Evaluation of the Functionality of the Networking of Sexual and Reproductive Health Rights Services in the Event of Gender-Based Violence in the Commune of Kpomasse in 2022

Introduction: In developing countries, gender-based violence (GBV) is a real public health problem. In Benin, GBV affects the majority of women and girls (69%). Benin has implemented strategies and set up integrated centers for the management of violence in order to reduce cases of violence and ensure the holistic management of victims. The objective of our study was to assess the functionality of the network of sexual and reproductive health rights (SRHR) in case of GBV in the commune of Kpomasse in 2022. Method: This descriptive and evaluative study was conducted from March 21 to April 11, 2022. The sampling method used was non-probabilistic. Reasoned choice and convenience were the techniques used for the different targets of the study. The functionality of the SRHR service network was assessed first by calculating scores at the structure, process and outcome levels, and then by analysis using the human rights-based approach. Results: Out of the 34 GBV victims identified, only one had received a full response and 54% of the victims had no knowledge of SRHR. The lack of knowledge about health care facilities was 41% for victims and 80% for non-victims in the community who participated in the study. In the case of gender-based violence, the community preferred to settle out of court rather than report it. The functionality of the networking of sexual and reproductive health rights services in the event of the occurrence of gender-based violence in the commune of Kpomassè is insufficient. Lack of knowledge of the roles of rights holders (DD) and duty bearers (DO) explains the insufficient functionality of networking. Conclusion: Training of SRHR service agents and community sensitization are essential to improve the functionality of SRHR service networking in the commune of Kpomasse.

Prevalence and Predictors of Double Burden of Malnutrition within Households in Africa: A Systematic Review

In recent years, there has been growing interest in the emergence of double burden of malnutrition (DBM) in Africa. In this study, we reviewed the literature on double burden of malnutrition in households, reviewing previous studies investigating the prevalence of DBM in Africa and the factors associated with it. To identify relevant studies, we consulted the PubMed and Cochrane electronic databases, using specific search terms. A total of seventeen articles met the eligibility criteria. These articles were published between 2012 and 2022, and their data were collected between 2000 and 2019. Twelve of these studies used secondary data, including demographic and health surveys. The age of children and adults varied from study to study. All studies used Body Mass Index as a nutritional indicator for adults. For children, the height-for-age Z-score was most commonly used, while weight-for-age, weight-for-height and Body Mass Index-for-age were less commonly used. The national prevalence of double nutritional burden in households ranged from 1.71% to 38.7%, depending on the country and the year. However, direct comparisons between studies were limited due to differences in combinations of undernutrition, overweight or obesity. Among the factors associated with double nutritional burden within households, the most frequently cited in the selected articles were urban/rural residence, income or socioeconomic status, age of child and mother, household size and mother’s level of education. However, no study assessed physical activity, and very few examined the diet of household members. It is essential to take these different parameters into account when designing and implementing interventions to prevent the DBM in Africa. Community and societal factors will also need to be studied and taken into account in these interventions.

Sanitary Quality of the Meals Served in the Canteens and Vicinity of the Lagoon Mother and Child University Hospital Centre and National Hospital and University Centre Hubert Koutoukou Maga of Cotonou (Benin)

Background: Food safety is an integral part of food security. It refers to the protection of the food supply against risks of microbial, chemical and physical origin at every stage of the food chain. The study aimed to investigate the sanitary quality of meals served in the canteens and vicinity of the Lagoon Mother and Child University Hospital Centre (CHU-MEL) and National Hospital and University Centre Hubert Koutoukou Maga (CNHU-HKM) of Cotonou. Methods: It was a cross-sectional with a data collection period from 16 March to 22 April 2021. There were 32 vendors surveyed and 40 meals sampled for microbiological examination. We collected data on the vendors’ socio-demographic characteristics, working environment, health status, knowledge and food hygiene practices. Results: About 83% of the meals contained pathogenic microorganisms. All meals had a temperature from 4°C to 60°C called the “danger zone”. The level of knowledge about food hygiene was low in 65.7% of the vendors. About 47% had poor personal and clothing hygiene, food storage and service practices. Conclusion: These results show that the meals sold in the CHU-MEL and CNHU-HKM could present a health risk for the consumer. There is a need for improvement of the sanitary quality of the meals delivered in the canteens and vicinity of these hospitals.

