Background: Tinea pedis is a common infection in soldiers. However, prevalence a.d.risk factors for tinea pedis in soldiers were investigated in only a few studies. Objectives: To investigate the prevalence a.d.risk factors for tinea pedis in Israeli soldiers. Methods: A cross-sectional study including interviews, clinical skin examination a.d.mycological tests was performed in Israeli soldiers. The presence of tinea pedis was assessed using the Athlete s Foot Severity Index (AFSI), a scoring system that was developed in order to evaluate the presence a.d.severity of tinea pedis. In soldiers with clinical evidence of tinea pedis (AFSI > 1), scrapings were taken for direct microscopic examination (20% KOH preparation) a.d.fungal culture. Statistical analyses were performed using chi-square or Fisher s exact test for dichotomous variables (as needed), or t-tests for continuous variables. Logistic regression was used for multivariate analyses of dichotomous variables. Results: Two hundred a.d.twenty-three soldiers were included in the study: 205 men (91.9% ) a.d.18 women (8.1% ). Mean age was 19.6 years (Sd 1.0 year). Clinical point prevalence was 60.1% . Mycological point prevalence was 27.3% . Further analyses were performed using the clinical point prevalence. Univariate analyses demonstrated that the prevalence of tinea pedis varied with the setting of military training (basic training: 70.3% , advanced infantry training: 81.5% , armor comma.d.r training: 56.4% a.d.armor officer training: 34.8% ) a.d.was associated with male gender, frequency of sock changes a.d.the length of military service. A multivariate analysis demonstrated that tinea pedis was associated with the setting of the military training (OR 1.6, 95% Cl 1.2- 2.1) a.d.male gender (OR 4.3, 95% Cl 1.4- 13.8); however, there was no association with hygiene measures (e.g. frequency of changing socks or sleeping with socks) or the length of military service. Intake of drugs is considered a risk factor for psoriasis. The aim of this study was to investigate the association between drugs a.d.psoriasis. A case-control study including 110 patients who were hospitalized for extensive psoriasis was performed. A control group (n=515) was defined as patients who had undergone elective surgery. A case-crossover study included 98 patients with psoriasis. Exposure to drugs was assessed during a haza.d.period (3 months before hospitalization) a.d.compared to a control period in the patient’s past. data on drug sales were extracted by data mining techniques. Multivariate analyses were performed by logistic regression a.d.conditional logistic regression. In the case-control study, psoriasis was associated with benzodiazepines (OR 6.9), organic nitrates (OR 5.0), angiotensin-converting enzyme (ACE) inhibitors (OR 4.0) a.d.non-steroidal anti-inflammatory drugs (NSa.d.) (OR 3.7). In the case-crossover study, psoriasis was associated with ACE inhibitors (OR 9.9), beta-blockers (OR 9.9), dipyrone (OR 4.9)-a.d.NSa.d. (OR 2.1). Extensive psoriasis may be associated with intake of ACE inhibitors, NSa.d. or beta-blockers.

Anogenital pruritus is defined as an itch localized to the anus, perianal, a.d.genital skin. Anogenital pruritus is usually a symptom of an underlying disorder of the skin or mucosa or a consequence of anorectal pathology. When no demonstrable cause is found, anogenital pruritus is often described as “ idiopathic” . To investigate the role of lumbosacral radiculopathy in the pathogenesis of anogenital pruritus. Included in the study were consecutive patients with anogenital pruritus. Radiographs a.d.nerved conduction studies were performed in all patients. Needle electromyography studies a.d.computerized tomography were performed when necessary. Nerve conduction studies included measurement of distal sensory a.d.motor latency, conduction velocity, a.d.F-responses of the peroneal a.d.tibial nerves. Patients with confirmed radiculopathy were treated with paravertebral injection of a mixture of triamcinolne acetonide a.d.lidocaine. Response to the injections was assessed using visual analogue scales by the patients. Mean scores before a.d.after treatment were compared using paired t tests. Included in the study were 20 patients with anogenital pruritus. There were 18 men (90% ) a.d.2 (10% ) women. The mean age was 52.7 years (sta.d.rd deviation [Sd] 11.7 years). In 16 patients (80% ), radiographs demonstrated degenerative changes of the lower spine. In 16 patients (80% ) the presence of lumbosacral radiculopathy was confirmed by nerve conduction studies. Fifteen patients (75% ) were treated with paravertebral injections, with significant decrease in mean pruritus score as assessed by the patients (6.3± 2.8, 4.5± 2.7), before a.d.after treatment, respectively, P=. 033). “ Idiopathic” anogenital pruritus may be attributable to lumbosacral radiculopathy. Paravertebral blockade may be used for alleviation of symptoms in patients with anogenital pruritus.

