Deep learning (DL) has experienced an exponential development in recent years, with major impact in many medical fields, especially in the field of medical image and, respectively, as a specific task, in the segmentation of the medical image. We aim to create a computer assisted diagnostic method, optimized by the use of deep learning (DL) and validated by a randomized controlled clinical trial, is a highly automated tool for diagnosing and staging precancerous and cervical cancer and thyroid cancers. We aim to design a high-performance deep learning model, combined from convolutional neural network (U-Net)-based architectures, for segmentation of the medical image that is independent of the type of organs/tissues, dimensions or type of image (2D/3D) and to validate the DL model in a randomized, controlled clinical trial. We used as a methodology primarily the analysis of U-Net-based architectures to identify the key elements that we considered important in the design and optimization of the combined DL model, from the U-Net-based architectures, imagined by us. Secondly, we will validate the performance of the DL model through a randomized controlled clinical trial. The DL model designed by us will be a highly automated tool for diagnosing and staging precancers and cervical cancer and thyroid cancers. The combined model we designed takes into account the key features of each of the architectures Overcomplete Convolutional Network Kite-Net (Kite-Net), Attention gate mechanism is an improvement added on convolutional network architecture for fast and precise segmentation of images (Attention U-Net), Harmony Densely Connected Network-Medical image Segmentation (HarDNet-MSEG). In this regard, we will create a comprehensive computer assisted diagnostic methodology validated by a randomized controlled clinical trial. The model will be a highly automated tool for diagnosing and staging precancers and cervical cancer and thyroid cancers. Rationale and Objectives: Accurately establishing the diagnosis and staging of cervical and thyroid cancers is essential in medical practice in determining tumor extension and dissemination and involves the most accurate and effective therapeutic approach. For accurate diagnosis and staging of cervical and thyroid cancers, we aim to create a diagnostic method, optimized by the algorithms of artificial intelligence and validated by achieving accurate and favorable results by conducting a clinical trial, during which we will use the diagnostic method optimized by artificial intelligence (AI) algorithms, to avoid errors, to increase the understanding on interpretation computer tomography (CT) scan, magnetic resonance imaging (MRI) of the doctor and improve therapeutic planning. Materials and Methods: The optimization of the computer assisted diagnosis (CAD) method will consist in the development and formation of artificial intelligence models, using algorithms and tools used in segmental volumetric constructions to generate 3D images from MRI/CT. We propose a comparative study of current developments in “DICOM” image processing by volume rendering technique, the use of the transfer function for opacity and color, shades of gray from “DICOM” images projected in a three-dimensional space. We also use artificial intelligence (AI), through the technique of Generative Adversarial Networks (GAN), which has proven to be effective in representing complex data distributions, as we do in this study. Validation of the diagnostic method, optimized by algorithm of artificial intelligence will consist of achieving accurate results on diagnosis and staging of cervical and thyroid cancers by conducting a randomized, controlled clinical trial, for a period of 17 months. Rationale and Objectives: Accurate diagnosis and staging of cervical precancers is essential for practical medicine in determining the extent of the lesion extension and determines the most correct and effective therapeutic approach. For accurate diagnosis and staging of cervical precancers, we aim to create a diagnostic method optimized by artificial intelligence (AI) algorithms and validated by achieving accurate and favorable results by conducting a clinical trial, during which we will use the diagnostic method optimized by artificial intelligence (AI) algorithms, to avoid errors, to increase the understanding on interpretation of colposcopy images and improve therapeutic planning. Materials and Methods: The optimization of the method will consist in the development and formation of artificial intelligence models, using complicated convolutional neural networks (CNN) to identify precancers and cancers on colposcopic images. We will use topologies that have performed well in similar image recognition projects, such as Visual Geometry Group Network (VGG16), Inception deep neural network with an architectural design that consists of repeating components referred to as Inception modules (Inception), deeply separable convolutions that significantly reduce the number of parameters (MobileNet) that is a class of Convolutional Neural Network (CNN), Return of investment for machine Learning (ROI), Fully Convolutional Network (U-Net) and Overcomplete Convolutional Network Kite-Net (KiU-Net). Validation of the diagnostic method, optimized by algorithm of artificial intelligence will consist of achieving accurate results on diagnosis and staging of cervical precancers by conducting a randomized, controlled clinical trial, for a period of 17 months. Deep learning (DL) has seen an exponential development in recent years, with major impact in many medical fields, especially in the field of medical image. The purpose of the work converges in determining the importance of each component, describing the specificity and correlations of these elements involved in achieving the precision of interpretation of medical images using DL. The major contribution of this work is primarily to the updated characterisation of the characteristics of the constituent elements of the deep learning process, scientific data, methods of knowledge incorporation, DL models according to the objectives for which they were designed and the presentation of medical applications in accordance with these tasks. Secondly, it describes the specific correlations between the quality, type and volume of data, the deep learning patterns used in the interpretation of diagnostic medical images and their applications in medicine. Finally presents problems and directions of future research. Data quality and volume, annotations and labels, identification and automatic extraction of specific medical terms can help deep learning models perform image analysis tasks. Moreover, the development of models capable of extracting unattended features and easily incorporated into the architecture of DL networks and the development of techniques to search for a certain network architecture according to the objectives set lead to performance in the interpretation of medical images.

