Introduction: Work environments create the basis for arduous nursing work. This is what led us to research the factors of arduousness in the exercise of the health profession in North benin, through this study. Methods: This is a cross-sectional and descriptive study which focused on health professionals in the health zones of Parakou-N’dali and Tchaourou. The sampling was an exhaustive census. All health workers who had given their free and informed consent were included. The data was collected using a questionnaire. The variables studied were the factors of arduous work and socio-professional characteristics. The data collected was processed and analyzed with the Epi info 7.***.0.1 software. Results: A total of 692 were surveyed, the participation rate was 85.11% and the sex ratio (M/F) equal to 0.7. The average age was 38 years old. Nurses made up 32.66% of the sample. They were 86.42% subject to night work and 82.66% to alternating work. Overall, 70.23% worked more than 40 hours per week. between 78.12% and 96.46% of non-specialist doctors, nurses, midwives and biomedical analysis technicians were required to work night shifts. Nurses, midwives, radiology and laboratory technicians were between 89.53% and 97.35%, organized in alternating work. In the sample, 55.20% complained about the insufficient number of work materials, 26.29% handled harmful chemical substances. Among those surveyed, 58.14% had been attacked. Conclusion: All professional categories of caregivers are subject to arduous work. Measures are needed to reverse the situation.

Freshwater bodies are natural resources that should be exploited to the fullest, while maintaining the sustainability of ecosystems and ecosystem services which they support. Riparian forests are more important as they contain rivers which are vital sources of fresh water for local populations. However, the quality and quantity of water issued from the watershed depend on the structural state of these forests. The aim of this work was to assess the physico-chemical and structural state of the Akono gallery forest. To achieve this, fieldwork consisted of selecting six major streams of the watershed including Ndjolong, Menyeng adzap, Emomodo, Mvila, Negbe and Ossoé kobok. On each of these, two stations, one intact and one degraded, were marked by transects. The method involved measuring Hydrometric parameters (depth, length, width) of the stream and Physico-chemical parameters of water in the streams while dendrometric parameters were measured along 100 m-transects laid using the point-centred quarter method modified for water bodies to collect tree, shrub and palm variables such as trunk diameter, crown diameter and height. Macrophytes and species identification were carried out using standard botanical procedures. Results showed that, the majority of physico-chemical parameters measured differed significantly between intact and degraded stations (P Pentachletra mancrophylla, whereas on degraded sites, this index was low and characterized by the relative dominance of species Piptadeniastrum africanum. Sorensen’s index (0.56) and CFA showed that the different stands were homogeneous. We can affirm that the riparian forests of Akono watershed are towards a state of stability notwithstanding the perpetuation of anthropological actions.

Assessing Community Health Interventions in Reducing Infant and Child Mortality in the Southeastern benin

background: To reduce infant and child mortality in benin, a package of high-impact interventions per healthcare level was implemented in 2009. This study aimed to assess the quality of community-based health interventions in reducing infant and child mortality within the municipality of Pobè in southeastern benin. Methods: This was a cross-sectional evaluative study carried out in November 2021 focused on children aged 0 - 59 months, their mothers, health workers, community facilitators, community health workers and the Town Hall health focal point. Mothers and their children were targeted by cluster sampling, and exhaustive selection was used to recruit all other participants. Predetermined scores based on rating criteria were used to assess the quality of community health interventions using the “input, process and outcome” of Donabedian approach. Results: Over 300 mother-child couples, 46 community health workers, 7 health agents, 1 community facilitator and 1 health focal point from Pobè town hall were surveyed. Intervention quality was judged as “average”, with a score of 73.80%. The “inputs” and “outcomes” components were the weakest links. Conclusion: Improving access to the inputs needed by community health workers can enhance the quality of PIHI interventions.

History: Pediatric management of HIV infection in children in the Central African Republic began in 2004 with the use of fractionated adult antiretrovirals and Cotrimoxazole. It has evolved over the years with the use of pediatric forms, oral suspensions and dispersible tablets. The transition to Dolutegravir took place in 2020. The active file of our patients will grow from 78 to over 1900 today. Follow-up examinations are carried out to assess adherence to treatment. Objective: To determine the immunovirological profile and factors associated with treatment failure during follow-up of children on ART at the bangui pediatric university hospital. Patients and Method: This was a cross-sectional, analytical study from May 30 to December 02, 2022. The study sample was drawn from a cohort of HIV-1-infected children followed up at the bangui pediatric university hospital and on ART for three semesters who met the selection criteria. Results: The prevalence of treatment failure varied from one semester to the next. Thus, the prevalence of therapeutic failure was 20% in the first semester, 10% in the second semester and 7% in the third semester. The prevalence of virological failure was 10.28% in the first half of the year, 6.91% in the second half and 4.98% in the third. Secondly, immunological failure was 0.48% in the first half of the year, 0.32% in the second 0.64% in the third half. Finally, clinical failure was 8.82% in the first half, 4.82% in the second half, 1.92% in the third half. Socio-demographic and clinical factors associated with treatment failure were male gender (p 1000 copies/ml (p Conclusion: The occurrence of treatment failures in children is a major problem, especially in our resource-limited countries, given the challenges facing antiretroviral therapy. It is therefore necessary to carry out a study on resistance genotyping in order to propose correct management protocols, as the future of treatment programs depends on it.

