brazilian patients with nonalcoholic fatty liver disease

BACKGROUND Nonalcoholic fatty liver disease(NAFLD)is becoming the most common chronic liver disease worldwide,with significant morbidity associated with nonalcoholic steatohepatitis(NASH).Genome-wide association studies demonstrated that the variants rs738409 C/G in the PNPLA3 and rs58542926 C/T in the TM6SF2 genes are determinants of inter-individual and ethnicity-related differences in hepatic fat content and NAFLD *** To investigate PNPLA3 and TM6SF2 genotype frequency and their association with NAFLD development and progression in Brazilian *** This cross-sectional case-control study enrolled 285 individuals from the Gastroenterology and Hepatology clinics at a university hospital in *** case patients(n=148)were confirmed to have NAFLD by the identification of hepatic steatosis on ultrasonography and exclusion of other causes of liver *** to the clinical protocol,patients underwent liver biopsy when at high risk for NASH and/or advanced fibrosis(n=65).Steatohepatitis was confirmed in 54 *** who did not have biopsy indication or NASH on histology were considered to have simple steatosis(n=94).The control group(n=137)was selected among patients that attended the Intestinal Disease clinic and was composed of subjects without abnormalities on abdominal ultrasonography and normal liver biochemical *** individuals underwent PNPLA3 and TM6SF2 genotype *** PNPLA3 CC,CG and GG genotype frequencies were 37%,44%and 19%,respectively,in NAFLD patients and were 58%,31%and 10%in controls(P<0.001).In a model adjusted for gender,age,body mass index and type 2 diabetes mellitus,the G allele increased the chance of NAFLD(OR=1.69,95%CI:1.21-2.36,P=0.002)and NASH(OR=3.50,95%CI:1.84-6.64,P<0.001).The chance of NASH was even higher with GG homozygosis(OR=5.53,95%CI:2.04-14.92,P=0.001).No association was found between G allele and the features of metabolic *** histological assessment,PNPLA3 genoty

This study aimed to elaborate on theoretical model of the perceptions, feelings, and meanings of early stress for people with depression. To perform a qualitative study based on the Symbolic Interactionism and Grounded Theory, we interviewed 12 patients diagnosed with depression. It was structured a model based on “walking between traumatic childhood and the adult life with depression,” integrating eight inter-articulated categories that represent the perceptions, feelings, and meanings of the experiences of the depressed individual with a history of early stress. The theoretical model constructed meanings and conditions that affect the individual from a traumatic childhood to depression in adulthood.

BACKGROUND Although the prognostic relevance of sarcopenia has been increasingly recognised in the context of liver disease,there is a paucity of data evaluating body composition in patients with chronic hepatitis B(CHB).Beyond virus-related factors,nutritional and metabolic aspects can be associated with skeletal muscle abnormalities in these patients and should not be *** To evaluate the association between components of sarcopenia and demographic,clinical,lifestyle,nutritional,and biochemical variables in CHB *** Dual-energy X-ray absorptiometry(DXA)was used to assess muscle mass by quantifying appendicular lean mass(ALM)adjusted for body mass index(ALMBMI).Muscle function was evaluated by hand grip strength(HGS)and the timed up and go ***-associated fatty liver disease(MAFLD)was defined according to the criteria proposed by an international expert panel.A body shape index and the International Physical Activity Questionnaire were used to assess central obesity and physical activity level,*** This cross-sectional study included 105 CHB outpatients followed at the tertiary care ambulatory centre(mean age,48.5±12.0 years;58.1%males;76.2%without cirrhosis;23.8%with compensated cirrhosis).The DXA-derived fat mass percentage was inversely correlated with the ALMBMI(r=-0.87)and HGS(r=-0.63).In the multivariable analysis,MAFLD,sedentarism and central obesity were positively and independently associated with low *** and central obesity were independently associated with low *** MAFLD and central obesity were associated with low muscle mass and strength in patients with chronic hepatitis B,independent of the liver disease stage.

Comparative Proteomics Reveals Set of Oxidative Stress and Thaumatin-Like Proteins Associated with Resistance to Late Blight of Tomato

Proteomics techniques were used to study the molecular mechanisms involved in the defense of tomato against late blight (Phytophthora infestans). Proteins were extracted from resistant access BGH-2127 and susceptible cultivar “Santa Clara”. Leaves of the inoculated and non-inoculated (control) genotypes were collected at 0, 2, and 48 h after inoculation and analyzed by two-dimensional electrophoresis (2-DE), followed by identification with mass spectrometry (MALDI TOF-TOF). A total of 56 differentially abundant proteins were identified, of which 39 were resistant genotypes and 17 were susceptible. These proteins were categorized into functional groups of energy and metabolism, photosynthesis, stress and defense, transcription, other proteins, and as un-characterized ones. For access BGH-2127, oxidative stress proteins (2-cis peroxiedoxin BAS1 and 2-cis peroxiredoxin) and thaumatin-like protein showed increase in the relative abundance at 0 and 48 h of inoculation, respectively, and were therefore considered important for the defense mechanism of this genotype. The expression standards evaluated by real-time PCR differed from the results of the proteomic analysis. The protein-protein interaction networks provided important information on the cellular activities involved in the resistance of BGH-2127 late blight.

