Predictors of rebleeding and in-hospital mortality in patients with nonvariceal upper digestive bleeding

BACkGROUND Nonvariceal upper digestive bleeding (NVUDB) represents a severe emergency condition and is associated with significant morbidity and mortality. Despite a decrease in the incidence due to the widespread use of potent therapy with proton pump inhibitors as well as the implementation of modern endoscopic techniques, the mortality rate associated with NVUDB is still high. AIM To identify the clinical, biological, and endoscopic parameters associated with a poor outcome in patients with NVUDB to allow the stratification of risk, which will lead to the implementation of the most accurate management. METHODS We performed a retrospective study including patients who were admitted to the Gastroenterology Department of Clinical Emergency County Hospital Timisoara, Romania, with a diagnosis of NVUDB between 1 January 2008 and 31 December 2016. All the data were collected from the patient’s records, including demographic data, medication history, hemodynamic status, paraclinical tests, and endoscopic features as well as the methods of hemostasis, rate of rebleeding, need for surgery and death;we also assessed the Rockall score of the patients, length of hospitalization and associated comorbidities. All these parameters were evaluated as potential risk factors associated with rebleeding and death in patients with *** We included a batch of 1581 patients with NVUDB, including 523 (33%) females and 1058 (67%) males with a median age of 66 years. The main cause of NVUDB was peptic ulcer (73% of patients). More than one-third of the patients needed endoscopic treatment. Rebleeding rate was 7.72%;surgery due to failure of endoscopic hemostasis was needed in 3.22% of cases;the in-hospital mortality rate was 8.09%, and the bleeding-episode-related mortality rate was 2.97%. Mechanically induced phase transformation of zinc sulfide

Molecular dynamics （MD） simulations of the consecutive compression-decompression cycles ot hexagonal zinc sulfide （wurtzite） nanoparticles predict an irreversible phase transformation to the cubic *** phase transformation commences at the contact area between the particle and the inden- ter and proceeds with the number of compression cycles. Dislocations are visible for a particle size above 5nm. Results from wet grinding and dry powder compression experiments on a commercial wurtzite pigment agree qualitatively with MD simulation predictions. X-ray diffraction patterns reveal that the amount of cubic polymorph in the compressed samples increases with pressure applied to the powder. In comparison with powder compression, wet milling leads to a more pronounced phase transformation. This occurs because the particles are exposed to a large number of stress events by collision with the grinding media, which leads to the formation of defects and new surface crystallites by particle fracture. According to the MD simulations, phase transformation is expected to occur preferentially in surface crystallites because they experience the highest mechanical load. Because of the phase transformation, the wet ground and compressed samples exhibit a lower photo- luminescence intensity than the feed material. In comparison with powder compression, milling reduces the photoluminescence intensity more substantially. This occurs because a higher defect concentration is formed. The defects contribute to the phase transformation and photoluminescence quenching.

Diseases caused by invasive pathogens are an increasing threat to forest health,and early and accurate disease detection is essential for timely and precision forest *** recent technological advancements in spectral imaging and artificial intelligence have opened up new possibilities for plant disease detection in both crops and *** this study,Dutch elm disease(DED;caused by Ophiostoma novo-ulmi,)and American elm(Ulmus americana)was used as example pathosystem to evaluate the accuracy of two in-house developed high-precision portable hyper-and multi-spectral leaf imagers combined with machine learning as new tools for forest disease ***-and multi-spectral images were collected from leaves of American elm geno-types with varied disease susceptibilities after mock-inoculation and inoculation with ***-ulmi under green-house *** traditional machine learning and state-of-art deep learning models were built upon derived spectra and directly upon spectral image *** learning models that incorporate both spectral and spatial features of high-resolution spectral leaf images have better performance than traditional machine learning models built upon spectral features alone in detecting *** and symptomatic spots on the leaves were highlighted in the deep learning model as important spatial features to distinguish leaves from inoculated and mock-inoculated *** addition,spectral and spatial feature patterns identified in the machine learning-based models were found relative to the DED susceptibility of elm *** further studies are needed to assess applications in other pathosystems,hyper-and multi-spectral leaf imagers combined with machine learning show potential as new tools for disease phenotyping in trees.

Consensus views on competencies and teaching methods for an interprofessional curriculum on complementary and integrative medicine:A Delphi study

Emotional regulation of pain:the role of noradrenaline in the amygdala

The perception of pain involves the activation of the spinal pathway as well as the supra-spinal pathway,which targets brain regions involved in affective and cognitive *** and emotions have the capacity to influence each other reciprocally;negative emotions,such as depression and anxiety,increase the risk for chronic pain,which may lead to anxiety and *** amygdala is a key-player in the expression of emotions,receives direct nociceptive information from the parabrachial nucleus,and is densely innervated by noradrenergic brain *** recent years,the amygdala has attracted increasing interest for its role in pain perception and *** this review,we will give a short overview of structures involved in the pain pathway,zoom in to afferent and efferent connections to and from the amygdala,with emphasis on the direct parabrachio-amygdaloid pathway and discuss the evidence for amygdala’s role in pain processing and *** addition to the involvement of the amygdala in negative emotions during the perception of pain,this brain structure is also a target site for many neuromodulators to regulate the perception of *** will end this article with a short review on the effects of noradrenaline and its role in hypoalgesia and analgesia.

Unboundedness properties of smoothness Morrey spaces of regular distributions on domains

We study unboundedness of smoothness Morrey spaces on bounded domains ? ? R^n in terms of growth envelopes. It turns out that in this situation the growth envelope function is finite—in contrast to the results obtained by Haroske et al.(2016) for corresponding spaces defined on R^n. A similar effect was already observed by Haroske et al.(2017), where classical Morrey spaces M_(u,p)(?) were investigated. We deal with all cases where the concept is reasonable and also include the tricky limiting cases. Our results can be reformulated in terms of optimal embeddings into the scale of Lorentz spaces L_(p,q)(?).

Continuous revision of the histologic and stage-wise classification of lung cancer by the World Health Organization (WHO) provides the foundation for therapeutic advances by promoting molecular targeted and immunotherapies and ensuring accurate diagnosis. Cancer epidemiologic data provide helpful information for cancer prevention, diagnosis, and management, supporting health-care interventions. Global cancer mortality projections from 2016 to 2060 show that cancer will overtake ischemic heart diseases (IHD) as the leading cause of death (18.9 million) immediately after 2030, surpassing non-small cell lung cancer (NSCLC), which accounts for 85 percent of lung cancers. The clinical stage at the diagnosis is the main prognostic factor in NSCLC therapies. Advanced early diagnostic methods are essential as the initial stages of cancer show reduced mortality compared to the advanced stages. Sophisticated approaches to proper histological classification and NSCLC management have improved clinical efficiency. Although immune checkpoint inhibitors (ICIs) and targeted molecular therapies have refined the therapeutic management of late-stage NSCLC, the specificity and sensitivity of cancer biomarkers should be improved by focusing on prospective studies, followed by their use as therapeutic tools. The liquid biopsy candidates such as circulating tumor cells (CTCs), circulating cell-free tumor DNA (cfDNA), tumor educated platelets (TEP), and extracellular vesicles (EVs) possess cancer-derived biomolecules and aid in tracing: driver mutations leading to cancer, acquired resistance caused by various generations of therapeutic agents, refractory disease, prognosis, and surveillance.