Introduction: Maternal mortality constitutes a public health problem and its rate is an indicator of a country’s development. Among the causes of maternal and perinatal death, high blood pressure associated with pregnancy occupies a significant part. It represents 5% of direct maternal deaths. Objective: to study the maternal and perinatal prognosis of high blood pressure during pregnancy in the Tenenkou reference health center in Mali. Methodology: This was a descriptive, analytical and retrospective cross-sectional study over a period of twelve months from January 1, 2021 to December 31, 2021 and involving 144 cases of high blood pressure associated with pregnancy. Results: We obtained a frequency of 11.75%. The majority of patients 70.9% were aged between 20 - 35 years. The important risk factor found was young age. During our study, 46.5% of patients had performed at least one CPN and only 13.9% performed 04 CPN. Pre-eclampsia was the most common type of high blood pressure during pregnancy, i.e. 61.1%. Eclampsia and retroplacental hematoma were the most common maternal complications, respectively 27.8% and 11.1%. The most common fetal complications were premature births and fetal distress with 20.9% and 17.4% respectively. Conclusion: Hypertension associated with pregnancy still remains a major cause of maternal-fetal morbidity and mortality in our context where diagnosis is often late. The main clinical form was preeclampsia. Eclampsia and retroplacental hematoma were the most frequent maternal complications. Fetal complications were mainly prematurity and fetal distress.

Introduction: Uterine fibroid is a mixed mesenchymal tumor, developing from smooth muscle cells of the myometrium, separated by connective tissue. The majority of fibroids are asymptomatic and do not require any intervention or other exploratory measures. However, in some cases fibroids are symptomatic, their treatment should aim to improve symptoms and quality of life. Objectives: Describe the epidemiological-clinical aspects of uterine fibroids in the gynecology and obstetrics department of the reference health center of commune VI of the bamako district. Methodology: This was a retrospective descriptive study carried out over a period of one year. It concerns all patients seen in consultation in the gynecology and obstetrics department of the reference health center of commune VI. Results: during the study period, the uterine fibroid frequency was 1.80%. During this same period, fibroids represented 5.59% of gyneco-obstetric pathologies operated on in the department. The 30 - 45 year old age group was the most represented with a frequency of 75.63%. The average age was 36.87 years ± 6.2 years with extremes of 25 and 63 years. Married women were the most represented 97.48%. The vast majority of our patients 95.8% were not postmenopausal. The multigravidas were the most represented, i.e. 37.50% with a large part of the pauciparous 41.29%. In our patients, 96.64% had a clinical symptom on their fibroid with the main reason for consultation being the sensation of a pelvic mass in 97.48%. We recorded three cases of infertility as associated factors. Ultrasound was performed in all patients. The location of the myxomatous nuclei was subserosal in the majority in 42.86% and with multiple nodules in 84.85% of cases. The main indication for myomectomy was the failure of medical treatment in 86.49% of cases. Surgical treatment was mainly a myomectomy 93.30%, a hysterectomy was indicated in 6.70%. The jasmonic acid （JA） pathway is the main signal-transduction pathway induced by insect folivory. Mutant plants affected in the jasmonate pathway （18：0 and/or 16：0-oxylipin routes） were studied to assess the effects of JA and its oxylipin intermediates 12-oxophytodienoic acid （OPDA） and dinor-OPDA （dnOPDA） on interconnected signal- transduction pathways that underlie induced defenses in Arabidopsis. Our data show that the oxylipin jasmonates dnOPDA, OPDA and JA have different roles in defense signaling induced after feeding by the chewing-biting caterpillar Pieris rapae. Jasmonic acid, and not OPDA or dnOPDA, is the major signaling compound required for the induction of the defense-related genes LOX2 （Lipo：~vgenase 2）, OPR3 （12-Oxophytodienoate reductase 3）, ACX1 （Acyl-CoA oxidase 1） and PAL1 （Phenvlalanine ammonia-lyase 1）. Monitoring PAL1 transcript levels clearly showed that accumulation of JA upon P rapae feeding results in the induction of the salicylic acid pathway. Furthermore, JA is the major signaling compound required for the P rapae-induced expression of the defense-related gene HPL1 （Hydroperoxide lyase 1）. The jasmonate dnOPDA influences the induction of the HPk- branch as well, yet its effect is antagonistic to the effect of JA. Our data show that these jasmonates may be used to fine-tune Arabidopsis＇ herbivore-induced responses in terms of the HPL-branch from the oxylipin pathway.

Objective: To study the epidemiological profile of stress among health professionals in North benin in 2021. Methods: It was a cross-sectional and descriptive study with health professionals in the Parakou-N’dali and Tchaourou health zones. These two areas have a population of 462,942 inhabitants served by two university hospitals, three area hospitals and 19 health centers. Sampling was exhaustive by free and informed consent. The data was collected through a questionnaire. Studied socio-professional characteristics and stress. The level of stress was assessed using the Karasek questionnaire. Results: Of 813 caregivers approached, 692 were investigated. The participation rate was 85.1%. The average age was 38 years old. Subjects aged 28 - 38 were more numerous, as were those aged between 1 and 10 years. The prevalence of stress was 73.8%. Its level was high at 17.8% and very high at 4.5%. Women were affected (76.6%) than men (70.7%). Within the professional category, midwives were the most affected (82.5%). Respondents from the denominational sector were 90.8% reached. Conclusion: Professional stress is endemic in health facilities in northern benin, particularly faith-based ones. Action must be taken to reduce it.
