Advanced hepatocellular carcinoma and sorafenib: Diagnosis, indications, clinical and radiological follow-up

Advanced stage hepatocellular carcinoma(HCC) is a category of disease defined by radiological, clinical and hepatic function parameters, comprehending a wide range of patients with different general conditions. The main therapeutic option is represented by sorafenibtreatment, a multi-kinase inhibitor with anti-proliferative and anti-angiogenic effect. Trans-arterial Radio Embolization also represents a promising new approach to intermediate/advanced HCC. Post-marketing clinical studies showed that only a portion of patients actually benefits from sorafenib treatment, and an even smaller percentage of patients treated shows partial/complete response on follow-up examinations, up against relevant costs and an incidence of drug related adverse effects. Although the treatment with sorafenib has shown a significant increase in mean overall survival in different studies, only a part of patients actually shows real benefits, while the incidence of drug related significant adverse effects and the economic costs are relatively high. Moreover, only a small percentage of patients also shows a response in terms of lesion dimensions reduction. Being able to properly differentiate patients who are responding to the therapy from non-responders as early as possible is then still difficult and could be a pivotal challenge for the future; in fact it could spare several patients a therapy often difficult to bear, directing them to other second line treatments(many of which are at the moment still under investigation). For this reason, some supplemental criteria to be added to the standard modified Response Evaluation Criteria in Solid Tumors evaluation are being searched for. In particular, finding some parameters(cellular density, perfusion grade and enhancement rate) able to predict the sensitivity of the lesions to anti-angiogenic agents could help in stratifying patients in terms of treatment responsiveness before the beginning of the therapy itself, or in the first weeks of soraf

Differential diagnosis of brainstem lesions,either isolated or in association with cerebellar and supra-tentorial lesions,can be challenging. Knowledge of the structural organization is crucial for the differential diagnosis and establishment of prognosis of pathologies with involvement of the brainstem. Familiarity with the location of the lesions in the brainstem is essential,especially in the pediatric population. Magnetic resonance imaging(MRI) is the most sensitive and specific imaging technique for diagnosing disorders of the posterior fossa and,particularly,the brainstem. High magnetic static field MRI allows detailed visualization of the morphology,signal intensity and metabolic content of the brainstem nuclei,together with visualization of the normal development and myelination. In this pictorial essay we review the brainstem pathology in pediatric patients and consider the MR imaging patterns that may help the radiologist to differentiate among vascular,toxico-metabolic,infectiveinflammatory,degenerative and neoplastic processes. Helpful MR tips can guide the differential diagnosis: These include the location and morphology of lesions,the brainstem vascularization territories,gray and white matter distribution and tissue selective vulnerability.

Many progresses have been done in the management of gastrointestinal(GI) lymphomas during last decades, especially after the discovery of Helicobacter pylori-dependent lymphoma development. The stepwise implementation of new endoscopic techniques, by means of echoendoscopy or double-balloon enteroscopy, enabled us to more precisely describe the endoscopic features of GI lymphomas with substantial contribution in patient management and in tailoring the treatment strategy with organ preserving approaches. In this review, we describe the recent progresses in GI lymphoma management from disease diagnosis to follow-up with a specific focus on the endoscopic presentation according to the involved site and the lymphoma subtype. Additionally, new or emerging endoscopic technologies that have an impact on the management of gastrointestinal lymphomas are reported. We here discuss the two most common subtypes of GI lymphomas: the mucosaassociated lymphoid tissue and the diffuse large B cell lymphoma. A general outline on the state-of-the-art of the disease and on the role of endoscopy in both diagnosis and follow-up will be performed.

Lesions of the brainstem have been reported in the clinical scenarios of hypoxic-ischemic encephalopathy(HIE), although the prevalence of these lesions is probably underestimated. Neuropathologic studies have demonstrated brainstem involvement in severely asphyxiated infants as an indicator of poor outcome. Among survivors to HIE, the most frequent clinical complaints that may be predicted by brainstem lesions include feeding problems, speech, language and communication problems and visual impairments. Clinical series, including vascular and metabolic etiologies, have found selective involvement of the brainstem with the demonstration of symmetric bilateral columnar lesions of the tegmentum. The role of brainstem lesions in HIE is currently a matter of debate, especially when tegmental lesions are present in the absence of supratentorial lesions. Differential diagnosis of tegmental lesions in neonates and infants include congenital metabolic syndromes and drug-related processes. Brainstem injury with the presence of supratentorial lesions is a predictor of poor outcome and high rates of mortality and morbidity. Further investigation will be conducted to identify specific sites of the brainstem that are vulnerable to hypoxic-ischemic and toxic-metabolic insults.