Factors Associated with Diet Quality in Women of Childbearing Age in Southern Benin in 2022

Background: The nutritional status of women while in pregnancy influences foetus growth, infant development, and the mother’s health. The objective of this study was to assess diet quality and associated factors among women of childbearing age in the Comè community, Benin. Methods: The cross-sectional and analytical study involved 215 participants randomly selected by multi-stage sampling technique. Two non-consecutive dietary recall data were collected as well as sociodemographic information. The International Food Quality Index (IQD-I) was used to assess diet quality. Factors associated with the nutritional quality of the diet were identified by using linear regression. Results: Among 212 women, a diet quality of 175 (82.55%) was fair while 14.62% had good diet quality. Factors associated with the diet quality were the level of household possession, health issues, and nutritional advice at the health care facilities. Conclusion: Nutritional education interventions and income-generating activities may improve the diet quality among women of childbearing age in the Comè in Benin.

Prevention of Maternal Low-Grade Chronic Inflammation: The Diet’s Inflammatory Potential in Pregnant Women in Southern Benin

Background: Pathogenic mechanisms link maternal systemic low-grade chronic inflammation to intrauterine development. A pro-inflammatory diet during pregnancy is associated with chronic maternal systemic inflammation. The objective of the study was to determine the diet inflammatory index and identify associated foods and nutrients in pregnant women of Ouidah city, Benin. Methods: The cross-sectional study conducted among pregnant women randomly selected by cluster sampling. Nutrient intakes and food consumed were estimated based on data of two 24-hour non-consecutive recalls. The inflammatory weightings of each nutrient were used to calculate the diet inflammatory index for each pregnant woman. Results: The average age of the 310 pregnant women surveyed was 27.20 ± 5.99 years. The median diet inflammatory index was -3.026 (-16.59;1.86). The pregnant women with inflammatory diet accounted for 11.30%. Vitamin A and zinc were negatively correlated with the diet inflammatory index. Fruits, roots, tubers and sweetened drinks consumed were positively correlated with the diet inflammatory index. On the contrary, the quantities of cereals, vegetables, legums and fish consumed were negatively correlated with the diet inflammatory index. Conclusion: More than one pregnant woman out of ten in the Ouidah city had a pro-inflammatory diet. Reducing maternal systemic low-grade chronic inflammation may include increasing the consumption of identified anti-inflammatory foods.

Assessing Community Health Interventions in Reducing Infant and Child Mortality in the Southeastern Benin

Background: To reduce infant and child mortality in Benin, a package of high-impact interventions per healthcare level was implemented in 2009. This study aimed to assess the quality of community-based health interventions in reducing infant and child mortality within the municipality of Pobè in southeastern Benin. Methods: This was a cross-sectional evaluative study carried out in November 2021 focused on children aged 0 - 59 months, their mothers, health workers, community facilitators, community health workers and the Town Hall health focal point. Mothers and their children were targeted by cluster sampling, and exhaustive selection was used to recruit all other participants. Predetermined scores based on rating criteria were used to assess the quality of community health interventions using the “input, process and outcome” of Donabedian approach. Results: Over 300 mother-child couples, 46 community health workers, 7 health agents, 1 community facilitator and 1 health focal point from Pobè town hall were surveyed. Intervention quality was judged as “average”, with a score of 73.80%. The “inputs” and “outcomes” components were the weakest links. Conclusion: Improving access to the inputs needed by community health workers can enhance the quality of PIHI interventions.

Introduction: Diabetes mellitus is a chronic disease that leads to premature death and disability. This disease increases healthcare costs and hinders the development of countries, compromising their ability to achieve the Sustainable Development Goals (SDGs) for health. The aim of the study was to investigate the determinants of diabetes mellitus in adults in the commune of Kpomassè in 2024. Methods: This was a cross-sectional, descriptive, analytical study carried out in March 2024. Residents aged 18 to 69 were selected using the World Health Organization (WHO) probability cluster sampling method. Data collection was carried out using an electronic questionnaire administered to respondents by interviewers, in compliance with ethical principles. Data were analyzed using STATA 15 software. Determinants of diabetes were identified using a multiple logistic regression model, with a threshold set at 5%. Results: A total of 308 subjects, including 156 men, were surveyed. The prevalence of diabetes was 14.94%. Five determinants were associated with diabetes: gender (p = 0.038), age (p = 0.006), sleep duration (p = 0.026), consumption of fried foods (p = 0.024) and stress (p = 0.013). Conclusion: Diabetes among adults in Kpomassè poses a serious public health problem. It is imperative to strengthen diabetes prevention efforts through concerted action by the competent authorities.

