Intraductal papillary neoplasm of the bile ducts: A case report and literature review

Intraductal papillary neoplasm of the bile duct(IpNB) is a rare bile duct neoplasm mostly found in far eastern nations where hepatolithiasis and clonorchiasis infections are endemic. In western countries,it is very rare and the etiology is unknown. In this article,we report the first IpNB patient we encountered in our clinic and a literature review. The patient is a 38-yearold female with a history of choledocholithiasis who presented with obstructive jaundice. She was found to have a papillary mass at the junction of the right hepatic duct and common hepatic duct with six masses in the liver parenchyma. The immunophenotypic and histologic features of the tumor are consistent with IpNB,gastric subtype. The patient had a partial hepatectomy and has been receiving palliative chemotherapy. In a search of pub Med database,we collected 354 IpNB patients reported in 22 articles. In these patients,52.8% were from Japan and 27.7% were from western countries including the United States(11.0%). The age of the patients ranged from 35 to 80 years old with an average of 64.6. Male/female ratio was 1.5. Macroscopically,57.5% of the tumors were in the left lobe and 29.5% were in the right lobe. The average size of the tumor were 4.2 cm at the time of diagnosis. Histologically,pancreato-biliary subtype accounted for 41.8%,intestinal 28.0%,gastric 13.5% and oncocytic 16%. An invasive component is most often present in the pancreato-biliary and gastric subtypes. Despite recent advanced technologies,diagnosis of IpNB is still challenging,especially in western countries due to its rarity. Defined clinicopathologic features are in demand for the accurate diagnosis and proper treatment.

Background Araucaria araucana is a mast species that presents a high variability in annual cone production. Researchers have recorded synchronization events in cone production in different populations, which allows the seed production to be concentrated, reducing the percentage of seeds consumed by different animal *** We sampled three populations located in the Andes Mountains, Araucanía Region, Chile. In 2004 we began the collection of data on cone production, for which we installed permanent plots（1200 m2each） at each location. We identified and labeled each female tree in each plot to monitor its cone production. In 2012 we selected a total of 30 trees near the plots to evaluate the number of seeds per cone. In each February from 2012 to 2014 we selected two mature cones and covered them with a porous mesh for subsequent collection and storage in March. At the beginning of June, we counted and weighed the seeds, determining the average weight, the number of seeds per cone, the germination capacity（GC）, and the germination speed（GS）.Results Cone production was synchronous across the three locations. We observed significant differences among the locations and years evaluated. The cones had fewer seeds in 2013（high production） compared to those in 2012（low production）, but their weights were similar. In 2014 the cones produced smaller seeds in fewer quantities. The difference between the years 2013 and 2014 resulted from the high-energy expenditure in 2013. Regarding GC and GS, there were significant differences among the three locations（GC: F = 45.41, p p produced in a given year. Nutritional Status of patients with Cutaneous Leishmaniasis from a Tropical Area of Bolivia, and Implications for Zinc Bioavailability

Macro and micronutrient deficiencies are a significant problem among people in rural areas in developing countries. Deficiencies may lead to an impaired immune system making the organism vulnerable to infections and diseases. In this paper, the dietary intake, anthropometric measurements, zinc and copper levels in serum, of patients with cutaneous leishmaniasis (CL) are compared with a group of healthy controls and reference values. Results showed no significant differences in most nutrient intake or anthropometrics between patients and controls. However, serum zinc level of patients (80 μg/dl) was significantly lower (p 0.001) than in controls (85 μg/dl), likely explained by the presence of leishmaniasis. The median serum zinc level in both groups was below the reference values, even though their median zinc intake met the zinc recommendations from WHO. Consequently, besides the presence of leishmaniasis, serum zinc levels may be compromised by inhibitory components in their diet, such as phytates, as it is shown by the molar ratio phytate:zinc (phy:Zn) that was between 11 and 19, while 15 is the level said to compromise zinc status. There were significant (p 0.05) negative correlations between serum zinc and phy:Zn, for patients (r = - 0.413) and controls (r = - 0.410). In conclusion this study shows that patients with CL in Chapare, Bolivia had low serum zinc levels due to the leishmaniasis per se and the decreased zinc bioavailability in their diet. CL infection was not in direct association with the nutritional status indicated by the anthropometric or dietary assessments. However, dietary intake showed 5 essential nutrients below the nutrient recommendation in both groups.

The equations of state for degenerate electron and neutron gases are studied in the presence of magnetic *** including quantum effects in the investigation of the structural properties of these systems,it is found that some hypermagnetized stars can be unstable according to the criterion of stability of *** magnetized white dwarfs should collapse producing a supernova type Ia,while superstrong magnetized neutron stars cannot stand their own magnetic field and must implode,too.A comparison of our results with a set of the available observational data of some compact stars is also presented,and the agreement between this theory and observations is verified.

Optimizing the delivery of combination therapy for tumors： A mathematical model

We present in this paper a new mathematical model for the scheduling of angiogenic inhibitors in combination with a chemotherapeutic agent for a tumor. Our model takes into account the process of angiogenesis and the quality of the vasculature discriminating between stable blood vessels and unstable blood vessels. We characterize theoretically the optimal controls on drug distribution to minimize the number of cancer cells at the end of the treatment in a free horizon time problem with restrictions on the total amount of drug doses. Finally, we solve the optimal control problem by using numerical simulations, obtaining as a result that, despite the number of the tumor cells decrease with anti-angiogenic treatment, the best results are reached at the end of the chemotherapy treatment.

