Mechanical lithotripsy and/or stenting in management of difficult common bile duct stones

BACKgROUND:Mechanical lithotripsy and/or stent insertion is the alternative therapeutic approach in difficult endoscopic retrograde cholangiopancreatography (ERCP)*** study was designed to investigate the appropriate treatment for extraction of bile duct stones in difficult cases of ERCP. METHODS:Between 2000 and 2008,744 ERCP procedures were performed in 592 patients with choledocholithiasis in our endoscopy *** demographic features,and clinical and laboratory findings were collected from a prospectively held *** duct calculi were extracted by basket and/or balloon catheter following ERCP and *** with retained stones were regarded as difficult *** patients were treated with mechanical lithotripsy and those with incomplete clearance of stones underwent stent placement. RESULTS:Two hundred and forty-five patients(41%) were male and 347(59%)were female with a mean age of 58 years(range 19-95 years).Stones were impacted in 27 patients(5%).Stone extraction was performed by basket and/or balloon catheter in 610 ERCP procedures,and lithotripsy was performed in 70 ERCP ***- four patients underwent stent insertion,and 20 underwent stent *** occurred in 39 patients (5%),with no mortality associated with the *** occurred in 9 patients and basket impaction in *** pancreatitis and cholangitis developed in 12 and 11 patients,respectively. coNCLUSION:Difficult cases of bile duct stones can be treated successfully with lithotripsy,and a stent should be applied when the common bile duct cannot be cleared completely.

Dear Editor, We would like to present a rare case ofpolyorchidism. Polyorchidism is an unusual abnormality of the genital tract and until date, there are approximately 178 case reports of polyorchidism in the literature？ However, the most of patients are asymptomatic and usually diagnosed incidentally. Some may present as inguinal hernia （30%）, maldescended testis （15% to 30%）, testicular torsion （13%） and hydrocele （9%）, varicocele （1%）, hypospadias （〈1%）, anomalous urogenital union （〈1%）, and malignancy （〈1%）.2 Management of polyorchidism is still controversial. Important factors of polyorchidism management are testis location, size and anatomical organization of the testicular drainage system and the age of the patient.

ABSTRACT CAPACITY OF REgIONS AND coMPACT EMBEDDINg WITH APPLICATIONS

The weighted Sobolev-Lions type spaces W pl,γ（Ω; E0, E） = W pl,γ（Ω; E） ∩ Lp,γ （Ω; E0） are studied, where E0, E are two Banach spaces and E0 is continuously and densely embedded on E. A new concept of capacity of region Ω ∈ Rn in W pl,γ（; E0, E） is introduced. Several conditions in terms of capacity of region Ω and interpolations of E0 and E are found such that ensure the continuity and compactness of embedding operators. In particular, the most regular class of interpolation spaces Eα between E0 and E, depending of α and l, are found such that mixed differential operators Dα are bounded and compact from W pl,γ（Ω; E0, E） to Eα-valued Lp,γ spaces. In applications, the maximal regularity for differential-operator equations with parameters are studied.

Hepatic cystic echinococcosis:Percutaneous treatment as an outpatient procedure

Effects of honey as a scolicidal agent on the hepatobiliary system

AIM： To examine the effects of 10% diluted honey, which has been shown to be scolicidal, on the liver and biliary system and determine whether it could be used as a scolicidal agent in the presence of biliary-cystic communication. METHODS： Thirty Wistar-Albino rats were divided into two groups. Honey with 10% dilution in the study group and 0.9% saline （NaCI） in the control group were injected into the common bile ducts of rats through a 3-mm duodenotomy. The animals were sacrificed 6 mo alter the procedure. Histopathological, biochemical, and radiological examinations were performed for evaluation of side effects. RESULTS： At the end of the sixth month, liver function tests were found to be normal in both groups. The tissue samples of liver and ductus choledochus of the honey group showed no histomorphologic difference from the control group. No stricture on the biliary tree was detected on the retrograde cholangiograms. coNCLUSION： According to these results, we concluded that 10% diluted honey could be used as scolicidal agent safely in the presence of biliary-cystic communication.

Embedding Theorems in J5-Spaces and Applications

This study focuses on the anisotropic Besov-Lions type spaces B^lp,θ（Ω;E0,E） associated with Banach spaces E0 and E. Under certain conditions, depending on l =（l1,l2,…,ln）and α=（α1,α2,…,αn）,the most regular class of interpolation space Eα between E0 and E are found so that the mixed differential operators D^α are bounded and compact, from B^l＋s p,θ（Ω;E0,E） to B^s p,θ（Ω;Eα）.These results are applied to concrete vector-valued function spaces and to anisotropic differential-operator equations with parameters to obtain conditions that guarantee the uniform B separability with respect to these parameters. By these results the maximal B-regularity for parabolic Cauchy problem is obtained. These results are also applied to infinite systems of the quasi-elliptic partial differential equations and parabolic Cauchy problems with parameters to obtain sufficient conditions that ensure the same properties.

Arrhythmogenic epilepsy and pacing need: A matter of controversy

There is increasing awareness among the cardiology community regarding ictal bradyarrhythmias as a cause of loss of consciousness. A high degree of suspicion is necessary when diagnosing ictal bradyarrhythmias, and delay in diagnosing this condition may lead to morbidity associated with falls and trauma. Ictal bradyarrhythmias have also been suggested to be associated with sudden unexplained death in epilepsy, although evidence related to this association is limited. There is no guidelinedirected therapy for symptomatic ictal bradyarrhythmias due to a lack of randomized, controlled trials. Cardiac pacemaker therapy is commonly used for these patients; however, currently, there is no universal agreement on the pacing indications for these patients. In this review, we focus on the pathophysiology and clinical presentation of ictal bradyarrhythmias and then discuss the pacing need based on the available literature data.

Prevalence of arrhythmias in heavy vehicle drivers

Objective: In our study we aimed to determine the frequency of arrhythmias that we believe may affect driving safety. Methods and Results: Two hundred drivers were randomly selected from the heavy vehicle driver population (82 bus and 118 truck drivers, p = 0.08, q = 0.92, N = 1200, α = 0.01, d = 0.045). A questionnaire was completed via face to face interviews with the individuals including questions about their personal socio-demographic characteristics and symptoms for arrhythmias. An electrocardiography (ECg) was taken of the study participants using the Cardioline Delta 3 Plus Digital ECg machine. The cardiologist at the clinic evaluated the questionnaire and ECg for presence of arrhythmias. When indicated, ambulatory electrocardiography (Holter Monitoring) was performed for 24 hours in 133 individuals (71 driver and 62 control). In cases that had Holter examination;ventricular ectopy was identified in 25.4% and 22.6%, and supra-ventricular ectopy in 45.1% and 35.5% in the driver and the control groups;respectively. Ventricular tachycardia was detected in 2 patients. Arrhythmia frequencies were 59.1%, 54.8% and 57.1% in drivers, control and both groups respectively. Statistical differences between drivers and control group for rhythm disorders were not detected. conclusions: Arrhythmias with lethal and devastating potential;need to be diagnosed and treated in professional drivers with extreme caution. The follow up and screening for heart diseases has a crucial role in preventing accidents and occupational diseases in drivers.
