Background. Dysp.reunia is frequently associated with a p.ychiatric origin, p.rticularly in p.tients with no obvious Sjogrensp. p.Sjogrensp. p.Sjogrensp. p.Sjogrensp.¨ p.Sjogrensp.vulvovaginal or p.lvic disease. The aim of this study was to assess the frequency of dry syndrome in p.tients with dysp.reunia and to evaluate the main clinical and biological features and follow-up.data for women with dysp.reunia and dry syndrome. p.tients and methods. Twenty-two p.tients p.esenting chronici diop.thic dysp.reunia (without clear vulvovaginal dermatosis or infection) were included in this retrosp.ctive study. All p.tients underwent history-taking, gynecological examination, a Schirmer tear test, a sugar test, labial salivary gland biop.y assessment and immunological examination. A diagnosis of syndrome was considered where histological examination of the salivary glands showed a lymp.ocyte infiltration corresp.nding to stage 3 or 4 in the Chisholm classification. Diagnosis of dry syndrome without syndrome was made in p.tients with xerostomia and/or xerop.thalmia without a sp.cific histological p.cture of syndrome or immunological abnormalities at salivary gland biop.y. Results. Based on our criteria, 10 p.tients (45% ) had dry syndrome, including 4 with syndrome and 6 with dry syndrome without syndrome. 9 of these 10 p.tients p.esented either xerostomia (7 cases) and/or xerop.thalmia (7 cases). Vaginal dryness was rep.rted by 3 of the 10 women with dry syndrome but also by 4 of 12 women without dry syndrome. Examination of the vulva showed no p.rticular clinical features and treatment with an emollient was not effective in all cases. Discussion. This study showed a high frequency of dry syndrome in p.tients with chronic “ idiop.thic” dysp.reunia. Background. We rep.rt a case of necrolytic migratory erythema in a p.tient with Waldmann’s disease. p.tients and methods. A 55-year-old male p.tient with a history of Waldmann’s disease was hosp.talized for a rash on the trunk and limbs comp.ising annular p.lycyclic lesions with p.rip.eral scaling evocative of necrolytic migratory erythema. High-p.otein and fatty-acid-sup.lemented p.renteral feeding led to rap.d imp.ovement of the p.tient’s cutaneous lesions. Discussion. Waldmann’s disease is characterized by intestinal lymp.atic abnormalities leading to exudative intestinal disease causing p.otein loss in the bowel lumen and deficient fatty acid absorp.ion. The p.thogenesis of necrolytic migratory erythema is not fully understood. Increased serum glucagon does not ap.ear to be the only mechanism involved. The occurrence of necrolytic migratory erythema in a p.tient with Waldmann’s disease sup.orts the current p.ysiop.thological hyp.thesis of the role of decreased p.asma p.otein and amino acid levels in necrolytic migratory erythema.

Introduction. The drug-induced hyp.rsensitivity syndrome or DRESS (drug reaction with eosinop.ilia and systemic symp.oms) is a severe toxiderma because it is accomp.nied by lethal visceral involvement in 6 to 10%of cases. Its p.ysiop.thology remains unclear. In order to sp.cify the immunological characteristics of this toxiderma we analyzed, p.osp.ctively, the rearrangement of the blood and cutaneous T-cell lymp.ocyte recep.or (TCR) genes of p.tients exhibiting a drug-induced hyp.rsensitivity syndrome between Ap.il 1998 and Ap.il 2000. p.tients and methods. The inclusion criteria were: age over 18 years, occurrence of a drug-induced generalized erup.ion, existence of associated systemic involvement (lymp. node or visceral),and p.esence of hyp.reosinop.ilia greater than0.5G/l and/or circulating atyp.cal lymp.ocytes. Six p.tients (3 men and 3 women), with a mean age of 54 years were included. The imp.table drug was an anti-seizure in 3 cases, allop.rinol in 2 and oxazep.m in one. Remission occurred within a delay of 10 to 30 days after the acute p.ase. Two p.tients p.esented several flares. Results. No clonal rearrangement in TCR genes was detected in the cutaneous samp.es. A clonal rearrangement of TCR genes was initially detected in the blood lymp.ocytes of 3 out of the 6 p.tients (allop.rinol: n=2 and oxazep.m: n=1). The latter remained detectable during the evolution, during the second or third flare of the drug-induced hyp.rsensitivity in 2 p.tients (allop.rinol: n=1 and oxazep.m: n=1). Discussion. The p.esence of circulating T-cell clones detectable for several months after the occurrence of a drug-induced hyp.rsensitivity shows the mono or oligoclonal exp.nsion of activated T-cells, induced by the drug imp.ted. Their p.rsistence over several months corresp.nds to a remnant activation of the immune systemthatcanexp.ainthep.olongedand/orrecurrentevolutionof the drug-induced hyp.rsensitivity syndrome in some p.tients.

Introduction. Hyp.p.gmented mycosis fungoides is a rare clinical form of the disease, first described in 1978. Since then, only a hundred odd cases have been documented. Case rep.rt. A young 19 year-old African woman had p.esented with hyp.chromatic macules since the age of 9 and for which the diagnostic enquiry had finally concluded in hyp.p.g mentedmycosis fungoides. Discussion. The p.rticularities of this form of mycosis fungoides, grade I according to the TNM classification, are p.incip.lly its onset in black-skinned p.rsons or of Asian origin, and the age of early onset with a p.edominance of p.diatric cases. Its course is indolent for several years and thus source of delayed diagnosis. The differential diagnoses to be evoked are basically vitiligo,achromate eczematides and p.rap.oriasisis. The p.st-inflammatory dep.gmentation, frequent in black-skinned subjects, is only an eliminating diagnosis. Its treatment is that of classical Grade I mycosis fungoides:top.cal mechlorethamine, p.ototherap. and top.cal corticosteroids.
