Burden of gallstone disease in the United States population:Prepandemic rates and trends

BACKGROUND Gallstone disease is one of the most common digestive disorders in the United States and leads to significant morbidity,mortality,and health care *** To expand on earlier findings and investigate prepandemic rates and trends in the gallstone disease burden in the United States using national survey and claims *** The National Ambulatory Medical Care Survey,National Inpatient Sample,Nationwide emergency Department Sample,Nationwide Ambulatory Surgery Sample,Vital Statistics of the United States,Optum Clinformatics®Data Mart,and Centers for Medicare and Medicaid Services Medicare 5%Sample and Medicaid files were used to estimate claims-based prevalence,medical care including cholecystectomy,and mortality with a primary or other gallstone *** were age-adjusted(for national databases)and shown per 100000 *** Gallstone disease prevalence(claims-based,2019)was 0.70%among commercial insurance enrollees,1.03%among Medicaid beneficiaries,and 2.09%among Medicare beneficiaries and rose over the previous ***,in the United States population,gallstone disease contributed to approximately 2.2 million ambulatory care visits,1.2 million emergency department visits,625000 hospital discharges,and 2000 deaths *** had higher medical care rates with a gallstone disease diagnosis,but mortality rates were higher among *** had higher ambulatory care visit and hospital discharge rates compared with Whites,but not mortality *** had lower ambulatory care visit and mortality rates,but similar hospital discharge rates compared with *** the study period,ambulatory care and emergency department visit rates with a gallstone disease diagnosis rose,while hospital discharge and mortality rates *** commercial insurance enrollees,rates were higher compared with national data for ambulatory care visits and hospitalizations,but lower for emergency department ***

Seasonal variations in dissolved organic matter concentration and composition in an outdoor system for bank filtration simulation

Dissolved organic matter(DOM)in surface waters can vary markedly in character depending on seasonal variations such as rainfall intensity,UV radiations and *** in DOM as well as temperature and rainfall intensity over the year can affect the biochemical processes occurring in bank filtration(BF).Identification and characterization of DOM in the surface water could help to optimize the water treatment and provide stable and safe drinking *** study investigated year-long variations of DOM concentrations and compositions in a surface water of a circulated outdoor pond(research facility)connected to a BF *** was dominated by humic substances and a changing pattern of DOM in surface water was observed throughout the year.A significant increase of DOM(~38%)in surface water was noted in August compared to *** fluorescent DOM showed that DOM in summer was enriched with the degradable fraction whilst non-degradable fraction was dominated in winter.A constant(1.7±0.1 mg/L)effluent DOM was recirculated in the system throughout the *** removal through BF varied between 4%to 39%and was achieved within a few meters after infiltration and significantly correlated with influent DOM concentration(R^(2)=0.82,pever,no significant(p>0.05)change in the removal of DOM was observed in two subsurface layers(upper and lower).This study highlights the presence of a constant non-degradable DOM in the bank filtrate,which was not affected by temperature,redox conditions and UV radiations.

Near Infrared Spectroscopy (NIRS) Model-Based Prediction for Protein Content in Cowpea

Cowpea (Vigna unguiculata L. Walp) is a multi-purpose legume with high quality protein for human consumption and livestock. The objective of this work was to develop near-infrared spectroscopy (NIRS) prediction models to estimate protein content in cowpea. A total of 116 cowpea breeding lines with a wide range of protein contents (19.28 % to 32.04%) were selected to build the model using whole seed and ground seed samples. Partial least-squares discriminant analysis (PLS-DA) regression technique with different pre-treatments (derivatives, standard normal variate, and multiplicative scatter correction) were carried out to develop the protein prediction model. Results showed: 1) spectral plots of both the whole seed and ground seed showed higher spectral scatter at higher wavelengths (>1450 nm), 2) data pre-processing affects prediction accuracy for bot whole seed and ground seed samples, 3) prediction using ground seed samples (0.64 R2 0.85) is better than the whole seed (0.33 R2 0.78), and 4) the data pre-processing second derivative with standard normal variate has the best prediction (R2_whole seed = 0.78, R2_ground seed = 0.85). The results will be of interest in cowpea breeding programs aimed at improving total seed protein content.

Comparative Performance Measurement of the Pareto Optimal Combination and Multi-Objective Combination Models for Controller Placement in Software-Defined Networks

The evolution of the current network has challenges of programmability, maintainability and manageability, due to network ossification. This challenge led to the concept of software-defined networking (SDN), to decouple the control system from the infrastructure plane caused by ossification. The innovation created a problem with controller placement. That is how to effectively place controllers within a network topology to manage the network of data plane devices from the control plane. The study was designed to empirically evaluate and compare the functionalities of two controller placement algorithms: the POCO and MOCO. The methodology adopted in the study is the explorative and comparative investigation techniques. The study evaluated the performances of the Pareto optimal combination (POCO) and multi-objective combination (MOCO) algorithms in relation to calibrated positions of the controller within a software-defined network. The network environment and measurement metrics were held constant for both the POCO and MOCO models during the evaluation. The strengths and weaknesses of the POCO and MOCO models were justified. The results showed that the latencies of the two algorithms in relation to the GoodNet network are 3100 ms and 2500 ms for POCO and MOCO respectively. In Switch to Controller Average Case latency, the performance gives 2598 ms and 2769 ms for POCO and MOCO respectively. In Worst Case Switch to Controller latency, the performance shows 2776 ms and 2987 ms for POCO and MOCO respectively. The latencies of the two algorithms evaluated in relation to the Savvis network, compared as follows: 2912 ms and 2784 ms for POCO and MOCO respectively in Switch to Controller Average Case latency, 3129 ms and 3017 ms for POCO and MOCO respectively in Worst Case Switch to Controller latency, 2789 ms and 2693 ms for POCO and MOCO respectively in Average Case Controller to Controller latency, and 2873 ms and 2756 ms for POCO and MOCO in Worst Case Switch to Controll