Aquaporin 5 in Alzheimer’s disease:a link between oral and brain pathology?

The involvement of aquaporins(AQPs)in the development of diseases has been widely described(Azad et al.,2021).AQP5 has been described in astrocytes changing after traumatic brain injuries(Chai et al.,2013),but the precise role of AQP5 in Alzheimer’s disease(AD)pathology is yet to be understood.We have recently reported that AQP5 expression changes during the development of AD(Antequera et al.,2022).The AQP5 expression in salivary glands is decreased in 6-month-old APP/PS1 mice and AD patients.This decrease in AQP5 expression could be involved in the mechanism of salivary gland dysfunction described in a previous study(Antequera et al.,2021).Now,we propose a new indirect role of AQP5 in the connection between infection-induced oral dysbiosis and AD(Sureda et al.,2020).Here,we suggest that the proinflammatory response induced by oral pathogen infection results in the downregulation of AQP5 contributing to the salivary gland secretory dysfunction.All these alterations destabilize the peripheral immune-inflammatory balance and exacerbate neuroinflammation and neurodegeneration leading to AD pathology.

This article describes the practice of co-evolutionary and transformative resilience through a case study con-ducted in Turin(Italy).According to a broad definition,resilience includes performing actions of urban design and planning,innovating community-based project procedures,and creating positive financial outcomes that are assessable because of the monitoring process of short-and long-term outcomes and impacts.Through the Turin-based case of the Bottom Up!Architecture Festival,this article observes processes in which resilience is in action in metropolitan areas,feeding urban projects and practices of self-organization of the social and financial actors involved.By applying the definition of community projects,the festival manages to take territorial problems and crises(the pandemic,inequality,etc.)and view them as an opportunity to change the system,recommending in-tegrated action on the natural,cultural,financial,and social capital,innovating practices and holding society and institutions more accountable.The transformation of spaces relies on collaborations between social and institu-tional actors,operating spatially concentrated transformations in the city of Turin,and using flexible governance tools based on co-planning and crowdfunding for project design and financing.