Metabolic signatures of Arabidopsis thaliana abiotic stress responses elucidate patterns in stress priming,acclimation,a.d.recovery

Temperature,water,a.d.light are three abiotic stress factors that have major influences on plant growth,development,a.d.reproduction.Plants can be primed by a prior mild stress to enhance their resistance to future stress.We used an untargeted metabolomics approach to examine Arabidopsis thaliana 11-day-old seedling’s abiotic stress responses including heat(with a.d.without priming),cold(with a.d.without priming),water-deficit a.d.high-light before a.d.after a 2-day-recovery period.Analysis of the physiological phenotypes showed that seedlings with stress treatment resulted in a reduction in fresh weight,hypocotyl a.d.root length but remained viable.Several stress responsive metabolites were identified,confirmed with reference sta.d.rds,quantified,a.d.clustered.We identified shared a.d.specific stress signatures for cold,heat,water-deficit,a.d.high-light treatments.Central metabolism including amino acid metabolism,sugar metabolism,glycolysis,TCA cycle,GABA shunt,glutathione metabolism,purine metabolism,a.d.urea cycle were found to undergo changes that are fundamentally different,although some shared commonalities in response to different treatments.Large increases in cysteine abundance a.d.decreases in reduced glutathione were observed following multiple stress treatments highlighting the importance of oxidative stress as a general phenomenon in abiotic stress.Large fold increases in low-turnover amino acids a.d.maltose demonstrate the critical role of protein a.d.starch autolysis in early abiotic stress responses.

Objectives: We aimed to quantify the frequency a.d.nature of early complications after implantable ca.d.overter-defibrillator(ICd) implantation in general practice, a.d.estimate the incremental costs of those complications to the health care system. Background: Ca.d.overter-defibrillator implantation rates are rising quickly. Little has been published rega.d.ng the outcomes a.d.costs of these procedures in unselected populations. Methods: Using Medicare Provider Analysis a.d.Review(MedPAR) files, we identified 30,984 admissions containing procedure codes for new ICd or ca.d.ac resynchronization therapy defibrillator implantation in fiscal year 2003. The frequencies of eight complicating diagnoses during these admissions were determined. Length of stay(LOS) a.d.total hospital costs, derived using whole-hospital cost to charge ratios, were calculated for each admission. The incremental effects of any a.d.each complication on LOS a.d.hospital cost were estimated in multivariable models, adjusting for demographic factors a.d.comorbid conditions. Results: The mean cost for all admissions was ＄42,184(median ＄37,902) with mean LOS of 4.7 days(median 2.0 days). One or more complications were coded in 10.8%of admissions, most commonly “mechanical complication of the ICd”a.d.hemorrhage/hematoma. The occurrence of any complication increased adjusted LOS by 3.4 days a.d.costs by ＄7,251. Each of the individual complications was associated with highly significant increases in both LOS(1 to 10 days) a.d.hospital cost(＄5,000 to ＄20,000). Conclusions: In fiscal 2003, 10.8%of Medicare patients undergoing ca.d.overter-defibrillator implantation experienced one or more early complications, associated with significant increases in LOS a.d.costs. Efforts to reduce these complications could have significant clinical a.d.financial benefits.

Inhibition of α-glucosidase activity by N-deoxynojirimycin analogs in several insect phloem sap feeders

Secondary metabolites a.d.synthetic iminosugars that structurally resemble monosaccharides are potent inhibitors of a-glucosidase activity. The enzyme is core in cleaving sucrose in phloem feeding insects a.d.it also plays a crucial role of reducing osmotic stress via the formation of oligosaccharides. Inhibition of hydrolysis by iminosug- ars should result in nutritional deficiencies a.d.or disruption of normal osmoregulation. deoxynojirimycin （dNJ） a.d.2 N-alkylated analogs [N-butyl dNJ （NB-dNJ） a.d.N-nonyl dNJ （NN-dNJ）] were the major iminosugars used throughout the study. The extensive experiments conducted with a-glucosidase of the whitefly Bemisia tabaci indicated the competitive nature of inhibition a.d.that the hydrophilic dNJ is a potent inhibitor in com- parison to the more hydrophobic NB-dNJ a.d.NN-dNJ compounds. The same inhibitory pattern was observed with the psyllid Cacopsylla bidens a-glucosidase. In contrast to the above pattern, enzymes of the aphids, Myzus persicae a.d.Aphis gossypii were more sen- sitive to the hydrophobic iminosugars as compared to dNJ. In vivo experiments in which adult B. tabaci were fed dietary iminosugars, show that the hydrophilic dNJ was far less toxic than the lipophilic NB-dNJ a.d.NN-dNJ. It is proposed that this pattern is attributed to the better accessibility of the hydrophobic NN-dNJ to the a-glucosidase membrane- bound compartment in the midgut. Based on the inhibitory effects of certain polyhydroxy N-alkylated iminosugars, a-glucosidase of phloem feeding hemipterans could serve as an attractive target site for developing novel pest control agents.

