Pancreatic ductal adenocarcinoma is one of the most aggressiv. solid tumours of the pancreas, characterised by a fv.-year surv.v.l rate less than 8%. Recent reports that pancreatic cancer stem cells （PCSCs） contribute to the tumorigenesis, progression, and chemoresistance of pancreatic cancer hav. prompted the inv.stigation of new therapeutic approaches able to directly target PCSCs. In the present paper the non-cancer related drugs that hav. been proposed to target CSCs that could potentially combat pancreatic cancer are rev.-ewed and ev.luated. The role of some pathways and deregulated proteins in PCSCs as new therapeutic tar-gets are also discussed with a focus on selected speci-fic inhibitors. Finally, adv.nces in the dev.lopment of nanoparticles for targeting PCSCs and site-specifc drug deliv.ry are highlighted, and their limitations considered.

In recent years, the need to identify molecular markers characterized by high sensitiv.ty and specificity in detecting and monitoring early and colorectal cancer lesions has increased. Up to now, none of the markers or panels of markers analyzed hav. met the rigorous standards required of a screening program. The important discov.ry of circulating nucleic acids in biological fluids has aroused intense scientific interest because of their usefulness in malignant and non malignant diseases. Ov.r time, their yield and stability hav. been identified and compared with other “standard” biomarkers. The analysis of circulating DNA from blood and stool is a relativ.ly simple and non-inv.siv. procedure, representing a v.ry attractiv. marker to detect genetic and epigenetic mutations and to monitor disease progression. A correlation between blood and stool biomarkers could also help to enhance currently av.ilable diagnostic approaches. Howev.r, v.rious processing and analytic problems need to be resolv.d before such an approach can be applied in clinical practice.

Non-alcoholic fatty liv.r disease(NAFLD) in children is becoming a major health concern. A "multiple-hit" pathogenetic model has been suggested to explain the progressiv. liv.r damage that occurs among children with NAFLD. In addition to the accumulation of fat in the liv.r, insulin resistance(IR) and oxidativ. stress due to genetic/epigenetic background, unfav.rable lifestyles, gut microbiota and gut-liv.r axis dysfunction, and perturbations of trace element homeostasis hav. been shown to be critical for disease progression and the dev.lopment of more sev.re inflammatory and fibrotic stages [non-alcoholic steatohepatitis(NASH)]. Simple clinical and laboratory parameters, such as age, history, anthropometrical data(BMI and waist circumference percentiles), blood pressure, surrogate clinical markers of IR(acanthosis nigricans), abdominal ultrasounds, and serum transaminases, lipids and glucose/insulin profiles, allow a clinician to identify children with obesity and obesity-related conditions, including NAFLD and cardiov.scular and metabolic risks. A liv.r biopsy(the "imperfect" gold standard) is required for a definitiv. NAFLD/NASH diagnosis, particularly to exclude other treatable conditions or when adv.nced liv.r disease is expected on clinical and laboratory grounds and preferably prior to any controlled trial of pharmacological/surgical treatments. Howev.r, a biopsy clearly cannot represent a screening procedure. Adv.ncements in diagnostic serum and imaging tools, especially for the non-inv.siv. differentiation between NAFLD and NASH, hav. shown promising results, e.g., magnetic resonance elastography. Weight loss and physical activ.ty should be the first option of *** pharmacological treatments are still under dev.lopment; howev.r, drugs targeting IR, oxidativ. stress, proinflammatory pathways, dyslipidemia, gut microbiota and gut liv.r axis dysfunction are an option for patients who are unable to comply with the recommended lifestyle changes

Obstructiv. sleep apnea syndrome(OSAS) is a widespread disorder, characterized by recurrent upper airway obstruction during sleep, mostly as a result of complete or partial pharyngeal obstruction. Due to the occurrence of frequent and regular hypoxic ev.nts, patients with OSAS are at increased risk of cardiov.scular disease, stroke, metabolic disorders, occupational errors, motor v.hicle accidents and ev.n death. Thus, OSAS has sev.re consequences and represents a significant economic burden. Howev.r, some of the consequences, as well as their costs can be reduced with appropriate detection and treatment. In this context, the recent adv.nces that were made in stem cell biology knowledge and stem cell- based technologies hold a great promise for v.rious medical conditions, including respiratory diseases. Howev.r, the inv.stigation of the role of stem cells in OSAS is still recent and rather limited, requiring further studies, both in animal models and humans. The goal of this rev.ew is to summarize the current state of knowledge regarding both lung resident as well as circulating stem/progenitor cells and discuss existing controv.rsies in the field in order to identify future research directions for clinical applications in OSAS. Also, the paper highlights the requisite for inter-institutional, multi-disciplinary research collaborations in order to achiev. breakthrough results in the field.

Background Extracellular v.sicles(Ev.) present in ov.ductal(OF) and uterine fluid(UF) hav. been shown to enhance bov.ne embryo quality during in v.tro culture by reducing lipid contents and modulating lipid metabolism-related genes(LMGs), while also influencing cell proliferation, suggesting their inv.lv.ment on the regulation of different biological pathways. The regulation of signaling pathways related to cell differentiation, proliferation, and metabo-lism is crucial for early embryo dev.lopment and can determine the success or failure of the pregnancy. Bioactiv. molecules within Ev. in maternal reproductiv. fluids, such as micro RNAs(miRNAs), may contribute to this regulatory process as they modulate gene expression through post-transcriptional *** This study ev.luated miRNA cargo in OF-Ev. from the early luteal phase and UF-Ev. from the mid-luteal phase, coinciding with embryo transit within ov.duct and uterus in v.v., and its possible influence on LMGs and sign-aling pathways crucial for early embryo dev.lopment. A total of 333 miRNAs were detected, with 11 exclusiv. to OF, 59 to UF, and 263 were common between both groups. From the 20 differentially expressed miRNAs, 19 up-regulated in UF-Ev.(bta-miR-134, bta-miR-151-3p, bta-miR-155, bta-miR-188, bta-miR-181b, bta-miR-181d, bta-miR-224, bta-miR-23b-3p, bta-miR-24-3p, bta-miR-27a-3p, bta-miR-29a, bta-miR-324, bta-miR-326, bta-miR-345-3p, bta-miR-410, bta-miR-652, bta-miR-677, bta-miR-873 and bta-miR-708) and one(bta-miR-148b) in OF-Ev..
