Loss of the vitamin D receptor in human breast and prostate cancers strongly induces cell apoptosis through downregulation of Wnt/β-catenin signaling

Vitamin D co-regulates cell proliferation, differentiation and apoptosis in numerous tissues, including cancers. The known anti-proliferative and pro-apoptotic actions of the active metabolite of vitamin D, 1,25-dihydroxy-vitamin D [1,25（OH）2D] are mediated through binding to the vitamin D receptor （VDR）. Here, we report on the unexpected finding that stable knockdown of VDR expression in the human breast and prostate cancer cell lines, MDA-MB-231 and PC3, strongly induces cell apoptosis and inhibits cell proliferation in vitro. Implantation of these VDR knockdown cells into the mammary fat pad （MDA-MB-231）, subcutaneously （PC3） or intra-tibially （both cell lines） in immune-incompetent nude mice resulted in reduced tumor growth associated with increased apoptosis and reduced cell proliferation compared with controls. These growth-retarding effects of VDR knockdown occur in the presence and absence of vitamin D and are independent of whether cells were grown in bone or soft tissues. Transcriptome analysis of VDR knockdown and non-target control cell lines demonstrated that loss of the VDR was associated with significant attenuation in the Wnt/0-catenin signaling pathway. In particular, cytoplasmic and nuclear β-catenin protein levels were reduced with a corresponding downregulation of downstream genes such as Axin2, Cyclin D1, interleukin-6 （IL-6）, and IL-8. Stabilization of 0-catenin using the GSK-3β inhibitor BIO partly reversed the growth-retarding effects of VDR knockdown. Our results indicate that the unliganded VDR possesses hitherto unknown functions to promote breast and prostate cancer growth, which appear to be operational not only within but also outside the bone environment. These novel functions contrast with the known anti-proliferative nuclear actions of the liganded VDR and may represent targets for new diagnostic and therapeutic approaches in breast and prostate cancer.

Characterization of the Bacillus subtilis Penicillin-Binding Protein PBP4

Purpose: The PBP4* is a Penicillin Binding Protein belonging to the class C of AmpH type whose function remains poorly understood. This study aimed to evaluate the biophysical and enzymatic properties of the Bacillus subtilis PBP4* to gain insights into its role in the context of bacterial cell wall recycling. Methods: To characterize the PBP4*, the full-length PBP4* and its N-terminal penicillin-binding domain have been produced in Escherichia coli and purified. Results: A comparison of biophysical properties has shown that both recombinant proteins are monomeric in solution and retain the same thermal stability. On the other hand, the D-alanine methyl esterase activity detected with the full-length PBP4* is impeded by the cleavage of the 92 amino acid C-terminal domain. The esterase activity of the full-length PBP4* strates a clear D-stereospecificity. The PBP4* is also active on B. subtilis cell walls bearing teichoic acids, compounds commonly substituted with D-alanine residues. Conclusions: Our results are in agreement with the hypothesis that PBP4* could play a role in recycling cell wall components, as previously suggested.

Objectives: To evaluate the long-term outcome of adolescents with anorexia nervosa and attempt to define prognostic factors for outcome. Methods: Transversal, descriptive epidemiological study with a strict methodology: minimum interval of 4 years since first admission, evaluation using clearly defined diagnostic criteria (EAT-40 self-evaluation, HSCL, global patient-driven self-evaluation and hetero evaluation: clinical interview and LECE). The 198 subjects are former adolescent anorexics treated at the Hôpital d’Enfants (Children’s Hospital) in Nancy (France). Results: Clinical evaluation of the 144 subjects traced showed positive outcome for 68.1%, intermediate outcome for 23.2% and poor outcome for 8.7%. 5 factors for a good prognosis were found (middle socioeconomic class of parents, no prior treatment, hospitalization > 1 month,

