Proprioception is significantly impaired in knee osteoarthritis(KOA),contributing to reduced *** training(ST)is essential in KOA by improving muscle strength,although it may also be effective in improving *** purpose was to determine the effect of ST on knee proprioception in KOA ***,CINAHL,Scopus,WOS,and PEDro were searched for randomized controlled trials(RCTs)(inception to March 2023).Comparisons for ST were physical exercise different from ST,non-exercise-based interventions,and no *** quality was assessed using the PEDro scale,and risk of bias(RoB)using the Cochrane ***-analyses were performed by comparison groups using the standardized mean difference(SMD)(Hedge's g)with random effects models,also considering subgroups by proprioception ***,six RCTs were *** mean PEDro score was 6.3,and the highest proportion of biases corresponds to per-formance,selection,and *** meta-analysis indicated that only when compared with non-intervention,ST significantly improved knee proprioception for the joint position sense(JPS)(activeþpassive),JPS(passive),and threshold to detect passive motion(TTDPM)subgroups(g=-1.33[-2.33,-0.32],g=-2.29[-2.82,-1.75]and g=-2.40[-4.23,-0.58],respectively).However,in the knee JPS(active)subgroup,ST was not significant(g=-0.72[-1.84,0.40]).In conclusion,ST improves knee proprioception compared to ***,due to the paucity of studies and diversity of interventions,more evidence is needed to support the effectiveness of *** RCTs may address the limitations of this review to advance knowledge about pro-prioceptive responses to ST and contribute to clinical practice.

Solid-pseudopapillary tumor (SPT) is a rare neoplasm of the pancreas that usually occurs in young females. It is generally considered a low-grade malignant tumor that can remain asymptomatic for several years. The occurrence of infiltrating varieties of SPT is around 10%-15%. Between 1986 and 2006, 282 cystic tumors of the pancreas were observed. Among them a SPT was diagnosed in 8 patients (2.8%) with only one infiltrating variety. This was diagnosed in a 49-year-old female 13 years after the sonographic evidence of a small pancreatic cystic lesion interpreted as a pseudocyst. The tumor invaded a long segment of the portal- mesenteric vein confluence, and was removed with a total pancreatectomy, resection of the portal vein and reconstruction with the internal jugular vein. Histological examination confirmed the R-0 resection of the primary SPT, although a vascular invasion was demonstrated. The postoperative course was uneventful, but 32 mo after surgery the patient experienced diffuse liver metastases. Chemotherapy with different drugs was started. The patient is alive and symptom-free, with stable disease, 75 mo after surgery. Twenty-five patients with invasion of the portal vein and/or of mesenteric vessels were retrieved from the literature, 16 recent patients with tumor relapse after potentially curative resection were also retrieved. The best treatment remains a radical resection whenever possible, even in locally advanced or metastatic disease. The role of chemotherapy, and/or radiotherapy, is still to be defined.

We read with great interest the article of Rodriguez et al.,"Which method of distal pancreatectomy is cost-effective among open,laparoscopic,or robotic surgery?"(1).In the current climate of limited hospital sources and control of medical costs,the authors afford the interesting topic of the cost-effectiveness evaluation of the robot-assisted distal pancreatectomy(RDP)in comparison to the laparoscopic(LDP)and open approaches(ODP).To accomplish this purpose,they focused their attention on both the clinical and the economical outcomes of the three different techniques.

Autophagy is an essential cellular homeostatic mechanism by which intracellular components are delivered into the lysosomes for degradation and *** has been related with a diversity of pathological or physiological dentary processes such as bone remodeling,skeletal aging,osteoclastogenesis,osteoblastogenesis and different types of oral ***-rich plasma(PRP),isolated from autologous blood,is a plasma preparation containing a higher concentration of platelets which contains numerous different growth factors and cytokines that activate several cellular signaling *** purpose of this study is to investigate the effect of PRP on autophagy stimulation in both osteoblast precursor 3T3-L1 and non-related osteoblastic *** results showed that PRP can increase the number of autophagic structures in 3T3-L1 and HeLa(cervical cancer cells)***,we have determined by Western blot a rise in the lipidated form of the autophagic protein LC3(***3-II)upon PRP *** together,our results suggest that PRP is able to induce a strongly autophagy response in osteoblast precursor and,to a lesser extent,in non-related osteoblastic cells,suggesting that PRP could be a potential therapeutic tool for some autophagy-related diseases associated with bone homeostasis.

The paper proposes a matrix-based approach that a designer can use to evaluate the sustainability of one or more design solutions in the conceptual phase of a product development process. The methodology is structured in two main phases. In the first one, a data structure, called Augmented-DSM （Design Structure Matrix）, has to be drawn in order to collect design information such as functional links among product components, indicators that provide information about the life cycle of each element, connections with the environment and toward other systems. The matrix is based on a DSM format, where new records were introduced to store and organize different types of data and an editing protocol was formalized. In the second stage, a set of procedures have to be applied in order to evaluate sustainability of a design solution and compare it with the others. The presented procedures, in fact, allow designers to assess a product in its proper characteristics, evaluate a device during its use stage and verify the coherence of links present in the functional model. This new approach is conceived to manage different performance indicators and, as a support tool, it is an attempt to aid designers in a quantitative evaluation.

We aimed to examine the major causes of isolated chronic hypertransaminasemia in asymptomatic children and develop a comprehensive diagnostic flow diagram. A MEDLINE search inclusive of publications throughout August 2012 was performed. We found only a small number of publications that had comprehensively investigated this topic. Consequently, it was difficult to construct a diagnostic flowchart similar to those already available for adults. In children, a "retesting panel" prescription, including gamma-glutamyl transpeptidase and creatine kinase in addition to aminotransferases, is considered a reasonable approach for proficiently confirming the persistence of the abnormality, ruling out cholestatic hepatopathies and myopathies, and guiding the subsequent diagnostic steps. If re-evaluation of physical and historical findings suggests specific etiologies, then these should be evaluated in the initial enzyme retesting panel. A simple multistep diagnostic algorithm incorporating a large number of possible pediatric scenarios, in addition to the few common to adults, is available. Accurately classifying a child with asymptomatic persistent hypertransaminas-emia may be a difficult task, but the results are critical for preventing the progression of an underlying, possibly occult, condition later in childhood or during transition. Given the high benefit/cost ratio of preventing hepatic deterioration, no effort should be spared in diagnosing and properly treating each case of persistent hypertransaminasemia in pediatric patients.

A brief description of the windmills from the second millennium BC to the Renaissance is presented. This survey is a part of several studies conducted by the authors on technology in the ancient world. The windmills are the first motor, other than human muscles, and are the ancestors of the modem wind turbines. Some authors＇ virtual reconstructions of old windmills are also presented. The paper shows that the operating principle of many modem machines had already been conceived in the ancient times by using a technology that was more advanced than expected, but with two main differences, as follows： Similar tasks were accomplished by using much less energy; and the environmental impact was nil or very low. Modem designers should sometimes consider simplicity rather than the use of a large amount of energy.
