AIM: To evaluate neutrophil gelatinase associated lipocalin(NGAL) in patients infected by hepatitis C virus(HCV) before and during treatment with directly acting antivirals(DAAs).METHODS: NGAL was measured in a group of patients with chronic HCV infection ranked, at baseline, by age, gender, anti-hypertensive therapy, HCV viral load, liver fibrosis stage and, either at baseline or after 1 year, estimated glomerular filtration rate(e GFR). Then, NGAL and e GFR evolutions were monitored in a subgroup of patients who started antiviral therapy with DAAs. differences of median NGAL levels were evaluated through Wilcoxon-Mann-Whitney test for nonparametric data. differences in dichotomous variables were evaluated through χ~2 test. At baseline, a univariate regression analysis was conducted to verify if NGAL values correlated with other quantitative variables [age, fibrosis four(FIB-4), AST to platelet ratio index(APRI), and e GFR]. RESULTS: Overall, 48 patients were enrolled, 8 of them starting HCV treatment. At baseline, statistically significant differences were found in median NGAL values only between patients with e GFR differences in NGAL were not significant among patients ranked by HCV viral load, FIB-4 score and APRI, when patients with NGAL > 118.11 ng/d L were compared with those of NGAL ≤ 118.11 ng/d L, not statistically significant differences were present for age, gender, chronic kidney disease classification and liver fibrosis(P > 0.05). Linear correlation was found between NGAL and both age(P = 0.0475) and e GFR(P = 0.0282) values. Not statistically significant predictions of NGAL at baseline were demonstrated for e GFR evolution 1 year later. Interestingly, in the 8 patients treated with DAAs, median NGAL significantly increased at week 12 compared to baseline(P = 0.0239).CONCLUSION: Our results suggest that NGAL should be further evaluated as an adjunct marker of kidney function in these patients.

A Novel Transient Current-Based differential Algorithm for Earth Fault Detection in Medium Voltage distribution Networks

This paper presents a novel transient current differential algorithm for earth fault detection in unearthed （isolated） and compensated neutral medium voltage （MV） networks. The proposed algorithm uses the transient residual currents, which are very sensitive for earth faults detection. The transient values of residual currents are calculated for each feeder in the network and used as an earth fault indicator. The flow of residual currents is investigated. It is found that the residual current for the faulted feeder is equal to the summation of all residual currents for all other healthy feedersl Based on this investigation, a differential technique is proposed. A percentage restrain performance is proposed to ensure the selectivity and security of the algorithm. The transient algorithm is very sensitive for earth fault incidence. To apply the proposed algorithm, the residual currents can be measured easily by one sensor for each feeder with no need to voltage measurement. The proposed algorithm is less dependent on the fault resistance and the faulted feeder parameters. The network is simulated by ATP/EMTP program. different fault conditions are covered in the simulation process： different fault inception angles, fault locations and fault resistances.

Evaluation of Clinical Utility of the Personal KinetiGraph<sup>®</sup>in the Management

INTRODUCTION: Parkinson’s disease (PD) is a disorder characterized by complex motor and non-motor symptoms that can be difficult for patients to accurately communicate. Wearable technologies portend improvements in assessment and monitoring of these symptoms, with their clinical utility currently being evaluated in routine clinical care. OBJECTIVE: To evaluate the clinical utility of the Personal KinetiGraph? (PKG?) Movement Recording System in the routine clinical care of persons with PD (PWP). METHODS: Clinically stable, non-demented PWP presented for two routine clinic visits that included: medication review, symptom review, neurological examination including the Movement disorder Society Unified Parkinson’s disease Rating Scale (MDS-UPDRS) III/IV, and completion of a clinical management plan by a movement disorder specialist prior to review of the PKG report. After reviewing the PKG report, the clinician completed a modified clinical management plan taking into consideration the findings of the PKG. This was repeated at a second visit to evaluate various outcome measures following PKG-enhanced management. RESULTS: The PKG improved the assessment of PD symptoms and the response to treatment, while increasing patient activity levels and compliance. Clinical management plans enhanced by PKG led to different recommendations in 29.4% of cases compared with standard of care due to higher rates of bradykinesia, dyskinesia, tremor, and fluctuations identified by PKG. Using the PKG in the clinical management plan led to a change in medications in 75% (21/28) of patients and both a statistically significant difference and a clinically meaningful reduction in MDS-UPDRS III score of 4.8 (p = 0.028). Additionally, positive changes in both the clinician (17/28;61%) and patient-reported (13/24;54%) Global Impression of Improvement were reported. CONCLUSION: The PKG is a valuable tool in augmenting clinical management when utilized along with a clinical assessment.

Investigating Factors Associated with Vaccine Hesitancy in Makkah, KSA

Introduction: Vaccination provides health and economic benefits to the individual and to society. In Saudi Arabia, public support for immunisation programmes is generally high and well received. However, the benefits of vaccines are often not fully known or appreciated. When public discussion on vaccine safety, quality or efficacy occurs, very often misinformation creeps into the debate through the internet and other media sources, significantly weakening immunisation programmes. Vaccine hesitancy is a concept frequently used in the discourse around vaccine acceptance. This study reflects on the factors associated with vaccine hesitancy, through a study focusing on knowledge, attitudes and beliefs among parents of young children. Methods: A cross-sectional study was carried out in seven primary schools in Makkah city, Saud Arabia. Data were collected in 2017 through a computer based-survey administered to 100 parents with children aged between 2 months and 17 years of age. Questionnaire responses were analysed using Minitab. Results: Many parents (with a total of 292 children) agreed on the importance of vaccinations, to protect their children’s health and to prevent the spread of diseases in the community. Most parents (68.7%, P discussion and Conclusions: Vaccine hesitancy is a common problem in Makkah, Saudi Arabia. Further research will be needed to better understand when, how and why these beliefs are formed.