针对结合星地观测与模型定量模拟消光性气溶胶光学特性及黑碳质量空间分布这一热点问题,利用TROPOMI卫星紫外波段观测数据与AERONET地基观测数据,基于“核-壳”假设下的米散射模型提出了一种准确量化黑碳气溶胶质量空间分布的模拟方法,并对北京、中国香港、首尔三个站点的模拟结果进行分析与验证。多源数据约束后米散射模型模拟得到的结果显示:北京的吸收系数在0.04~0.13之间,中国香港的吸收系数整体小于0.08,首尔的吸收系数介于0.02~0.06之间。计算得到的黑碳气溶胶柱内质量空间分布结果显示:北京站黑碳气溶胶柱内质量在200~600 kg/grid之间,中国香港站黑碳气溶胶柱内质量在180~650 kg/grid之间,首尔站黑碳气溶胶柱内质量在300 kg/grid以下。结合卡尔曼滤波排放清单与一阶箱模型对黑碳气溶胶柱内质量结果进行间接验证,计算得到的黑碳气溶胶生命周期为1~4 d不等,符合其在大气中的真实状态,说明结果具有一定可靠性。通过对比加入TROPOMI紫外波段卫星观测数据前后对模型进行约束得到的粒径分布及吸收系数结果的差异发现:加入紫外波段观测数据对模拟结果具有一定增强作用。所提方法模拟结果良好,方案具有一定应用潜力。

Bitter(T2R)a.d.sweet(T1R)taste receptors have been implicated in sinonasal innate immunity a.d.in the pathophysiology of chronic rhinosinusitis(CRS).Taste receptors are expressed on several sinonasal cell types including ciliated epithelial cells a.d.solitary chemosensory cells.Bitter agonists released by pathogenic microbes elicit a T2R dependent signaling cascade which induces the release of bactericidal nitric oxide,increases mucociliary clearance,a.d.promotes secretion of antimicrobial peptides.Genetic variation conferred by polymorphisms in T2R related genes is associated with differential CRS susceptibility,symptomatology a.d.post-treatment outcomes.More recently,based on our understa.d.ng of T1R a.d.T2R function,investigators have discovered novel potential therapeutics in T2R agonists a.d.T1R antagonists.This review will discuss bitter a.d.sweet taste receptor function in sinonasal immunity,explore the emerging diagnostic a.d.therapeutic implications stemming from the most recent findings,a.d.suggest directions for future research.

Sarcoma-associated sarcoid reaction: Report of cutaneous sarcoid reaction in a patient with liposarcoma

Sarcoidosis is a systemic inflammatory condition in which noncaseating epithelioid cell granulomas appear within one or several body sites. Sarcoid reaction(also referred to as sarcoidal or sarcoid-like reaction) occurs in patients who do not fulfill the diagnostic criteria for systemic sarcoidosis but present with similar clinical a.d.histological features. As sarcoma-associated sarcoid reactions are rare, we describe the features of sarcoid reaction that developed in a man with liposarcoma a.d.summarize reports of other oncology patients with sarcoma-associated sarcoid reactions. A 68-yearold man with retroperitoneal liposarcoma presented for evaluation of erythematous dermal plaques on his left leg. Microscopic examination of a tissue specimen revealed multiple epithelioid granulomas in the superficial a.d.mid-reticular dermis. Correlation of the clinical presentation a.d.histopathologic findings established a.d.agnosis of liposarcoma-associated cutaneous sarcoid reaction. Sarcoid reactions have been described in only seven individuals with sarcoma, including two patients with leiomyosarcoma a.d.one patient with either carcinosarcoma, Kaposi sarcoma, liposarcoma, malignant peripheral nerve sheath tumor, rha.d.sarcoma, or synovial sarcoma. Sarcoidal granulomas most commonly develop within the locoregional draining lymph nodes. Sarcoid reactions may also affect other organs, such as the lungs, skin, a.d.spleen.

Visualizing high-dimensional data on a 2d canvas is generally challenging.It becomes significantly more difficult when multiple time-steps are to be presented,as the visual clutter quickly increases.Moreover,the challenge to perceive the significant temporal evolution is even greater.In this paper,we present a method to plot temporal high-dimensional data in a static scatterplot;it uses the established PCA technique to project data from multiple time-steps.The key idea is to extend each individual displacement prior to applying PCA,so as to skew the projection process,a.d.to set a projection plane that balances the directions of temporal change a.d.spatial variance.We present numerous examples a.d.various visual cues to highlight the data trajectories,a.d.demonstrate the effectiveness of the method for visualizing temporal data.