Simplified seismic scenario analysis of existing masonry buildings accounting for local site effects

This study discusses the effects of local sites and hazard amplification on the seismic vulnerability assessment of existing masonry *** this context,a rapid seismic evaluation procedure was implemented on an old masonry building stock in the historical center Galata,located inİstanbul,to determine the seismic risk priority of the built *** scenarios were generated for all soil classes,different moment magnitudes,and source-to-site distances to obtain more accurate results for the seismic vulnerability assessment of the studied building ***,damage distributions estimated under nine different scenarios with/without site effects were compared and illustrated in maps to discuss changes in vulnerability owing to amplification *** this study,by re-examining the rapid seismic evaluation procedure by including geo-hazard-based assessment,the importance of site effects on the vulnerability and risk assessment of built heritage was *** proposed framework integrating field data and local site effects is believed to advance the current applications for vulnerability assessment of masonry buildings and provide an improvement in the application of rapid seismic assessment procedures with more reliable results.

FSCIL-eACA: Few-Shot Class-Incremental Learning Network Based on embedding Augmentation and Classifier Adaptation for Image Classification

The ability to learn incrementally is critical to the long-term operation of AI systems. Benefiting from the power of few-shot class-incremental learning(FSCIL), deep learning models can continuously recognize new classes with only a few samples. The difficulty is that limited instances of new classes will lead to overfitting and exacerbate the catastrophic forgetting of the old classes. Most previous works alleviate the above problems by imposing strong constraints on the model structure or parameters, but ignoring embedding network transferability and classifier adaptation(CA), failing to guarantee the efficient utilization of visual features and establishing relationships between old and new classes. In this paper, we propose a simple and novel approach from two perspectives: embedding bias and classifier bias. The method learns an embedding augmented(eA) network with cross-class transfer and class-specific discriminative abilities based on self-supervised learning and modulated attention to alleviate embedding bias. Based on the adaptive incremental classifier learning scheme to realize incremental learning capability,guiding the adaptive update of prototypes and feature embeddings to alleviate classifier bias. We conduct extensive experiments on two popular natural image datasets and two medical datasets. The experiments show that our method is significantly better than the baseline and achieves state-of-the-art results.

Surgical Treatment of Nephroblastoma in a 4-Year Male Child: A Case Report and Literature Review

Herein we report a patient with nephroblastoma which was successfully removed at the Sunyani Teaching Hospital CJ Oppong theatre in Ghana in sub-Saharan Africa by extended below umbilical incision. Our patient had a family history of Wilms tumour predisposing him to the disease. His main symptoms were haematuria and abdominal mass which was noticed later. examination and investigation were suggestive of a late-stage unilateral Nephroblastoma resulting in the need for nephrectomy. To reduce the need for such radical surgeries among children at an early stage, there is the need for early screening of children for Wilms tumours especially, those with family predisposition as in our case study. The case report presented here constitutes a rare case of nephroblastoma in the literature.

Investigating the impact of the universal function of the nuclear proximity potential in heavy-ion fusion cross sections

The fusion barriers and cross sections of 15 colliding systems with 320≤Z_(1)Z_(2)≤1512 are investigated in detail to understand the influence of the universal function of proximity potential formalism in the heavy-ion fusion *** realize this goal,we select three versions of the phenomenological proximity potentials,including Prox.77,Zhang 2013,and Guo 2013,to calculate the nucleus-nucleus *** experimental fusion cross sections for the selected reactions are analyzed using the standard coupled-channel calculations,including couplings to the low-lying 2^(+)and 3^(-)states in the target and *** calculated results show that the universal functions of the Guo 2013 and Prox.77 models provide the lowest and highest fusion barriers,*** addition,it is found that the height of the fusion barriers is enhanced by increasing the mass number of the projectile from light to heavy *** highest sensitivity to the mass number of the projectile belongs to the results of Prox.77.A discussion is also presented on the influence of the universal function on the radial behavior of the interaction potential in the allowed region for overlapping *** results reveal that the best fit to the experimental data of the fusion cross sections for the reactions involving light and medium nuclei is obtained using the universal function of the Zhang 2013 *** the heavier systems,the results of the Guo 2013 model at sub-barrier energies provide a good description of the available data.

event-triggered H_(∞) consensus control for input-constrained multi-agent systems via reinforcement learning

This article presents an event-triggered H_(∞) consensus control scheme using reinforcement learning (RL) for nonlinear second-order multi-agent systems (MASs) with control constraints. First, considering control constraints, the constrained H_(∞) consensus problem is transformed into a multi-player zero-sum game with non-quadratic performance functions. Then, an event-triggered control method is presented to conserve communication resources and a new triggering condition is developed for each agent to make the triggering threshold independent of the disturbance attenuation level. To derive the optimal controller that can minimize the cost function in the case of worst disturbance, a constrained Hamilton–Jacobi–Bellman (HJB) equation is defined. Since it is difficult to solve analytically due to its strongly non-linearity, reinforcement learning (RL) is implemented to obtain the optimal controller. In specific, the optimal performance function and the worst-case disturbance are approximated by a time-triggered critic network;meanwhile, the optimal controller is approximated by event-triggered actor network. After that, Lyapunov analysis is utilized to prove the uniformly ultimately bounded (UUB) stability of the system and that the network weight errors are UUB. Finally, a simulation example is utilized to demonstrate the effectiveness of the control strategy provided.
