BACKGROUNd:Obstructive jaundice caused by an intraductal hepatocellular carcinoma is a rare initial *** report a rare case of an extrahepatic icteric type hepatocellular ***:A 75-year-old patient was admitted to our hospital because of obstructive jaundice 3 months after resection of multilocular hepatocellular carcinoma.A postoperative bile leakage was treated by placement of a decompressing stent in the common bile *** retrograde choledochoscopy showed extended blood clots filling the bile duct system and computed tomography revealed a local swelling in the common extrahepatic bile *** level of alpha-fetoprotein(AFP)was only slightly elevated but that of CA19-9 was dramatically *** showed an intraductal filling defect typical of a cholangiocellular ***:Bile duct brushing cytology showed no cholangiocellular carcinoma but hepatocellular carcinoma cells in the extrahepatic bile *** extrahepatic bile duct resection was *** examination confirmed the diagnosis of extrahepatic intraductal growth of hepatocellular ***:Ectopic hepatocellular carcinoma is a rare but important differentially diagnosed of extrahepatic bile duct filling defect.

An unusual case of early dislocation of a mobile bearing posterior stabilized total knee arthroplasty in a 48-year-old Caucasian woman is described. dislocation occurred one day postoperatively, attributed to a gap mismatch. Revision surgery reduced posterior dislocation, increased bearing plate thickness and rebalanced ligaments. A second dislocation occurred after revision surgery. The patient’s history was retaken and a hamstring spasm disease identified. A new revision utilized a more constrained design, without perioperative local nerve block. Two years following surgery, no further dislocation had occurred. A numerical musculoskeletal model of the case and implant configuration identified no trend to mobile bearing dislocation when regular muscle forces were applied. Muscle spasm is a risk factor for mobile bearing total knee arthroplasty dislocation, especially with femoral nerve block.

目的　评价在ACI大鼠肝细胞癌模型进行多种介入治疗方案的疗效及该模型在介入治疗实验中的应用价值。方法　在 5 8只雄性ACI大鼠肝包膜下植入MorrisHepatoma 3 92 4A肝癌瘤块( 1mm3) ,移植术后 13d行MR检查 ,测量肿瘤体积 (V1) ,第 14天时 ,经腹部切开术及胃十二指肠动脉逆行插管而采取以下介入治疗方案 :(A)丝裂霉素 4只 ;(B)降解淀粉 4只 ;(C)碘油 5只 ;(d)肝动脉结扎 4只 ;(E)丝裂霉素 +降解淀粉 4只 ;(F)丝裂霉素 +动脉结扎 5只 ;(G)丝裂霉素 +碘油 5只 ;(H)降解淀粉 +动脉结扎 4只 ;(I)碘油 +动脉结扎 4只 ;(J)丝裂霉素 +聚丙交酯复合乙交酯 (poly lactid co glycolid ,Plcg)微球 4只 ;(K)丝裂霉素 +碘油 +动脉结扎 4只 ;(L)丝裂霉素 +降解淀粉 +动脉结扎4只 ;(M) 0 9%生理盐水 (对照组 ,7只 )。 13d后再次行磁共振检查以确定肿瘤体积 (V2 )变化 ,并对各组间V2 /V1的比值进行比较。结果　肝癌移植率为 10 0 %。介入治疗后肿瘤体积与治疗前肿瘤体积之比 (V2 /V1)分别为A组 4 5 0 ,B组 12 73 ,C组 15 84,d组 10 17,E组 90 2 0 ,F组 7 16,G组 4 0 8,H组 3 45 ,I组 9 99,J组 2 86,K组 3 76,L组 7 71,M组 2 7 12。与对照组相比 ,采取A、G、H、J和K组方法能明显抑制肿瘤生长 ( χ2 值分别为 5

本文研究微合金钢49M_nVS3的一种专门用途——对汽车曲轴的适用性。微量合金添加剂的作用。为了合理地选择某一专用钢的成份,应当充分研究钢的强化机理。钢的强化机理,可用下述公式表示:σy=σd+σs+σG+σA。式中σd=位错密度引起的强化;σS=固溶引起的强化;σS=晶粒度细化引起的强化;σA=时效硬化引起的强化。钢的总强化是这些强化共同作用的结果。其中第一、第三和第四种因素,影响较大。为了利用这些强化作用需要一定的淬火回火条件。