The paper develops a methodology for the optimal sizing of a generic wind farms, focusing primarily on reliability aspects, besides the traditional economic ones. A specific objective function is proposed in order to select the better wind farm configuration, based upon the profits related to the economic trading in the deregulated electric market and the costs due to investment, operation & management, and to system unavailability. This objective function is accurately investigated as a function of the turbines number in order to derive the most convenient alternative, which implies also the optimal choice of the single wind generators size. The ranking coming out from this assessment is then compared with that one established in terms of expected load not supplied. A compromise choice, between the best alternatives provided by the two criteria has finally adopted. A simple numerical application is reported in the last part of the paper for testing the validity of the proposed approach. Also uncertainty aspects in the basic input parameters are taken into account and possible way of dealing with them is briefly illustrated.

AIM:To evaluate the hospitalization rate of femoralneck fractures in the elderly Italian population over ten ***:We analyzed national hospitalizations records collected at central level by the Ministry of Health from 2000 to ***-and sex-specific rates of fractures occurred at femoral neck in people≥65 years *** performed a sub-analysis over a three-year period(2007-2009),presenting data per five-year age groups,in order to evaluate the incidence of the hip fracture in the oldest ***:We estimated a total of 839008 hospitalizations due to femoral neck fractures between 2000 and2009 in people≥65,with an overall increase of 29.8%over 10 *** incidence per 10000 inhabitants remarkably increased in people≥75,passing from158.5 to 166.8(+5.2%)and from 72.6 to 77.5(+6.8%)over the ten-year period in women and men,*** oldest age group(people>85 years old)accounted for more than 42%of total hospital admissions in 2009(n=39000),despite representing only 2.5%of the Italian ***,women aged>85accounted for 30.8%of total fractures,although they represented just 1.8%of the general *** results of this analysis indicate that the incidence of hip fractures progressively increased from 2000 to 2009,but a reduction can be observed for the first time in women≤75(-7.9%between 2004 and 2009).CONCLUSION:Incidence of hip fractures in Italy are continuously increasing,although women aged 65-74years old started showing a decreasing trend.

Use of bovine pericardium graft for abdominal wall reconstruction in contaminated fields

AIM:To employ, in such conditions, a biological graft such as bovine pericardium that offers resistance to infection. METHODS:In our surgical department, from January 2006 to June 2010, 48 patients underwent abdominal wall reconstruction using acellular bovine pericardium; of these 34 patients had a contaminated wound due to diffuse peritonitis (complicated diverticulitis, bowel perforation, intestinal infarction, strangled hernia, etc. ) and 14 patients had hernia relapse on infected synthetic mesh. RESULTS: In our series, one patient died of multiorgan failure 3 d after surgery. After placement of the pericardium mesh four cases of hernia relapse occurred. CONCLUSION: Recurrence rate is similar to that of prosthetic mesh repair and the application of acellular bovine pericardium (Tutomesh , Tutogen Medical Gmbh Germany) is moreover a safe and feasible option thatcan be employed to manage complicated abdominal wall defects where prosthetic mesh is unsuitable.

The objective of this study was to compare the soil seed bank in an area under natural regeneration(F1)and another reforested through seedling planting(F2),seven years after bauxite mining,in Southeast,*** each area,10 sampling points were randomly distributed,using three samples of topsoil to represent each *** samples were transported to a shade house and evaluated for six months,where emerging individuals were identified every two *** similarity was evaluated with cluster analysis based on the Jaccard index(SJ).Species richness and abundance of individuals were compared using Student's t *** F1,2,114.94 propagules m-2 were registered,belonging to 51 species and 24 botanical families,Shannon-Wiener diversity index H’=2.770 and Pielou evenness index J’=*** F2,1,168.62 propagules m-2 were registered,belonging to 42 species and 22 families,H’=2.618 and J’=***(SJ)between F1 and F2 was 0.61,indicating a high *** results showed high density of propagules,good diversity and absence of ecological *** high number of individuals and diversity of tree species with zoochoric dispersion found in the areas indicates that both natural regeneration and reforestation were efficient in the ecological restoration of bauxite mined areas.
