A Prospective Study Comparing Quality of Life and Cosmetic Results between Single-Port and Conventional Laparoscopic Cholecystectomy

Background: Multiple studies from the last five years have demonstrated that single-incision laparoscopic surgery cholecystectomy (SILS) is not only feasible but also produces a result comparable with that of conventional laparoscopic cholecystectomy (CLC). Methods and results: In this bicentric study, we used the Short Form (36) (SF-36) and Nottingham Health Profile (NHP) surveys to estimate and compare the post-operative quality of life up to the 28th post-operative day for 66 patients who had undergone either CLC (n = 32) or SILS (n = 34). Additionally, we investigated patient satisfaction with the cosmetic results after one year. The curves summarizing the eight sections in the physical sum scale and in the mental sum scale (SF-36) were the same within the confidence interval with a confidence level of 95%. Thus, post-operative quality of life could be assumed to be equivalent for the two groups. Evaluation of the NHP survey produced similar results. The perceived cosmetic results were significantly better for the SILS group (1.3 on a scale of 1 - 5 with 1 being the best) than for the CLC group (1.9) (p = 0.016). Conclusions: This study did not demonstrate better quality of life for the single-port procedure as it had been expected. Instead, the single-port procedure produced subjectively better cosmetic results.

Anthropological analyses of 30 insertion/deletion autosomal markers in five major ethnic groups of Pakistan

We investigated the forensic efficacy of the 30 insertion/deletion(Indel)markers included in the Qiagen Investigator■dIPplex kit in 529 Pakistani individuals from five major subpopulations in Pakistan(Punjabi,Pashtun,Sindhi,Saraiki,and Baloch).In the Sindhi population,the distribution of HLd81 and HLd97 alleles deviated from Hardy-Weinberg equilibrium after Bonferroni *** combined match probability ranged from 2.0E-12(Pashtun and Baloch)to 1.0E-12(Sindhi),and the mean paternity exclusion power varied from 0.995(Punjabi,Sindhi,and Saraiki)to 0.996(Pashtun and Baloch).The high combined power of discrimination(0.99999999999997)and low combined match probability(1.7E-12)for all subpopulations studied support the utility of the 30 Indel markers for forensic identification in the studied *** allele frequencies of the Indel markers in the Pakistani subpopulations were compared with those from 18 other *** results show that the populations clustered according to *** subpopulations investigated in this work showed a close genetic relationship with others from Pakistan,as well as with South Central Asian and Middle Eastern *** results suggest that the Investigator■dIPplex kit can be useful as a supplementary tool for human identification in the five Pakistani subpopulations investigated in this study.

Hepatic steatosis as the most prevalent liver disorder can either be related to alcoholic liver disease (ALd) or non-alcoholic fatty liver disease (NAFLd). In both conditions, hepatocytes excessively accumulate fatcontaining vacuoles within their cytoplasm, which is the key histological feature. In contrast to ALd, NAFLd is commonly associated with metabolic syndrome, obesity and insulin resistance. To determine increased liver fat content, liver biopsy is currently considered the gold standard. Besides the invasive technique, various other non-invasive techniques have been developed, such as ultrasound, computed tomography (CT), magnetic resonance spectroscopy (MRS) and magnetic resonance imaging (MRI) based methods. Among these techniques, ultrasound and CT provide only qualitative information about hepatic steatosis, whereas MRS-or MRI-based methods are able to determine even small amounts of fat accurately. These non-invasive magnetic resonance techniques have already proven their great potential, especially in longitudinal and cross-sectional studies regarding various metabolic conditions and medical treatment regimens. In this review, the most common, non-invasive MRS/MRI techniques for assessment of intrahepatic lipid content are described with their inherent advantages and limitations.

A High Order Sharp-Interface Method with Local Time Stepping for Compressible Multiphase Flows

In this paper,a new sharp-interface approach to simulate compressible multiphase flows is *** new scheme consists of a high order WENO finite volume scheme for solving the Euler equations coupled with a high order pathconservative discontinuous Galerkin finite element scheme to evolve an indicator function that tracks the material *** the interface our method applies ghost cells to compute the numerical flux,as the ghost fluid ***,unlike the original ghost fluid scheme of Fedkiw et al.[15],the state of the ghost fluid is derived from an approximate-state Riemann solver,similar to the approach proposed in[25],but based on a much simpler *** formulation leads only to one single scalar nonlinear algebraic equation that has to be solved at the interface,instead of the system used in[25].Away from the interface,we use the new general Osher-type flux recently proposed by dumbser and Toro[13],which is a simple but complete Riemann solver,applicable to general hyperbolic conservation *** time integration is performed using a fully-discrete one-step scheme,based on the approaches recently proposed in[5,7].This allows us to evolve the system also with time-accurate local time *** to the sub-cell resolution and the subsequent more restrictive time-step constraint of the dG scheme,a local evolution for the indicator function is applied,which is matched with the finite volume scheme for the solution of the Euler equations that runs with a larger time *** use of a locally optimal time step avoids the introduction of excessive numerical diffusion in the finite volume *** different fluids have been used,namely an ideal gas and a weakly compressible fluid modeled by the Tait *** tests have been computed to assess the accuracy and the performance of the new high order scheme.A verification of our algorithm has been carefully carried out using exact solutions as well as a comparison with other numerical re

Effects of temperature,pH and O_(2) on the removal of hydrogen sulfide from biogas by external biological desulfurization in a full scale fixed-bed trickling bioreactor(FBTB)

Hydrogen sulfide(H_(2)S)is a critical component of biogas formed under anaerobic conditions by sulfur and sulfate reducing bacteria from animal manure and renewable energy crops.H_(2)S causes high corrosion in equipment,has a negative environmental impact,inhibits the biogas formation process and is furthermore odorous and *** several methods for internal and external desulfurization found their way into practice and had been explored at laboratory scale,no data were available on the performance of such methods in full scale practice,especially for an external fixed-bed trickling bioreactor(FBTB).The effects of temperature,pH and air ratio on H_(2)S removal efficiency(RE)were *** study was conducted at a research biogas plant with a given output of 96 m^(3) biogas per hour,and an H_(2)S concentration ranging between 500 ppm and 600 ppm(1 ppm=1 cm^(3)/m^(3))on *** FBTB column has been designed to hold a packing volume of 2.21 m^(3) at a gas retention time of 84 seconds being loaded at an average of 32.88 g H_(2)S/(m^(3)·h).The highest H_(2)S RE of 98% was found at temperatures between 30℃ and 40℃.A major decline in RE to 21%-45%was observed at temperatures from 5℃ to 25℃.The results clearly showed a temperature optimum range for sulfate reducing *** results reveal that RE is little affected by different pH values and air *** the experimental period,the practical suitability of the FBTB system could be proved while avoiding the disadvantages of internal biological desulfurization methods.
