BACKGROUND Tumor necrosis factor-alpha inhibitors,including infliximab and adalimumab,are effective medical treatments for perianal fistulising Crohn’s disease(CD),but not all patients achieve fistula *** To determine the correlation between perianal fistula healing and closure with infliximab and adalimumab trough *** In this multicentre retrospective study conducted across four tertiary inflammatory bowel disease centres in Australia,we identified CD patients with perianal fistulae on maintenance infliximab or adalimumab who had a trough level within twelve weeks of clinical *** collected included demographics,serum infliximab and adalimumab trough levels(mg/L)within 12 wk before or after their most recent clinical assessment and concomitant medical or surgical *** primary outcome was fistula healing,defined as cessation in fistula *** secondary outcome was fistula closure,defined as healing and closure of all external fistula *** between patients who did or did not achieve fistula healing were compared using the chi-square test,t test or Mann-Whitney U *** One hundred and fourteen patients(66 infliximab,48 adalimumab)were ***-eight(72.7%)patients on maintenance infliximab achieved fistula healing and 18(27.3%)achieved fistula ***-seven(77%)patients on maintenance adalimumab achieved fistula healing and 17(35.4%)achieved fistula *** who achieved fistula healing had significantly higher infliximab and adalimumab trough levels than patients who did not[infliximab:6.4(3.8-9.5)vs 3.0(0.3-6.2)mg/L,P=0.003;adalimumab:9.2(6.5-12.0)vs 5.4(2.5-8.3)mg/L,P=0.004].For patients on infliximab,fistula healing was associated with lower rates of detectable anti-infliximab antibodies and younger *** patients on adalimumab,fistula healing was associated with higher rates of combination therapy with an *** trough levels for patients with and without fistu

Cretaceous amber from Myanmar(～99 Ma Burmese amber) has become a valuable supplement to the traditional skeletal record of small theropod dinosaurs preserved in sedimentary rocks, particularly for coelurosaurs and enantiornithines. The specimens recovered from this deposit preserve skeletal material and soft tissues in unmatched detail. This provides opportunities to study three-dimensional preservation of soft tissues, microstructure, and pigmentation patterns that are seldom available elsewhere in the fossil record. Ultimately, this line of research provides insights into life stages that are difficult to preserve, the ecology and appearance of the groups involved, and the evolutionarydevelopment of structures such as feathers. Here we describe the most recent discovery from Burmese amber, an articulated skeleton of an enantiornithine bird. This individual has been sectioned along the coronal plane, providing a unique view inside multiple body regions. Osteological observations and plumage patterns support placement within the Enantiornithes, and suggest that the animal may have been a juvenile at the time of death. The specimen has a complex taphonomic history that includes exposure at the surface of a resin flow prior to encapsulation, and may include scavenging by some of the insects trapped within the same amber piece. The chemical composition observed along surface exposures and shallowly buried regions of the body indicate that the specimen has not undergone significant exchange with its surroundings. High iron concentrations are present in regions that preserve soft tissues as carbon films, and calcium distribution corresponds to regions where bones breach the surface of the amber.

Consciousness is the unified, structured, subjective experience that we all share. The Default Space Theory has been proposed as a unified theory of consciousness that includes the brain and body in describing the infrastructure of consciousness. We have presented the theory in journals with a variety of academic specialties;however, the model is continually being developed. Due to the current state of science on the nature of conscious experience lacking hard data, the theory’s concepts must thoroughly explain and shoulder phenomenological observations including the phenomenology of deficits of consciousness. According to the theory, the thalamus serves as a central hub which networks the globally distributed, and continuous fast oscillations not only among the brain, but also the eyes, ears, skin, and other sensory organs. These oscillations form the virtual template of external space within the mind in which external sensory information is integrated into this pre-existing, dynamic space. In this article, we explore phenomenological support for our theory of conscious experience in which such experience resides entirely in such a virtual space, termed the default space. In order to provide such support, we discuss simple personal experiments and observations which anyone can partake along with phenomenal symptoms of clinical deficits. We encourage readers to perform the personal experiments we describe in order to gain an understanding of the various concepts of the theory. The neurological deficits we elucidate not only support the theory but clarify obscurities surrounding these conditions. We assert the support we give in our theory here will advance the uphill struggle many paramount theories face in gaining initial acceptance. Further research is needed in order to acquire empirical evidence for the veracity of our theory.

