Post-Obstructive Diuresis: Physiopathology, Diagnosis and Management after Urological Treatment of Obstructive Renal Failure

Obstructive acute renal failure is both a medical and a surgical emergency. The first treatment modality is urological by relief of obstruction. Post-obstructive diuresis is an abnormal condition of prolonged polyuria, involving both excessive solute and water loss, after acute drainage of obstructed urinary tract system. Physiopathology mechanisms are multiples. In most patients, diuresis will resolve once the kidneys normalize the volume and solute status and homeostasis is achieved. Post-obstructive diuresis can occur in up to 50% of patients with substantial urinary tract obstruction and can be life-threatening if it becomes pathologic. It can be detected by hourly monitoring of diuresis. Medical treatment of post-obstructive diuresis consists of oral or intravenous fluids adjusted to the findings of clinical examination, diuresis (volume and electrolytes) and close monitoring of patient. Fluid compensation should be tapered off over several days. Patients at high risk of post-obstructive diuresis should be identified and appropriately monitored.

Over the past decade extensive development of advanced high strength steel (AHSS) was driven by the demand from the automotive industry for stronger materials that can enable lightweighting to meet increasing fuel efficiency requirements. However, achievement of higher strength in many AHSS grades comes with reductions in ductility, leading to geometric constraints on formability and limiting their application. In this paper, a 3rd Generation AHSS with a compelling property combination of high tensile strength of ~1200 MPa and total elongation > 40% was used for laboratory and stamping studies. Various auto related laboratory tests were done including tensile testing, 180 degree bending, bulge testing, and cup drawing to estimate the steel’s formability under different applied conditions. Additionally, since laboratory testing provides only an estimation of the potential stamping response, the 3rd Generation AHSS sheet was stamped into b-pillars under industrial stamping conditions. Non-destructive and destructive analysis of the resulting stampings were done to evaluate the microstructural and property changes occurring during stamping. Significant strengthening of material in the stamped part is attributed to the structural changes through the complex Nanophase Refinement and Strengthening mechanism.

目的分析1995-2005年青海省人与动物棘球蚴病的流行病学调查结果。方法人群棘球蚴病感染情况以bu-ELISA、EM18-ELISA和b超、X线进行检查和评价。动物棘球蚴病/棘球绦虫感染调查采用解剖学方法和寄生虫学方法。结果①女性人群的血清阳性率和患病率显著高于男性人群(14.41%和4.81%、10.22%和3.25%);随年龄增长,人群血清阳性率和患病率升高(见表5);以牧民、喇嘛的血清阳性率和患病率最高(17.73%和9.33%、18.56%和10.0%)。②青海省人与动物棘球蚴病/棘球绦虫的血清阳性率和感染/患病率以青南高原的果洛、玉树、黄南三州最高(见表1、7、8),祁连山地和河湟谷地的海南、海北两州次之,海东地区、西宁及柴达木盆地的海西州较低。③青海省是以囊型棘球蚴病/细粒棘球绦虫为主的囊型和泡型棘球蚴病的混合流行区,并在其南部发现存在石渠棘球绦虫的动物感染。结论青海省棘球绦虫的生活史循环链十分复杂,家养动物相互之间、野生动物相互之间、家养动物和野生动物之间均参与其中。

Sex dimorphism and gene expression were studied in developing catkins in 159 F 2 individuals from the bioenergy crop Salix purpurea,and potential mechanisms and pathways for regulating sex development were *** expression,eQTL,bisulfite sequencing,and network analysis were used to characterize sex dimorphism,detect candidate master regulator genes,and identify pathways through which the sex determination region(SDR)may mediate sex *** genes are presented as candidates for master regulators of sex,supported by gene expression and network *** include genes putatively involved in hormone signaling,epigenetic modification,and regulation of *** analysis revealed a suite of transcription factors and genes involved in secondary metabolism and floral development that were predicted to be under direct control of the sex determination ***,data from bisulfite sequencing and small RNA sequencing revealed strong differences in expression between males and females that would implicate both of these processes in sex dimorphism *** data indicate that the mechanism of sex determination in Salix purpurea is likely different from that observed in the related genus *** further demonstrates the dynamic nature of SDRs in plants,which involves a multitude of mechanisms of sex determination and a high rate of turnover.

目的　了解乙型肝炎病毒 (HbV)前C区基因突变与HbeAg阴性的原发性肝癌患者HbV感染的关系。方法　应用套式PCR方法对广西 16例原发性肝癌患者血清HbVDNA前C区进行扩增 ,对阳性者用SangerDNA测序法分析。结果　 16例肝癌患者中有 14例HbVDNA阳性 ,阳性率为87 5 % (14/16 )。其中 1例HbeAg阳性 ,1例虽然HbeAg为阴性 ,但其前C区序列正常 ;2例为HbV野毒株和突变株混合感染 ;其余 8例为HbV突变株感染。该 8例中 ,仅第 4、5、12例的第 2 8密码为终止密码 ,有 1例为HbV终止密码株和无终止密码突变株混合感染。结论　经典的 1896位核苷酸点突变株感染在广西原发性肝癌患者中并不常见 ,提示可能存在前C区以外的基因突变 ,并导致HbeAg阴性的HbV感染。
