Fondaparinux用于预防老年急症内科患者静脉血栓栓塞的有效性和安全性:一项随机、安慰剂对照试验

Objective: to determine the efficacy and safety of the a.t.coagula.t.fondaparinux in older acute medical inpatients at moderate to high risk of venous thromboembolism. Design: Double blind randomised placebo controlled trial. Setting: 35 centres in eight countries. Pa.t.cipa.t.: 849 medical patients aged 60 or more admitted to hospital for congestive hea.t.failure, acute respiratory illness in the presence of chronic lung disease, or acute infectious or inflammatory disease and expected to remain in bed for at lea.t.four days. Interventions: 2.5 mg fondaparinux or placebo subcutaneously once daily for six to 14 days. Outcome measure: the primary efficacy outcome was venous thromboembolism detected by routine bilateral venography along with symptomatic venous thromboembolism up to day 15. Secondary outcomes were bleeding and death. Patients were followed up at one month. Results: 425 patients in the fondaparinux group and 414 patients in the placebo group were evaluable for safety analysis(10 were not treated). 644 patients(75.9% ) were available for the primary efficacy analysis. Venous thrombembolism was detected in 5.6% (18/321) of patients treated with fondaparinux and 10.5% (34/323) of patients given placebo, a relative risk reduction of 46.7% (95% confidence interval 7.7% to 69.3% ). Symptomatic venous thromboembolism occurred in five patients in the placebo group and none in the fondaparinux group(P=0.029). Major bleeding occurred in one patient(0.2% ) in each group. At the end of follow- up, 14 patients in the fondaparinux group(3.3% ) and 25 in the placebo group(6.0% ) had died. Conclusion: Fondaparinux is effective in the prevention of asymptomatic and symptomatic venous thromboembolic events in older acute medical patients. the frequency of major bleeding was similar for both fondaparinux and placebo treated patients.

Cdc42 regulates the cellular localization of Cdc42ep1 in controlling neural crest cell migration

the member of Rho family of small GtPases Cdc42 plays importa.t.and conserved roles in cell polarity and motility. the Cdc42ep family proteins have been identified to bind to Cdc42, yet how they intera.t.with Cdc42 to regulate cell migration remains to be elucidated. In this study, we focus on Cdc42epl, which is expressed predomina.t.y in the highly migratory neural crest ceils in frog embryos. through morpholino-mediated knockdown, we show that Cdc42epl is required for the migration of cranial neural crest cells. Loss of Cdc42epl leads to rounder cell shapes and the formation of membrane blebs, consistent with the observed disruption in a.t.n organization and focal adhesion alignment. As a result, Cdc42ep1 is critical for neural crest cells to apply tra.t.on forces at the correct place to migrate efficiently. We further show that Cdc42ep1 is localized to two areas in neural crest celts： in membrane protrusions together with Cdc42 and in perinuciear patches where Cdc42 is absent. Cdc42 directly intera.t. with Cdc42epl （through the CRIB domain） and changes in Cdc42 level shift the distribution of Cdc42epl between these two subcellular locations, controlling the formation of membrane protrusions and directionality of migration as a consequence. these results suggest that Cdc42ep1 elaborates Cdc42 a.t.vity in neural crest cells to promote their efficient migration.

Channel dropping waveguide filters based on single and technology are of great interest due to their compa.t.ess multiple resonators in silicon-on-insulator （SOI） and high wavelength selectivity, which is a desirable feature for photonic modulators, detectors, and other optically integrated components in telecommu- nication systems, in pa.t.cular for wavelength division multiplexing （WDM） systems. Utilizing Solar thermal Energy for Post-Combustion C02 Ca.t.re

Post-combustion amine absorption and stripping can remove 90% of the CO2 from power pla.t.flue gas, but systems can reduce electrical output by approximately 30% due to energy requirements for stripping CO2 from solvent and CO2 compression. the CO2 ca.t.re energy pena.t. can be reduced while developing renewable energy technologies by meeting CO2 ca.t.re energy requirements with a solar thermal energy system, pa.t.cularly when electricity demand and prices are the highest. this study presents an initial review of solar thermal technologies for supplying CO2 ca.t.re energy, with a focus on high temperature systems. Parabolic troughs and central receivers are technically able to provide energy for CO2 ca.t.re. However, the solar system＇s capital costs would be roughly half that of the base coal-fired pla.t.with CO2 ca.t.re, and high electricity prices are required to offset the costs of operating the solar thermal system. For high temperature solar thermal systems, direct electricity generation is likely a more efficient way to use solar energy to replace output lost to CO2 ca.t.re energy. However, low temperature solar thermal systems might integrate better with solvent stripping equipment, and more rigorous analysis is required to definitively assess the feasibility of using solar energy for CO2 ca.t.re.