Purpose: Magnetic resonance imaging (MRI) is the gold standard in visualizing brain tumors and their effects on adjacent structures. However, no reliable information concerning different tumor components and borders between perifocal edema and infiltration areas can be received. The aim of the study was to establish and evaluate a multimodal imaging concept, in order to differentiate different biological tumor components and to determine tumor borders. Materials and Methods: 12 patients with cerebral gliomas (four low and eight high grade) received a “morphological” MRI, a 3d MR spectroscopy and a T2* MR perfusion examination prior to surgery. data was evaluated by defining different tumor components, which were entitled based upon their multimodal characteristics and histological data. Results: In high grade gliomas different components can be differentiated, which were described as: “true edema”, “cellular proliferation”, “vascular proliferation”, “cellular infiltration”, “tumor” and “necrosis”. In low grade gliomas, four different tumor components were found: “true edema”, “cellular infiltration”, “cellular proliferation” and “tumor”. Conclusion: With the applied multimodal imaging and a novel evaluation concept, it was possible to detect different tumor components, which could be helpful in detecting the optimal sites for tumor biopsy. Especially in morphological “edema appearing” sites, this knowledge could be important for the adaption of tumor resection borders and the planning of radiation therapy. Further studies with more patients and histological correlation are needed.

The GP130 cytokine receptor subunit encoded by IL6ST is the shared receptor for ten cytokines of the IL-6 family. We describe a homozygous non-synonymous variant in IL6 ST(p.R281 Q) in a patient with craniosynostosis and retained deciduous teeth. We characterize the impact of the variant on cytokine signaling in vitro using transfected cell lines as well as primary patient-derived cells and support these findings using a mouse model with the corresponding genome-edited variant Il6 st p.R279 Q. We show that human GP130 p.R281 Q is associated with selective loss of IL-11 signaling without affecting IL-6, IL-27, OSM, LIF, CT1, CLC, and CNTF signaling. In mice Il6 st p.R279 Q lowers litter size and causes facial synostosis and teeth abnormalities. The effect on IL-11 signaling caused by the GP130 variant shows incomplete penetrance but phenocopies aspects of IL11 RA deficiency in humans and mice. Our data show that a genetic variant in a pleiotropic cytokine receptor can have remarkably selective defects.

Adiponectin and its receptors in rodent models of fatty liver disease and liver cirrhosis

AIM: To determine circulating and hepatic adiponectin in rodents with fatty liver disease or liver cirrhosis and investigate expression of the adiponectin receptors AdipoR1 on the mRNA and protein level and AdipoR2 on the mRNA level. METHOdS: Fat fed rats were used as a model for fatty liver disease and bile duct ligation in mice to investigate cirrhotic liver. Expression of AdipoR1 and AdipoR2 mRNA was determined by real time RT-PCR. AdipoR1 protein was analysed by immunoblot. Adiponectin was measured by ELISA. RESULTS: Systemic adiponectin is reduced in fat- fed rats but is elevated in mice after bile duct ligation (BdL). Hepatic adiponectin protein is lower in steatotic liver but not in the liver of BdL-mice when compared to controls. Adiponectin mRNA was not detected in human liver samples or primary human hepatocytes nor in rat liver but recombinant adiponectin is taken up by isolated hepatocytes in-vitro. AdipoR1 mRNA and AdipoR1 protein levels are similar in the liver tissue of control and fat fed animals whereas AdipoR2 mRNA is induced. AdipoR2 mRNA and AdipoR1 mRNA and protein is suppressed in the liver of BdL-mice. CONCLUSION: Our studies show reduced circulating adiponectin in a rat model of fatty liver disease whereas circulating adiponectin is elevated in a mouse model of cirrhosis and similar fi ndings have been described in humans. diminished hepatic expression of adiponectinreceptors was only found in liver cirrhosis.
