BACKGROUND QTc interval prolongation with an increased risk of torsade de pointes(Tsd)has been described in coronavirus disease 2019(COVID-19)patients treated with hydroxychloroquine(HCQ)and azithromycin(AZI)in Western *** the DR Congo,few studies have evaluated the safety of this association or proposed new *** To determine the incidence of QTc prolongation and Tsd in COVID-19 patients treated with HCQ-AZIs vs doubase C(new molecule).MeTHODS In present randomized clinical trial,we have included patients with mild or moderate COVID-19 treated with either HCQ-AZI or doubase ***(eCG)changes on day 14 of randomization were determined based on pretreatment *** QTc was defined as≥500 ms on day 14 or an increase of≥80 ms compared to pretreatment *** with cardiac disease,those undergoing other treatments likely to prolong QTc,and those with disturbed eCG tracings were excluded from the *** The study included 258 patients(mean age 41±15 years;52%men;3.4%diabetics,11.1%hypertensive).Mild and moderate COVID-19 were found in 93.5%and 6.5%of patients,*** baseline,all patients had normal sinus rhythm,a mean heart rate 78±13/min,mean PR space 170±28 ms,mean QRS 76±13 ms,and mean QTc 405±30 *** complaints suggesting cardiac involvement were reported during or after *** four patients(1.5%)experienced QTc interval prolongation beyond 500 ***,only five patients(1.9%)had an increase in the QTc interval of more than 80 *** prolongation was more significant in younger patients,those with high viral load at baseline,and those receiving HCQ-AZI(Pe of the patients developed *** QTc prolongation without Tsd was observed at a lower frequency in patients treated with HCQ-AZI vs doubase *** absence of comorbidities and concurrent use of other products that are likely to cause arrhythmia may explain our results.

We present a new experimental platform for studying laboratory astrophysics that combines a high-intensity, highrepetition-rate laser with the Large Plasma Device at the University of California, Los Angeles. To demonstrate the utility of this platform, we show the first results of volumetric, highly repeatable magnetic field and electrostatic potential measurements, along with derived quantities of electric field, charge density and current density, of the interaction between a super-Alfv′enic laser-produced plasma and an ambient, magnetized plasma.

BACKGROUND Basal joint arthritis is a common disease which is especially present in postmenopausal women. A variety of procedures have been described once conservative treatment has failed. The purpose of our study is to present a novel surgical technique based on a clinical case with three-year follow-up which is highly effective, even after failure of previous procedures. CASe SUMMARY We present a case of a 39 year-old-woman who suffered from recurring, persistent, sharp left thumb pain for several years After two operative procedures, she presented again with intolerable, sharp pain over the trapeziometacarpal joint, and wished to proceed with a pain relieving procedure or arthroplasty. Intraoperatively, the articular surfaces appeared well, without any bare areas of bone or eburnation, resulting in the decision to perform a “subtraction hemiarthroplasty”. A shortening osteotomy of the os trapezium was performed by removing a slice of 8-9 mm bone, without damaging the joint cartilages. The osteotomy was stabilized using two screws, the ligament and capsular tissue was suture imbricated, and a cut down on the translation and a dorsal radial ligament reconstruction were performed. Three years after the final procedure, a long-term follow-up demonstrates excellent results, pain free with full range-of-motion. CONCLUSION Subtraction hemiarthroplasty with ligament tightening imbrication and reconstruction of the radial ligament led to excellent results in motion and pain.

Posterior instability of the shoulder is a rare condition and represents about 10% of shoulder instability. It has become more frequently recognized in the last year,even though it is more difficult to diagnose than anterior shoulder instability. As this form of shoulder pathology is somewhat rare,biomechanical knowledge is limited. The purpose of our study was to perform an extensive literature search,including PubMed and Medline,and to give an overview of the current knowledge on the biomechanics of posterior shoulder instability. The Pub Med/Medline databases were utilized,and all articles related to posterior shoulder instability and biomechanics were included to form a comprehensive compilation of current knowledge. A total of 93 articles were deemed relevant according to our inclusion and exclusion criteria. As expected with any newly acknowledged pathology,biomechanical studies on posterior shoulder instability remain limited in the literature. Current biomechanical models are performed in a static manner,which limits their translation for explaining a dynamic pathology. Newer models should incorporate dynamic stabiliza-tion of both the rotator cuff and scapulothoracic joint. There is a current lack of knowledge with regards to the pathomechanism of posterior shoulder instability,with no consensus on appropriate treatment regimens. Further investigation is therefore required at both basic science and clinical levels.

The basement of the Romanian Carpathians is made of Neoproterozoic to early Paleozoic peri- Gondwanan terranes variably involved in the Variscan orogeny, similarly to other basement terrains of europe. They were hardly dismembered during the Alpine orogeny and traditionally have their own names in the three Carpathian areas. The Danubian domain of the South Carpathians comprises the Dragsan and Lainici-Paiius peri-Amazonian terranes. The Dragsan terrane originated within the ocean surrounding Rodinia and docked with Rodinia at -800 Ma. It does not contain Cadomian magmatism and consequently it is classified as an Avalonian extra-Cadomian terrane, The Lainici-Pfiius terrane is a Ganderian fragment strongly modified by Cadomian subduction-related magmatism, It is attached to the Moesia platform. The TisoviD terrane is an ophiolite that marks the boundary between Drfagsan and Lainici-Paius terranes. The other basement terranes of the Romanian Carpathians originated close to the Ordovician North- African orogen, as a result of the eastern Rheic Ocean opening and closure. except for the Sebes-Lotru terrane that includes a lower metamorphic unit of Cadomian age, all the other terranes （Bretila, Tulghes, Negrisoara and Rebra in the east Carpathians, Somes, Biharia and Baia de Aries in the Apuseni mountains, Fagaras, Leaota, Carat and Pades in the South Carpathians） represent late Cambrian-Ordovician rock assemblages. Their provenance, is probably within paleo-nortbeast Africa, close to the Arabian-Nubian shield. The late Cambrian-Ordovician terranes are defined here as Carpathian-type terranes. According to their lithostratigraphy and origin, some are of continental margin magmatic arc setting, whereas others formed in rift and back-arc environment and closed to passive continental margin settings. The current challenges that society faces call for actions fostering climate change adaptation and long-term sustainability grounded in effective policies. Consequently, to raise environmental resilience and sustain human wellbeing, we need to overhaul the policy-making process towards a ‘holistic approach’. The nexus approach has proven a valuable tool for identifying gaps and incoherencies in current policies, for understanding major interlinkages across sectors and scales, and for promoting the Sustainable Development Goals (SDGs). Nevertheless, a ‘silo approach’ to decision making currently prevails, thus achieving cross-sectoral and cross-scale harmonization still remains a challenge in policy-making. In response to this challenge, the next step in the science–policy–practice interface is to integrate the nexus approach in the ‘climate services’ arena, in order to support integrated policies. Climate services embody a bridge between researchers and stakeholders, and by supporting integrated policies they will ensure synergies between sectors and scales, reduce potential trade-offs, and enable co-benefits.