La0.7Sr0.3Cr0.4Mn0.6O3-δperovskite nanoparticles have been synthetized using an ethylene glycol modified solgel method and impregnated by Cu0.75Ni0.25nanoparticles. The phase transitions and nucleation kinetics were studied by time resolved x-ray diffraction, Rietveld, Raman spectroscopy and high resolution transmission electron microscopy. Findings have shown that, an atomic disorder dominates from room temperature to 400 °C. Nanoparticles with crystal size pm3m cubic and p21/n monoclinic symmetry respectively in the range of 600–650 °C. Symmetry reduction from the pm3m→R3c space group had been obtained at a lower temperature（750 °C） than those reported in the literature in oxygen atmosphere and it was confirmed by an octahedral distortion, which takes place in the crystallographic direction[221]/（122）. The Johnson-Mehl-Avrami model had described the whole process, where the existence of two phases was shown before it reached its complete D3d6point group symmetry.

Overcast sky condition prevails on and influences the biometeorology of the tropical Andean páramos

Clouds play a major role in modulating the biometeorological processes. We studied the influence of cloudiness on four biometeorological variables:daily air temperature(Tair), relative humidity(RH),reference evapotranspiration(ETr),and photosynthetic active radiation(pAR), recorded at four sites of Andean páramos in southern Ecuador during 2.5 to 5.5 years. First, we quantified both the cloud cover percentage(Cloud%) creating cloud masks over the visible bands of Landsat 7 images and the sky condition(K_(T)) using the records of solar and extraterrestrial radiation. Second, we estimated KTfrom Cloud%. Finally, we quantified T_(air), RH, ET_(r), and pAR under clear, cloudy, and overcast K_(T) and their dependence on KT. The average Cloud% ranged between 65%–76%, and KTcorroborated the prevailing overcast sky(between 55% and 72.5% of the days) over the páramos. The proposed model performed well in the sites of calibration(R^(2)= 0.80;MBE = 0.00;RMSE = 0.05) and validation(R^(2)= 0.74;MBE =-0.07;RMSE = 0.11). The overcast sky diminished T_(air)(≤ 10℃), ET_(r)(≤ 1.6 mm day-1), and pAR(4 MJ m^(-2)day^(-1)) and increased RH(≥ 88%),while the variables showed the opposite behavior during the uncommon clear sky(≤ 5.5% of the days).Thus, mostly the dynamic of RH(R^(2)≥ 0.62), ETr(R^(2)≥ 0.85), and pAR(R2≥ 0.77) depended on K_(T). Hence,the prevailing overcast sky influenced the biometeorology of the páramos.

Can testosterone therapy be offered to men on active surveillance for prostate cancer？ preliminary results

This report presents our experience with T therapy in a cohort of T-deficient men on active surveillance （AS） for Gleason 3 ＋ 3 and Gleason 3 ＋ 4 prostate cancer （pCa）. A retrospective chart review identified 28 men with T deficiency who underwent T therapy （T group） for at least 6 months while on AS for pCa. A comparison group of 96 men on AS for pCa with untreated T deficiency （no-T group） was identified at the same institution. The AS protocol followed a modified Epstein criteria and allowed inclusion of men with a single core of low-volume Gleason 3 ＋ 4 pCa. Mean age was 59.5 and 61.3 years, and mean follow-up was 38.9 and 42.4 months for the T and no-T groups, respectively. Of all 28 men in the T group, 3 （10.7%） men developed an increase in Gleason score while on AS. Of 22 men in the T group with Gleason 3 ＋ 3 disease, 7 （31.8%） men developed biopsy progression including 3 men （13.6%） who developed Gleason 3 ＋ 4 pCa. Of 6 men with Gleason 3 ＋ 4 disease at baseline, 2 （33.3%） men developed an increase in tumor volume, and none developed upgrading beyond Gleason 3 ＋ 4. All 96 men in the no-T group had Gleason 3 ＋ 3 disease at baseline and, 43 （44.7%） developed biopsy progression, including 9 men （9.38%） with upgrading to Gleason 7 （3 ＋ 4）. Biopsy progression rates were similar for both groups and historical controls. Biopsy progression in men on AS appears unaffected by T therapy over 3 years. prospective placebo-controlled trials of T therapy in T-deficient men on AS should be considered given the symptomatic benefits experienced by treated men.

Homotypic Vacuole Fusion Requires VTI11 and Is Regulated by phosphoinositides

Most plant cells contain a large central vacuole that is essential to maintain cellular turgor. We report a new mutant allele of VTI11 that implicates the SNARE protein VTI11 in homotypic fusion of protein storage and lytic vacuoles. Fusion of the multiple vacuoles present in vtill mutants could be induced by treatment with Wortmannin and LY294002, which are inhibitors of phosphatidylinositol 3-Kinase （pI3K）. We provide evidence that phosphatidylinositol 3-phosphate （ptdlns（3）p） regulates vacuole fusion in vtill mutants, and that fusion of these vacuoles requires intact microtubules and actin filaments. Finally, we show that Wortmannin also induced the fusion of guard cell vacuoles in fava beans, where vacuoles are naturally fragmented after ABA-induced stomata closure. These results suggest a ubiquitous role of phosphoinositides in vacuole fusion, both during the development of the large central vacuole and during the dynamic vacuole remodeling that occurs as part of stomata movements.

Assuming that the external forces of the system are small enough, the reference temperature being a periodic function, we study the existence, the uniqueness and the regularity of time-periodic solutions for the Boussinesq equations in several classes of unbounded domains of Rn. Our analysis is based on the framework of weak-Lp spaces.