Rhizopus is fungus of the order Mucorales, responsible for invasive diseases called mucormycosis. The correct identification of these fungi is one of the main questions raised in the literature since the diagnosis is not simple, being the necessary detection in biopsies of infected tissues and isolation of the microorganism for identification by morphological and molecular methods. In vitro susceptibility testing results are not fully clear since there are reports showing that in vitro resistance does not always mean therapeutic failure. This paper describes a case of rhino-orbito-cerebral mucormycosis in a patient with alcoholic chronic pancreatitis and secondary mellitus diabetes involving microbiological diagnostic, antifungal susceptibility tests for conidia and hypha and clinical correlation. The causative agent was identified by morphological characteristics and DNA sequencing. Minimal Inhibitory Concentration (MIC) for amphotericin B (AMB), itraconazole (ITZ) and terbinafine (TERB) was determined by broth microdilution method. Standardization of the dynamic growth evaluation patterns using the automated system BCT (BioCell-Tracer?) was obtained allowing to determining hyphae growth control and evaluating the effect of different concentrations of amphotericin B, itraconazole and terbinafine directly on growing hyphae. The fractional inhibitory concentration (FIC) for the combination of the two antifungals that the patient received at the same time, terbinafine and itraconazole, was also determined trying to correlate with clinical outcome. Strains isolated of retro-orbital abscess were identified as Rhizopus oryzae. MIC results (MIC) testing conidia showed high sensibility to amphotericin B and itraconazole in comparison to the obtained for terbinafine. The tests in the automated system BCT using hyphae showed sensibility to all tested antifungal agents and the FIC results for combination in vitro between terbinafine and itraconazole showed synergism consisten

Hemangioma is the most common benign tumor affecting the liver in 7% of general population. One of the rare presentations of this pathology is gigantic liver hemangioma, reported in the literature as anecdotal cases. In selected cases, liver transplantation has been a therapeutic option. We herein describe a case of massive liver hemangioma which was submitted to liver transplant, and the main indication for liver transplant in this case was a severe psychosocial impact of the disease in the patient’s life, altogether with Kasabach-Merritt syndrome and abdominal discomfort.

In the last decade, Brazil was the scene of two of the world’s biggest disasters related to mining tailings dams. The collapse of the Mariana and Brumadinho dams was catastrophic event that brought to light major environmental and humanitarian crises. Facts like this incite debate on the safety of dam construction methods and highlight the need for a broad discussion of dam safety legislation in the country. From the above, this paper promotes a review of various forms of construction of tailings dams and their associated problems, as well as discusses the techniques for soil reinforcement and their construction methods. The work also brings an overview of the Brazilian dams from April to July 2019, tracing the context of the Brazilian tailings dams months after the tragedy of Brumadinho. The methodology includes a robust literature review and discourse analysis. In Brazil, the number of ruptured dams is alarming, especially in the state of Minas Gerais, in which more than 7 have broken in the last two decades. Among the damages, the deaths impact society abruptly and were cataloged 455 deaths, of which 216 correspond to the rupture of the dam of the Córrego do Feijão mine in Brumadinho. To evaluate and diagnose a dam, one should consider ordinance N ° 70,389, 2017 revoked by Resolution ANM n ° 95, of February 7, 2022. In this context, in March 2019, we experienced several disturbing situations regarding dam instability in the state of Minas Gerais, being categorized as risky structures. To exemplify, one can cite dams classified as level 2 risk that became level 3. Due to the above facts, several studies are being conducted to improve soil aspects. For example, we highlight the increase in soil resistance that can be done using commercial and alternative materials available in the environment.

Acute Appendicitis after Liver Transplant: A Case Report and Review of the Literature

Acute appendicitis is rare in the postoperative period of liver transplantation;only 23 cases were described in the literature to date, including late and immediate postoperative. Our case reports a patient who was presented with acute appendicitis in the immediate post-transplant and died in the subsequent postoperative period. The article reviews the available literature and all cases known until now, commenting on incidence, casual factors, symptoms, diagnostic and management.

Do Cytokines Play a Role in Predicting Some Features and Outcome in Infective Endocarditis?

Objective: This study aimed to measure the serum concentrations of several cytokines in patients with infective endocarditis (IE), and correlate them with the C-reactive protein serum levels, echocardiographic findings and infecting microorganisms. Methods: Forty-five patients with definite IE according to the Duke’s criteria were included in the study. Ten healthy blood donors formed the control group. Serum levels of interleukin (IL) 1β, IL-6, IL-8, IL-10, IL-12 and tumor necrosis factor-α (TNF-α) were measured and compared between the groups, and also with some clinical and laboratory parameters of IE. Results: Patients with IE had significantly higher serum concentrations of the inflammatory mediators than the control patients. Median IL-12 and IL-1β levels were higher in staphylococcal than streptococcal IE. Except for TNF-α, the levels of all the other cytokines correlated with the C-reactive protein concentrations. In multivariate analysis, IL-10 and IL-12 serum levels remained as independent factors associated with the C-reactive protein concentrations. There was a significant correlation between IL-10 concentration and vegetation length. Conclusion: The inflammatory serum cytokines levels were elevated in IE in comparison to healthy controls. C-reactive protein concentrations correlated with cytokines levels. IL-1β and IL-12 serum concentrations were higher in staphylococcal when compared to streptococcal IE. IL-10 levels were associated with vegetation size.

Bromeliaceae are important in ecological restoration, showing easy adaptation to different environments, and constantly interacting with each other through ecological relationships, whose effects may or may not benefit the species involved. The hypothesis tested whether the population density of Hohenbergia augusta is higher in areas at a more advanced successional stage (brown trail), since the species is typical of more structured forest environments, occurring especially in the lower strata of the forest. Six plots of 10 meters (6 × 10 m) were installed, parallel to the brown and yellow trails, and with a distance of 5 m from the edge. Plots were allocated alternately between the two sides of the trail, 10 meters apart from each other when on the same side. In each plot, all H. augusta (Vell.) E. Morren bromeliads were sampled, without a minimum inclusion criterion, provided they were epiphytic phorophytes. The logarithm of the value was used for data normalization and later, the data were tested in a student’s t-test. The study pointed out that the highest population density of H. augusta occurred in the brown trail because the area is in a more advanced successional stage.