While a large number of studies have described animal social networks, we have a poor understanding of how these networks vary with ecological and social conditions. For example, reproductive periods are an important life-history stage that may involve changes in dominance relationships among individuals, yet no study to date has compared social networks of do- minance interactions （i.e. dominance networks） across reproductive contexts. We first analyzed a long-term dataset on captive so- cial groups of the cooperatively breeding cichlid Neolamprologuspulcher, and found that eviction events were significantly more common around reproduction than expected by chance. Next, we compared the structure of dominance networks during early pa- rental care and non-reproductive periods, using one of the first applications of exponential random graph models in behavioral biology. Contrary to our predictions, we found that dominance networks showed few changes between early parental care and non-reproductive periods. We found no evidence that dominance interactions became more skewed towards larger individuals, became more frequent between similar-sized individuals, or became more biased towards a particular sex during parental care. However, we did find that there were relatively more dominance interactions between opposite-sex dyads in the early parental care period, which may be a by-product of increased sexual interactions during this time. This is the first study in behavioral ecology to compare social networks using exponential random graph modeling, and demonstrates a powerful analytical framework for future studies in the field [Current Zoology 61 （1）： 45-54, 2015].

In this article, the mechanisms of central pain syndrome (CPS) are examined for the purpose of gaining insight into how a unified conscious experience arises from brain and body interaction. We provide a novel etiology for CPS via implementation of the previously proposed 3D Default Space (3DDS) consciousness model in which consciousness and body schema arise when afferent information is processed by corticothalamic feedback loops and integrated via the thalamus. Further, we propose the mechanisms by which CPS represents deficits in dynamic interactions between afferent and efferent signaling. Modern hypotheses of CPS suggest roles for maladaptive neuroplasticity, a deafferentated somatosensory cortex and/or thalamus, and reorganization along the sensory pathways of the spinothalamic tract in the pathogenesis of the painful sensations. We propose that CPS arises when painful sensory signals originating along the maladapted and/or dysfunctional spinothalamic tract become accentuated by the dominant top down mechanisms of the brain.

The default space model is a unified theory of consciousness that posits the brain and body together form the foundation of conscious experience that exists as a three dimensional internally generated simulation of reality termed the 3D dynamic default space. We have explored and developed the model in many publications and journals with a variety of academic specialties and its scope and concepts continue to broaden. In these publications, we have supported the concepts of the model through its ability to explain neuropsychological disorders, illusions, and everyday observations on consciousness. The model’s foundations in which the thalamus serves as a central hub networked with the brain and body by continuous, fast, membrane potential oscillations have been greatly expanded since its initial publication which we review within this article. Profound leaps forward in our theory include the nature of lateral inhibition in sensory perception, the nature of sensory organs acting as “smart screens”, and the correlation of respiration with mental atmosphere. Through reviewing the developing concepts expanding the theory since our major 2015 publication that laid the foundation of our theory, we hope to give readers a summarized update of where the theory currently stands in terms of its structure. We encourage readers to investigate these previous publications to gain further insight into our propositions. Through accurate models of consciousness, we may develop etiologies for countless neurological disorders, as well as improve treatments.

This research builds upon the authors’ previous work that introduced and modeled a novel Gallium-Arsenide, Emitterless, Back-surface Alternating Contact (GaAs-EBAC) thin-film solar cell to achieve >30% power conversion efficiency. Key design parameters are optimized under an Air-Mass (AM) 1.5 spectrum to improve performance and approach the 33.5% theoretical efficiency limit. A second optimization is performed under an AM0 spectrum to examine the cell’s potential for space applications. This research demonstrates the feasibility and potential of a new thin-film solar cell design for terrestrial and space applications. Results suggest that the straight-forward design may be an inexpensive alternative to multi-junction solar cells.

A Systematic Review of the Literature on the Relationships between Chronic Diseases and Food Insecurity

Background: The association between food insecurity (FI) and obesity is now so well documented that scholars have dubbed it the “‘new’ food insecurity” and cited it as a leading cause of increased chronic disease (CD) risk. Here, the chain of causality is assumed to be FI → obesity → CD. However, this model overlooks the possible direct associations between FI and CD independent of obesity. Aim: This study assessed the literature on relationships between FI and CD. Methods: We conducted a systematic literature review of electronic databases. Selection criteria were designed to elicit studies that assessed FI and CD using a measure of CD other than obesity. Results: Fifty-one articles met the inclusion criteria. Forty-five studies (87%) reported a statistically significant association between FI and CD, but only 15 adjusted for obesity. The association was less consistent for asthma and dyslipidemia than for diabetes, hypertension, and other dietrelated CDs, and most were conducted in the USA or Canada. Conclusion: There is a body of literature documenting relationships between FI and CD, but it is heavily biased toward Western nations, ecological study designs, and type 2 diabetes as the CD of focus. A small subset of the literature controls for BMI, demonstrating that a portion of the FI-CD relationship cannot be fully explained by obesity. Possible direct pathways linking FI and CD include systematic effects of poverty that accompanies FI, micronutrient deficiencies, and environmental exposure to toxins;however, exploration of these alternative pathways is limited by study designs that fail to include obesity as a control variable.