α-Synuclein oligomers and fibrils:partners in crime in synucleinopathies

The misfolding and aggregation of a-synuclein is the general hallmark of a group of devastating neurodegenerative pathologies referred to as synucleinopathies,such as Parkinson’s disease,dementia with Lewy bodies,and multiple system atrophy.In such conditions,a range of different misfolded aggregates,including oligomers,protofibrils,and fibrils,are present both in neurons and glial cells.Growing expe rimental evidence supports the proposition that solu ble oligomeric assemblies,formed during the early phases of the aggregation process,are the major culprits of neuronal toxicity;at the same time,fibrillar confo rmers appear to be the most efficient at propagating among interconnected neurons,thus contributing to the spreading ofα-synuclein pathology.Moreover,α-synuclein fibrils have been recently repo rted to release soluble and highly toxic oligomeric species,responsible for an immediate dysfunction in the recipient neurons.In this review,we discuss the current knowledge about the plethora of mechanisms of cellular dysfunction caused byα-synuclein oligome rs and fibrils,both contributing to neurodegeneration in synucleinopathies.

Lactoferrin is an antimicrobial prote in characterized by the exertion of many protective functions,including antibacterial,antifungal,antiviral,and antiparasitic properties,as well as anti-inflammatory and immunomodulatory activities(Kruzel et al.,2017).Lactoferrin is one of the major proteins present in exocrine secretions,including saliva,and is therefore associated with host defense against oral pathogens and control of the oral microbiome.In recent years,it has become clear that alterations in the oral microbiome may contribute to opportunistic pathogen infections in the brains of Alzheimer’s disease(AD)patients and thus participate in or contribute to the development of this neurodegenerative disease(Sureda et al.,2020).Pathogenic oral microbes can affect neurological processes by entering brain tissue through various pathways and directly damaging the central nervous system.In the central nervous system,oral microbes may trigger an immune response that increases amyloidβ(Aβ)production and may even trigger the Aβcascade to promote the onset of AD,as we discuss in our previous study supporting the“infectious hypothesis”in AD(González-Sánchez et al.,2020).

Introduction:Parkinson's disease PD is a common neurodegenerative disorder mainly characterized by the progressive decline of motor function with a prevalence that is greatly increasing since the pathology is principally driven by aging.The currently available treatments only mitigate motor symptoms;therefore,a great deal of studies are aimed at finding novel drugs that are able to arrest the evolution of the disease.The progressive death of nigrostriatal dopaminergic(DA)neurons and the formation of the typical Lewy bodies neuronal inclusions containing misfolded alpha-synuclein are two key histopathological features of the disease.However,it is now evident that PD has to be considered as a heterogeneous disorder that includes subtypes with distinctive clinical and biological characteristics(Ryden and Lewis,2019).

Uncovering the multifaceted roles played by neutrophils in allogeneic hematopoietic stem cell transplantation

Allogeneic hematopoietic stem cell transplantation(alloHSCT)is a life-saving procedure used for the treatment of selected hematological malignancies,inborn errors of metabolism,and bone marrow failures.The role of neutrophils in alloHSCT has been traditionally evaluated only in the context of their ability to act as a first line of defense against infection.However,recent evidence has highlighted neutrophils as key effectors of innate and adaptive immune responses through a wide array of newly discovered functions.Accordingly,neutrophils are emerging as highly versatile cells that are able to acquire different,often opposite,functional capacities depending on the microenvironment and their differentiation status.Herein,we review the current knowledge on the multiple functions that neutrophils exhibit through the different stages of alloHSCT,from the hematopoietic stem cell(HSC)mobilization in the donor to the immunological reconstitution that occurs in the recipient following HSC infusion.We also discuss the influence exerted on neutrophils by the immunosuppressive drugs delivered in the course of alloHSCT as part of graft-versushost disease(GVHD)prophylaxis.Finally,the potential involvement of neutrophils in alloHSCT-related complications,such as transplant-associated thrombotic microangiopathy(TA-TMA),acute and chronic GVHD,and cytomegalovirus(CMV)reactivation,is also discussed.Based on the data reviewed herein,the role played by neutrophils in alloHSCT is far greater than a simple antimicrobial role.However,much remains to be investigated in terms of the potential functions that neutrophils might exert during a highly complex procedure such as alloHSCT.
