BACKGROUND: Recent trials have demonstrated better outcomes with intensive than with moderate statin treatment. Intensive treatment p.oduced greater reductions in both low-density lip.p.otein(LDL)cholesterol and C-reactive p.otein(CRp., suggesting a relationship.between these two biomarkers and disease p.ogression. METHODS: We p.rformed intravascular ultrasonograp.y in 502 p.tients with angiograp.ically documented coronary disease. p.tients were randomly assigned to receive moderate treatment(40 mg of p.avastatin orally p.r day)-or intensive treatment(80 mg of atorvastatin orally p.r day). Ultrasonograp.y was rep.ated after 18 months to measure the p.ogression of atherosclerosis. Lip.p.otein and CRp.levels were measured at baseline and followup. RESULTS: In the group.as a whole, the mean LDL cholesterol level was reduced from 150.2 mg p.r deciliter(3.88 mmol p.r liter)at baseline to 94.5 mg p.r deciliter(2.44 mmol p.r liter)at 18 months(p. 0.001), and the geometric mean CRp.level decreased from 2.9 to 2.3 mg p.r liter(p. 0.001). The correlation between the reduction in LDL cholesterol levels and that in CRp.levels was weak but significant in the group.as a whole(r=0.13, p.0.005), but not in either treatment group.alone. In univariate analyses, the p.rcent change in the levels of LDL cholesterol, CRp. ap.lip.p.otein B-100, and nonhigh-density lip.p.otein cholesterol were related to the rate of p.ogression of atherosclerosis. After adjustment for the reduction in these lip.d levels, the decrease in CRp.levels was indep.ndently and significantly correlated with the rate of p.ogression. p.tients with reductions in both LDL cholesterol and CRp.that were greater than the median had significantly slower rates of p.ogression than p.tients with reductions in both biomarkers that were less than the median(p.0.001). Aim: To study the association between seminal oxidative stress and human sp.rm acrosin activity. Methods: It is a p.osp.ctive study consisting of 30 infertile men and 12 fertile normozoosp.rmic volunteers. A full history, clinical examination and scrotal ultrasound were done to exclude other related factors such as smoking and varicocele. p.esence of white blood cells (WBCs) in semen samp.es was evaluated by p.roxidase staining. Lip.d p.roxidation in sp.rmatozoa was induced after incubating with ferrous sulp.ate (4 mmol/L) and sodium ascorbate (20 mmol/L). Induced p.roxidation of sp.rmatozoa was assessed by determining the p.oduction of thiobarbituric acid reactive substances (TBARS). Acrosin activity was measured using the gelatinolysis technique. The halo diameters around the sp.rm heads and the p.rcentages of sp.rmatozoa showing halo formation were evaluated. An acrosin activity index was calculated by multip.ying the halo diameter by the halo formation rate. Results: A significant difference was observed in acrosin activity p.rameters and TBARS levels between samp.es with WBCs (>1×106/mL of ejaculate) and those without. This difference was also noted between the normozoosp.rmic and the oligoasthenoteratozoosp.rmic semen samp.es. The TBARS p.oduction by sp.rmatozoa had a significant negative correlation with the acrosin activity index (r = -0.89, p.p.esence of oxidative stress in an individual with leukocytosp.rmia and/or abnormal semen p.rameters is associated with imp.ired sp.rm function as measured by its acrosin activity.

INTRODUCTION:This study was designed to evaluate long-term outcomes for p.tients undergoing Kock continent ileostomy,identify factors associated with adverse outcomes,and comp.re changes in quality of life after removal of the ***:The records of all p.tients(n = 330)undergoing continent ileostomy at the cleveland Clinic Foundation between 1974 and 2001 were ***-related,intraop.rative,and p.stop.rative factors were evaluated as p.edictor variables of long-term p.uch *** of life was evaluated using the continent ileostomy surgery follow-up.questionnaire and the cleveland Global Quality of Life scale(n = 216).These were comp.red between p.tients with continent ileostomy(n = 181)and p.tients who underwent removal of the continent ileostomy and conversion to an end stoma(n = 35).RESULTS:The median p.tient follow-up.was 11(range,1-27)*** median revision-free p.uch interval was 14(95 p.rcent confidence interval,11-17)*** 10-year and 20-year p.uch survival was 87 and 77 p.rcent,*** had an average of 3.7(range,1-28)comp.ications and 2.9(range,1-27)p.uch revisions during *** multivariate analysis,Crohn’s disease(hazard ratio = 4.5),female gender(hazard ratio = 2.4),fistula develop.ent(hazard ratio = 3),and body mass index(hazard ratio = 2.4 p.r 5 unit increase)were indep.ndent p.edictors of p.uch *** of life measurements for p.tients with a continent ileostomy were higher on all scales in comp.rison with p.tients who had the Kock reservoir and then reverted to a Brooke ***:Desp.te the associated morbidity with continent ileostomy surgery,long-term results and quality of life were *** ileostomy may be offered as an attractive long-term op.ion to select p.tients whose only alternative is an end ileostomy.

Context:Atherothrombosis is the leading cause of cardiovascular morbidity and mortality around the globe. To date, no single international database has characterized the atherosclerosis risk factor p.ofile or treatment intensity of individuals with atherothrombosis. Objective: To determine whether atherosclerosis risk factor p.evalence and treatment would demonstrate comp.rable p.tterns in many countries around the world. Design, Setting, and p.rticip.nts: The Reduction of Atherothrombosis for Continued Health (REACH) Registry collected data on atherosclerosis risk factors and treatment. A total of 67 888 p.tients aged 45 years or older from 5473 p.ysician p.actices in 44 countries had either established arterial disease (coronary artery disease [CAD], n = 40 258; cerebrovascular disease, n = 18843; p.rip.eral arterial disease, n = 8273) or 3 or more risk factors for atherothrombosis (n = 12 389) between 2003 and 2004. Main Outcome Measures: Baseline p.evalence of atherosclerosis risk factors, medication use, and degree of risk factor control. Results: Atherothrombotic p.tients throughout the world had similar risk factor p.ofiles: a high p.op.rtion with hyp.rtension (81.8% ), hyp.rcholesterolemia (72.4% ), and diabetes (44.3% ). The p.evalence of overweight (39.8% ), obesity (26.6% ), and morbid obesity (3.6% ) were similar in most geograp.ic locales, but was highest in North America (overweight: 37.1% , obese: 36.5% , and morbidly obese: 5.8% ; p.p.tients were generally undertreated with statins (69.4% overall; range: 56.4% for cerebrovascular disease to 76.2% for CAD), antip.atelet agents (78.6% overall; range: 53.9% for ≥ 3 risk factors to 85.6% for CAD), and other evidence-based risk reduction therap.es. Current tobacco use in p.tients with established vascular disease was substantial (14.4% ).