Effects of a Community-Based Intervention on the Lifestyle and Health of People at Risk of Type 2 Diabetes in Benin

Introduction: Worldwide, the prevalence of type 2 diabetes (T2D) is increasing at an epidemic rate. The objective of this study was to measure the effects on lifestyle intervention in subjects at risk of T2D in a West African country. Methods: This study involved Beninese adults at risk of T2D randomly selected from 2 groups of villages: control villages and villages exposed to nutrition education, cooking demonstrations and physical activity sessions. Anthropometric, clinical, and biological measurements were taken in both groups at the beginning and end of the intervention, which lasted 12 months. The paired sample t-test was used to assess the effect of the intervention on the lifestyle of people at risk of T2D. Results: After 12 months of exposure to nutrition education, cooking demonstrations and physical activity sessions in the intervention villages, the mean body mass index, waist circumference, body fat percentage, blood glucose, triglycerides, total cholesterol and amount of alcohol consumed per week had significantly decreased. Conversely, the mean intensity of physical activity and food consumption score had significantly increased. In the control group, 5.6% of the people at risk developed to diabetes. The relative risk was RR = 0.20 [0.14 - 0.56]. Conclusion: The intervention significantly improved metabolic indicators, intensity of leisure activity and dietary score. We suggest larger studies to better assess the effect of community-based interventions on the lifestyle and health of people at risk of T2D in Africa.

Epidemiology of Low Birth Weight in the Lake Areas of Cotonou (Benin Republic)

Introduction: Low birth weight is a significant public health problem worldwide, particularly in low-and-middle-income countries. This study aimed to investigate the epidemiology of low birth weight in the lake areas of Cotonou. Methods: A cross-sectional analytical study included 931 records of women who gave birth in 2022 in the Cotonou 1-4 health zone. Completed maternity records were included in this study, except for premature births and congenital malformations. Birth weight was the dependent variable. Sociodemographic, obstetrical, preventive care and child-related data were collected using a digitized form. These data were analyzed using SPSS software to determine the prevalence and factors associated with low birth weight using multiple logistic regression analysis. Results: The prevalence of low birth weight was 16.30%. Factors related to this prevalence were non-use of mosquito nets during pregnancy (OR = 2.72;CI: 1.275 - 5.805), twin pregnancy (OR = 9.97;CI: 3.869 - 25.696), previous abortion (OR = 1.61;CI: 1.034 - 2.515), low number of pregnancy (OR = 3.97;CI: 1.276 - 12.393), trimester of first antenatal visit (OR = 3.47;CI: 1.821 - 6.638) and birth size less than 45 cm (OR = 5.98;CI: 2.965 - 12.083). Conclusion: The prevalence obtained from this study justifies the need to support pregnant women in this health zone. It is, therefore, essential to strengthen communication and health promotion strategies for women before, during, and after pregnancy.

Functional Constipation in the General Population in Cotonou:Prevalence and Associated Socio-Demographic Factors

Background: Constipation is considered rare in sub-Saharan Africa because of the high fiber content of the local diet. However, certain factors could intervene in this pathology. The purpose of this study was to assess the prevalence of constipation in the general population in Cotonou and to determine the associated socio-demographic factors. Methods: This was a cross-sectional descriptive and analytical study conducted from July to August 2017 in 7 districts of the township of Cotonou, according to a cluster sampling. We included anyone over the age of 15 living in the city of Cotonou for at least 6 months. Constipation was either self-reported or functional (meeting the Rome IV criteria). Results: A total of 1058 participants were included: 574 men (sex ratio of 1.2), mean age of 29 years [range 15 - 92]. Self-reported constipation was noted in 512 patients (48.5%). It was functional in 256 cases (24.2%). In the latter category, there was a predominance of women (138, 53.9%), of secondary education (46.5%), of self-employed professionals (50.4%), from the Ouémé-Plateau (41.8%). Were associated with the occurrence of functional constipation (univariate analysis), age ≥ 60 years (p = 0.049), female gender (p = 0.003), and subjects living alone (166, 64.8%) vs. those living in a relationship (90, 35.2%, p = 0.001). Conclusion: Constipation was not as rare in Cotonou. It was more common in women, subjects over 60 years old or living alone.