目的与常规服务相比,评估健康教练使用动机谈话(MI)的干预对改善2型糖尿病(T2DM)患者的血糖控制,以及临床、心理社会和自我保健结果的影响。设计实用性群组随机对照试验。研究地点中国北京市丰台区社区卫生服务站(CHSs)。参与者在41家随机选取的CHSs(21家干预组和20家对照组),有21家干预组CHSs(372名患者)和18家对照组(296名患者)在开始时参与试验。干预干预组患者接受电话和面对面MI健康教练服务,并同时在CHSs接受常规的服务。对照组只接受常规的服务。2组的医疗费用均得到减免。结局指标在基线、6个月和12个月进行结果评估。主要指标是糖化血红蛋白(HbAlc)。次要指标包括一组人体测量学指标、血压、空腹血糖、社会心理和自我保健。结果在12个月时,没有发现HbA1c的治疗效果差别,[调整后差别为0.02,95%可信区间(CI)-0.40~0.44,P=0.929],干预组和对照组都显示出明显的改善。不过,2个次要指标:心理忧郁(调整后差别为-2.38,95%CI-4.64~-0.12,P=0.039)和收缩压(调整后差别为-3.57,95%CI-6.08~-1.05,P=0.005)显示为明显改善的结果,与明显的治疗效果差别相一致,并得到敏感性分析的支持。有意义的是,在HbA1c之外,两组均显示出甘油三酯、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇上的明显改善。结论在当今中国初级卫生保健改革的形势下,本研究是在中国第一个在CHSs实际现场开展的、特别针对T2DM的大规模群组随机对照研究。虽然没有在HbA1c上发现治疗效果差别,但在2组的很多结果(包括HbAlc)上呈现改善,该结果支持在中国对T2DM患者进行常规和免费的临床健康检查。

Self-Management of Type 2 Diabetes in Middle-Aged Population of Pakistan and Saudi Arabia

This research protocol design is aimed at exploring the qualitative health research in self-management of Type 2 diabetes and patient’s perceptions and experiences of undertaking physical activity and eating behaviour as part of their diabetes self-management. In addition, the study would analyze how the health issue related to diabetes is viewed and addressed in the community (Pakistan and Saudi Arabia) and would use the concepts of socio-ecological approach to self-management of Type 2 diabetes and explore the factors affecting the self-management practices in these countries. The other objective of this protocol is to examine the role of physical inactivity and obesity in the development of Type 2 diabetes and its self-management in middle-aged population living in rural area of Pakistan and to evaluate a lifestyle intervention (Physical Activity and Diet) in the management of Type 2 diabetes. The brief review conducted in this protocol design will identify the potential areas of health care which need attention including the overall functioning of community healthcare clinics to diabetes care in terms of recognizing the symptoms of diabetes to early detection and diagnosis, easy access to community doctors. This review will impress upon the need to recognize that in developing strategies and interventions to address diabetes, self-care, family support, community education and community ownership are important and it will be demonstrated by the comparison of two culturally diversified populations of Pakistan and Saudi Arabia in relation to the self-management of Type 2 diabetes.

目的探讨在中国传统文化背景下,社会交往模式对2型糖尿病患者自我管理行为的影响。方法采用社会学研究方法,对北京市丰台区10家社区卫生服务中心参与快乐生活俱乐部TM项目的 33名糖尿病患者进行半结构式个人深入访谈。结果我国的2型糖尿病患者的疾病自我管理行为受到了传统中国文化下社会交往模式的影响,这种模式以利他倾向、集体主义倾向、补充或替代主流信息源为主,具体表现为患者选择性公开自己患病的事实,出于对社会交往的需要而缺乏对饮食、酒精和烟草的控制,但加强了集体锻炼的机会,通过朋友或熟人补充正式服务系统的信息传播和同理心不足。结论中国传统文化下的社会交往模式会对糖尿病患者的自我管理行为产生消极或积极的影响。建议初级卫生保健专业人员在帮助患者管理糖尿病时,理解社会交往的文化属性,并在帮助患者改变行为过程中,收集和使用患者的社会交往信息来启发他们找到适合自己的慢性病管理方案。
